On February 1, 2026, at approximately 9:09 AM, while driving from a stop and continuing on main roads, the vehicle accelerated to extremely high speeds and would not properly slow down. When I removed my foot from the accelerator pedal, the vehicle did not decelerate as expected. Regenerative braking was nonexistent. Normally, releasing the accelerator provides noticeable deceleration, but in this
... incident the vehicle continued to propel forward instead of slowing. Braking was extremely difficult. The brake pedal felt abnormally stiff (“thick”) and required significant effort to depress. Even while actively braking, it felt as though the vehicle was still attempting to accelerate forward and did not slow normally. This occurred for a couple hundred yards before I was able to pull over due to safety concerns. At the same time, the vehicle was in All-Purpose drive mode, but the ride height was stuck at kneel height and would not adjust, which is abnormal. After pulling over, the vehicle would not shift into Park. I had to press the brake pedal with both feet while fully depressing it and lifting myself off the seat to keep the vehicle from moving. I then had to open the driver door in order to force the vehicle into Park. There were no warning lights or messages displayed on the dashboard. The vehicle was towed to the Rivian Service Center but has not yet been inspected because the service center is currently closed. This malfunction created a serious safety risk due to unintended acceleration, loss of normal deceleration, difficulty braking, abnormal suspension behavior, and inability to shift into Park.Read more

I am sending this email to report that my 2023 Chevrolet Silverado had its rear brakes lock up momentarily on consecutive days (1/13/26 & 1/14/26) driving 70-72 mph. Each occurrence came on without warning and only lasted a brief moment, but locked up hard enough for my rear wheels to screech and slam me forward in the truck, thankfully there were no other vehicles near me. I took the truck t
... o the dealership first thing in the morning on 1/15/26 to be informed later that day that the truck didn't show any codes. The service department at my dealership contacted the TAC Dept. at GM and they determined that the problem was caused by the brake light wiring from my truck topper (that was after they asked if the customer had added any accessories to the truck). I have had the topper for 3 years without any issues. I reached out to the company that installed the topper and the owner said that GM is giving me the run around and trying to point the blame elsewhere. He himself has installed 100's of toppers and they follow the wiring instructions from Leer Toppers. He did a 30 second Google search and said [XXX] , the problem you have experienced is well documented online, GM knows what the problem is with your truck". My truck was at the dealership for 5 days before they released it to me, even after I told them that there are known and documented cases of their trucks experiencing momentary rear wheel lockups. I have called GM Customer Service twice and left messages regarding my truck and TAC # and haven't received a return call. I am fearful that when this happens again and if it does so in the wrong place at the wrong time the end of this story may be tragic. If I was pulling a trailer, driving on ice or in rush hour traffic I can't imagine the outcome. GM knows they have a serious problem and they is comfortable in releasing a dangerous truck back onto the roads. I am writing to provide supplemental information regarding my existing safety complaints (IDs XXXand XXX). This update concerns a documented failure by Ford’s authorized dealer network to perform mandated Customer Satisfaction Programs (CSPs) that directly correlate to the catastrophic powertrain failures I have experienced. Timeline and Documentation of Failure: • August & September 2025: As
... previously reported, my vehicle suffered sudden "Reduced Power" malfunctions while in motion (P0734/P07F7), leading to a full transmission replacement. Discovery of Non-Disclosure: My records now confirm that on November 11, 2022—just weeks before I purchased this vehicle as a Ford Blue Advantage Gold Certified unit—CrossRoads Ford (Dunn, NC) documented that 21N03 (Cam Phasers), 21N08 (PCM Reprogram), and 21M06 (Tailgate) were outstanding. • Breach of Duty: Despite this knowledge, the dealership sold me the vehicle on December 3, 2022, without completing these safety-related service actions. Furthermore, as the registered owner, I never received the required notification from Ford Motor Company regarding the 21B10 program or its supplements. Safety Implication: The failure to apply the 21B10/21N08 updates directly contributed to the mechanical failure of the cam phasers and transmission. This resulted in the vehicle entering "Limp Mode" during highway operation, creating a severe risk of rear-end collisions due to the sudden loss of motive power. I request that these facts be added to my existing case files. The lack of manufacturer oversight regarding "Certified" vehicle standards has resulted in a vehicle that was represented as safe and fully maintained while actually harboring known, unaddressed mechanical defects.INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The Original Equipment (OE) Bridgestone Alenza Sport A/S 255/50R20 105H M+S tires on my brand new 2024 Nissan Pathfinder suffer from groove wander on roads in which grooves are cut into them to mitigate hydroplaning in rainy conditions. ... ere is a mismatch between the tire design and the vehicle's weight or suspension design - Nissan should not be placing this OE tire on new Pathfinders. Groove wander causes the vehicle to follow road grooves involuntarily, which is a handling defect that compromise driver control. This condition is a safety hazard, especially at highway and interstate speeds or in inclement weather. Groove wander does not allow the vehicle to operate safely, causing the driver to constantly correct the involuntary path of the vehicle on such roads. The driver's and others' safety are at risk if the driver is unable to quickly correct the path of the vehicle and keep the vehicle in the lane of traffic to avoid lane departure into another vehicle or object. The problem has not been reproduced or confirmed by both the Nissan dealer from I bought the vehicle and a local Bridgestone/Firestone service center because both are unwilling to do so. Both have refused to accompany me on a test drive on Interstate 90 where this occurs because this road is approximately 12 miles one way from their locations (their claim it is too far away; if it was only 5 miles, they would be willing to do this). The tires have been visually inspected by both companies, and naturally both have found nothing wrong with the tires (normal wear at this time). Visually inspecting he tires will not diagnose the issue, and a test ride needs to be completed to document this issue - again both Nissan and Bridgestone refuse to do this. Other brand tires on other vehicles I own do not suffer groove wander.Read more

