I filed a complaint when the first engine seized. It's a miracle we got off the road safely that day. While it was at the dealership, the engine was replaced with a used engine and covered by my extended warranty from Vroom. I believe the cost was approximately $14,000. I was told the engine had around 30,000 miles on it. I've put around 20,000 miles on this engine and the same thing happened agai
... n. This time, the engine had been knocking for a few months. When it started knocking, I took it to the dealership because the new extended warranty by Hyundai was in effect and I assumed it would be covered. Immediately, Autonation Hyundai in Savannah, GA said it would not be covered because it was a used engine. I don't understand why when the engine can be traced and the mileage known by the number on the engine. On XXX, my [XXX] granddaughter took her driving test in the vehicle. About 2 hours later, the engine seized with her driving. We were on the same road and it was about the same time of day. Once again, it was a miracle we got off the road safely. I just talked to Hyundai and we'll see what they say. From the stories I've seen, they'll do anything they can to avoid covering these engines and they get away with it. The people who get hurt the most from this are the ones who can least afford it. I'm a [XXX] widow raising my 5 grandchildren by myself and believed I was getting a good vehicle, mainly due to the report by Consumer Reports magazine. Even after the first engine failed and was replaced, I still wasn't in a financial position to get another vehicle and assumed if something happened again, it would be covered under the extended warranty Hyundai was made to provide. Obviously, that means nothing. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

[XXX] [XXX] [XXX]; [XXX] [XXX] [XXX] Date: XXX To: National Highway Traffic Safety Administration Office of Defects Investigation (ODI) Subject: Formal Safety Defect Complaint – 2017 Hyundai Sonata Theta II Engine Failure, Excessive Oil Consumption, Intake/Engine Component Failure and Safety Hazard Dear Sir or Madam: I am submitting this formal complaint and request for immediate investigat
... ion into the dangerous and ongoing mechanical failures involving my 2017 Hyundai Sonata equipped with the Theta II engine. I believe this vehicle contains a serious manufacturing and/or engineering defect that presents a substantial safety hazard to consumers. My vehicle has experienced a pattern of progressive mechanical failure beginning with abnormal and excessive oil consumption, which appears to be directly associated with malfunctioning engine airflow and intake-related components. Specifically, the lower engine airflow system and intake functionality appear defective, placing excessive strain on the intake manifold system and contributing to cascading engine failures. As a result of these failures, the vehicle has experienced: Severe and excessive oil burning/consumption Intake manifold malfunction and abnormal operating stress Suspected overheating and/or premature head gasket failure Ignition coil failure and electrical shorting/misfires Progressive engine deterioration rendering the vehicle unsafe and unreliable Increased risk of sudden engine shutdown, stalling, fire hazard, or catastrophic engine failure while operating The vehicle has become increasingly unreliable and inoperable due to these conditions. These failures raise significant concerns regarding public safety, especially given the known history of complaints and recalls associated with Hyundai Theta II engine defects. I am deeply concerned that this vehicle may reflect a broader pattern of defective manufacturing, insufficient quality control. I heard knocking as I was leaving gas station. I was scared about what was going wrong and if it was safe to drive home. I was alone and it was after dark. I took the next day off and took the car to my mechanic. They stated that the noise was coming from main shaft, unable to repair, will need to replace engine. Cost estimate to replace engine is $15,964.36. I do not remember a check engine light coming on, only the lights for the driving sensing items. Which I read were also problems on this vehicle. Photos of the inspection report have been uploaded. A mistake was made on the mileage it is 103,203 not 103,703. Thank you for your time.Read more

