I am reporting a serious safety concern involving a 2018 Volvo XC90 T8 PHEV. On [XXX], at approximately [XXX] CT, the vehicle experienced a sudden multi-system safety and actual brake failure while in a parking-lot/low-speed setting near [XXX} in Austin, Texas. The vehicle displayed multiple urgent warning messages and indicators, including power steering failure / stop safely, brake/ABS warning,
... SRS airbag service urgent warning, stability/traction warning, battery/electrical warning, check-engine warning, and tire-pressure warning. The vehicle ended in an irregular/distressed parking position after trying to locate a concrete block to force stop the car due to ABSOLUTE BRAKE FAILURE, E BRAKE SEEMED TO NOT WORK EITHER. The concern is not simply that a warning light appeared. The event involved steering, braking, airbag/SRS, stability, and electrical warnings at the same time, making the vehicle unsafe to operate AND SOMEHOW THE BRAKES WEREN’T OPERATING DUE TO THE LACK OF FUNCITONING. Photos were taken of the warning messages and vehicle position. Prior related history:
This vehicle has prior safety-system history involving SRS Airbag Service Urgent warnings and braking/vehicle malfunction and movement concerns. Earlier review and discussion involved Volvo engineering, VDDM replacement, CAN/network concerns, calibration requirements, voltage/12V battery considerations, software/module updates, and post-repair validation. The VDDM is understood to participate in vehicle dynamics, stability control, brake coordination, and PHEV regenerative/hydraulic brake interaction. Prior documentation also raised concern that intermittent brake, SRS, VDDM, CAN, and calibration issues may not reproduce during a short dealer test drive. Safety risk:
The safety risk is that the vehicle may experience sudden or intermittent loss, degradation, or warning of systems involved in steering, braking, stability control, and airbag readiness. A simultaneous power steeri INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIARead more

Traveling on highway with no other cars anywhere around. Going 80 mph on an 80 degree day when sun roof on 2025 Toyota 4 runner exploded. Sun roof was closed at the time, but could have had devastating consequences if it had been open. Toyota rep did not take a look at the broken glass to check for imperfections or manufacturing issues, just told us we needed to make an insurance claim.

Vehicle: 2023 Jeep Wagoneer Mileage at time of incident: 43,400 miles Component: Engine Cooling System / Thermostat Incident Description: This vehicle has experienced two separate thermostat failures since purchase. The first failure occurred the day after I purchased the vehicle, in April 2024, at only 104 miles. This time the vehicle overheated /stall pattern even after confirming coolant was at
... a sufficient level. The second failure occurred on Monday, June 22, 2026, at 43,400 miles. While driving with my wife, son, and son's friend as passengers, the temperature warning indicator activated, signaling an engine overheating condition. Shortly after, the engine lost power and stalled completely while the vehicle was in the middle of a busy intersection. With the engine off, I had no power assist for steering or braking, which made it extremely difficult to maneuver the vehicle out of the path of oncoming traffic. My family and I were nearly struck by other vehicles while attempting to get the disabled Wagoneer clear of the intersection. The recurrence of this failure at relatively low mileage (43,400 miles on a 2023 model) indicates a potential design or manufacturing defect in the cooling system, not an isolated incident. The second failure resulted in a complete loss of engine power in active traffic, creating an immediate safety hazard to my passengers and to other drivers on the road. Repair history: Failure #1: April 2024, 104 miles Failure #2: June 22, 2026, 43,400 miles Cost of most recent repair: Approximately $2,000Read more

Vehicle shudders when driving between 18mph and 40mph. Brought into dealership, was told that the Torque Converter is coming apart, and that it would require an entire Transmission replacement. This vehicle has been immaculately maintained, and only has 34745 miles on it. There is no reason that a main component of a powertrain should fail at such a low milage. I was told that the Warranty onl

Hello, I recently brought my 2014 Ford F-150 (VIN: [XXX]) to the dealership regarding Recall 24S37. The dealership i

