MOST CONCERNING: 6/25/26 I was pulling out of a parking lot, with no other cars or obstacles in my path, going slowly, and the automatic braking system ground the car to a stop. There was no warning on the dash. It took a few seconds before the car would allow me to accelerate. This is the very problem the software update was supposed to fix. 7/2/26 I was using GoogleMaps navigation at night and as I was driving, the infotainment system went blank and I had no navigation. After a minute or so the map came back on, but it was just spinning in circles and couldn’t right itself. Additional issues: the infotainment system randomly keeps popping up the question, asking if I want to open Google Maps. Sometimes when using the parking camera, an informational window keeps popping up stating “Press the button to display the camera view, or press and hold to activate the parking assist system”. I use this function every day, but for some reason, at times it keeps trying to teach me how to use it. (These types of glitches have happened before after software updates.) 6/26/26 I opened a case with Hyundai. Case number 42790134. I have yet to hear from them. The posted speed limit was approximately 70 mph; however, FSD limited the vehicle to approximately 65 mph and displayed warnings indicating that the higher speed was unavailable. While FSD remained engaged, the system initiated a lane change from the far-right lane into the center lane. After entering the center lane, the vehicle began displaying repeated warning messages and FSD disengaged multiple times. I attempted to locate settings related to the warnings on the touchscreen but could not identify an available adjustment. During the third disengagement, I perceived that the vehicle continued braking without releasing while steering resistance increased significantly. The steering wheel then rotated abruptly, and I was unable to regain normal directional control before the vehicle departed its intended travel path and struck a concrete median. The collision caused personal injuries and substantial vehicle damage. I have not been able to reproduce the condition. The incident was investigated by the Kentucky State Police. Based on the sequence of repeated FSD warnings, disengagements, continued braking, and abrupt steering behavior, I believe this event warrants investigation. I request that NHTSA review the vehicle’s Full Self-Driving logs, telemetry, Event Data Recorder (EDR) data, steering and braking commands, diagnostic records, and software history to determine whether a vehicle or software malfunction contributed to the collision. Only six days later, on July 6, 2026, the check-engine light returned and we brought the vehicle back to the dealership. We were initially told the issue was a sensor; the following day, we were informed that the replacement engine — which the dealership had disclosed to us at the time was a refurbished unit — was itself defective and would need to be replaced a second time. As of this writing, that repair remains open. In other words, the remedy provided for this vehicle's most serious failure did not resolve the problem, and we are once again without our vehicle. This pattern of failures has caused real, ongoing hardship for our family. We specifically purchased a full-size, top-trim Suburban for its seating and cargo capacity: we have two teenagers at home who depend on us for transportation to school and activities, we regularly care for grandchildren, and our [XXX] daughter has Complex Regional Pain Syndrome (CRPS), a debilitating condition that often requires her to use a wheelchair. They advised it could stop at any time or not stop. I also experienced pulling and unprompted braking while driving. On XXX, I called Island Mazda service multiple times (calls went to a call center). No callbacks. I went in person. General Manager [XXX] was unavailable; Sales Manager [XXX] advised towing to [XXX]. The car was towed XXX at 1:29 PM. No confirmation call. I called again at ~2 PM. Technician [XXX] called first. Service Manager [XXX] later called and was rude/dismissive. He yelled when I mentioned lemon law saying he can no longer speak to me when i mention that, downplayed the issue, refused to discuss documentation, and suggested I test drive the unsafe vehicle with them to figure out whats wrong. He spoke over me and said it would take a long time to fix because they are busy. He said he would forward my case to someone else and i agreed he is not capable of handling this appropriately. This is a brand new vehicle under warranty. I have a [XXX] son with congenital adrenal hyperplasia (rare condition requiring stress-dosing for injury). I do not feel safe driving it since the alert never even appeared in my actual car just the mazda app on my phone i do not trust the dealership after this treatment to fix it. Two week Monday July 6, around 3:15pm while driving down on highway passing lane suddenly the vehicle start loosing speed and after few seconds I noticed that the engine stalled. No warning light or anything, the steering wheel turns to very heavy and the break pedal is really hard as well. Fortunately near by there was a U-turn lane for authorized vehicles only and I was able to merge to it and

Behind me there were at least 10 other cars trying to pass a line of semi trucks and I was lucky to nobody crash on my rear. I took the vehicle to the dealer, they run tests and tried to reproduce but no success. Besides the dealer no one checked anything due to there was no substantial damage or anything.
