When the AC system is turned on, the idle increases from 740rpm to 760rpm. It engages the torque converter in the automatic transmission causing the car to move forward on its own. This makes it difficult, whether you are in traffic or at a stop light where there are vehicles stopped infront of you. The driver has to press hard on the brake pedal to keep it from moving forward. The local Toyo
... ta dealership (Hendrick service manager and two service technicians) said it was fine. Toyota Brand Engagement sent Eric who is their corporate regional engineer to inspect the car. He said it was fine. I have spoke with Toyota Brand Engagement several times and at various levels who all say it functions fine as the idle is within limits. I asked Toyota for an idle specification sheet but no one (the local Toyota Dealership, Engineer from Toyota Brand Engagement, or CEO office of Toyota Brand Engagement) is able to produce an idle specification sheet for the vehicle. They continue to state it is normal. Toyota Executive Office Case XXX INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
Incident date: Dec 2, 2025
I am reporting ongoing safety concerns with my 2018 RAM 2500 (6.7L Cummins) related to repeated emissions/aftertreatment failures and the lack of timely repair options. These failures directly affect drivability and can create hazardous situations if the vehicle enters derate mode while towing or driving at highway speeds. Over the past year, my truck has suffered multiple emissions-related compo
... nent failures, including DEF dosing issues, SCR/DPF system faults, repeated warning messages, and the threat of speed-limiting derates. The vehicle is used for normal personal use, is well-maintained, and has not been modified. Despite proper maintenance, the same types of failures continue to recur. A major concern is that local RAM dealers have extremely limited ability to service these issues in a timely manner. Appointments are often weeks out, and when the truck is already in a failure state it is unsafe to continue driving. I have previously reported safety-related defects to NHTSA (ABS/HCU), and those issues later resulted in recalls—so I know that reporting matters. The combination of emissions failures, derate risks, and lack of dealer support creates a situation where the vehicle may suddenly lose power or become unsafe to operate, especially when towing or traveling in rural areas with limited service availability. I am requesting that NHTSA review whether there is a broader pattern of emissions-system failures on RAM heavy-duty trucks with the 6.7 Cummins and whether these failures pose a safety risk due to the forced-derate logic and inability to obtain timely repairs.Read more

My car is currently under two recall notices however only one of the issues has been communicated to me of resolution. One being emissions control 25E02 and second being Recall 25S87 REAR BRAKE FLEXIBLE HOSE LEAK. Recall 1- emissions has been a problem for about 2 years now. Emissions are connected to my engine thus having my check engine light come on. My car failed an emissions test but was only
... 4 years old and the catalytic converters are not broken, have hole, anything leading to failed test, car repair fees and non-registered vehicle. The second recall is not being fixed, and I have had to replace my brake house, line and rear right brake twice this year. Lastly, I have collision warning however it does not work properly if at all. I was in a minor fender bender when I went to push on brakes the sound to warn me of a collision did not go off until after the crash! I am unsure of the process but if connected to my braking system it failed and if it is a preventative measure it sucks.Read more

[XXX] Problem: Multiple tire blowouts causing safety hazards and air-suspension damage Complaint Summary: My 2024 BMW i7 has experienced multiple tire blowouts, including the most recent , two within days of each other (12/8/2025 and 12/10/2025). On 12/8/2025, BMW replaced the right rear tire and advised me that the vehicle was safe to drive. Two days later, the left rear tire suffered the s
... ame sudden failure, requiring another tow. These failures occurred during normal driving and immediately after BMW dealership inspection and clearance. In addition to these recent events, earlier blowouts on this vehicle have already caused damage to the air-suspension system, requiring repairs. This indicates the issue is not normal tire wear but a recurring safety defect involving the vehicle’s weight, load distribution, tire load rating, or compatibility with the i7’s unusually high curb weight. The repeated failures and resulting suspension damage raise concerns about a broader defect in the design, load-bearing capability, or inspection procedures for this model. This vehicle is unsafe to operate, and the recurring blowouts place my family and other motorists at risk. I am requesting that NHTSA investigate whether the BMW i7 has a systemic safety issue related to tire integrity, load capacity, or suspension stress under normal driving conditions. The safety-critical driver-assistance systems on my 2024 Ford Escape have remained in a failed state since March 2025. The Blind Spot Detection (BLIS), Cross-Traffic Alert, Park Aid, Pre-Collision Assist, and Front Camera systems all display warnings and do not function reliably. The vehicle has now been in the dealership for over 62 cumulative days for this safety defect, including 38+ continuou
... s days on the current repair order. Ford engineering has taken control of the case and has stated there is no fix available and no estimated timeline for repair. A major electronic module was replaced under engineering direction in November, but the same safety-system faults continue. The vehicle’s alert history and FordPass data have remained frozen and non-updating for months, indicating a continued system-level electronic failure. The vehicle is still at the Ford dealership under engineering control with no repair available, which confirms the defect is ongoing, unresolved, and unrepairable at this time. These persistent failures impact multiple crash-avoidance systems and create a significant safety risk during lane changes, cross-traffic backing, parking maneuvers, and forward-collision situations. I request NHTSA review this as a potential defect trend involving the 2024 Ford Escape driver-assistance systems.Read more

