On June 23, 2026, while operating my 2021 Genesis GV80, the vehicle's entire infotainment and system software rebooted without warning as I was leaving a parking lot. As a direct result of this reboot, the driver seat automatically moved out of my saved position. I was no longer able to reach the pedals and could not properly control the vehicle. This incident occurred while the vehicle was in mot
... ion. It took me several minutes to safely bring the vehicle to a stop, at which point I turned the car off and restarted it. Upon restart, all of my saved settings had been wiped. This is a safety defect, not a minor software issue. A seat repositioning without driver input while a vehicle is in motion constitutes a loss of vehicle control. Had this occurred at highway speed the consequences could have been fatal. I am aware that Genesis issued Safety Recall 031G for mid-drive infotainment reboots on other GV80 model years. I am requesting NHTSA investigate whether this defect extends to 2021 model year vehicles and whether a recall expansion is warranted.Read more

The factory-installed tow mirrors on this vehicle present a severe safety hazard due to a major engineering and ergonomic design flaw. The physical size and placement of the side mirrors are excessively large, creating massive blind spots at the front pillars of the windshield. This completely obstructs the driver's forward-diagonal visibility, creating a "blind wall" effect that fully hides oncom
... ing mid-size sedans, motorcycles, and pedestrians when approaching urban intersections, roundabouts, and crosswalks. This layout is entirely unsuitable for normal daily use and causes extreme driver distress due to near-miss situations. Additionally, the advanced safety electronics housed within these mirrors malfunction constantly. The Blind Spot Monitoring (BSM) warning lights freeze or lock up on the glass, or the dashboard displays a permanent "Blind Spot System Unavailable" error message shortly after starting the vehicle or deploying the power-folding mirrors. The 360-degree perimeter camera feed also experiences intermittent signal loss. The hardware components are available for inspection upon request. The electrical faults have been confirmed by an authorized dealer but a permanent design fix for the forward visibility hazard is required from the manufacturer.Read more

We bought a 2022 Polaris Slingshot R with Roll Cage and Canvas Top from Private Club Motors. The rims were a little beat up, but as far as we knew, that was just cosmetic. It was closing time and pouring down rain when we left the dealership. I was driving behind the Slingshot and could tell there was a definite problem with the wheels, as they looked like they were wobbling. When we stopped to ge
... t gas, the driver said it was vibrating like crazy. I texted the sales guy immediately to let him know there was a problem with the tires. He said to bring it back in and they would take care of it. So, we came back on Thursday, 2 days later, because of rain and the service guy said Thursday would be better for them. They looked at it and then asked us what we hit. We didn't hit anything, but they refused to replace the rims and tires. I called another dealership that we bought our previous Slingshot from and they told me to bring it to them and they would take care of us. Turns out, 2 of the 3 tires' rims were bent beyond repair, there were gouges in 2 of the tires, plus, the drive belt had a gouge in it. Used Bikes Direct Houston took tires and rims off of a Showroom Floor unit that they had and put them on our unit plus ordered a belt and installed it. They did all this at cost and no labor. So even with the lowest cost possible, which was $1,132.48, Private Club Motors refused to reimburse us. The Slingshot never should have left their dealership in the condition it was in. It wasn't road safe. I have reached out to them numerous times and they have me blocked on their cell phones and don't take my calls at the service and financial departments. I even went there in person yesterday to give them one final chance before I start reporting them. And, they still refused to take responsibility and reimburse me. And, just to show how much abuse that car had before we ever got behind the wheel, it has a hole in the oil pan that was repaired using a bunch of JB Weld.Read more

My check engine light came on, and my car was also feeling "sluggish" and slow to accelerate. I scheduled a service appointment at my local dealer 4 days later. At the appointment, the technician stated everything checked out okay and the check engine light was likely an "anomaly" or "outlier" but otherwise nothing to be concerned about since everything checked out "okay" and no problems were iden
... tified during their test drive. However, that very afternoon, I was driving down the interstate, where the posted speed limit is 70mph, when my check engine light came on again and suddenly lost power, drastically reducing my speed and could not accelerate faster than 39mph. Luckily, the interstate was not busy at the time (about 10:30-11:00 a.m. Saturday morning) and there were only less than 4 cars behind me that were able to easily switch lanes and pass me. I moved over to the shoulder to a complete stop and restarted my car. My check engine light remained illuminated and was still slow to accelerate and "sluggish" but was able to (slowly) reach interstate speed (60-70mph) until I reached an acceptable exit and safely made it to my destination. I have since used roadside assistance to tow the vehicle from my home to the the same dealership for inspection and service (again).Read more

