Vehicle purchased 9/1/24 from Courtesy Hyundai of Tampa. Dealership failed to file registration/tags until 10/4/24. 7/12/25: Vehicle experienced involuntary acceleration and braking, resulting in impact with my mother’s home. 7/16/25: Dealer issued written report stating vehicle was operating normally and documented a recall-related display/audio/camera software update. 8/14/25: Hyundai enginee
... ring inspection performed without my knowledge. No report has ever been released. 8/27/25: Hyundai issued corporate denial, blaming driver error and stating braking/acceleration systems were normal. 12/5/25: Second involuntary braking incident at highway speed on I-275. Five uncommanded braking events over ~20 miles. Service Manager stated failure was due to sensor/camera malfunction and instructed that the vehicle should not be driven and to tow immediately. Despite this, the vehicle had been released into roadway operation. VIN lookup shows an open, unrepaired federal recall (24V879000 / Recall 271) for the rearview camera system, marked Recall Incomplete, despite the dealer’s documentation that recall-related camera/software work was completed on 7/16/25. BBB Auto Line denied my claim only because the vehicle was outside Florida Lemon Law eligibility. No safety determination was made. Hyundai released no engineering, sensor, or camera data despite an engineering inspection. This vehicle has experienced repeated involuntary braking and acceleration at highway speed, involving braking, camera, and automatic emergency braking systems, creating a serious risk of loss of control, collision, injury, or death. Despite repeated failures, contradictory diagnostics, withheld engineering data, and an open federal recall, the vehicle was released back into public roadway operation. I request that NHTSA formally investigate this safety defect.Read more

The contact owns a 2011 Hyundai Sonata. The contact stated that while driving at an undisclosed speed, the brake warning light illuminated, and the vehicle shut off. The vehicle was taken to the dealer, where it was confirmed that the failure was due to an unrepaired recall. The contact received notification of NHTSA Campaign Number: 23V651000 (Service Brakes, Hydraulic). The repair was performed.
... The failure reoccurred. The dealer was made aware of the failure; however, the failure was not duplicated. The contact was informed that the recall was closed. The vehicle was taken to an alternate dealer, where it was diagnosed with a failed hydraulic ABS booster sensor. The contact was provided DTC: C1237/HPU. The contact was informed that the hydraulic ABS booster sensor needed to be replaced. The vehicle was repaired. The vehicle was released to the contact; however, the failure reoccurred. The contact stated that the ABS warning light illuminated and then went off. The manufacturer was made aware of the failure, and a case was filed. The contact was advised to take the vehicle for a second inspection. The dealer declined to perform the second inspection. The contact was informed that the repair was not associated with the recall repair. In addition, the contact referenced several unknown recalls; however, the contact was informed that the repair was not related to a recall. The failure mileage was approximately 149,000.Read more

Accident Statement – Instrument Panel Cluster Failure & Collision While driving my 2025 Ford Outerbanks, the Instrument Panel Cluster (IPC) screen suddenly went completely black without warning. At the time of the malfunction, I was attempting to merge into traffic on my left with my left turn signal activated. Simultaneously, an 18-wheeler in the lane beside me was merging to his right. The s

