Trucks driver side rear window which is a three-piece window with a sliding window in the middle has shattered apparently due to a defect or fault in the rear window fogger. I had my son in the back at one point and put the defogger on, and it started smoking and nearly caught on fire. No warning on either incidences and they did not occur at one time but about two days apart.

I am submitting this complaint to report a serious safety defect involving my Honda Odyssey equipped with Honda Sensing technology. During normal operation and without any accident, impact, windshield damage, repair, or environmental event, all Honda Sensing safety systems suddenly and simultaneously stopped working. The disabled systems include Collision Mitigation Braking System, Adaptive Cruis
... e Control, Lane Keeping Assist, Road Departure Mitigation, Forward Collision Warning, and related camera- and radar-based features. Multiple warning messages appeared, and the vehicle effectively lost all advanced driver-assistance and collision-avoidance capabilities at once. This occurred without warning and without any action on my part. This failure is extremely concerning because Honda Sensing features are marketed and relied upon as critical active safety systems intended to help prevent or mitigate crashes. A sudden, total loss of these systems materially reduces vehicle safety and places occupants and other road users at increased risk. A safety system that can fail entirely and unexpectedly—absent any triggering event—indicates a potential systemic defect, not routine maintenance or wear. I am concerned this issue may affect other Honda vehicles equipped with Honda Sensing and may be related to a common failure in cameras, radar units, software, or vehicle electronics. I respectfully request that NHTSA review and log this complaint, investigate this failure pattern, and determine whether a defect investigation, service bulletin, or recall is warranted in the interest of public safety. Vehicle details: Make: Honda Model: Odyssey Model Year: 2020 Mileage: 87000 VIN: [XXX] Thank you for your attention to this serious safety matter. The contact owns a 2015 Audi Q5 equipped with Firestone Tires, Tire Line: Firehawk A/S, Tire Size: P235/55/R19, DOT Number: 1RBE793E9. The tires were purchased in 2022. While the contact’s wife was driving at an undisclosed speed, there was an abnormal sound coming from the vehicle, and the rear driver’s side tire had a blowout. The contact's wife was able to pull over to the side of the road.
... The contact was unsure if the tire was replaced or if the vehicle was driven to the residence nearby with the blowout tire still on the vehicle. The vehicle was taken to a tire specialty shop, where the rear driver’s side tire was replaced. On another occasion, while the contact's wife was driving at an undisclosed speed, the driver heard an abnormal sound before rear passenger’s side tire had a blowout. No warning lights were illuminated. A passerby assisted the contact's wife in changing the tire. On another occasion, while the contact’s wife was driving at an undisclosed speed, the driver’s side front tire had a blowout. The contact changed the flat tire once the vehicle was driven back to the residence. Upon inspection, the contact noticed that the inside of the tire, where the bead of the tire meets the rim, was cracked. The contact noticed tire crazing all over the tire, and that the inside of the tires showed deep cracks. The contact stated that the other two tires showed the same failure. The tire manufacturer was made aware of the failure. The tire failure mileage was approximately 130,000. The vehicle failure mileage was unknown.Read more