I am reporting a severe safety hazard regarding an immediate, drastic increase in fuel consumption and erratic fuel range calculations following a manufacturer recall repair. My 2025 Ford Explorer ST-Line was operating perfectly until a Ford Mobile Service technician performed safety recall 26S21 (IPMA Reset). According to official Ford manufacturer specifications, this SUV has an expected drivi
... ng range of up to 501 miles on a full 17.9-gallon tank of fuel. However, immediately following this software update, the vehicle's total driving range drastically dropped. On a 100% full tank of gasoline, the dashboard's "Distance to Empty" (DTE) display now only reads 316 miles left to empty—nearly 200 miles below the manufacturer's expected capability. Furthermore, during a subsequent continuous, short 1.8-mile drive near my residence with no stop-and-go traffic, the DTE display rapidly plummeted from 15 miles down to 5 miles remaining, dropping visibly every two seconds. The vehicle is physically burning through gasoline at an accelerated, unsafe rate. This creates an immediate safety risk. If the vehicle is suffering from an uncalibrated software glitch in the powertrain/fuel modules due to the recall update, or if the procedure triggered a mechanical fault, the vehicle is at high risk of a sudden highway stall or leaving the driver stranded in live traffic due to inaccurate fuel signaling. I request that NHTSA look into whether the software installation process for recall 26S21 can corrupt engine/fuel management maps or trigger fuel delivery faults on the 2.3L EcoBoost platform.Read more

Van has repeatedly stalled during the idle stop restart process when stopped at a stoplight. The van will stall and the dashboard lights up with many symbols. Description of the Safety Defect & Failure Mechanism: The rear (bank 1) variable va
... lve timing spool valve assembly is mounted directly above the vehicle's alternator. Due to thermal cycling and material degradation, the molded rubber gasket on the upper portion of this spool valve assembly experiences premature compression set and fails, resulting in a pressurized engine oil leak. Because of the physical packaging and architecture of the J35 engine, the oil dripping from this failed gasket falls directly into the open housing of the alternator located immediately below it. The engine oil contaminates the alternator's internal brushes, slip rings, and diodes, leading to a catastrophic electrical short circuit and total failure of the charging system. Description of the Safety Risk: When the alternator fails due to oil contamination, the vehicle experiences an immediate drop in system voltage, resulting in a sudden and un-warned loss of electrical power while the vehicle is in motion. This triggers a cascade of safety hazards: Loss of Power Steering Assist: The sudden loss of electrical generation drops the vehicle into battery-reserve power, frequently causing the Electronic Power Steering (EPS) to shut down immediately, drastically increasing steering effort and the risk of a crash. Engine Stalling: Total depletion of the battery occurs rapidly without alternator output, leading to sudden engine stalling in active traffic without the ability to restart. Loss of Safety Systems: Reduced system voltage disables critical safety components, including Advanced Driver Assistance Systems (ADAS), supplemental restraint systems (airbags).Read more

The engine stalled when I was exiting the freeway due to the charging system failure. This is the second time the charging system failed. The first failure has been documented on NHTSA complaint number 11746092. I paid the Harley Davidson SLC dealership in April 2024 to have the charging system repaired and replaced. Harley Davidson had released a service bulletin acknowledging charging system def
... iciencies on 2021-2023 Pan America models and subsequently redesigned the entire charging system for the2024 model year. This is a documented pattern and not just an isolated failure. Regarding the repair in April 2024, the dealership replaced the failed stator with the same poorly designed part number instead of upgrading to the redesigned charging system. Now only two years and 9,000 miles later I'm experiencing the same problems as before.Read more

Rear Door / lift gate will not go all the way up or stay up without manual assistance, causing 2 head injuries and 2 arm injuries on recent occasions. We have confirmed with the dealer & one Nissan Certified Mechanic that this is a common problem. The dealer & 2 mechanics have said they do not know how to fix this without replacing the entire lifting assembly. We did not ask for estimates. We shou
... ld not have to, but we can live with the problem now that we are aware of it. Transmission was replaced with new Nisson equipment by the previous owner, costing $5,000+. The problems we are having are the same they were having. The vehicle runs rough, hesitates, vibrates & shakes a lot. It is not very safe in traffic. There is a place for a dipstick to check transmission fluid. We have confirmed with 3 parts stores, 2 mechanics & finally with Nissan, they did not make a dipstick for this car or several other models. One way to check the transmission fluid is to jack up the car, remove a port on the transmission & fill it until if overflows (Cost is approximately $1,100.00, vs a dipstick costing less than $30.00 retail for comparable cars) Another way to check is to jack up the car, drain the transmission fluid, change the filter, replace the seal & cover, then pour in the exact correct amount of transmission fluid through the dipstick hole. However, the only way to know it is actually full is to leave the above-mentioned port out until the fluid overflows. (Our 3 cost estimates range from $1,400.00 to $1,850.00) Had we known of the transmission problems occurring with many if not most Nissan cars, we would not have bought the vehicle. Our research shows these are very common problems with this car & Nissan. We don't know if this applies to what you do, but this car also burns through spark plugs & ignition coils very quickly. We have had to replace all spark plugs 2 times plus 8 ignition coils in the 2-1/2 years we have owned it & driven it only 28,000 miles.Read more