This created an immediate hazardous situation and made it extremely difficult to manage both my own speed and the surrounding traffic patterns. As I recognized the danger, I attempted to brake and avoid a collision. During this moment, my steering wheel began to shake slightly—possibly due to the lane-keeping or lane-safety system responding. Despite my efforts to correct and avoid impact, the 18-wheeler collided with my vehicle. At the time of the crash, I had no visibility of any dashboard warnings, alerts, vehicle speed, or safety notifications due to the IPC failure. 9/25/25 Car would not accelerate on exit ramp and engine seized just as I pulled into Romeo Kia Kingston Car Repair Center, causing a dangerous driving situation. No prior noise or warning lights for the 6 years I have owned the car. 10/6/25 per Jared Illg, District Kia Mgr., “Although, my 2018 Kia Soul 2.0 engine is under the Class Action Settlement, my car is not under extended warranty,
... because I did not have the KSDS software update.” I did not receive any mailings over the years, but two August, 2025 re-notifications. •Kia KSDS software 2018 update (172/953), why wasn't update performed by Romeo Auto Group Dealership before I purchased car? •Kia’s parent company, Hyundai Motor Group removed the KSDS update requirement for its lifetime engine warranty, why hasn’t Kia? •The Romeo Auto Group Dealership owned this car and used it as a Lease Car. I am second owner, why didn't I get a Transferable / Powertrain warranty? •Why would I get a limited warranty knowing the Kia Soul has faulty 2.0 engines since 2012, defects that arise in the car that originate from the factory; resulting in numerous lawsuits and class action settlements. This is not a case of owner neglect; I have taken excellent care of my 2018 Kia Soul. •I did not receive TSB-19 and TSB 222 which refers to known defects of all these connected components: excessive oil consumption, defective piston rings, and rod bearing failure; resulting in catastrophic engine failure. My mechanic has installed a used 53,000 mi. used engine. I now have a debt of $8,817.91 for a 2018 Kia Soul I have owned for six (6) years. I am so stressed this has a disastrous impact on my financial situation. Desired resolution: I am asking for Kia America, Inc. to reimburse me my expenses of $8,817.91 (replacement engine, towing, and car rentals; attached) and provide the extended Warranty for 150,000 mi.Read more

The vehicle is subject to NHTSA Safety Recall 25V572 (Manufacturer Recall Number 25S89) regarding the rear-view camera wiring harness. The manufacturer has notified owners of the safety risk but indicates that the remedy parts are currently unavailable and has failed to provide a specific timeline for repair. I have contacted the authorized dealer and the manufacturer requesting a remedy or an in
... terim inspection, as the rear-view camera is currently functioning correctly with no blank screen or distortion. However, the manufacturer and dealer have refused to perform an inspection to verify the system's safety and close the recall status temporarily. Because the manufacturer cannot provide parts and refuses to offer an inspection-based solution, the vehicle remains flagged with an open safety recall indefinitely. This represents an unreasonable delay in the remedy process, rendering the vehicle unusable for its intended purpose while the manufacturer fails to address the safety defect status.Read more

Description of Problem: INSTRUMENT CLUSTER COMPLETE FAILURE My 2016 Tesla Model X instrument cluster has completely failed and permanently displays "CAR OFF" regardless of the actual vehicle status, including while actively driving at highway speeds. Federal Safety Violations: This defect clearly violates Federal Motor Vehicle Safety Standards (FMVSS) No. 101 and No. 102. Gear position indicators must be "permanently displayed in view of the driver" Critical safety information must be accessible to a properly seated and belted driver Instrument panels must provide essential vehicle status information Tesla's Pattern of Similar Recalls: Tesla has previously recalled vehicles for similar instrument cluster issues: 2023 Recall (NHTSA Campaign 23V-404): Tesla recalled 2012-2023 Model S and 2016-2024 Model X vehicles for incorrect instrument panel warning light font size, specifically stating that warning lights with smaller fonts "can make critical safety information on the instrument panel difficult to read, increasing the risk of a crash." 2022 Recall (Multiple campaigns): Tesla recalled Model S and Model X vehicles for touchscreen failures affecting backup camera, window defroster/defogger, and instrument cluster displays.Read more

Nearly a complete loss of braking downhill. I am very familiar with the stretch of road I was traveling and have traveled it in other vehicles without issue. When I did need to use my brakes I noticed they were not working as they should, I had to basically stand in the brake petal and the truck shook barely slowing. I eased off the brakes and applied pressure again with the truck still shaking a
... nd barely slowing again. I was finally able to downshift and slow down enough to stop the vehicle and pulled off the side of the road allowing the brakes and rotors a chance to cool down. Had I not been able to stop the vehicle, it would have gone off the side of the mountain likely resulting in death. I did take the vehicle to the local Toyota service center complaining of loss of brakes and they returned with everything is within specs. I have not had another issue like that yet but I do still experience a brake squealing when braking especially in reverse. There were no prior issues and didn’t notice any warning lamps. Component/System: Passenger Occupant Detection System (PODS) / Airbag system — Recall 74E3 / NHTSA 21V874. What Happened: The passenger airbag may switch off even when the seat is occupied due to a faulty PODS wiring connection. The warning message “Passenger Airbag Off” appears with a chime. This is an active safety recall that was issued before I purchased the vehicle, but the recall was
... not disclosed at the time of sale. Safety Risk: If the system shuts off the airbag incorrectly, the passenger may have no airbag protection in a crash, creating a serious risk of injury or death. Inspection Availability: Vehicle is available for inspection. I live on the Big Island of Hawaii, where there is no Audi-authorized repair facility. Reproduction/Confirmation: Symptoms match the known recall defect. Audi has confirmed the recall applies to my VIN, but no local dealer exists to inspect or repair it. Manufacturer/Dealer Inspection: Audi has refused to cover shipping to the nearest authorized dealership on Oahu, which is the only location where the recall can be performed. Therefore, the defect has not been inspected due to lack of accessible remedy. Warnings/Symptoms: Airbag warning light illuminates and a message appears indicating the passenger airbag is off, along with a chime. Additional Notes: I cannot safely use or sell the car because the required safety repair is inaccessible. Audi’s refusal to provide transportation makes it impossible to complete the mandated recall.Read more

