My car started overheating on my way home. I pulled over to find that there is a hole in my lower radiator hose, which was caused by the lower radiator hose being rubbed against the belt that is turned by the water pump. I had my wife and child bring me coolant on the side of the road where I was broken down. I eventually was able to get my car to my brothers but it required multiple stops and ref
... illing of coolant. Once we got the car jacked up and the lower radiator hose replaced, we found the bottom of the radiator is loose and is pushing the lower radiator hose against the belt that rubbed the hole into the lower radiator hose. I then had my car towed to Dan Cummins in Georgetown Kentucky, located at 1470 Cherry Blossom Way, Georgetown, KY 40324. The mechanic drove my car that I had towed to the shop which damaged the new radiator hose that I had just put on the car. Dan Cummins of Georgetown Kentucky, who is an authorized Chevrolet Dealership, was informed (by me) that some 2016 Chevy Malibu's have been recalled because of a faulty lower radiator mount causing the lower radiator hose to push against the belt which causes the lower radiator hose to get a hole and leak. Dan Cummins suggested that I report my issue because my VIN is not covered by the recall. Authorized Chevy dealer Dan Cummins also suggested that I take my car elsewhere to be repaired while refusing to cover the damage they caused to the brand new lower radiator hose that I had purchased and put on the car. I would like for my Chevy Malibu with VIN [XXX] added to the recall so that I can have my Chevy Malibu repaired by an authorized Chevy dealership at no cost to me for something I did not do or cause. An intermittent electrical failure occurred to my wife causing my 2021 Volvo XC60 to stop and then later go into limp mode while driving. The service light went on immediately and was very startling to her and made it difficult for the drivers following as the car speed was significantly reduced/ I suspect limp mode though it was not on the dashboard. When I called Volvo and then brought the car
... in... it is a known issue to Volvo. They purposively produce a vehicle that is known for sunroof leaks which knowingly leads to electrical issues exposing their customers to harm. Volvo (Geely) inspected the car, found the electrical issues from the front interior to the battery compartment that had water over an inch deep. the VGM (Vehicle Gateway Module) was replaced and reloaded, all connectors were separate/dried and several had to be cut/spliced and rewired due to corrosion damage. Carpets were removed, the gasket was replaced, the front and rear sun roof drains were cleaned out. Aside from the safety concern the shrinking of the rubber gasket caused the issue. Not funny, but Volvo/Geely states that the repair now has a lifetime warranty after I was charged over $4,000 USD. The vehicle is only about 5 years old!!Read more

SUBJECT: Formal Safety Defect Complaint: 2016 Subaru Outback – Manufacturing Nondisclosure of Latent "Day 1" Design Flaw Causing Ignition Key Entrapment, Uncommanded Steering Column Lockout, and Severe Family Safety Risk I am writing to formally report a severe, unaddressed safety defect and a deceptive corporate practice by Subaru of America, Inc. affecting my 2016 Subaru Outback. This vehicle
... suffers from a "Day 1" latent engineering flaw that causes the physical ignition key to become permanently trapped in the ignition cylinder, triggering an uncommanded, rigid locking of the steering column assembly. Subaru has possessed documented knowledge of this failure for years, cataloging it under Technical Service Bulletin TSB 16-112-18R. Rather than fulfilling its statutory obligation to issue a comprehensive federal safety recall, Subaru deployed a voluntary, time-bound 8-year warranty extension. Subaru conveniently did not notify me of this warranty extension. By doing so, the manufacturer unconscionably ignored this defect, placing its customers at direct peril. They capitalized on asymmetric information to shift both the financial burden and the severe physical safety risks of a known factory flaw onto my family, the moment an arbitrary time threshold expired. My family and I have been at active, continuous risk in this vehicle since the day we purchased it due to an engineering error that was present when the car rolled off the assembly line. The root cause of this failure is a fundamental engineering defect within the Continuously Variable Transmission (CVT) shifter assembly, specifically, the premature degradation of the electrical contacts within the select lever park-range switch.Read more

Incident date: May 29, 2026
Description of Safety Defect: The Automatic Emergency Braking (AEB) / Forward Collision Avoidance system is severely malfunctioning. On multiple occasions, the vehicle has experienced "phantom braking"—abruptly slamming on the brakes for no reason whatsoever. There were no obstacles, pedestrians, or vehicles ahead to trigger the system. This malfunction is happening at high speeds on major high
... ways. Safety Risk and Impact: This issue represents an immediate and terrifying safety hazard. Unexpectedly slowing to a sudden stop at highway speeds completely compromises vehicle control and leaves me at an extreme risk of being rear-ended by high-speed traffic. It is a severe danger to myself, my passengers, and every other driver on the road. While the vehicle is currently subject to a manufacturer recall regarding the backup camera and other software updates, this specific, dangerous braking behavior makes the vehicle incredibly unsafe to operate. I request that the NHTSA review this issue immediately to determine if a broader safety investigation or expanded recall is required for the 2024 Honda Prologue.Read more

During the second day of ownership I was driving with no vehicles in front of me and only oncoming traffic, and I heard a loud cracking sound from the windshield. It did not sound like something impacted the windshield, and I did not see any object coming toward the vehicle. Two parallel cracks immediately developed in the windshield beginning from the headliner directly in front of the driver's s
... eat, and they spread down toward the dash approximately 4", then the cracks turned, remaining parallel, and spread horizontally toward the passenger side of the vehicle. The lower crack stopped, while the upper crack spread the following day to the rearview mirror assembly. Dealership determined it to be caused by impact, but the manner in which the crack spread and the path it took suggests improper windshield installation from the factory and improper tension on the glass, perhaps from a faulty or improperly installed windshield clip. BMW online forums suggest many customers are having identical problems with windshield tension cracking on this generation of the X3, complete with narratives and photographs.Read more

