I am reporting a potential safety recall violation by Honda Mall of Georgia (3699 Buford Dr, Buford, GA 30519). My vehicle experienced engine noise, vibration, and warning indicators, which the dealership attributed to 'fuel contamination' to deny warranty coverage (attached), and charged approximately $3,392.99 for repairs. However, I have a certified laboratory analysis (attached) from the Georg
... ia Department of Agriculture confirming the fuel met all state quality standards and had no contamination at Costco Gas. Costco also provided supporting document (attached) that there was no contamination and there were no other customers reporting such issue. The vehicle required a second repair (attached) for the engine issue immediately after the first, further proving the failure was not 'bad fuel' related. Despite my state-certified proof, the dealer refuses to provide any documentation for their claim of 'bad fuel.' I believe the dealership has intentionally misdiagnosed a known defect covered under Federal Safety Recall 24V-763 / QK8 (cracked high-pressure fuel pump) to avoid performing a free recall repair.Read more

I just didn't know it was something we could report. This h

My 2024 Ram 1500 TRX has recurring safety-related failures of the infotainment system affecting both Uconnect 5 (12” screen) and earlier Uconnect 4 units. These issues are widespread in 2021–2024 Ram TRX and Ram 1500 models and began or worsened after forced over-the-air updates. The primary defect is black-screen failures while driving. When the screen goes black or freezes, the truck loses
... all functions controlled through the display, including HVAC/defrost, backup camera, vehicle settings, seat controls, and performance gauges. This creates an immediate safety hazard: I cannot adjust climate/defrost, cannot see the required rear-view camera when shifting to reverse, and cannot access key vehicle information. Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. My vehicle experiences the same issues, including: Cameras not working, screen freezing or blacks out, navigation failing, carplay unstable won’t stay connected or randomly drops and more. Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 4 and Uconnect 5 systems. I request NHTSA open a defect investigation and require Ram to correct these failures.Read more

I am filing a safety complaint regarding my 2026 Honda CR-V. The front-mounted sensor used for advanced driver-assistance systems (including lane keeping assist and automatic emergency braking) is positioned in a way that makes it easily accessible and vulnerable to theft. My vehicle’s sensor was stolen, which immediately disabled critical safety features such as collision mitigation braking and
... lane assistance. While the vehicle may still be drivable, the loss of these systems significantly reduces safety and increases the risk of an accident. This does not appear to be an isolated incident. Many other Honda CR-V owners across the country are reporting similar thefts of the same sensor. The issue has become so common that third-party manufacturers are now producing protective covers to prevent theft, indicating a widespread and known vulnerability. As a result of this incident: •My vehicle has been inoperable or unsafe to drive with full safety functionality. •The car has been in a repair shop for over a month due to the sensor being on backorder. •I am continuing to make payments on a vehicle that I cannot safely use as intended. I believe this represents a design defect that creates an unreasonable safety risk. A critical safety component should not be so easily removed by theft, especially when its absence disables key driver-assistance systems. I respectfully request that NHTSA investigate this issue and determine whether a recall or mandated design change is necessary to protect consumers. Thank you for your attention to this matter.Read more

Infotainment Center: I am submitting this complaint regarding my 2021 Honda Accord Sport and the ongoing issues I’ve experienced with its infotainment system. This problem has been persistent, unpredictable, and increasingly concerning from both a usability and safety standpoint. Since purchasing the vehicle, the infotainment center has repeatedly malfunctioned without any warning. The screen
... will freeze, switch from setting to setting, or become unresponsive to touch input. At times, it will randomly reboot while I am driving. These issues occur regardless of whether I am using navigation, Bluetooth audio, or even when the system is idle. This also affects the ADAS, periodically, warnings will be displayed, and then not show up. What makes this especially troubling is how it affects my ability to safely operate the vehicle. For example, when the system freezes, I lose access to navigation directions mid-route, which forces me to either drive without guidance or attempt to troubleshoot the issue while on the road. Similarly, when using hands-free calling or music controls, the glitches can be distracting and require repeated attempts to regain control, taking my attention away from driving. I rely on my vehicle daily, and this ongoing issue has caused a great deal of frustration and concern. A modern vehicle’s infotainment system is not just a convenience—it is integrated into essential driving functions. When it fails, it directly impacts driver awareness and safety. I am requesting that this issue be investigated further, as it appears to be a systemic problem rather than an isolated defect. I hope that by reporting this, it may help lead to a resolution for myself and other drivers experiencing similar problems.Read more

