However, within three days of having the vehi

I am the driver's husband and the driver was the only one in the car. I assist her to file this report The accident happened around 6 pm on 12.28.2025 My wife was involved in a single car accident in a parking lot. She made a left turn at the intersection of 192nd ave and 3rd st, and then entered into the parking lot from the north entrance. The car hit the curb as she entered the parking lo
... t. Then the car (RAV4) started accelerating by itself, and was out of control. She cannot stop the car. To avoid going straight and hit the shops at the other end of the parking lot, she turned the wheel to one side. She hit a light post and then hit something else. The vehicle flipped over and landed on its roof. The vehicle was totaled. She was trapped inside the car unable to free herself until the ambulance, tow truck and the police arrived. By God's grace, she did not hit any other car or person and she only suffered minor injury. She was taken to the emergency room at Peace Health, examined and released after 3 hours. If you look at the RAV4, you cannot believe that she only suffered minor injury. Praise the Lord. She is still mentally shaken up by this traumatic event. Looking at the pictures of the damage to the vehicle, the car must have been traveling at a very high speed and there is no way that the driver stepped on the gas pedal intentionally. The insurance Company is Liberty Mutual Claim# [XXX] -XX The vehicle is considered total loss by the insurance company. EDR was inspected but data in the EDR was not collected. I suspect this incident was not caused by the driver, The data in the EDR will tell the true story. I asked Liberty Mutual to retrieve the data from the EDR to prove that the gas pedal was not depressed by the driver. If the car accelerated without the driver stepping on the gas pedal, then the vehicle has safety issues. I am waiting for Liberty Mutual to retrieve the data. Description of the Safety Problem: In the early morning hours of January 8, while driving my Hyundai vehicle, I ran off the road into a ditch. I was extremely disoriented and activated the vehicle’s built-in Blue Link SOS emergency assistance system for help. I successfully connected with a Blue Link operator and repeatedly told her that I did not know where I was and that I was scared and nee
... ded assistance. While I was actively requesting help, the SOS call was disconnected. After the disconnection, I attempted to reconnect using the SOS system but was unable to do so, despite the vehicle having power and an active, paid telematics subscription. The system did not reconnect or provide further assistance. I was forced to call 911 from my personal cell phone to obtain emergency help. This resulted in a delayed response and a $1,300 towing expense that I would not have incurred if the SOS system had functioned as marketed. This incident raises serious safety concerns. The vehicle’s emergency SOS system failed during an active emergency, disconnected while the driver was disoriented and requesting help, and provided no ability to reconnect. This defeats the purpose of an integrated emergency safety feature and could place drivers at risk of serious injury or death. I am reporting this incident so NHTSA can evaluate the reliability and safety of Hyundai’s Blue Link SOS emergency system during real-world emergencies.Read more

INCIDENT SUMMARY: On January 9, 2026, at approximately 1:30 PM, while traveling at 50 MPH in North Carolina, the vehicle experienced a sudden, catastrophic engine seizure without any prior "Limp Mode" warning or Check Engine Light. This resulted in an immediate, total loss of motive power in active traffic. I was forced to navigate a dead multi-ton vehicle across lanes without power steering or va
... cuum-assisted braking, narrowly avoiding a high-speed collision. TIMELINE & NEGLIGENCE: This vehicle was purchased from an independent retail dealer on December 20, 2025—less than 21 days prior to this life-threatening failure. The vehicle was sold with an "Open" status for Safety Recall 23V-707. The failure occurred exactly as described in the recall’s warning: a catastrophic stall at speed. TECHNICAL EVIDENCE: A non-invasive borescope inspection of the cylinders has confirmed the presence of metallic "silver chunks" (sheared bolt heads) and "half-moon" impact marks on the piston crowns, indicating a total timing failure and internal mechanical collision (piston-to-valve contact). REQUIRED REMEDY: This incident is a direct manifestation of the safety risk documented in Recall 23V-707. Per BMW Service Bulletin SIB 11 09 23, the presence of sheared bolt heads and seized internals constitutes "Catastrophic Consequential Damage." I am filing this report to ensure federal oversight of BMW North America’s obligation to provide a complete engine replacement, as the safety defect was present at the time of sale and resulted in a near-fatal incident within 300 miles of ownership.Read more