This vehicle is affected by NHTSA Recall 25V644 (starter motor – fire risk). The recall remedy has been listed as “Remedy Not Available” since September 25, 2025, and remains unavailable more than two months later. BMW did not notify me about this safety recall. I only discovered it after a third-party platform automatically deactivated the vehicle due to the open recall. In addition, this
... vehicle has recently developed starting issues — it now struggles to start and requires multiple attempts. This may indicate that the defective starter component described in the recall is already failing. Combined with the stated fire risk, this creates a serious daily safety concern. BMW advises owners to park outside and away from structures, but provides no remedy, no timeline, and no assistance. Multiple inquiries to BMW Recall Support were answered with the same message: there is no known timeframe for parts or repair. I am filing this complaint because the manufacturer has not provided a remedy, has not communicated adequately, and the defect appears to be progressing in a way that increases the risk described by NHTSA. I respectfully request that NHTSA review the extended delay, lack of remedy, lack of notification, and the presence of active symptoms related to this safety defectRead more

... d. Details of Each Incident: While driving under normal conditions or coming to a stop, the vehicle triggered the red “Stop Safely Now” alert with powertrain and brake warnings. Propulsion shut down instantly and the vehicle locked in Park, leaving it immobilized in traffic lane. I had to exit the vehicle and wait for it to complete a full power-down cycle before it would restart. After restarting, the vehicle operated with reduced functionality and limited range. Ford Roadside Assistance was contacted for every incident. Safety Concern: A sudden loss of forward motion in moving traffic poses a serious crash risk to me and to other drivers. The repeated nature of this failure suggests a defect in the propulsion system, sensors, electrical components or related modules. Current Status: The vehicle is now at the local Ford dealership for evaluation. I have also notified Ford Corporate Customer Care. The root cause has not yet been identified or resolved. Requested Action: I request that NHTSA document this complaint and consider further review due to the recurring safety-related failure and the potential risk to other owners.Read more

Different cylinder proves systemic defect—50% of engine affected. Third (12/6/24): Highway breakdown. "Repaired" in hours with spark plugs only—inadequate for explosive failure. Service advisor admitted "whole part might have potential issue" but Honda policy prohibits preventive replacement "until really broken"—knowingly allowing defective vehicles until catastrophic failure. Dealership attempted release without highway test; conducted only after I demanded it. No explosive sound explanation. Refused diagnostics, compression testing, internal inspection. First two failures had dashboard warnings at low speed. Third had ZERO immediate warning—normal to explosive failure instantly at highway speed (warnings appeared 2 hours later). Unpredictable and extremely dangerous. Explosive sound indicates severe internal damage—not just spark plugs. Evidence: 11,700 miles too young. Specific cylinders (2, 4) suggest manufacturing defect. In May 2024, I purchased an EVIQO Gen 1 Level 2 electric vehicle charger from Amazon. In November 2025, I noticed the charger would go dormant after charging, which prevented vehicle pre-conditioning. When I contacted EVIQO support, they advised me to remove the faceplate to access an internal reset button while the unit was energized, creating a significant electrical hazard. I raised concerns a
... bout the danger of accessing live, high-voltage components. EVIQO provided mixed and contradictory guidance, at times claiming it was safe due to insulated components and later quoting their manual warning of electric shock. Their instructions evolved repeatedly, creating confusion and exposing me to potential harm. This incident highlights a serious design flaw in the Gen 1 unit: users were required to access high-voltage components while energized to perform a basic reset, creating an unavoidable risk of electric shock. EVIQO’s subsequent release of the Gen 2 unit, which features a safe, externally accessible reset button, demonstrates that the hazard in the Gen 1 model was avoidable and that the company was aware of the flaw. No injuries occurred, but the situation underscores the risk posed by the Gen 1 unit and multiple instances of contradictory and unsafe guidance from EVIQO support. Despite this, the Gen 1 unit continues to be sold on Amazon.Read more

The specific part that requires replacement is often documented in General Motors Technical Service Bulletin (TSB) 19-NA-206 or 23-NA-119. The faulty component is available for inspection if the dealership removes the old part and retains it. My safety and the safety of others are put at risk because th
... e vehicle often fails to register that it is fully in "Park," which prevents the vehicle from shutting off properly, potentially leaving it in accessory mode. This could lead to a dead battery, leaving me stranded. More importantly, the failure to confirm the 'Park' state introduces a risk of the vehicle not being securely immobilized, creating a roll-away hazard if parked on an incline. The problem has been confirmed by local Chevrolet dealership service centers (receipts available). The issue is identical to the defect that prompted official safety recalls for numerous other GM models, including the 2017-2018 GMC Acadia. Furthermore, the symptoms described match those outlined in the active class-action settlement for 2016-2019 Malibu owners (STPLawsuit.com).Read more

