Safety defect involves an extreme structural hazard caused by the improper execution and assembly of components under open Federal Safety Recall Campaign 23V905 (Ford Recall 23S64) on a 2018 Ford EcoSport. The vehicle previously underwent the authorized interim engine replacement mandated by Safety Recall 23S64. Following a recent diagnosis by certified technicians to investigate severe vehicle noi
... se, it was discovered that all but one of the main front engine subframe (cradle) bolts are completely missing from the vehicle. The front subframe must be completely dropped and reinstalled to execute this specific recall's interim repair. The missing hardware is a direct result of critical structural fasteners being left loose or entirely uninstalled during the official recall service. As a direct consequence of the detached subframe shifting dynamically while driving, the vehicle has suffered severe structural and tire damage and tracking misalignment. The vehicle has been officially deemed entirely unroadworthy and unsafe to drive due to an imminent risk of structural collapse or catastrophic steering and control failure. The consumer is currently facing a refusal of accountability regarding this safety failure. The manufacturer's customer service division is classifying this severe hazard as a localized dealership workmanship dispute rather than a critical failure of federal safety recall integrity, leaving an unsafe, recalled vehicle unaddressed and grounding the consumer.Read more

The transmission was shifting extremely rough. When attempting to accelerate from a stop, the RPMs would increase without shifting for several seconds causing me to be stuck in the middle of intersections. It would also slip from a higher gear to a lower gear for no reason. This included while driving on the interstate causing a sudden loss of speed. A local transmission shop confirmed the issue a

The engine (connecting rod bearings in the 2.0L Nu GDI engine) failed. The vehicle entered limp mode with DTC P1326, then died on the road and the engine seized completely (will not turn by hand when attempted; only clicks on starter). It is currently at First Hyundai dealer in [XXX], under Repair Order #XXXand available for inspection. The vehicle suddenly lost power and died while driving on th
... e road. This created a significant risk of an accident or being stranded in traffic. I was on a busy highway and almost got hit twice. There is also a known fire risk associated with this exact defect (oil leaking onto hot engine components), as stated in Recall 209 (NHTSA 21V727). The vehicle was towed to First Hyundai. They have documented the no-start/seized condition on Repair Order #XXX. A prior independent scanner reading showed P1326 before the code was cleared. The dealer has not performed the full diagnostic inspection (scheduled around June 11). Hyundai Corporate (Claim #XXX) has been notified but has not responded after multiple escalation emails. The vehicle first went into limp mode ("Engine Protection Mode") with DTC P1326 on the scanner. Shortly after, the engine died completely and would not restart. This occurred after Recall 209 (bearing/KSDS campaign) was completed on XXX. Additional Details: This is a known issue with premature connecting rod bearing wear. The dealer is refusing to provide a loaner/rental upon drop-off, which contradicts TSB 22-01-023H (Dealer Best Practices for P1326 vehicles). The vehicle has been inoperable since 6/4 ..with no resolution. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am reporting a severe safety hazard on my 2020 Audi Q8 directly caused by the software update for NHTSA Recall 25V900 (Audi 90TV). The recall shows as "completed" on my VIN, but it left the vehicle with an active driver assistance defect that did not exist before. No accident has occurred, but there is a severe risk of a crash. Following the recall, the Audi Side Assist (blind-spot monitoring)
... behavior changed dangerously. When I signal a lane change while a vehicle is in my blind spot, the mirror LEDs blink only 3 times and then return to a solid, dim state—even though the vehicle is still actively there. If you miss that brief 3-blink window, the dim LED falsely implies the lane is clear. This has caused 3 separate highway close calls (no crash occurred) where I almost merged into a vehicle because the warning period is inadequately short. This defect followed a catastrophic failure during the recall execution. I dropped the car at Audi Chantilly on Jan 13, 2026. When picked up on Jan 29, the lane-keep assist was broken (bouncing the vehicle left and right between lanes) and an SOS malfunction light was on. It took the dealer over 2 additional months to "fix" the corrupted camera/localization modules, finally returning the car in early April. While the major module errors are gone, the underlying side assist logic remains broken and hazardous. Because of a bitter, 3-month waiting experience where the vehicle was degraded, I do not trust Audi Chantilly to fix this. I need guidance on how to compel Audi to investigate this post-recall software defect and allow me to have it corrected at a different dealership.Read more

I have been having issues with my check engine light being on for about 6 months now. Took the car to Honda to get it diagnosed and was told that I needed to get spark plugs, fuel injectors, and ignition coils. Got all of that changed and the issues still remains. Had an outside mechanic take a look and he specializes in Hondas and was told that it is the head gasket, and that would be the only w

I was getting on the interstate and my car just started stalling, had to immediately pull over before getting hit. I was quoted $3000 to have the head gasket done. This obviously is an ongoing issue with my model Hondas, and I am already out of pocket $1000 with the issue not even being resolved.Read more

