My 2016 Kia Soul (1.6L GDI engine) experienced a catastrophic, sudden engine failure while driving, preceded by Check Engine Light code P0420 (Catalyst System Efficiency Below Threshold). This exact failure sequence is the precise subject of active NHTSA Safety Recall Campaign 19V120 (Kia Campaign SC176).Although the vehicle received the mandated SC176 preventative software update in 2019, the und
... erlying mechanical defect was not cured. High exhaust gas temperatures caused the catalytic converter to degrade and fracture internally. The fractured abrasive substrate particles were then siphoned backward into the engine combustion chamber during normal valve operation. This caused immediate, severe scoring of the cylinder walls and piston damage, leading to total engine seizure and failure. This defect presents an extreme safety risk. Sudden engine failure at highway speeds causes an immediate loss of motive power, braking assistance, and power steering, significantly increasing the risk of a high-speed collision. Furthermore, fractured connecting rods or oil leakage stemming from this specific engine failure pose a severe vehicle fire hazard. The vehicle is currently held for diagnostic verification at Kia Riverside in California. I am filing this complaint because the manufacturer's 2019 software remedy was entirely ineffective at preventing the mechanical destruction of the engine and catalytic converter. I am requesting that the NHTSA hold Kia accountable to fulfill the physical repair and replacement mandates of Recall SC176 at zero cost to the consumer, regardless of the prior software logging.Read more

The cam carriers for my vehicle are leaking oil. This is an incredible common, seemingly almost guaranteed, issue with these vehicles from Subaru at the 75-90k mile mark. The carrier are dripping oil, so worse than seeping, but not pouring. This runs the risk of the engine bleeding oil fast and running dry without checking oil levels every week or so and carrying extra emergency oil. This is a hug
... e safety risk and given the proliferation of this issue across subaru vehicles and the CVT it needs to be addressed by SOA. There are no warning lights for this issue and this would not have been caught if I hadn't gone to the dealership for a general car inspection as I suspected something may be wrong. The problem has been reproduced at other shops/dealerships and is a very common almost guaranteed issue with these vehicles.Read more

Beginning on 04/17/2026, my vehicle began displaying multiple safety-related dashboard warnings simultaneously, accompanied by audible alerts. The warnings included the Forward Collision Safety System, Blind-Spot Monitoring, Electronic Parking Brake (EPB), Stability and Traction Control, Lane Keep Assist, and AUTO HOLD — all activating at the same time. These warnings reappeared on several occas
... ions and were photographed with date and location data embedded in each image. My vehicle was brought to Burlington Hyundai dealership, where a repair order was opened and a loaner was provided. The car was returned without the issue being fixed. Within days, the same warnings reappeared and the situation worsened. On multiple occasions, the engine shut off completely and the transmission shifted into neutral on its own while my vehicle was being driven at approximately 20 to 40 mph. On at least one occasion, the parking brake engaged automatically without any input from the driver. A dashboard photo clearly documents the message "Engine Off! Shift to N, then start engine." These failures are a serious danger to everyone on the road. When the engine shuts off while driving, normal steering and braking ability is lost. When the parking brake engaged on its own, the car stopped abruptly with no warning. Any one of these incidents could have caused a serious accident or injury. My vehicle was returned to the dealership for a second repair visit and a second loaner was provided. The dealership confirmed via text that the car was still in the shop but never followed up with a diagnosis or timeline. When the dealership was called directly, no update was provided. As of 06/04/2026, my vehicle has been at the dealership for at least 30 days across two visits — 11 days for the first and at least 19 days for the second — with no confirmed root cause, no completed repair, and no clear communication.Read more

The vehicle is experiencing excessive engine oil consumption. The engine burns approximately one quart of oil per week during normal driving conditions. There are no visible oil leaks, and no warning lights, check engine light, or other dashboard messages have appeared. The oil level drops rapidly between oil changes and requires frequent topping off to prevent the engine from running low on oil.
... This condition creates a safety risk because the engine could suffer sudden damage, loss of power, or engine failure while driving if the oil level becomes too low. The problem has been ongoing and is reproducible. The oil consumption appears similar to issues reported in recalls and investigations involving Hyundai engines; however, when the VIN was checked, it was reported as not included in any recall. No external oil leaks have been identified. The component available for inspection is the engine and related lubrication system. Prior to discovering the excessive oil consumption, there were no warning lamps, messages, unusual noises, or other symptoms. The issue was first noticed when the oil level was found to be significantly low despite regular maintenance and no evidence of leaks. The cause is believed to be internal engine oil consumption. UNKNOWN whether the manufacturer has inspected the vehicle. Additional inspection and investigation are requested due to the similarity of this condition to known Hyundai engine oil consumption complaints.Read more

