I am reporting a dealership conduct issue that directly affected vehicle safety and caused financial loss. Valley Ford of Columbus, Ohio told me by phone that a specific safety-related part for my vehicle was in stock and available. Based on that representation, I traveled to the dealership and took time away from work. When I arrived, I was told the part was not available and had not been there.
... Shortly after this, I went out of town. During that time, Valley Ford stated they were unable to reach me. After returning, I again contacted Valley Ford and was once more told that the same safety-related part was available. I again traveled to the dealership, taking additional time off work, and once again was told upon arrival that the part was not available. This resulted in the same situation happening twice. Because of this repeated misinformation, I lost wages and hard-earned paid time off (PTO), was delayed in repairing my vehicle, and had to continue driving while waiting for a safety-related component I was told was available. I use my vehicle for work every day for approximately eight hours per day. Being forced to continue operating my vehicle without this safety-related repair places myself and other motorists at risk. After these incidents, I contacted Germain Ford of Columbus, Ohio in an effort to resolve the issue. I was informed that the same safety-related part could take up to eight weeks to become available. This extended delay has further prolonged my inability to complete necessary safety-related repairs and has continued to place me in a potentially unsafe driving situation. I believe this repeated misinformation and prolonged unavailability of a safety-related vehicle component is dangerous and unacceptable. I am submitting this complaint so NHTSA is aware of the actions of Valley Ford of Columbus, Ohio and the ongoing delay reported by Germain Ford of Columbus, Ohio.Read more

Subject: URGENT: Investigation Request – Systemic Loss of Propulsion in Jaguar Land Rover (JLR) Vehicles (2017–2026). 1. Description of Defect I am formally reporting a critical, life-threatening safety defect involving the Powertrain Control Module (PCM) and the Fuel Pump Driver Module (FPDM) in Jaguar Land Rover (JLR) vehicles. These modules suffer from spontaneous software "resets" or
... "reboot" loops during operation. When the PCM resets, it severs communication with the fuel system, causing an immediate and total loss of propulsion (motive power). 2. Safety Hazard and Real-World Impact This defect triggers without warning at highway speeds, resulting in vehicles stopping abruptly in high-speed lanes. This creates a "sitting duck" scenario, rendering the vehicle unable to move to a shoulder and significantly increasing the risk of catastrophic rear-end collisions and multi-vehicle pileups. Reported incidents include near-misses and accidents directly attributed to this sudden stalling. 3. Timeline of Manufacturer Awareness (2017 – Present) JLR has been aware of this specific PCM/FPDM communication failure since at least 2017. Internal documentation, such as NHTSA Technical Bulletin SB-10095121-1020, identified short circuits in the PCM/FPDM line for 2017 F-PACE and XE models. Subsequent bulletins, including MC-10218879-0001, admit that software "flashing" is ineffective and that FPDM hardware replacement is often required. Despite nearly a decade of internal knowledge, JLR has failed to initiate a comprehensive global safety recall for all affected VINs, opting instead for localized "Service Actions" that do not permanently resolve the hardware instability. 4. International Context and Manufacturer Inaction The Chinese State Administration for Market Regulation (SAMR) has already summoned JLR officials regarding these stalling failures. However, JLR has not provided a verified, permanent solution. 5. Demand for Action We demand an immediate, multi-agencyRead more

On November 15, I was traveling with my mother and seated in the front passenger seat of a 2023 Kia Sportage while she was driving approximately 65 mph on the interstate when the vehicle struck a deer. The impact caused significant front-end damage, including the bumper, radiator area, and underbody components. I was wearing a seatbelt. Despite the severity of the collision and the high rate of s
... peed, neither the driver nor passenger airbags deployed. No airbags deployed in any position in the vehicle. I sustained injuries, including a right shoulder sprain and having the wind knocked out of me, and required medical care. As a result of this crash, my mother developed post-traumatic stress symptoms, including fear of driving and anxiety related to the accident. The component that failed appears to be the Supplemental Restraint System (SRS)/airbag system, which did not deploy in a crash that should have met deployment thresholds. The vehicle is available for inspection upon request. This failure put our safety at risk by leaving us without the primary restraint protection designed to prevent head, chest, and upper body injuries in a high-speed frontal collision. The issue has not yet been reproduced or confirmed by a dealer. The vehicle has been seen by insurance, but no airbag system inspection or explanation has been provided. There were no airbag warning lights or dashboard messages prior to the crash. If there were any faults stored in the system, they were not displayed to the occupants. I believe this is a serious safety defect involving airbag non-deployment.Read more

I am writing to report a serious safety incident involving my 2022 Hyundai Ioniq 5 and to request a thorough investigation and guidance on next steps. On Jan. 3, 2026, at approximately 6:15 p.m. on a snowy day, we parked our car in the driveway of a weekend rental home near Lake Tahoe, CA. The driveway had a slight slope toward the house, and we backed the car down the driveway with the rear of th
... e car facing the house. My husband turned off the car, then all five occupants exited the vehicle, closed the doors, and began unloading items from the trunk. The car began rolling backwards toward us and the house. It only stopped after pinning my husband into a snowbank created by a snowplow. The other three adults and our [XXX] niece narrowly avoided being struck. While pinned behind the vehicle, my husband had to dig out some snow to reach into his pants pocket and retrieve the key fob. I then moved the car forward a few feet to free him. Although he was pinned in the snow, my husband was (thankfully) not seriously injured. After freeing my husband, I put the car in Park and turned it off. In this model, the parking brake engages automatically; there is no separate parking brake button or lever. A few minutes later, without anyone in or near the vehicle, the car again rolled backwards down the driveway into the same snowbank. Tracks in the snow clearly indicate that the wheels rotated and the car rolled; it was not sliding on ice with the brakes applied. In other words, the parking brake and/or Park mechanism failed to hold the vehicle in place in sub freezing, snowy conditions. The temperature at this location and time was in the mid 20s °F. After returning home from our trip, we took the car to our Hyundai service center, which has performed all maintenance since we purchased it in 2022. The service center performed all standard diagnostics but was unable to replicate the failure conditions.