the vehicle was driven for approximately 30 minutes, operating with zero dashboard warnings or performance indicators. The vehicle was then parked on my property and turned off completely. Approximately two hours and thirty minutes later, the vehicle spontaneously ignited while stationary and unoccupied. The local fire department responded and successfully extinguished the blaze. Both physical obs
... ervations and my home security camera footage confirm that the point of origin was the front right passenger side of the engine compartment. This exact location houses the factory electric water pump and its electrical plug connector, directly matching the failure mechanism detailed in NHTSA Campaign 24V608000, wherein fluid intrusion causes a severe, persistent electrical short circuit capable of igniting a fire hours after the vehicle is parked. The car is completely totaled because of this.Read more

In January I experienced a “backlite leak” that allowed water to enter the cab of my vehicle. As a direct result of this water intrusion, my RFH (Radio Frequency Hub) module failed due to electrical shorting caused by moisture exposure. This failure was sudden and completely outside of my control and was not the result of misuse, neglect, or normal wear and tear. Not to mention the risk it put
... my family in as I was driving with my wife and daughter when the module failed and caused total power failure. I was driving down the road with my family in the vehicle when without any warning the truck completely shut down and didn’t have any power. I brought it to the dealership I bought it from and they stated they fixed the leak and replaced the RFH. Fast forward to this past week, there was another leak in the rear window. The dealership then stated it was a faulty back window unit that needs to be completely replaced. Stellantis (formerly FCA) previously acknowledged this defect through extended warranty coverage. However, my vehicle’s model year was later excluded despite being affected by the same known issue. This exclusion is particularly concerning given that the defect ultimately caused collateral electrical damage beyond the window itself. Given the documented nature of this issue, its prior recognition under extended warranty, and the risk of safety it put my family in I wanted to document this issueRead more

I recently had my front subframe replaced as part of your recall, however, the rear subframe and all components attached are suffering from the exact same rust and potential component failure as the front. I was replacing my brakes today and noticed that the entire structure is on the verge of complete failure. The right rear suspension already makes unusual noises when going over even small bumps
... and I imagine if I were to hit a large pothole that would probably be the end of this car. I imagine all similar Mazda 6s in the Midwest suffer from the exact same degradation and it is a major safety concern. There are places where there's chunks of subframe missing because it has rusted completely through and just disintegrated.Read more

We have a video of the incident from our ring camera: [XXX] My daughter pulled our 2022 Bronco Sport out of our driveway onto [XXX] and then made a right turn onto [XXX] After completing the turn, my daughter said the car pulled to the right like the lane assist kicked in and locked and would not allow her to turn the wheel. My daughter does not remember if any warning lights or messages lit up
... on her dashboard because the whole thing happened within a few seconds. She crashed into our next door neighbors concrete and cinderblock mailbox at low speed and then the Bronco immediately flipped over. The vehicle is at a local tow yard and available for inspection. I am reporting a serious safety concern involving a 2018 Volvo XC90 T8 PHEV. On [XXX], at approximately [XXX] CT, the vehicle experienced a sudden multi-system safety and actual brake failure while in a parking-lot/low-speed setting near [XXX} in Austin, Texas. The vehicle displayed multiple urgent warning messages and indicators, including power steering failure / stop safely, brake/ABS warning,
... SRS airbag service urgent warning, stability/traction warning, battery/electrical warning, check-engine warning, and tire-pressure warning. The vehicle ended in an irregular/distressed parking position after trying to locate a concrete block to force stop the car due to ABSOLUTE BRAKE FAILURE, E BRAKE SEEMED TO NOT WORK EITHER. The concern is not simply that a warning light appeared. The event involved steering, braking, airbag/SRS, stability, and electrical warnings at the same time, making the vehicle unsafe to operate AND SOMEHOW THE BRAKES WEREN’T OPERATING DUE TO THE LACK OF FUNCITONING. Photos were taken of the warning messages and vehicle position. Prior related history:
This vehicle has prior safety-system history involving SRS Airbag Service Urgent warnings and braking/vehicle malfunction and movement concerns. Earlier review and discussion involved Volvo engineering, VDDM replacement, CAN/network concerns, calibration requirements, voltage/12V battery considerations, software/module updates, and post-repair validation. The VDDM is understood to participate in vehicle dynamics, stability control, brake coordination, and PHEV regenerative/hydraulic brake interaction. Prior documentation also raised concern that intermittent brake, SRS, VDDM, CAN, and calibration issues may not reproduce during a short dealer test drive. Safety risk:
The safety risk is that the vehicle may experience sudden or intermittent loss, degradation, or warning of systems involved in steering, braking, stability control, and airbag readiness. A simultaneous power steeri INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIARead more