I’ve heard all the horror stories about faulty electrical issues. I’m chasing an issue now on my 2018 with ONLY 87,000 miles after spending a bunch of money on batteries hub assemblies random control modules to try to fix an issue of stabilitrak, traction control, abs, trailer brakes, power steering etc all going off every 3 seconds. Description On 12-15-2025, in 18°F weather, the vehicle was parked on a residential sloped driveway with the manual parking brake fully engaged. While the driver was outside the vehicle, the parking brake system failed to hold the car’s weight. The vehicle rolled backward, crashed through a wooden railing, teetering on a 4-foot retaining wall, and came to rest over a culvert. Two young children
... were inside the vehicle at the time of the failure. Component Failure & Availability: The manual parking brake system failed to provide sufficient holding force to secure the vehicle. The rear brake calipers are currently available for inspection. Safety Risk: The safety of the two children inside the vehicle was put at extreme risk due to the unexpected movement of the car. This failure matches the exact conditions and consequences described in Safety Recall 1217F (NHTSA Campaign 17V-393), which states that "unexpected movement of the vehicle if parked on a slope" increases the risk of a collision. Reproducibility & Confirmation: The failure occurred under the specific environmental triggers (cold temperature and slope) identified in the manufacturer's own safety recall documentation. However, the manufacturer has officially refused to authorize the internal teardown of the actuator shafts—the only diagnostic method capable of verifying the rust and sliding resistance that causes this loss of holding force. Inspections: The vehicle has been visually inspected by an authorized dealership and insurance representatives. The manufacturer has declined the necessary internal mechanical diagnostics to verify the internal defect. Warning Symptoms: There were no warning lamps, messages, or symptoms prior to the failure. The parking brake felt fully engaged at the time the vehicle was parked.Read more

I am filing this complaint regarding a persistent and unresolved defect with my 2024 Chevrolet Traverse, VIN: [XXX] . The first occurrence of this issue was in June 2025. Since then, the vehicle has repeatedly experienced a “Check Engine” warning, engine misfires (cylinder misfire), loss of power while driving, and severe engine shaking. These symptoms make the vehicle unsafe to operate. The v
... ehicle has been taken to an authorized Chevrolet dealership approximately 20 times for the same ongoing issue. Despite multiple repair attempts, the problem has never been permanently resolved. In total, the vehicle has spent about three months out of service at the dealership. As of January 14, 2026, after the vehicle has been at the dealership for over two weeks, I was informed that the dealer is unable to repair the vehicle. They have escalated the case to the manufacturer, General Motors, because they cannot identify a permanent solution. The current mileage of the vehicle is approximately 41,700 miles. This ongoing defect makes the vehicle unreliable and unsafe for normal use. The repeated failures to repair suggest a potential manufacturing or design defect rather than an isolated incident. I respectfully request that the National Highway Traffic Safety Administration review this matter and determine whether an investigation or further action is warranted. While driving the car an alert pops up with a red steering wheel and an exclamation point. The console states "Power Steering Unavailable Service Required". I brought the vehicle to a dealer and they stated they thought it was from low voltage due to a bad battery. They returned the car and the issue happened again. Oddly the you get the symptom first. Then you see an alert saying "LaneSense
... Off" then a ding where the console says "Power Steering Unavailable Service Required". So for a few seconds you might think lane sense off is the problem even though the reality is you have no more power steering. My family's life was at risk. Without power steering the vehicle was incredibly hard to control. It is very heavy and I had to use my full force to turn it, if it was my wife or someone less muscular they would have crashed. It has been inspected by the dealer and they returned it after replacing the battery and the issue re-occurred. I noticed this was an issue on a previous year model but it occuring on ours as well.Read more