However, the vehicle continues to exhibit transmission-related symptoms, and I am concerned that the underlying condition may be related to or aggravated by the conditions addressed in Recall 24S37. I am requesting that Ford review this case and determine whether additional diagnostic evaluation or repairs should be considered. However, they refuse to accept that the codes have anything to do with the recall, even though it is the same part and the affected sensor that is triggering the error codes on the scanner. They are asking me to pay for the transmission repair because the codes are not the ones approved for the recall, but based on the signs and symptoms the truck is showing, it is a chain reaction that triggered the previously mentioned failures. The failure had occurred on several occasions. On June 18, 2024, the vehicle failed to accelerate or shift properly. On January 15, 2025, the vehicle shuddered violently and nearly stalled. On April 14, 2026, the veh

The dealer replaced the spark plugs and performed an induction cleaning and transmission valve service. The dealer was unable to duplicate the failure after a 36-mile test drive, after the service. From May 27 to May 29, 2026, the vehicle failed to start and was towed to the dealer again, where a new short-block engine assembly was installed. On June 2, 2026, a technician discovered that there was no oil in the engine, resulting in a critical crankshaft failure code. Despite the short block replacement and oil correction, the powertrain failures persisted, and the vehicle failed to exceed 50 MPH. Subject: Complaint Regarding Improper Handling of Safety Recall Repair Hello, I would like to file an official complaint regarding a dealership that failed to properly follow required procedures for a federally mandated safety recall repair on my vehicle. Despite the recall requiring specific corrective actions to ensure the vehicle is safe to operate, the dealer either: • failed to perform the full recall procedure, • refused or delayed the repair without a valid reason, or • returned the vehicle without completing required steps to ensure safety (such as proper inspection, adjustment, or alignment). This violates my rights as a vehicle owner, as federal law requires manufacturers and authorized dealers to correct safety-related defects at no cost and in full compliance with recall instructions. I am requesting NHTSA’s assistance in ensuring that the recall is properly completed and that the dealership’s non-compliance is investigated. After coming to a stop and while making a routine right turn at under 10 mph, the vehicle unexpectedly accelerated on its own, causing an immediate loss of control. The entire incident unfolded in seconds, leaving no opportunity to react or correct the vehicle’s behavior. The incident happened at the intersection of [XXX]. The "Autopilot" feature in Tesla was on at the time of the incident. The impact was severe. The airbags deployed, the vehicle was declared a total loss by insurance, and first responders—including the Frisco Police Department—were required to clear the wreckage. My wife sustained injuries in the crash and continues to recover. Witnesses at the scene confirmed that the acceleration was abrupt and not driver‑initiated. The emotional and physical toll has been significant. More importantly, the nature of this incident raises serious concerns about a potential safety issue that could affect other Tesla owners. We feel compelled to ensure this incident is thoroughly examined. If there is a systemic issue—software‑related or otherwise—it is critical that it be identified and addressed to protect other drivers and uphold the integrity of the Tesla name. What component or system failed or malfunctioned, and is it available for inspection upon request?: The right-side tailgate hinge assembly on my 2019 Ram 2500 failed and became increasingly difficult to operate. The tailgate now requires excessive force to open and close throughout its entire range of motion. An independent body shop inspected the vehicle and determined that the hinge assembly req
... uires replacement. The vehicle and failed component are available for inspection upon request. How was your safety or the safety of others put at risk? The failure significantly affects the normal operation of the tailgate and requires excessive force to open and close. This creates a risk of injury while operating the tailgate and raises concerns regarding the continued safe operation of the tailgate assembly. The failure of a primary tailgate support component could also affect the tailgate's ability to function as intended. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The problem was inspected and confirmed by an independent body shop. The shop determined that the tailgate hinge assembly has failed and requires replacement. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle has been inspected by an independent body shop. At this time, the vehicle has not been inspected by the manufacturer, police, or insurance representatives. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? No warning lamps, messages, or diagnostic indicators were present. The only symptom was a progressive increase in the force required to open and close the tailgate. I started to notice the issue about a year ago. My car experienced a sudden internal engine failure due to a known manufacturer design defect causing coolant intrusion into the engine cylinders. A Ford dealer service center diagnosed the issue with Diagnostic Trouble Code (DTC) P0303, confirming a Cylinder 3 misfire caused by coolant leaking into the block. The vehicle requires a total engine replacement at just ~66000 miles at a cost of over $
... 9000. This internal coolant leak poses a severe safety hazard, including sudden engine stalling, loss of motive power while driving, and potential thermal runaway/engine fire risks. The vehicle is currently rendered unsafe to drive.Read more