I was driving down the highway, and the dash started to flash a message about the vehicle being unsafe to drive, stop driving immediately, call dealer. I was able to stop and then turn left across traffic with limited power; and continued to make my way back to work thinking I could probably get to my parking lot. The dash continued to display warning lights, starting with brake failure, then par
... king brake failure, and then the whole series of possible failures. I continued to drive about one mile. I shut the power off an powered up twice thinking that might reset the sensors; but they continued to fail. As I continued to drive there was limited power to the throttle, and I eventually coasted through a red light and into my work parking lot where the vehicle shut down entirely, and eventually fully locked up (couldn't start or event put in neutral to push). I had the vehicle towed to back to the dealer, and they diagnosed the problem as a failed transmission cooler. Of note, I had purchased the vehicle brand new less than 3 weeks before this incident and the vehicle had less than 600 total miles on it. While driving at approximately 45 mph, the steering system suddenly failed without any warning. The steering wheel became extremely stiff and nearly impossible to turn, resulting in a sudden loss of control of the vehicle. I was unable to safely steer, especially when attempting to turn right, which nearly caused a crash. There were no warning lights, alerts, or prior symptoms before this failure.
... The issue occurred abruptly and without any indication, creating a serious and unexpected safety hazard. Due to the severity of the problem, the vehicle had to be towed and is unsafe to drive. This appears to be a failure of the electronic power steering (EPS) system. A sudden loss of steering assist at driving speed presents a significant risk of injury or death to the driver, passengers, and others on the road. This is a critical safety issue that should be investigated immediately.Read more

All spark plugs fouled again; all spark plugs replaced. Valve cover gasket was leaking, so that was replaced as well. Yup, I paid for the repairs out-of-pocket. Episode Four: May 02, 2025. Vehicle goes into limp mode again, with CHECK ENGINE LIGHT turning on. Another spark plug fouled up, so it was replaced. Ignition coil replaced as well. Episode Five: June 19, 2025. Limp mode and Check Engine Lamp illuminates yet again, so Hyundai of North Charleston (SC) initiates an oil consumption test. With the recent completion of recalls 68&78c , and a new HV battery, APPEARS TO ME, thatJeep has changed the software for the throttle response when in electric mode. The result creates a safety hazard as you lose pedal response and power when the battery reaches below 35% or about 9 miles remaining. On the highway the jeep loses all power and will not accelerate over 62 mph. And the same around t
... own when you need that extra umph, the pedal becomes dead. More Scary when you’re on the highway!! The result? Since the electric power output is unavailable, you have to mash the pedal to the flow to kick it out of electric mode. But this is also unsafe as now you start the engine (often cold as I was running electric mode) and you smash the rpm’s over 6k to launch the gas engine (That can’t be good for the engine long term) . Before battery replacement and recall software updates, this process was very smooth and occurred with 5% or less battery life. This had occurred a few times and could probably reproduce it when this scenario is presentRead more

I am submitting a safety complaint regarding the driver monitoring/driver assistance system in the 2025 Kia Carnival. The system repeatedly issues “take a break” or inattentive driver warnings during normal, attentive driving conditions. These alerts are overly sensitive, frequent, and intrusive. Rather than improving safety, the warnings are highly distracting. The visual and audible alerts
... demand attention at critical moments, pulling focus away from the road. This creates a counterproductive and potentially hazardous situation, especially in traffic or complex driving environments. There appears to be no reasonable way to adjust the sensitivity or disable these alerts without impacting other safety features. A driver assistance system should support the driver—not introduce additional distraction or frustration. I request that this issue be investigated, including the calibration, trigger thresholds, and user control options for the system. A redesign or update may be necessary to ensure the feature enhances safety instead of undermining it.Read more