Vehicle: 2016 Kia Sorento, VIN [XXX] Incident Date: November 13, 2025 Incident Location: Durham, NC (3828 Guess Rd) Cause of Loss: Engine Fire, confirmed by Durham Fire Department Report (Incident #253663) as "originated from under the hood." TECHNICAL NARRATIVE & SYMPTOMS (CRITICAL FOR LIABILITY): I am filing this complaint to report a catastrophic vehicle fire that I believe is directly linked
... to multiple acknowledged manufacturing defects that create a fire risk in Kia vehicles. ABS/Traction Control Light Warning (Direct Fire Risk Symptom): Immediately prior to the loss, the ABS and Traction Control warning lights were constantly illuminated. This is a known precursor symptom for the defective Hydraulic Electronic Control Unit (HECU) or ABS Module short-circuit, which has been cited in numerous Kia fire recalls (e.g., NHTSA 20V019, 20V519). The fire's origin under the hood is consistent with this component failure. Prior Engine Failures (Systemic Defect History): This vehicle has a history of catastrophic manufacturing defects: The engine assembly was replaced in 2023 due to the known Theta II Rod Bearing Failure. The knock sensor was replaced in September 2025 as part of the KSDS Product Improvement Campaign, demonstrating the vehicle was still under active service for critical engine monitoring defects just weeks before the fire. Potential Tow Hitch Wiring Fire Risk: The vehicle was fully engulfed, but the fire may also be linked to the defect cited in NHTSA Recall 22V703 (tow hitch wiring harness short circuit, which also poses a fire risk). I require a full investigation into this defect leading to compensation for the total loss of the vehicle and consequential damages. Central computer (motherboard), cameras (front/rear/side), autopilot/auto steer system. Error code: APS_w132. Description of Problem: On or around [XXX], Tesla pushed a mandatory over-the-air software update to my vehicle. Immediately after, multiple safety-critical systems failed: all cameras (including backup) became intermittent or non-functional, auto steer/autopilot stopped working, and the c
... ar emitted constant distracting beeping alerts due to a critical motherboard failure. This made driving hazardous—impaired visibility for parking/reversing, loss of collision avoidance features, and constant audio distractions that could lead to accidents. I had to drive in this unsafe condition for weeks while waiting for service. Tesla attempted a remote fix but it failed. I scheduled service on November 19 for December 3 (19-day delay). Diagnostics confirmed the update induced the hardware failure, a known issue reported by other 2021 Model 3 owners on forums like Reddit (e.g., software bricking motherboards post-update). Despite this, Tesla refused warranty/goodwill coverage, quoting $2,576.83 for repairs. Service advisors Josh and Andy were unprofessional: Josh was rude/condescending; Andy was aggressive, sarcastic, and retaliatory. After I mentioned filing complaints with the AZ Attorney General and News Channel 3, Andy threatened $100/day storage fees with an impossible deadline (called at 4 PM Friday, close at 5 PM, demanded pickup by morning, then noon). He closed my ticket without authorization, delaying fixes and forcing me to contact another location—prolonging my exposure to the unsafe vehicle. This defect poses serious safety risks: sudden loss of cameras/autopilot could cause crashes. Tesla's awareness (from prior reports) and poor handling (intimidation delaying repairs) exacerbates the danger. No crash/injury yet, but potential is high. Please investigate this update-induced failure as a widespread defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