I am filing a complaint regarding the premature head gasket failure on my 2021 Honda Accord, which failed on May 28, 2026, with only 89,000 miles on the vehicle. A head gasket is a critical engine component that should last well beyond 89,000 miles under normal driving conditions and proper maintenance. I purchased this vehicle because of Honda's reputation for reliability and longevity, and I am
... extremely disappointed to experience such a major engine failure at this mileage. After researching the issue, I found numerous reports from other Honda Accord owners experiencing similar head gasket failures, particularly in newer models equipped with the 1.5L turbocharged engine. Many owners report engine misfires, coolant loss, overheating, and costly repairs at relatively low mileage. This suggests there may be a broader issue affecting these vehicles rather than normal wear and tear. My vehicle has been properly maintained, and I did not expect to face a repair of this magnitude before reaching 100,000 miles. The cost of replacing a failed head gasket is substantial and places an unfair financial burden on owners who reasonably expect Honda vehicles to provide dependable service. I respectfully request that Honda review this matter, investigate the growing number of similar complaints, and provide assistance with repair costs. I believe Honda should stand behind its products and support customers who experience premature component failures. Thank you for your attention to this complaint. I look forward to a prompt response and a fair resolution.Read more

... camshaft (p001400, p001404, p001500) may be stored. Cause continuously fluctuating oil pressure in the central valve may lead to damage to the filter screen on the central valve. This can result in damage to the filter screen or the filter screen breaking loose. The resulting loss of oil pressure prevents proper adjustment of the camshaft adjuster. If the filter screen breaks loose, parts of it may settle in the camshaft adjuster or in the camshaft. If there are parts in the camshaft, they can pass through the oil supply bore to the last camshaft bearing (i.E., the first camshaft bearing behind the adjuster) and cause damage there. Remedy 1. In cases in which only the screen of the central valve is damaged and no metal particles are found in the oil filter, the following parts must be replaced: - always both central valves - the camshaft adjuster of the damaged central valve 2. In cases where the screen is completely missing and metal particles are found in the oil filter, the camshaft bearings must be checked. June 3, 2026, approximately 5:30 PM EDT, I entered my 2025 Toyota Sienna to move it from the street beside my home, to my driveway. Upon closing the driver's door, the rear liftgate window exploded into the vehicle. This occurred without any warning or adverse action by myself or anyone else. The vehicle has been in no collisions, and has only 6429 miles on the odometer. I phoned Toyota's "Cust
... omer Experience Center" to report the incident and to seek warranty service for this problem. I was told to drive the vehicle to my Toyota Dealer, which is located 115 miles away, for inspection. I explained that it is unsafe to drive a vehicle filled with broken glass and with no rear window 115 miles. If I vacuum up the glass, there is nothing for the dealership technician to inspect. If I tape plastic over the opening, it could come loose at highway speed, and cause an accident, and the tape adhesive could damage the vehicle finish. The Toyota agent advised me that the vehicle must be inspected at a Toyota Dealership, and if the vehicle is unsafe to drive to the dealer, we could have it towed. But in a subsequent call to the "Customer Experience Center", my wife was informed that Toyota would not cover a tow of that distance in their warranty, and it was our fault for buying a car and taking it to an area that far from a dealer. The phone number we called is located inside the vehicle owner's manual cover under a heading, "If there is not a Toyota dealer near you, please call the following number:" In the morning, once they are open, I will call the nearest dealership, 115 miles distant, to seek further guidance. Based on my background in both mechanical engineering and automotive failure analysis, it is my opinion that the window and its support system was not designed to withstand the prolonged sun and heat exposure experienced. Vehicle: 2019 Nissan Versa SV VIN: [XXX] My vehicle experienced intermittent transmission problems including hesitation during acceleration, shuddering, rough deceleration, loss of the Drive ("D") indicator, unexpected rear-view camera activation, and entry into fail-safe (limp) mode. Diagnostic code P0705 (Transmission Range Sensor / PRNDL Input) was stored. A Nissan dealership determined that
... the cause was a faulty left rear tail lamp assembly. After repairing the tail lamp, all transmission-related symptoms disappeared. My concern is that a failure, corrosion, or poor connection in a rear tail lamp should not be capable of affecting transmission operation, gear recognition, or vehicle propulsion. In this case, a tail lamp electrical fault caused transmission fault codes, loss of proper gear indication, and reduced vehicle performance. This design appears to allow a rear lighting circuit to interfere with transmission control functions. Such a condition may result in hesitation, reduced power, fail-safe mode activation, or loss of propulsion while driving, creating a potential safety hazard when entering traffic, merging, crossing intersections, or responding to emergency situations. I have also located multiple reports from other Nissan Versa owners and Nissan technicians describing similar incidents involving tail lamp corrosion or electrical backfeeding that resulted in P0705 fault codes, transmission-related symptoms, incorrect gear indication, and drivability problems. I just bought my Subaru outback brand new in October 2025. I’ve maintained my oil changes at the dealership. I bought it from in the last one was last may 26. I filled up my tank with a full tank of gas June 1. On June 2 when I was driving home from work on the I 94 Highway in Detroit Michigan I was going about 50 mph when my check engine light went on and within seconds the engine failed. The c
... ar was still on, but the gas pedal wouldn’t work and I was in the far left-hand side lane of the highway. I was able to slowly pull over to the fire right hand shoulder, even though it was during peak traffic time. As I pulled over, I started seeing smoke come from under the hood. I then seen a big flame which made me quickly get out of the vehicle and then seconds two minutes later the entire vehicle was in golf in flames and had many explosions throughout. I had no prior indications notifying me that there was something wrong with my vehicle. I had no problems with it. There was only 12,500 miles on the vehicle. There was no obvious cause to this happening. It was only me in the car however if there were small children in car seats or less mobile elderly people in the car, I don’t think that they would have made it out safely. The fire department came and put out the fire. A police report was made with the Michigan State troopers. My insurance is progressing. They are working on a claim. I also contacted Subaru of America and opened up a case with them. I notified the Subaru of Troy dealership that this happened.Read more

