Since purchasing my 2022 Honda HR-V in 2024, I have experienced ongoing unintended or “phantom” braking while driving at highway speeds with adaptive cruise control, lane keeping assist, and the Collision Mitigation Braking System activated. During each incident, I disengage adaptive cruise control using the steering wheel controls to regain manual control. Although no accidents or injuries have occurred, these sudden braking events have resulted in multiple near-miss situations and present a significant safety concern.Read more

My 2019 Honda Civic has experienced repeated air conditioning system failures over multiple years. The failures include confirmed refrigerant leaks, condenser replacement, and evaporator replacement. The system has again failed due to a refrigerant leak traced to the evaporator. This vehicle has a documented history of recurring A/C system issues, including abnormal pressure conditions and repeate
... d refrigerant loss. Honda has acknowledged defects within this system by extending warranty coverage for certain components, including the compressor. Despite this, the system continues to fail, requiring multiple major component replacements. Loss of air conditioning in high-temperature conditions presents a safety concern, particularly during extreme heat, as it can affect driver alertness, passenger safety, and overall vehicle usability. This appears to be a systemic defect involving the A/C system rather than isolated component failures.Read more

On April 13, 2026, I brought my 2019 Honda Odyssey to a Honda dealership in Chicago, IL for a dealer-initiated recall service. The dealer contacted me and asked me to come in. Before the recall service, the vehicle had zero warning lights, zero error codes, and was operating completely normally. Immediately after the recall service was completed, the following warning lights appeared on the dashbo
... ard: Brake System warning, Hill Start Assist Problem, VSA (Vehicle Stability Assist) warning, and additional stability/steering system warnings. None of these warnings existed before the service. I returned to the dealership the same day. The service team cleared all error codes. However, within minutes of driving off the lot, all warning lights reappeared. This confirms the issue is persistent and not a one-time post-update self-check. I returned a second time. The service manager stated the warnings were not caused by the recall service and refused to investigate further. The dealer quoted $800 for diagnostic work. I also contacted American Honda by phone and was told they could not intervene. With the Brake System and VSA warnings active, the anti-lock braking system and vehicle stability control may not be functioning properly. This puts me and other drivers at risk, especially in emergency braking situations and on slippery roads. This pattern of multiple safety warnings appearing immediately after recall service on 2019 Honda Odysseys has been reported by other owners. The vehicle and its warning lights are available for inspection upon request.Read more

Subject: Safety Complaint – 2024 Honda Prologue – Clicking Noise When Turning (Possible CV Axle Defect) I am submitting a safety complaint regarding my 2024 Honda Prologue Touring due to a persistent clicking noise when turning, which appears to be related to the front axle or CV joint. Vehicle Information: Year: 2024 Make: Honda Model: Prologue Touring Mileage:10300 VIN: [XXX] Description
... of Issue: The vehicle produces a noticeable clicking/popping noise when turning left or right, especially at low speeds such as in parking lots or during tight turns. The noise is consistent and reproducible. The issue appears to originate from the front axle/drivetrain area and raises concern about potential failure of a critical drivetrain component. Safety Concern: This defect may indicate premature wear or failure of the CV joint or axle assembly. A failure of this component while driving could result in loss of power to the wheels and create a dangerous driving condition, increasing the risk of an accident. Dealer Response: I brought the vehicle to an authorized Honda dealership. The dealer acknowledged the issue but stated that no repair or fix is currently available and advised me to contact Honda directly. The issue remains unresolved. Additional Notes: Based on my research, multiple other owners are experiencing similar issues with the same vehicle, suggesting this may be a widespread defect rather than an isolated incident. I am requesting that NHTSA investigate this issue as a potential safety defect affecting the 2024 Honda Prologue. Thank you for your attention to this matter. Tire Integrity Failure — The tire experienced a catastrophic sidewall blowout on a brand new vehicle. No road hazard, debris, or obstacle was struck prior to the failure. The structural integrity of the tire failed without cause. 3. Vehicle Safety System Communication Failure — The vehicle’s onboard diagnostics failed to detect and communicate a critical safety condition to me as the driver. On a 2026 model year vehicle, multiple redundant systems should have caught and communicated this danger, giving me the opportunity to respond and prevent the incident. These three failures created a dangerous chain of events that left me with no ability to take preventive action. The incident caused significant emotional distress. I have retained the damaged tire as physical evidence and have documented the incident with photographs. I am requesting a full investigation into whether this represents a pattern of TPMS and tire failures in the 2026 Honda Civic, and whether a safety recall is warranted.