While driving my 2018 Kia Optima S, the car suddenly jerked, lost power, and stalled in traffic. Multiple warning lights came on at the same time including the oil light, battery light, and check engine light. I had no acceleration and the brakes became very hard to press. The vehicle had to be towed for safety reasons. The Kia dealership inspected the vehicle and confirmed internal engine failur
... e due to “cylinder failure” and stated the engine needs complete replacement. This failure occurred without any external leaks and without warning other than the sudden loss of power. The dealer stated the issue is not covered, even though it is clearly an internal engine malfunction. This is a major safety hazard because the stall happened while I was in active traffic. A sudden loss of power and braking ability puts me and others at risk of an accident. The problem has been confirmed by the Kia dealership through diagnostic testing. The vehicle is still available for inspection. Warning lamps appeared only at the moment of failure, giving no advance warning. This appears to be related to the ongoing engine defects in Kia vehicles involving oil starvation, loss of compression, cylinder scoring, and internal engine failure. Cylinder failure and sudden stalling should be investigated as part of the known safety-related engine defects affecting this model.Read more

I am reporting a safety-related engine defect on my 2019 Ford Escape (2.0L EcoBoost). On September 23, 2025, at 72,233 miles, the engine failed due to coolant leaking into the combustion chamber (cylinder #3). Ford’s own dealer confirmed coolant intrusion and referenced Ford TSB 22-2239. This issue is associated with known EcoBoost coolant-intrusion failures documented in multiple Ford bulletin
... s and class-action suits. The engine failure required a complete long-block replacement costing $8,644.38. Ford denied assistance despite the failure occurring within the extended 7-year/84,000-mile coverage window under Customer Satisfaction Program 21N12. Coolant intrusion can cause sudden engine failure, stalling, loss of power, or inability to accelerate, creating a hazardous driving condition. I request NHTSA review this defect as part of the widespread EcoBoost coolant-intrusion problem. Owner: [XXX] , [XXX] . VIN: [XXX] . # **NHTSA Complaint Answers** ### **1. What component or system failed or malfunctioned, and is it available for inspection upon request?** The **engine** failed due to **sudden and extreme oil consumption** that resulted in the engine being completely dry of oil despite regular maintenance. This caused internal damage including **CVVT (Continuous Variable Valve Timing) system failure**, bearing d
... amage, and a complete engine seizure. Yes, the failed engine is fully available for inspection upon request. The vehicle is currently located at Dick Smith Hyundai in Greenville, South Carolina, where the dealership diagnosed the failure and recommended a full engine long block replacement. ### **2. How was your safety or the safety of others put at risk?** The engine failure occurred **suddenly and without warning**, causing the vehicle to lose power while operating. The loss of engine function created the risk of: * sudden deceleration in traffic, * inability to accelerate or maneuver, * potential rear-end collision, * complete engine stall in unsafe conditions. The vehicle had **no oil warning light** prior to the failure, meaning I had no indication that the engine was at risk until the failure occurred. ### **3. Has the problem been reproduced or confirmed by a dealer or independent service center?** Yes. The engine failure was confirmed by: • **Schwab’s Automotive** in Fayetteville, NC – determined the engine suffered catastrophic internal damage and was not repairable. • **Dick Smith Hyundai** dealership – confirmed **high oil consumption and CVVT damage**, and recommended a full **engine long block replacement** (estimate: $8,160.60). The problem has been fully verified by both an independent mechanic and an authorized Hyundai dealership. ## **4.Yes.The vehicle was formally inspected by Hyundai Motor America through the dealership’s diagnostic report. My vehicle is under an open safety recall 01C involving the PTU system, and there has been no remedy available for nearly a year. This defect caused a dangerous and sudden loss of acceleration while I was driving, and the manufacturer has still provided no fix, and no support through the dealership. The incident occurred when I was stopped at a red light. When the light turned green,I pressed the
... gas and the vehicle immediately made a loud grinding/shaking noise, shuddered violently, and lost all propulsion. The car would barely move. I was extremely lucky that there was a parking lot directly next to the intersection, and with the tiny amount of movement left,I was able to coast into it. If this failure had happened anywhere else—on a highway, in the middle of an intersection, or without a place to pull off—I could have been rear-ended or caused a collision. My safety was put at immediate risk due to this recall defect. I contacted the dealership, but they stated they could not repair the vehicle because Jeep/Stellantis had not released a remedy, parts, or service procedure for the recall. I was told multiple times that there was nothing they could do until Jeep provided a fix. After the breakdown, an independent shop confirmed the PTU failure, since the dealer still had no ability to repair it. All failed components and documentation are available for inspection. I lost use of my vehicle and was forced to take safety into my own hands. When I submitted reimbursement documentation to Jeep Customer Care, they denied it because the repair was not done at a certified dealer and did not use MOPAR parts—even though no certified repair or MOPAR parts exist for this recall. This leaves me without a safe vehicle, without a remedy, and without support. I am requesting NHTSA intervention due to the ongoing safety risk and the manufacturer’s failure to provide a timely fix and proper reimbursement for their admitted issue with the vehicle.Read more