This make and model Honda with a 2.4L engine have several known defects which brings potential harm and high operational and repair costs to its owners. Honda is well aware of the engine problems but refuses to issue recalls to correct the problem. Two of the major problems are: 1. due to a design error, the engine burns excessive amounts of oil, and 2. Honda installed a defective VTC Actuator i
... n this engine. Either problem can cause the engine to suddenly sieze/fail which could lead to serious injury or loss of life. I purchased a used 2014 Honda CR-V from a Honda dealer in 2022 with about 50,000 mi. After a short time owning the vehicle I realized that the engine was burning excessive oil (about 2 quarts every 2 to 3 weeks). When I discussed this issue with my local Honda dealer, they told me that according to a Honda Service Bulliten, my car was not losing oil at a rate that warranted repair. I've owned around 10 Hondas in my life and I've never had a car burn oil where I had to add ANY oil between oil changes. So, burning multiple quarts of oil a month is EXTREMELY UNUSUAL and should have been repaired by Honda! The 2nd problem, defective VTC Actuators, actually may be related to the 1st problem due to a constant fluctuation of the oil level, and lack of oil. The defective VTC Actuators lead to a stretched/worn Timing Chain which can cause engine failure at any time and any place. After experiencing a loud rattling coming from my engine on start-up for some time, I just had several dashboard lights come on. Running a scan on the vehicle it showed a P0341 code. I took my car to my local Honda Dealer where they informed my that I had to replace the stretched Timing Chain and failed/defective VTC Actuator at a cost of over $2600. I believe that fraud was committed by Honda from the day I purchased the vehicle as Honda was aware of both significant engine problems and did not inform us a unexpecting Buyers.Read more