The vehicle experiences a constant cooling system failure that appears to be related to the thermostat. Beginning at approximately 40,000 miles, the engine cooling fan started running continuously at high speed whenever the vehicle is operating. At the same time, the air conditioning stopped functioning properly and the vehicle began displaying a warning message indicating that steering assist is
... unavailable or reduced. The vehicle is available for inspection upon request. This condition creates a safety concern because the steering assist warning indicates a potential reduction or loss of power steering assistance, which could increase steering effort and make the vehicle more difficult to control, particularly during low-speed maneuvers or emergency situations. The warning appears during normal vehicle operation and can distract the driver. The ongoing failure of multiple vehicle systems raises concerns about the safe and reliable operation of the vehicle. The problem has been present continuously since first appearing at approximately 40,000 miles and is fully reproducible. The condition occurs every time the vehicle is driven and has not corrected itself. Symptoms include the engine cooling fan running continuously, loss of air conditioning, and a steering assist warning message. Based on reports from other Chevrolet Blazer owners experiencing similar thermostat-related failures, this may represent a common defect that warrants investigation and consideration for a safety recall.Read more

Component/System Malfunction & Availability: The vehicle's Air Conditioning (A/C) system failed completely. The primary component that failed is the REAR A/C compressor/evaporator core. The vehicle and its NEW and REPLACED components are available for inspection upon request. The original damaged components are with the dealership. Safety Risk: The sudden and total loss of climate control occ
... urred during extreme weather conditions. This caused an immediate and rapid rise in cabin temperature to unsafe levels, posing a severe risk of heat exhaustion, dehydration, and heatstroke, particularly for vulnerable passengers. Furthermore, the loss of the A/C system compromised the vehicle's ability to clear window fogging and condensation, rapidly reducing visibility and creating an immediate crash hazard. Diagnosis & Inspection Status: The issue has been reproduced and officially confirmed by an authorized dealer/independent service center. However, the vehicle and the failed components have not yet been inspected by the manufacturer, police, or insurance representatives. Prior Symptoms & Warning Signs: There were no warning lamps, error messages, or diagnostic trouble codes displayed on the dashboard prior to the failure. UNKNOWN exactly when the internal component degradation began, but the total failure occurred suddenly without prior symptoms.Read more

Engine knocking vechile had loss of power. Had towed to dealer. Throwing all kinds of codes 6.2 L v8. Truck passed GM crap test in September and here we are at 59,000 with a blown motor. Lucky it did not go out under power and kill me or my small kids and family that was going to vacation with this truck the next day

Driving in the rain on the interstate and during acceleration, engine just lost all power. Recovered within 2-4 minutes. After 1 hour of driving, it repeated again. Hard rain conditions. Engine lost all power and wanted to die. I had my family with me and the first incident was merging onto [XXX] with a semi truck quickly closing onto me. Fortunately, he saw I was having issues and slowed do
... wn and gave me room. Then when it happened again, pouring rain, night time conditions and the dash lights were flickering and engine was just bogging down during any acceleration. Nursed it to a well lit gas station. Turned off and when re-started- no further issues but drove with the CEL on for well over an hour. Took to Jefferies Honda in Roseville, Michigan where they charged me $200 just to diagnose the issue, even though the vehicle is still under warranty. Provided a printout of the "results" from the computer but it only is of the "Live Data"- not the recorded data. Not only that but I suspect they never really checked as the mileage listed on that page does not match the mileage on their report. I suspect my vehicle is about to come out of warrenty so easy to state "no faults found" even though we almost caused multiple accidents every time the engine was failing on the interstate. Check-engine lamp flashed and went off and then eventually came on solid for well over an hour so I know data exists but I suspect Jeffries deleted it when they stated "no faults found"- impossible. My wife and I were in the car and know what we experienced. HUGE safety concern and now I don't trust Honda. They did not diagnose nor fix anything. Only charged me and sent me on my way. Explained when I complained that their service people may have made a mistake "with the mileage" section. I suspect they took my car out for a spin. I have the print out- why have a "Mileage In/Out" section if they won't use? Doesn't add up. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The CV axel is bad and needs replaced. I am only at 72,000 miles. If I had not gotten an oil change and the technician saw it, it could have broken while driving this coming weekend for 14 hours. Independent service center is replacing. No warning. This could have caused a serious accident.
