My less than a year old Lexus GX550 was sitting in the driveway and the sunroof exploded. The car was not moving and it exploded outwards into a ton of pieces. The dealership claims that it is not a warrantable part. No foreign object hit the sunroof, even then it shouldn't shatter, only crack. It is my assumption that defective glass from the factory was installed into my vehicle and caused the

Incident date: May 12, 2026
Description of Defect: When the vehicle is shifted into reverse, the rearview camera display freezes for several seconds before going black. This malfunction creates a direct and serious safety hazard — during the period the display is frozen, the image shown does not reflect actual real-time conditions behind the vehicle. Any pedestrian, child, or object that moves into the path of the vehicle
... after the display freezes will not be visible to the driver. This significantly increases the risk of a backover collision. Dealership Inspection Outcome: I brought the vehicle to an authorized Ford dealership for diagnosis. Upon inspection, the technician was unable to detect the diagnostic trouble code associated with Recall 25S89, and as a result, the vehicle was deemed to have passed inspection. However, the malfunction continues to occur. The failure to trigger the recall code does not resolve the underlying issue — the camera display still freezes and blacks out during reverse operation, leaving the driver without a functioning rearview safety system. Safety Concern: A rearview camera is a federally mandated safety feature. A display that intermittently freezes and then goes dark is functionally equivalent to having no camera at all during those critical moments. This is not a minor inconvenience — it is a safety system failure that puts pedestrians and other individuals behind the vehicle at risk every time the vehicle is put in reverse. Requested Action: I am requesting that NHTSA investigate whether Recall 25S89 adequately captures all failure modes associated with this defect, and whether vehicles that present with this symptom but do not trigger the associated diagnostic code are being improperly cleared at dealerships. A remedy should be identified and applied to all affected vehicles regardless of whether the recall code is detected during inspection.Read more

This truck has a recall on it but by specific VIN numbers mine doesn’t fall into the VIN number but had the same catastrophic oil cooler line failure, which is in the recall which caused the engine tool expire with 40,000 miles on it. Truck is like brand new General Motors will not honor the warranty even though it’s the failure that’s in the warranty makes no sense.

The car has always undergone maintenance timely, and upon inspection by the dealership technician, the report state the vehicle was up to adequate level. There were no symptoms before the issue occurred, only the sudden catastrophic engine failure.

Steering sticks/locks while driving requires excessive input. Feels like the lane departure is controlling the steering and fighting you for control. Engine was replaced in 2023. In March 2026 there was a fuel line leak to include fuel misting near heat components. The vehicle was in a fender bender but Hyundai determined this did not cause the leak which eludes to this being a faulty installation of the engine replacement associated with the recall. I’ve reviewed my 2023 engine replacement invoice. I see the Service Kits were used to transf
... er parts from my old engine, but the High-Pressure Fuel Pipe (35340-2G710) is not listed. Per Hyundai's GDI service protocols and Recall 180, that pipe is a one-time-use safety component that should not be reused during an engine swap due to fire risk. Since it is leaking now, it’s clear the old pipe was reused and has failed. I'm asked for a goodwill replacement because this was a procedural error during the 2023 warranty work and was denied. This is a safety issue.Read more

Two different incidents involving the transmission. The first was going at a moderate speed, it’s a nine speed transmission. It shifted down into second gear, revving the engine horribly and throwing me forward somewhat. If this had happened at highway speeds it could have locked up the back wheels causing me to loose control of the vehicle. I have picture of the dash panel showing it shifted do

When I took it to the dealership they said since it wasn’t actively doing it they could not help me. It was still under warranty at this point. The second incident was when I was at a stop sign on an uphill grade. The service transmission light came on and the vehicle started to roll backwards very fast. I hit the brake but had nowhere to go but forward. I had to accelerate quite a bit just to get it to barely move forward and get out of traffic. Luckily I was near my house. I returned home and having some automotive knowledge I disconnected the battery and it reset the computer and transmission. Once again it drove normal. Had this happened in heavy traffic at high speeds I could have been stranded on a busy freeway. Once again the dealership said if it wasn’t actively doing the problem they couldn’t help me. I have since read their is a lawsuit pending against Nissan for these very issues, telling me I’m not the only one having these problems. The second incident occurred just a few weeks after the 5 years warranty was out. The vehicle still only has 45,000 miles on it. Both these defects could cause injury to myself or others.Read more