While slowing to a near-complete stop and preparing to shift into Park at a low-speed private property location, the vehicle suddenly and violently accelerated without any driver input. The operator, a [XXX] experienced driver, had no time to react. The vehicle travelled approximately 200-300 meters uncontrolled, narrowly missed a grounds keeper operating a tractor on the paved road, left the pav
... ed road, drove over a fallen log and between trees, and stopped autonomously just before hitting a tree directly ahead. The driver did not brake at any point. The vehicle stopped itself. Physical evidence at the scene shows deep tire marks dug into soft ground consistent with autonomous emergency braking activation directed straight toward the tree. Four witnesses were present including a bystander who nearly was struck by the vehicle. A formal safety case has been opened with the manufacturer's corporate office. Federal safety complaint also filed with Transport Canada. A nearly identical sudden unintended acceleration incident involving a 2023 Porsche Macan while parking is already on record with NHTSA. This appears to represent a pattern on the same platform and warrants formal investigation. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I own a 2022 Nissan Rogue with approx 71,139 miles on. While driving on the freeway at 65mph, the vehicle suddenly lost power and became disabled. Prior to the failure, the vehicle did not dispaly an overheating warning, coolant warning, or other indication the engine was overheating or experiencing coolant loss. The vehicle was being driven normally and was on the way to routine maintenance and f
... uel stop when the failure occurred. An independent repair facility diagnosed coolant intrusion into the combustion chamber and determined the engine had overheated and required replacement. The vehicle's sudden loss of propulsion occurred without warning creating a safety hazard for myself and my daughter as I was forced to coast to a stop in the Gore Area of the freeway just as you entered the [XXX] from [XXX] due to the inability to maintain speed on the freeway. This was the safest area for us to park but it was terrifying as cars zoomed past us from both sides. I am concerned that the engine failed without adequate warning to the driver and that similar complaints have been noted by several others describing the same issue with the same year, make and model as my car. Multiple reports have said it occurred while on the freeway, making this a greater concern for safety. I took my car to Nissan who stated they see the coolant intrusion, but that it was not covered under recall. I asked what caused the internal leak, to which they replied that would require them to take apart the engine to examine further at my cost. I advised the issue with the car not indicating it was even out of coolant to cause it to overheat was concerning because even if I replace the engine, if the electrical failure isn't addressed I could run into the same issue and this time actually get into an accident that could cause death or serious injury. My car is still at Nissan Mission Hills Dealership and available for inspection upon request. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The driver’s seat lower cushion in my 2023 Toyota Highlander provided inadequate support during normal driving, particularly on the left side of the seat cushion where support for my left leg and hip area appeared insufficient. This caused uneven seating posture, increased discomfort, pressure, and difficulty maintaining a proper driving position during extended periods of driving. The seat bec
... ame increasingly uncomfortable during prolonged use and caused pain, numbness, and difficulty maintaining a proper driving position. I am a professional driver and spend many hours operating the vehicle. Over time, the lack of support contributed to left-sided sciatica symptoms that required medical evaluation and physical therapy. Medical records document that prolonged sitting in the vehicle aggravated my condition and limited my ability to sit and drive comfortably. An independent upholstery specialist inspected the seat and determined that additional foam had to be added to the driver’s seat lower cushion to improve support and comfort. After the modification, the seat became significantly more comfortable. This indicates that the original seat cushion support may be inadequate. I believe this condition may affect other vehicles of the same model and should be investigated as a potential safety concern because it can affect driver posture, comfort, concentration, and safe vehicle operation during extended driving.Read more

On Thursday, May28 I was driving my 2025 Ford Mach-2 on Schrock Rd. in Columbus, Ohio at approximately 3:30 PM when the hood (the "frunk") of the car unexpectedly unlatched and opened, flying up to a vertical position and obscuring my view of traffic. I was driving in the left lane of this road at 40-45 mph. My view of the road was immediately obstructed, but I was able to crouch sufficiently to s
... ee enough of the road ahead and the mirrors on each side to navigate to the right lane and then to the side of the road, pull off safely, turn the car off, exit, and close the frunk firmly. I was away from home when this occurred. When I got in the care to drive home, there was a warning that came on, for the first time, that read as follows: FRUNK FAULT DETECTED; SERVICE REQUIRED. There was no advice to avoid driving the car. I checked the hood/frunk to be sure that it was firmly closed, then started for home, avoiding my usual freeway drive at 65mph for a slower trip on local roads. It is a good thing I did, because the risk of a major accident with injury and serious property damage would have been far greater at higher speed. I arrived home without the frunk opening again, called for Ford Roadside Assistance, and arranged to have the car towed to Germain Ford of Columbus, where it still is, awaiting the delivery of a new frunk latch system. The dealer has determined that there is likely a defective sensor in the frunk latch. This is a known issue with the Ford Mach-e, affecting not only the frunk but also sometimes the hatch lid in the rear. This is a serious safety hazard, and a review of reports over several models years from Mach-e owners indicates that I am not alone in having experienced it. At highway speeds a suddenly unlatched frunk/hood would not only open to the vertical, but conceivable also be smashed against the windshield or ripped off the vehicle and become airborne. Ford is aware of the issue, for there is are technical service bulletins covering it.Read more