The tow couldn't be completed by the assigned wrecker service because it was busy with other tows because of the severe storms. Repair took just over 3 months to complete under warranty/recall at dealership, didn't get vehicle back until the beginning of September 2025. Engine replacement Inspection passed by GMC on the recall inspection at local dealership. Oil changed from factory 0W-20 to 0W-40 around January or February 2026 if I'm not mistaken. Driving down the highway on 06/03/2026 and all the sudden loss of propulsion. Zero warning! Lucky I was able to coast onto an exit ramp without getting in an accident with hwy. traffic! Luckily i was able to find a church parking lot and asked a person that i knew to give me a ride home. Because it was after business hours, i waited till the local dealership opened the next morning and spoke with service personnel and they arranged a tow to take it to the dealership. As of the end of the business day 06/05/26 I have no word from dealership on the status of vehicle, but the vehicle experienced the same symptoms, a sudden total loss of power and no start, just a single click and no electrics. My 2016 GMC Terrain 2.4L Ecotec first stalled while idling at a red light in Raleigh, NC. I restarted it and it sounded different, so I drove it home. The next morning it would not start. The failure occurred at approximately 69,939 miles with no meaningful warning. The dealer’s diagnostic report states there were no current relevant codes and no relevant codes in history. The dealer documented
... major timing-system failure: timing chain tensioner failure, timing chain guide failure, a timing chain guide bolt pulled out of the cylinder head, aluminum contamination in the cylinder head/oiling system, and engine replacement recommended. The vehicle was found unsafe to operate. This vehicle regularly transported my family, including my children. If the stall/no-start failure had occurred at highway speed, while merging, or in heavier traffic, it could have created a serious crash risk. The oil was maintained outside the dealership network and had been serviced before failure. I also topped off the oil shortly before this occurred. The 2.4L Ecotec engine family is known for oil-consumption issues, yet this vehicle gave me no direct warning that oil consumption had reached a dangerous level. A reasonable owner would not know to check oil every day when there were no relevant stored warning codes. Other NHTSA complaints involving 2016 GMC Terrain vehicles describe similar patterns: oil consumption, no warning lights, sudden stalling or no-start conditions, timing-chain failure, and engine replacement recommendations. I am filing because this appears to be an ongoing safety risk involving oil consumption without adequate warning, timing-chain/timing-guide failure, internal contamination, and sudden engine failure. I request NHTSA review this issue for investigation or recall expansion before this failure mode causes a preventable serious crash, injury, or fatality. The vehicle was inspected by a GMC dealer and remains unrepaired.Read more

To Whom It May Concern: I am submitting this complaint to report ongoing safety concerns involving my 2013 Hyundai Sonata. I purchased this vehicle in 2022 through financing and paid it off in 2025. Since acquiring the vehicle, I have experienced persistent mechanical issues that I believe may pose a safety risk to myself, my child, and others on the road. The most concerning issue involves the
... braking system. I have had the brakes replaced and serviced multiple times, yet the brakes begin grinding again within a very short period, including an instance where the grinding returned approximately one month after replacement. Despite spending money on repairs, the problem continues to reoccur. I am concerned that there may be an underlying defect affecting the braking system that is not being properly identified or corrected through normal maintenance. In addition, the vehicle consumes an excessive amount of oil, requiring frequent monitoring and replacement between oil changes. I am concerned that this issue could potentially lead to engine failure or other dangerous mechanical problems while the vehicle is being operated. As a single mother, I rely on this vehicle daily for transportation to and from work and for caring for my family. I have experienced significant financial hardship attempting to keep the vehicle operational and safe. Despite my efforts to maintain the vehicle, these issues continue to occur. I respectfully request that the National Highway Traffic Safety Administration review this complaint and determine whether these issues may be related to known defects, recalls, investigations, or patterns reported by other owners of 2013 Hyundai Sonata vehicles. I am willing to provide maintenance records, repair invoices, mileage information, and any additional documentation that may assist in evaluating this matter. I am writing to report a severe, escalating safety defect regarding the "ghost touching" / phantom input malfunction of the infotainment head unit on my Subaru Outback. This electronic defect has escalated from a minor distraction into a severe hazard that directly compromises the operational safety features of the vehicle, putting myself and my children at imminent risk of a crash. When this prob
... lem initially began, I investigated whether a safety recall had been issued. At that time, I was informed that my vehicle was outside of its standard warranty coverage period. Since then, the defect has degenerated significantly. The system now suffers from continuous, uncontrollable phantom inputs ("ghost touching"). Even when I manually power the radio off, the system overrides my command, turns itself back on, and initiates constant, rapid beep sequences. This continuous, high-volume auditory glitch creates a dangerous critical safety failure: the nonstop beeping totally masks and completely drowns out the vehicle's actual auditory collision avoidance and driver-assist safety alerts. Because of this defect, I am entirely unable to hear the safety tones meant to warn me of immediate hazards, including blind-spot detection alerts, lane-departure warnings, and cross-traffic alerts. This vehicle has become an active hazard to drive, as I am deprived of the core safety features I rely on to protect my family. I contacted Subaru of America to resolve this dangerous equipment failure, but they refused to offer any financial assistance or repair support. Subaru Corporate documented this refusal under Subaru Case #260603-1600711. Because Subaru refuses to address a known component failure that actively disables crash-avoidance warning systems, I am petitioning the NHTSA to investigate this defect as a systemic safety hazard before it results in severe injuries or a fatal accident. I understand it a 20 yo car but no rust anywhere on the body or the undercarriage but the engine cradle is almost completely rusted out to the point that the car is not safe to drive something is definitely wrong and reading others reporting the same issue I don’t know there car’s condition but like I said mine is rust free even the undercarriage but the engine cradle is totally rusted out something is wrong here Chrysler had to know something about this it should be recalled or at the very least make the engine cradle available for purchase and fix the rusting issue with it!! I was using Full self driving (FSD), but I notice the autopilot gets "impatient" when (correctly) stopped for a "long" period of time (~2 minutes), but will then try to proceed in dangerous conditions. 1. I had FSD engaged while waiting to make an uncontrolled left turn. The street I'm turning onto is busy, so both lanes are going at a steady pace. After about 2 minutes of waiting, the autopilot
... decided to make the left turn while BOTH directions had cars approaching. It turned towards a motorcyclist (which might be why it thought the lane was "empty"), and cut off a rather large car behind the motorcycle which almost caused a collision. I was honked from both sides (rightfully so) and put in a very dangerous position as my car was now stopped in the middle of the row of both lanes. 2. I was at a red light intersection with no traffic coming from either side with FSD engaged. After about 2 minutes of waiting, the car tried to go on its own while the light was still red. After manually stopping the autopilot, the light naturally turned green a few seconds later.Read more