OnXXX, as we approached what I call the 6 o’clock position of a round-about in [XXX], we had braked almost to a full stop (0-5 mph) to wait on the car ahead of us to enter the round-about when we felt increasing movement. Despite braking, to wait on that car, our car with increasing speed started shoving that car into the circle. Fortunately, he was able to drive away from us, but that car had
... been damaged on the back and right side. With that obstacle out of the way, our car rapidly and exponentially increased speed - all while we were fully braking. Two witnesses said they saw our brake lights as we sped along. We hit the curb as we approached the 12 o’clock exit, and that turned us so that we crossed both lanes of the road and headed towards the far sidewalk and storefronts. When we hit that curb it pushed us diagonally to face the street again. Hard to estimate but we must have been doing 50 mph when we hit a very large brick planter and we finally were wedged to a stop when we slammed in between two vehicles parked parallel to the sidewalk. The motor ran for a few seconds after we stopped, and finally quit on its own. We KNOW the brake was applied the whole time; there were no visual or audible alarms from the car as we entered “UNINTENDED ACCELERATION”. Despite the incredibly strong impact (our car has been deemed totaled) the front air bags did not deploy. Only the driver’s side curtain deployed. The bumper of the parked transport van on our front left was damaged, and the small SUV on our front right was deemed totaled. No people were in those two parked cars; the first car was hit at the initial slower speed so those people were not injured. By some grace, no pedestrians were in our path or they would surely have been killed. The driver of our car (my husband) was miraculously not injured, but CT scans and x-rays showed I had several contusions and a fractured tibia that will require surgery due to brake, propulsion, no air bags. Subject: Urgent Safety Concern – Driver Seatbelt Malfunction I am writing to formally report a serious safety concern regarding the driver-side seatbelt in my vehicle. The seatbelt does not retract properly and frequently becomes caught in the door. In addition, the retractor fails to pull the seatbelt back completely, leaving slack in the belt. More concerning, there have been multiple instance
... s where the seatbelt did not properly engage or lock when braking was applied, even though it was securely fastened. I brought the vehicle to the dealership for inspection, but I was advised that this condition is “normal.” I strongly disagree with this assessment. A seatbelt that does not retract correctly or fails to lock during braking poses a significant safety risk and does not meet expected safety standards. This issue raises serious concerns about the reliability of the restraint system in the event of an accident. I am requesting a thorough inspection of the driver-side seatbelt system, including the retractor mechanism, and that any defective components be repaired or replaced immediately. Please document this concern formally and provide written confirmation of your findings and recommended corrective actions. I am very concerned for my safety and expect this issue to be handled with the urgency it deserves.Read more

I am reporting a serious safety defect involving the door locking and electrical systems in my 2024 Audi SQ8. On two separate occasions the vehicle experienced a complete electrical malfunction. During the first incident, I and my child were trapped inside the vehicle. The doors would not open from the inside and the vehicle would not start. After the first incident, the vehicle was taken to an Au
... di dealership for inspection and repair. I was informed that the issue had been identified and repaired. Despite those repairs, the same safety issue occurred again. During the second incident, which was captured on video, the vehicle would not start despite repeated attempts. The vehicle malfunctioned again after the dealership had already stated the problem was fixed. Dealership personnel acknowledged that they could not guarantee the problem would not happen again. I was also provided a video demonstrating a manual emergency exit procedure because the electronic door locking system could become inoperative again. The dealership showed how occupants would have to use a mechanical override to exit if the electronic system failed again. This defect creates a significant safety risk because occupants can become trapped inside the vehicle unable to exit during an emergency. The inability to start the vehicle combined with failure of the electronic door locking system could delay escape in the event of a fire, collision, medical emergency, or other hazardous situation. The vehicle has been inspected by an authorized Audi dealership multiple times. The problem was reproduced by the vehicle itself on multiple occasions, and I have video evidence documenting one of the incidents. Because of these repeated failures and the safety risk posed to me and everyone on the road I no longer feel it is safe to drive it.Read more

In November 2025, with approximately 6,900 miles on the vehicle, I reported brake issues to the dealership. After inspection, the dealership resurfaced (turned) the brake rotors at no charge. At that time, I was led to believe that the issue had been corrected. However, in June 2026, with only 14,300 miles on the vehicle, I returned to the dealership because the same symptoms had reappeared. I was
... informed that the rotors again require resurfacing and that the brake pads now need replacement as well. why has the same issue returned so quickly? Is there a defect in the rotors, brake pads, calipers, brake system design, or another component that has not been identified and corrected? I do not believe that my driving habits are the cause of this problem. Prior to purchasing this Sportage, I owned two vehicles from other manufacturers. Neither vehicle required brake rotor resurfacing until well over 50,000 miles, despite being driven under similar conditions. The fact that this vehicle has required rotor service twice before reaching 15,000 miles strongly suggests that an underlying issue exists. I believe that that premature rotor wear puts me and others at risk of collision. The Sportage is available for inspection.Read more

On May 11th I was driving my 2019 Mazda CX-5 with approximately 35,000 miles when my cabin filled with the smell of gasoline. Upon inspection, the technicians discovered there was a "crack at rear cylin

I was advised that this must be a manufacturer defect as you should not see this issue at such low mileage. The vehicle was then towed to the nearest Mazda dealership/service center, where a cracked rear cylinder head was diagnosed and confirmed. I was informed that continued operation of the vehicle could have resulted in catastrophic engine failure, engine seizure, fire risk from leaking oil contacting hot engine components, or sudden loss of power while driving, creating an extremely dangerous driving situation.