I own a 2019 Honda Odyssey equipped with Honda Sensing. The vehicle has repeatedly displayed multiple driver-assistance system failure warnings, including “Adaptive Cruise Control Problem,” “Lane Keeping Assist Problem,” “Collision Mitigation Braking System Problem,” “Road Departure Mitigation System Problem,” and “Auto High Beam Problem.” The warnings initially appeared inter
... mittently and then began recurring. In some cases, the warnings appeared at startup and remained active while driving. When the warnings are present, multiple Honda Sensing safety features are disabled simultaneously. To address a possible low-voltage condition, I replaced the vehicle’s battery with a new battery. After replacement and normal driving, the warnings continued to occur. The issue therefore does not appear to be solely related to battery condition. I also observed clouding and dark spotting within the front Honda emblem that covers the forward radar sensor. The discoloration appears to be inside the emblem assembly rather than on the exterior surface. Because the radar sensor is located behind this emblem, I am concerned that deterioration of the emblem or radar assembly may be affecting the operation of critical safety systems. This failure impacts several safety-related functions, including adaptive cruise control, collision mitigation braking, lane keeping assistance, road departure mitigation, and automatic high beams. The loss of multiple advanced driver-assistance systems creates a potential safety risk because the vehicle’s collision avoidance and lane assistance features become unavailable without warning. I am submitting this complaint to document a recurring failure affecting multiple Honda Sensing safety systems on my 2019 Honda Odyssey. There are numerous YouTube and social media posts on the same failure for same vehicle. It seems to be an issue for 2019 yearRead more

REAR PASSENGER SEAT BELTS ON BOTH DRIVERS AND PASSENGER SIDE: The seat belt would not fully pull out/extend, and allow the passenger OR the child safety seat to be properly secured. It would lock up, and once the belt was pulled out, it would lock up on the retraction and not allow any extension. I took to the KIA dealership - which took a total of 3 visits- DEMONSTRATED and recreated the issue-
... which confirmed there was a problem. This did not happen on each use, but once I realized the child safety seat could not be installed properly (fully extended seat belt, retraction/ratcheting of belt to secure seat) I returned, and asked the service manager also observe along with at least one of the service techs. They were skeptical and not even fully aware of how a child seat should be installed, and the full extension of the seat belt, and the SOUND of the ratcheting/locking of the belt once fully extended. A repair/replacement was eventually done- parts had to be ordered and I could not find this “defect” on any KIA or other websites. It took a number of days for the replacement/repair. There were no warning lights that there were any problems with the safety belts. I (retired) police officer with NHTSA safety seat instructor certification (not current) and have knowledge of this, and it wasn’t until I explained and demonstrated the system to these dealership employees the proper function did they “believe” me. I just read of the a KIA recall on their Telluride models, and it sounds like a similar problem. Until it was fixed, I had a couple of occasions where the passenger was unable to secure their belt, which was before I realized that there was a manufactured problem with this system.Read more

Incident date: Jun 1, 2026
The systems affected with failure notifications were Electric Parking Brake, TPMS, Emissions, Brake, Brake Hold, Power Steering, Vehicle Stability Assist, Hill Start Assist, Adaptive Cruise (including regular cruise), Collision Mitigation/Avoidance, Road Departure Mitigation. The stored codes in the vehicle that I was able to read were P0300, P0303, P0304. This failure bears a striking resemblance
... to that of TSB 21-010, directed at Keihin manufactured fuel injectors, PN: 16010-RLV-315, which was later replaced with updated ...-305 after the failure was determined, and the vehicles were given a 10 year 150,000 mile warranty. The CRV had injector PN: 16010-5PA-305, also manufactured by Keihin, which was also later revised to 06160-..., but the CRV was not given the same warranty extension, suggesting Honda acknowledged the same defect pattern across both injector families but chose not to extend equivalent coverage to the CRV. 21-010 also details that debris from the high pressure pump may contribute, and the CRV experienced the same fuel pump recall for low density impellers that these vehicles did. Honda is also obligated under TSB 98-081 to perform emissions services due to OBD inadequacy, which this vehicle experienced. Misfires were felt for months with no CEL. The Norm Reeves Honda parts catalog classifies injectors as emissions components, implying they should be covered. However, a Honda dealer in CA, mandated by CARB and EPA, refused to do the repairs or make any goodwill attempt on my behalf. I then reached out to American Honda, who delayed assigning a case manager as long as possible, then required phone contact after initially accepting alternate communication methods, then assigned a case manager whose hours conflicted with my stated availability, then assigned a second who has not answered the phone. I am still awaiting a callback. The vehicle has been immobile and in limp mode for the past two weeks while Honda's callback process remains unresolved.Read more

The Service Director initially pushed back significantly, attempting to dismiss the safety keywords and label a major steering defect as a vague 'multi-system failure' on the printed Repair Order. After consumer refusal to accept the vehicle, the Service Director capitulated, personally authorizing a Repair Order explicitly documenting in handwriting: 'Also Steering Sticky while CAR in motion. Also vehicle jerky at low Speeds.' The Service Director physically signed next to these terms. The Service Director explicitly acknowledged that the symptoms sound like the NHTSA recall 24V-744, but stated they must put up to 300 miles on the vehicle to 'replicate' the issue before replacing the steering rack due to fear of corporate financial chargebacks, actively attempting to force a consumer to keep an unsafe vehicle unless able to replicate.Read more