My 2016 Ford F-150 is currently subject to Recall 26S28 / NHTSA Recall 26V237. Prior to the recall remedy becoming available, the vehicle began experiencing erratic transmission shifting and illuminated the check engine light, wrench symbol, and codes P0720 and P0722 were present. I brought the vehicle to an authorized Ford dealership for diagnosis. The dealership documented that the vehicle was
... experiencing erratic shifting and was considered not drivable. The technician determined that the erratic shifting condition was related to DTCs P0720 and P0722 and recommended replacement of the transmission lead frame/conductor plate. The technician also noted the need for additional diagnostics involving the Output Speed Sensor (OSS) circuit within the transmission. The vehicle remains available for inspection at the dealership. This condition created a safety concern because the transmission was shifting erratically and the vehicle could not be relied upon for normal operation. Unpredictable transmission behavior creates a risk during acceleration, merging, highway travel, and other driving situations where consistent vehicle response is required. At the time, I was preparing for a cross-country military PCS move and did not believe the vehicle was safe or reliable to drive long distance. The problem was confirmed by an authorized Ford dealership who documented the condition, diagnostic codes, and repair recommendation. The findings were provided to Ford Motor Company through Corporate Case Number CXH-08096574-L9M6T4. Ford representatives advised that the Automotive Safety Office was notified regarding the technician findings and the open recall. My concern is that the vehicle developed transmission-related symptoms involving P0720, P0722, erratic shifting, and a recommended lead frame/conductor plate replacement while subject to an active safety recall affecting related transmission systems. Ford has stated that the recall remedy is not available until July 2026.Read more

The contact owns a 2016 Ford Escape. The contact stated that while stopped at a traffic light and depressing the accelerator pedal, there was a clanging sound coming from the front of the vehicle. The vehicle then started to work as intended before coming to a complete stop at the second traffic light. The contact stated that once the traffic light turned green, the accelerator pedal was depressed
... and the vehicle rolled backwards. The contact was informed by her husband to shift the gear shifter into park(P). The husband jumped into the driver’s seat and shifted the gear shifter to low and driven to a safe place and stopped the vehicle. The contact stated that while the vehicle was stopped, the vehicle was turned off for thirty minutes then restarted and operated as intended. The transmission shifter failure warning light was illuminated while her husband was driving. A dealer was contacted. The vehicle was towed to a dealer where it diagnosed and determined that the transmission needed to be replaced and provided an estimate for the repair. The vehicle was not repaired. The manufacturer was notified of the failure but offered no assistance. The approximate failure mileage was 99,000.Read more

The inspection sticker mechanic inspected the car and found the rear brake hoses rubbing against the sway bar indicating that there should be a clip somewhere to prevent rubbing. See picture attached. See if a possible safety recall can be done on all Toyota RAV4 vehicles.

I am writing to formally request reconsideration of Hyundai’s denial of assistance for the transmission failure in my 2021 Hyundai Palisade. I purchased this vehicle with the expectation that Hyundai vehicles are built to provide dependable transportation and that major components such as the transmission would last well beyond the current mileage and age of the vehicle. Unfortunately, I have e
... xperienced a significant transmission failure that has resulted in substantial repair costs. I have made every effort to properly maintain this vehicle and have acted responsibly as an owner. Given the relatively recent model year of the vehicle, I believe a transmission failure of this magnitude is unreasonable and may indicate a defect in materials or workmanship. I respectfully request that Hyundai review my case again and consider providing goodwill assistance, cost-sharing, or full coverage of the transmission repair. As a long-time consumer, I am extremely disappointed with the current outcome and hope Hyundai will stand behind its product. I would appreciate a written response explaining the reason for the denial and any options available for further review.Read more