I was driving my 2020 Kia Sportage recently and experienced a safety issue 3 times within 30 minutes. I was driving on a highway (Pennsylvania Turnpike/I-76) at night. While driving through the snow and with exposure to the sa
... lt, my car notified me that the forward Collision Detection system could not operate because the sensors were blocked. Some time later in the drive, after that notification had turned off and remained off for a bit of time, I engaged my cruise control which uses the forward Collision Detection system. At 3 different times, I either approached a barrier (which run a large portion of I-76 to separate the east and west bound lanes) or a vehicle (I believe it was a semi truck) in the lane next to me while entering a curve. While entering the curve, the sensor saw these items and falsely detected a vehicle in front of me, and responded by rapidly slowing my vehicle. Two times the vehicle speed dropped about 20 mph within 30 seconds. On each occasion, I was fortunate that there was no vehicle close behind me that likely would have rear-ended me.Read more

In 9/2025 I noticed that the vehicle software was defaulting to a speed limit of 55 mph. I noticed that when in any form of autopilot and FSD it would accelerate to that speed. This happens in any situation before the car encounters a sign or an area that it's internal map corrects it, which is not common outside of major routes in Idaho. This includes paved marked roads, graded roads, single trac
... k dirt roads, my driveway and garage. Sometimes the car controls it's speed consistent with conditions, sometimes not. It can happen unexpectedly, and that's the issue. Where I have learned this is a problem, I of course turn off the AP or FSD and drive myself, however, if I am in driver assist mode and things are going well and the speed limit in the vehicle changes suddenly to the default 55 mph, it can take a moment for me to react. In October 2025 I reported this to my Tesla Service center as well as Tesla's over the air and on line email reporting options (from which I got no response). The service center entered a ticket directly with Tesla, and told me that that Tesla was aware. The explanation that came back was that it was due to a mapping update for the internal map of the car. They said it was the result of Google Maps setting the default speed limit in Idaho, which is where I live, to 55 and that this was due to a September 2025 update. They said it might be corrected with the next update, which has not occurred at this point (January 2026). This speeding behavior can happen suddenly and unexpectedly and seemingly randomly, and has the potential to injure pedestrians, the driver, and property, not to mention that it is also exceeding by far the legal speed limit. Drivers from other areas that may not be aware of the issue are likely to be caught unaware. There is absolutely no reason for the default speed to be 55, ever. It's unsafe, and unnecessary. It is rare to see a speed limit of 55 at all in Idaho. Most of the roads are above or below that speed limit.Read more

My 2025 Toyota Camry XSE (Hybrid) is subject to an open safety recall issued December 16, 2025. The recall involves a serious safety risk, and I was advised by the dealership not to drive the vehicle. At the time of the recall, the vehicle was approximately two months old with about 1,500 miles and in like-new condition. Because there is currently no available remedy, I was required to leave the
... vehicle stored at the dealership. I have been without use of my own vehicle since January 6, 2026 and am using a loaner vehicle solely due to the safety recall, not by choice. Toyota has stated a fix may not be available until at least Q1 2026, with no defined repair timeline, and customers are being told to wait without interim mitigation. I am concerned about the inability to safely operate my vehicle and the long-term impact of extended storage on a nearly new hybrid vehicle, including hybrid battery health, fuel and fluid systems, seals, tires, and interior and exterior materials. Prior to the recall, the vehicle was routinely garaged and properly maintained, and extended outdoor storage due to the recall introduces risk of premature wear unrelated to normal ownership use. While the dealership has indicated the vehicle may be started periodically, this does not replace normal driving and ownership use. In addition to loss of use, I am required to maintain a $300 monthly loaner vehicle deposit while my vehicle remains unusable due to the recall. Toyota Customer Service has stated they cannot escalate or provide assistance and are instructed to close cases without resolution, creating ongoing financial burden through no fault of my own. Toyota has acknowledged similar recalls on other models lasting extended periods, yet no interim remedy, mitigation plan, or customer relief is offered here. I believe this raises concerns regarding recall remedy timeliness, consumer safety, and loss of use.Read more

This turns off some of the vehicle's safety features. Toyota was directly notified of this incident but refused to cover the issue under warranty and expects consumers to pay for a new sensor and programing for a part that is faulty or not durable enough to perform under normal driving conditions.