My 2025 Tesla Cybertruck AWD (VIN: [XXX] ) was returned to me after the first service visit (Invoice XXX Tesla Palo Alto, 8/22/2025) with the Pedestrian Warning System (PWS) speaker disconnected. The disconnection occurred during front-end service work at that visit, which included front fascia inspection and removal. The technician did not reconnect the PWS speaker before returning the vehi
... cle. The disconnection was not discovered until the second service visit (Invoice XXX, Tesla Palo Alto, 9/18/2025), at which point Tesla's own invoice documented: "The issue was caused by a disconnected front pedestrian warning speaker. Technician reconnected the front pedestrian warning speaker to resolve the issue." The [XXX] s therefore operated with a disconnected PWS for approximately 4 weeks — from August 22 to September 18, 2025 — without my knowledge. During this entire period, the vehicle was not emitting the required audible low-speed alert. The PWS emits an audible alert at low speeds to warn pedestrians, cyclists, and visually impaired individuals of the vehicle's approach. This system is required under FMVSS 141 (Minimum Sound Requirements for Hybrid and Electric Vehicles). This is a service-inflicted deactivation of a federally-required pedestrian safety system. The deactivation was caused by Tesla's Palo Alto service center (4180 El Camino Real, Palo Alto, CA 94306) during routine warranty repair work. The vehicle was returned to me without disclosure of the disconnection. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

My 2025 Tesla Cybertruck AWD (VIN: [XXX] ) has a recurring failure of the right rear seat occupant detection system, which governs airbag deployment and is certified by Tesla to meet FMVSS 208. ... 24, 2025. 2nd repair (Invoice XXX, 9/18/2025): harness assembly replaced. Tesla then performed a Visual Quality Check, recording in the invoice: "Technician performed a visual quality check to ensure its condition meets our standards." Vehicle returned as repaired. Relying on Tesla's written certification, I reinstalled my [XXX] son's car seat in that position. He rode there daily for approximately 7 months (September 2025 - April 2026). On April 21, 2026, I tested the seat and found the sensor still actively failing - unable to detect an adult occupant. My spouse witnessed this. I have 30-second video. Alert RCM2_a698 remains active in Tesla's systems today, confirmed by screenshot. This is my family's only vehicle. My wife currently uses the right rear seat. To Whom It May Concern, I am submitting a formal complaint regarding the handling of an unresolved safety recall involving my vehicle by Nissan, as well as the lack of communication, escalation support, and resolution provided by Nissan Customer Affairs. Despite multiple attempts to resolve an active safety recall, I have not received a follow-up call or any meaningful update regarding the statu
... s of the repair. The recall remains unresolved. I was previously informed that the issue may be related to the vehicle being “driven too much,” which I do not find to be an acceptable or relevant explanation in the context of a manufacturer-issued safety recall. Additionally, Nissan Customer Affairs has failed to provide adequate support. I requested escalation to a manager and/or a direct contact number to continue addressing the issue and was refused both. Calls are not returned, and there is no clear or functional escalation path provided. At present, Nissan is still in possession of my vehicle while I am in a loaner vehicle. However, I have not been given a clear timeline, status update, or resolution plan regarding the completion of the recall repair. This ongoing situation leaves me without transparency or assurance regarding the safety or status of my vehicle. Due to the unresolved recall, lack of communication, and inability to obtain proper escalation through internal channels, I am formally escalating this matter and submitting a complaint to the NHTSA for review. I respectfully request: Immediate clarification on the status of the recall repair A confirmed timeline for completion and return of my vehicle Proper escalation to a qualified manager or recall specialist Written documentation outlining next steps and resolution plan This matter involves a potential safety-related defect and requires prompt and appropriate attention.Read more

I am filing a safety complaint regarding Safety Recall 01C (NHTSA 25V-011) and the handling of my vehicle by both the dealership and Stellantis. ... failure status (“TestFailed: True”) Excessive clutch temperature readings exceeding 400°F These are consistent with PTU-related failure conditions the recall is intended to detect and address. Despite this, the dealership concluded that the PTU was not the cause and instead replaced the rear differential. I repeatedly requested written documentation explaining how the PTU was ruled out and how the rear differential was determined to be the root cause. This was not clearly provided. Additionally, the recall remained marked as “incomplete” for over a month while the vehicle was in the dealership’s possession. Immediately after I escalated the issue to corporate, the recall was suddenly marked “complete,” without clear explanation or supporting documentation. I was also not clearly informed that the Drivetrain Control Module (DTCM) had been replaced. I do not know: When the DTCM was replaced relative to testing Whether recall testing was performed with a faulty DTCM Whether the new DTCM was properly updated under Recall 01C Whether testing was repeated after replacement If recall validation was performed using a faulty DTCM, this raises concerns that the PTU testing procedure may have been invalid. The vehicle remained at the dealership for nearly two months during this process. Throughout this time, I experienced significant difficulty obtaining clear answers, and my concerns were not fully addressed before the recall was marked complete.
