I purchased this vehicle on [XXX], with approximately 170,150 miles. On [XXX], during a routine oil change at approximately 172,636 miles, it was discovered that the engine had no oil present. Shortly afterward, the oil warning light began flickering intermittently while driving. On November 4, 2025, at approximately 175,495 miles, a certified mechanic again confirmed that there was no oil presen
... t in the engine. At no point were there visible external oil leaks, indicating excessive internal oil consumption. This condition creates a serious safety risk, as sudden engine failure while driving could lead to loss of power, stalling, or accidents. Based on my experience and publicly known issues involving Kia engines, this appears to be a systemic defect rather than normal wear. I am requesting that NHTSA document and review this issue as a potential safety defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The vehicle experienced vibration symptoms and stalled on the road. The day before it stalled, there were rattling noises which prompted me to take the car to the dealership the next day. I had just received the letter about the recall so I knew it was likely related. The car was towed to the Acura dealership but the dealership denied recall coverage, stating the engine number did not match the
... recall list. During the discovery process, it was later discovered that the engine had been swapped by the dealership in 2018 — a fact that was not disclosed to me at the time of my purchase. The installed engine still falls within the recall defect interval of 2015-2020 but they are resistant to repair. Upon initial inspection of the vehicle, the dealership found metal shavings in the oil indicating internal engine damage. The manufacture requested my oil change records suggesting that this was my fault. I presented the oil change receipts but they still refused recall coverage and the dealership requested approximately $5,000 for diagnostics, if determined my fault. If it is determined that the defect damaged the engine, they will cover the 5k. After several weeks of inspection and going through the manufacture recall process, the dealership determined the recall issue applies my car. However, due to the engine number not on the “list” they are denying my recall coverage. They are seeking $25k for service repairs. The engine warning light illuminated and the vehicle began to hesitate and lose power while driving, making the vehicle unsafe to operate. The problem involves the engine and powertrain. Each time they repair something different 1st- replace ABS module and hydraulic brake, 2nd - happened out of state at my mother funeral and replaced my ECM, 3rd couldn't figure it out and guessed it was my Forward collision camera and replaced it, 4th - is today and awaiting dealership diagnosis. This issue has left me stranded several times local and long distance, occured in middle of driving including on highway, and overall uncertain of the safety and reliability of the vehicle as each time it has been the same type of issue. I have been in communication with Hyundai Corporate since October and they stated that even tho I only have 24k miles on the vehicle that it is outside the state of IN lemon laws for a buyback. If I hadn't purchased an extended warranty I would be in even more debt or unable to put it in the shop even tho I don't feel like this should be my burden when I have kept this vehicle in great shape, low mileage, frequent oil changes, etc . however the safety of my vehicles goes unresolved. At this point I am uncertain that Hyundai is properly diagnosing my vehicle and not certain how these issues are impacting other components of my vehicle such as brakes, transmission, engine, sensors. I am the only owner of this vehicle bought brand new. I have checked repeatedly for any recalls that I have not addressed and at this time none are pending. NHTSA Complaint Narrative I am filing this complaint regarding a 2010 Mazda 3, VIN [XXX] . The vehicle experiences intermittent transmission control failures, including difficulty shifting out of first gear and warning indicators triggered by vibration. The transmission and transmission control module have already been replaced. The dealership has identified the wiring harness as the cause of th

However, Mazda has not provided a written determination of whether the known wiring harness recall (engine wiring harness chafing affecting control systems) applies to this VIN, nor confirmed recall completion history. Mazda has advised the customer to seek aftermarket or salvage parts, but has not provided an approved repair remedy or goodwill assistance despite the issue affecting drivability and safety. I am requesting that NHTSA review whether Mazda has met its obligations regarding: 1.recall applicability determination by VIN, and 2.providing an adequate remedy when OEM repair parts are no longer available for a safety-related defect. On [XXX], while operating my 2024 Tesla Model Y in Full Self-Driving (Supervised) mode, the vehicle experienced an Uncommanded Steering Input leading to a collision. The vehicle was traveling at a very low speed of approximately 5 mph in a clear, simple environment. Despite this walking pace, the FSD software failed to identify a visible concrete curb and initiated a sudden, sharp turn directly in
... to the obstacle without any prior warning or driver input. The fact that the system executed such a violent and erroneous maneuver at 5 mph indicates a fundamental failure in the Neural Network's object detection and path planning logic. If this logic failure had occurred at higher speeds or near pedestrians, the results could have been catastrophic. The local Tesla Service Center (Watertown, MA) refused to conduct a physical inspection or a log review of the steering torque. They issued a Repair Estimate of $1,217.26 while claiming 'no hardware fault codes' were present, thus ignoring the documented software malfunction. As a leaseholder of this vehicle with only 15,489 miles, I am reporting this as a significant safety defect that the manufacturer refuses to investigate. Secondly I am curious why the service manager didn’t mention about a recall possibly concerning our car. Even though it isn’t in the window of the recall ( but shows all symptoms). Guess I’m feeling a litt
... le jaded with the way the service manager conveniently left out they didn’t have any parts for our car and they would have to order them. Anyway they knew I was driving the car home and they only had 6 hours to mess with it.Read more

Back up camera stopped working, no blown fuses and the system updates I can do on the car from the touch screen say everything is up to date. My car has no open/not repaired recall for this.

My engine light came on shortly after my first warranty oil change August 23,2025 at the Tonkin Hillsboro dealership. During my 2nd warranty oil change January 9,2026 at Tonkin Hillsboro dealership I let them know my engine light came on after their last service and they wanted to charge me the diagnosis fee. After continued discussion learned they already checked the code indicator and mentioned
... possible gas cap seal and it was not fixed. Today I received an Updated Carfax of my recent service and saw a recall for my vehicle dated on October 30, 2025 re: Urgent Fire Risk When Parked. NHTSA Recall Number 25V741000 Manufacturer Recall Number68C NHTSA Recall Number25V-741 Recall StatusRecall Incomplete This safety was not checked, fixed or completed within the consumer regulatory requirement of recalls while my vehicle was in service and in their observation. If the recall notice is dated October 30, 2025, there should be no reason for an oversight in providing clear communication and corrective action immediately while the vehicle was in service on January 9,2025 and communication/information before I picked up the vehicle and drove it off the lot. This is a certified vehicle that so far has not given me any confidence of proper dealership quality and car for vehicle safety and consumer co concern.Read more

While driving, my brakes became mushy and ineffective. Brake warning lights came on, also. It was very scary driving in rush hour traffic with little to no brakes. There was no reliability. Just keeping as much as possible space between cars, using Hazard lights &prayer.

My 2025 Kia Carnival Hybrid produces externally audible audio while the vehicle is locked, with all doors and windows fully closed. Private phone calls, navigation instructions, and other in-vehicle audio can be clearly heard from outside the vehicle indefinitely, even after locking the car. This behavior occurs without warning to the driver and cannot be disabled or controlled through any user se
... tting. The dealership and manufacturer have confirmed this behavior is “by design” and have declined to correct it. This presents a safety and security concern, as it exposes private communications, signals whether a vehicle is occupied, and may attract unwanted attention or increase the risk of harassment, stalking, or theft—especially in parking garages, residential areas, or public locations. The vehicle appears secure to the driver (locked, windows closed), yet continues to broadcast audio externally, which is unexpected and undocumented behavior. I believe this constitutes an unintended external output of vehicle systems while secured, creating a potential safety vulnerability.Read more
