Over the past nine months, the vehicle has again developed severe electrical faults while driving: sudden loss of power steering assist, all dashboard lights illuminating, windshield wipers activating spontaneously, and the engine failing to shut off using the Start/Stop button. The vehicle has also stalled without warning while driving in traffic or idling at red lights, restarting only after 5–10 minutes and blocking traffic lanes, placing me and surrounding drivers at significant risk. Two major repairs in the last two weeks failed to resolve the defects: (1) repair of the CAN-bus system after the vehicle would not start and acted erratically while driving, and a week later (2) replacement of the brake-booster vacuum pump, which was shorted internally and caused CAN-bus shutdown, complete steering loss, sudden braking, and no engine power. The vehicle is currently at Dependable Chrysler Dodge Jeep Ram, Canoga Park, CA, awaiting further diagnosis despite repeated repair attempts. These symptoms match Chrysler TSBs 08-007-17 and 08-033-21 concerning Body Control Module water intrusion and communication faults. This ongoing electrical defect has caused steering loss, random stalling, ignition failure, and unpredictable vehicle behavior—issues widely reported by Pacifica owners. The authorized Volvo retailer diagnosed failure of the WAM and other modules due to water intrusion originating from the sunroof drainage system. This is a known, recurring defect acknowledged by Volvo in multiple Technical Service Bulletins: - TJ 31372-2 (Water leakage via sunroof drain hoses) - TJ 35382 (Updated drain hoses - new part number) - TJ 29322-3 (Water ingress passenger footwell / CEM area) The original drain hoses are too narrow and prone to clogging, causing water to overflow into the passenger compartment instead of exiting the vehicle. ... pad friction material was completely gone, resulting in metal-on-metal contact with the rotor. The rotor showed deep scoring and unsafe wear consistent with a possible stuck caliper piston, seized caliper slide pins, or other component malfunction. This failure occurred without warning. The vehicle had been at the dealership for a customary inspection shortly before this issue, and the dealership did not report any brake wear concerns at that time. All brake services and major repairs on this vehicle have always been performed by the same Cadillac dealership. The last documented brake service with chanigng of the breaks involved was completed by the dealership in 2022. This is a critical safety issue that ]caused total brake failure. I am requesting that NHTSA review this defect, as premature brake destruction of this nature may indicate a systemic issue with the brake system, caliper operation, or inspection protocol on this model. The dealership was not sure of the reason why this happened that I was adviced that the bolt was missing and could have come off with vibration which is again a critical safety issue. This is a known defect documented by Tesla in Service Bulletin SB-21-39-001, which describes excessive front drive unit half-shaft vibration during acceleration and defines the repair with updated components. The latest revision of this bulletin (R3, dated January 19, 2024) explicitly adds “Customer Pay billing” instructions. Despite this, my local Tesla Service Center in Tempe, AZ (Supervisor: Lewis Holguin) refused to apply the SB-21-39-001 Customer Pay billing and instead charged full retail for the repair. On 11/28/2025, I was billed $1,694.86 out of pocket to replace both front half-shafts again (invoice shows approx. $1,046 in parts and $564 in labor). I have documentation showing that other Tesla customers with the same defect were billed only $350 under SB-21-39-001, which demonstrates that Tesla does apply a discounted bulletin rate in some cases. In my case, they refused to honor it even though this is a repeat failure of the same known defect and the prior warranty repairs did not permanently fix the problem. This is a repeat driveline/suspension defect that affects drivability and safety under acceleration. The shaking and vibration make it difficult to maintain stable vehicle control, especially during highway merging or lane changes. I am requesting that NHTSA investigate recurring front half-shaft failures and vibration/shudder issues on Tesla Model X vehicles and Tesla’s inconsistent handling of this known defect and service bulletin.Read more

This vehicle is going through coolant constantly. I couod not find a leak. I did find a recall FORD had for a cup plug and the cylinder head that leaks. ... ter a crash, I saw a straight “strip flame” shoot out of the HVAC dash vent, exactly like a gas heater. The fire began INSIDE the dash/HVAC ducting within seconds of impact, not from the battery. This shows an arc-flash failure of the HV heater or wiring inside the cabin. The flame entered the cabin instantly because there is no firewall separating HV components from occupants. During the fire, all electronic door releases failed. Tesla hides the manual front releases and provides NO rear mechanical releases. I was trapped inside a burning cabin and suffered major injuries trying to escape by kicking the rear passenger window before bystander helped me brake out the rear passenger window to get me out. Most people cannot escape this design. This is a dangerous combination of defects: (1) HV heater and wiring located inside the cabin with no firewall; (2) HVAC ducting becomes a flame pathway; (3) Electronic door releases fail during fire/crash; (4) No labeled manual rear-door exits; (5) No backup 12-V actuator power to open doors. Tesla must: relocate the HV heater outside the cabin (frunk), add a real firewall, add fire-retardant HVAC materials, add labeled manual releases on all doors, and include a 12-V backup to door actuators. She avoided a direct eye injury only due to quick reflex. This design presents a serious risk to passengers, especially those of shorter stature. I have photographic documentation showing the sharp door edge and the injury mechanism. Hyundai Motor America was informed, but they dismissed the case and advised us simply to “be more careful.” They offered a $150 virtual compensation card, which I did not use, because accepting it could be interpreted as accepting their proposed settlement. Hyundai insisted on dealer inspection, which is irrelevant because this is not a defect of a single unit — all vehicles of this model have the same door-edge geometry. Steering sometimes catches momentarily at the 1 o'clock and 11 o'clock positions. Seems most common during turns at highway speeds. Been monitoring intermittent occurrences for about a year. On 11/26/2025, it occurred several times in close succession while making gradual turns going 35 to 40 mph.

On 11/26/2025, I was driving on the NJ Parkway, Southbound, in the left lane. The Drivetrain Malfunctioned Warning Light came on and within second my car began loosing power. I did not notice any smells. My car stalled as I stopped along the left side of the roadway. I did not have time to maneuver safely to the right, onto the shoulder of the roadway. ... of the road. My car was pinned against the guardrail to avoid being struck by southbound vehicles. The warning light disappeared. I attempted to restart my car, it sputtered a few time with black smoke coming out of the exhaust. I called 911 and State Police Responded to protect my vehicle from being struck. About an hour later, a tow truck responded and my vehicle was towed to the BMW Dealership, 500 Route 37, Eaton Town, NJ. I believe this catastrophic failure is connected to my vehicles EGR’s system. I had my vehicle serviced at Sterling BMW, Sterling, VA, in early 2025. During that visit I was told that my vehicle’s Auxiliary EGR cooler needed to be replaced. In October 2025, I took my vehicle to BMW of Westbrook, Maine, after I received a notice from BMW that they had extended the warranty on my Auxiliary EGR Cooler and it could be replaced if needed. I provided the Dealership with the information that Sterling BMW provided me about the need to have my Auxiliary EGR cooler replaced. Westbrook BMW examined my car and said there was nothing wrong the my Auxiliary EGR cooler. My vehicle will be looked at tomorrow by Eaton Town BMW. I suspect, I will be told the failure will be related to the EGR system.Read more