1. The under carriage has corroded creating a safety hazard. Senior dealer representative indicated verbally that 2010-2014 models years are the most prone. 2. ... e until the dealer brought the issue to my attention. 2014 Subaru Outback. The car has been taken care of in a manner that exceeds the manual recommendations including always flushing the underside of the chassis during washes; especially the winter season. The car was taken to a dealer to have the intake manifold gasket looked at. The dealer took the opportunity to raise the car in the air, conduct an inspection and reported that the front carriage assembly is rusting away and considered dangerous. The Service Manager indicated that the model years 2010 to 2014 exhibit a high-degree of corrosion to the supportive member which holds the engine and transmission in place and if it fails could cause an extremely dangerous situation. What if any information has NHTSA compiled regarding these issues and Subaru's use of an under-designed, defectively manufactured part that could potentially cause a serious accident if the support structure collapsed? And if the NHTSA has conducted any research into this issue is there any option for a consumer to recoup or require Subaru to cover the cost of replacement which according to two Subaru Dealers is approximately $4,200. On [XXX] I was driving from Opelika, AL to Birmingham, AL. About 20 miles from home while driving on [XXX] in the left lane, my car would not accelerate. I pushe9 it down to the floor and there was no gas, so I had to get over to the right lane while rolling to get out of danger of the traffic and large trucks. MY car has been towed three time in about 3 month in 2025. I had it towed from my work
... parking lot twice because it would not crank. We bought new batteries, changed out the key batteries and this last incident. We had just got the car out of the shop for a fuel pump & fuel pump control module. (see receipt). I learned there was a Safety Recall from the NHTSA. However, when I called only to learn that my car had not been recalled. Therefore, I am quite confused because the safety issue recall no 09V343000 for 2010,S80 appears to be having the same issue. the software within the Central Electronic module (CEM) is not sending the signal to the fuel pump that was just replaced to the electronic module (PEM). The missing signal to the PEM inhibits the start of the fuel pump. This was a very frightening experience having a car stop on the highway while in the fast lane. Now the fear of it happening again is quite frightening, that's why I confused as to the seriousness of this electrical system software issue is not being taken for granted on my vehicle. If it was a problem for some, why would it not be a problem for all. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Headlights will appear to flicker during lite rain. Light reflected off of objects flicker. On coming traffic interpret the flickering as flashing lights trying to signal an emergency. This is caused by the location of the projector headlight light path being directly under the seam where the hood meets the fenders. During rain water is directed right in the path of the narrow beam beam thrown by
... the optics used to project the light in front of the cat. As water drips the beams of light are deflected sideways. It causes split second darkness in front of the car and confusion amongst drivers in the oncoming lanes. Mazda has recognized this and has a kit that fixes it but they are not calling it a safety issue and as a result are charging people including me upwards of $600 for the corrective kits installation. It is a safety issue. As a safety issue they should be compelled to install their fix for free. I have included file containing my request for help and Mazda's refusal. In addition I have included the technical bulletin describing the issue and the fix published by Mazda.Read more

I was driving slowly in heavy rain and pulled up to a red light behind another vehicle. Just as I was almost fully stopped, my car suddenly accelerated on its own and struck the vehicle in front of me. The car is equipped with an automatic braking system that should apply the brakes when there is an obstacle. Instead, not only did it fail to brake, it accelerated fully. If the car in front of us
... had not stopped us, we would have entered a very busy intersection, where we could have been seriously injured and might have hurt others. We were about to enter the freeway, and if this had happened there, the outcome could have been catastrophic. There were no warning lights or alerts of any kind. It happened completely out of the blue. We had the car towed to the dealer in Santa Barbara, where no problem was found. We then took it to the dealer in Thousand Oaks, where we purchased it, and again no problem was found. The vehicle’s black box was checked, but because the car was traveling under 5 mph at the time, nothing was recorded. We contacted Chrysler and reported the issue. So far, no action has been taken, and for the past week they have not even returned our calls. The incident occurred on [XXX]. We cannot drive this car. We also cannot sell it, because we cannot knowingly expose someone else to such a life-threatening risk. Since the problem has not been identified and fixed, it could happen again. My wife was pulling her car into a parking area consisting of a few small storefronts. When pulling in, the brakes completely failed as did the forward collision warning system and the automatic emergency braking system. As she continued to press the brake pedal it just went to the floor of the car. The end result was that the car struck one of the storefronts causing damage to the building as wel
... l as her car. Fortunately the store was closed at the time and no one was injured, nor was she. There were not any warning lamps or messages prior to the brakes failing. The brakes worked fine in the approx. 15 minute drive prior to the accident. I realize that drivers sometimes press the accelerator in error instead of the brakes and I can assure you that this was not the case (we wife is not elderly and is a very safe driver). We spoke to the auto body shop who will be repairing the car and they told us that the brakes currently seem to work. A online search revealed that this problem as been reported / discussed by others in the past. We expect to have the car repaired and returned to us in the next week but are very concerned with driving it as another accident could be fatal.Read more

Tesla Supervised Self Driving (FSSD) update 14 removed the ability for the vehicle operator to set speed targets. Additionally, their system is unable to accurately determine speed limits for many roadways. The discrepancy between FSSDs perceived speed limit and actual speed limit can be as much at +/- 25mph. The discrepancy can occur suddenly and at any time during a drive, even on stretches of r
... oad with a consistent speed limit. This results in moments of dangerous acceleration and/or deceleration that is not requested by the vehicle operator or can be reasonably anticipated. This causes erratic driving behavior to both the Tesla and to other motorist in the vicinity. Erratic, unpredictable driving is a major cause of traffic accidents that can lead to serious damage, injury, or death. Prior versions allowed the operator to set a speed target, similar to standard cruise control, that the vehicle attempted to maintain and would not exceed. FSSD v14 does not allow the vehicle operator to have any control over the speed of the vehicle to any degree that meaningfully contributes towards safe, legal driving.Read more

My 2024 Jeep Wagoneer Series 2 frequently stalls while coming out of idle, such as at traffic lights. This is not the auto stop-start feature and happens whether or not auto stop-start is deactivated. Usually, after being by stopped, as I am getting ready to start moving again, the vehicle will slightly rev or try to lurch forward before stalling out. This turns the engine off, throws the car in
... to park, and activates my e-brake. It happens before I press the accelerator so the car is not moving more than a mile or two an hour. This problem is dangerous because it creates a risk of accidents with other vehicles. When the car stalls in this way, I must confirm the car is in park, press the brake, restart the engine, deactivate the e-brake, and shift the car into drive before proceeding through the intersection. This takes a few seconds and if the driver in the car behind me isn’t paying attention and think I am starting to move, he or she could rear-end me. This problem has happened more than a dozen times (so not every time I drive but frequent enough to be a pattern). It can happen when I have been driving for a while and the engine is warm. There is no way for me to predict in advance when it will happen. It can go weeks without happening or happen twice in the same day. It does not activate any dash warnings (such as service engine light or an error code) that I can see. I have taken my car into the dealership twice for idle related issues including once for this exact problem. The last time, the dealership found misfires on all cylinders, both catalytic converters coming apart from the inside and 02 sensors that had melted from overheating. The dealership replaced all of this. My vehicle still stalled on the way home from the dealership. This time, I detected a strong rubber or plastic burning odor when I got home. The vehicle has less than 12,000 miles on it and was bought new less than a year ago. I attach last repair invoice.Read more

While driving my 2016 Kia Optima, the vehicle experienced a sudden and complete loss of engine power and stalled without warning. At the time, I was Door Dashing for income. The engine seized and the vehicle came to an immediate stop in the middle of 4925 Wolf Creek Dr SW, Atlanta, GA, a narrow two-lane road with limited street lighting. I was forced to remain stopped in the roadway in darkness f
... or over an hour while multiple oncoming vehicles passed by, creating a serious safety risk. The vehicle could not be restarted and had to be left on the roadside overnight. The following day, the vehicle would only start briefly when jump-started, ran for approximately 15 seconds, and shut off again. It could not move and was towed to an authorized Kia dealership. At the dealership, technicians jump-started the vehicle, performed diagnostics, added engine oil, cleared codes, reset adaptive values, and conducted a test drive. During that test drive, the engine seized again, and the vehicle had to be towed back. Kia approved a full engine replacement under its extended engine warranty, acknowledging engine failure consistent with known defects. However, after the engine replacement, the vehicle would not start at all. Kia stated oil intrusion from the failed engine damaged electronic components, leaving the vehicle non-operational. Kia denied coverage for the resulting damage and issued an estimated $6,000 repair bill for engine harness, injector sub-harness, and ECU replacement. The vehicle remains inoperable following an approved warranty repair. This incident involved sudden loss of propulsion while driving, occurred without warning, resulted in repeated engine seizure, and left the vehicle disabled in traffic at night. This appears consistent with known Kia engine defects and presents a serious safety concern that warrants investigation.Read more