My Jeep Wrangler 4xe underwent Safety Recall 68C, which included high-voltage battery replacement and associated electrical system work. The vehicle was held at the Jeep dealership for approximately one month for completion of this recall. After being notified that Recall 68C was completed, I picked up the vehicle. Immediately upon pickup, I experienced electrical issues that were not present pri
... or to the recall. Specifically, the passenger-side power running board failed to retract. The service manager accessed the area under the dashboard and restored operation, stating it was likely related to the battery replacement or electrical work performed during the recall. Upon driving the vehicle, I also noticed the steering wheel was not centered when driving straight, and the front end appeared visually misaligned. Less than 24 hours after pickup, while driving at approximately 65 mph on the freeway, the vehicle experienced sudden and violent steering oscillation consistent with death wobble. This required me to immediately pull over for safety. The vehicle had no prior history of death wobble, steering instability, or electrical running board issues before Recall 68C was performed. Due to the severity of the condition, the vehicle was towed back to the Jeep dealership for further inspection.Read more

NHTSA COMPLAINT - 2026 BMW X3 WINDSHIELD DEFECT Our brand new 2026 BMW X3 has experienced two complete windshield failures within six weeks of purchase. Both windshields spontaneously cracked from the perimeter bonding edge with NO rock strikes or impacts: ∙First failure (Week 3): Crack from driver-side A-pillar edge ∙Second failure (Week 6): Crack from top horizontal edge near rearview mirror
... Critical Pattern: Both cracks originated from different points along the windshield-to-body bonding periphery, indicating a systemic defect in the entire bonding system, not isolated damage. Safety Risk: This violates FMVSS 212 windshield retention requirements. Risks include: windshield detachment during crashes, airbag deployment failure, sudden visibility loss while driving, and water intrusion causing electrical fires. Key Evidence: BMW has already recalled FOUR related 2026 models for identical windshield seal defects: ∙Campaign 25V556: X5/X7 (1,406 vehicles) - improperly sealed windshields ∙Campaign 25V671: X4/X6 - same defect The X3 shares the same platform, manufacturing facility, and production timeframe but was excluded from recalls. Statistical Impossibility: Two spontaneous windshield failures in six weeks with zero impacts on a new vehicle cannot be coincidence. Normal defect rates are <0.1% annually. Dealer Confirmation: BMW dealer confirmed and replaced first windshield. Second failure proves systematic defect. Request: Investigate whether 2026 X3 has the same defect as recalled X4/X5/X6/X7 models and expand recalls accordingly. On April 7, 2025, the dealership attempted to sell me a vehicle that was experiencing an engine condition running too lean. I declined that vehicle due to safety concerns. On April 8, 2025, the salesperson contacted me again and stated that a different vehicle had been located that would meet my needs. At that time, I was nine months pregnant and urgently needed a vehicle with third-row seating f
... or my children. The dealership was aware of my condition and urgency. I took delivery of the vehicle that day. Unexpectedly, I went into labor the following day and gave birth shortly after. Because of childbirth and medical recovery, I did not immediately operate the vehicle extensively or discover the safety issues. After being discharged from the hospital, around April 11, 2025, I began using the vehicle and discovered serious mechanical problems that made the vehicle unsafe to operate. The vehicle exhibited engine-related issues and a major suspension defect that affected vehicle stability and safe operation. The vehicle was returned to the dealership, where repairs were performed and parts were ordered, confirming that the defects existed prior to or at the time of delivery. Despite the vehicle being driven a significant distance immediately before sale, the dealership did not perform a basic oil change or standard post-delivery inspection. Operating a vehicle under these conditions raises concerns for engine reliability, overheating, and failure. The suspension defect created risk of loss of control, and the engine condition created risk of drivability issues. These conditions posed a safety risk to the driver and passengers, including a newborn child. The vehicle remains available for inspection if requested.Read more

I am submitting this complaint to report a potential safety defect with the windshield glass on my 2025 Subaru Forester. Since purchasing the vehicle in May 2025, I have experienced repeated windshield damage that seems excessive and abnormal for a new vehicle. Within the first two months of ownership, the windshield developed a rock chip. Approximately three months later, I experienced another r
... ock chip that spread into a larger crack when I attempted to have it professionally repaired. Then, approximately two months after that, the windshield developed a large crack that is not repairable and requires full replacement. In total, my windshield has failed three times in less than one year of ownership under normal driving conditions. I have never experienced this frequency or severity of windshield damage with the cars I have owned in the past. This pattern suggests that the windshield glass may be unusually fragile or improperly designed for normal road conditions. The windshield is a critical safety component that supports the vehicle’s structural integrity and ensures proper airbag deployment. Repeated cracking raises serious concerns about occupant safety, visibility while driving, and the vehicle’s crashworthiness. I believe this may indicate a manufacturing or design defect in the windshield glass used in the 2025 Subaru Forester and warrants investigation before more drivers are affected.Read more

The forward collision avoidance system in this generation of Hyundai tucsons is dangerously flawed. We previously had a 2020 and no issues, but this one slams the brakes at the drop of a hat and has nearly caused several accidents as there is no good reason for the intervention (ample space to leading vehicle) and it brakes so hard that cars behind typically need to veer to avoid rear end collisi
... on. This is not a one-off event or linked to particular driver/driving style - both adult owners of this vehicle have experienced this on multiple unrelated occasions. All sensitivity setting have been set to minimum and the only way to drive with peace of mind is to completely disable the system - but here is where the next massive design flaw is: the system automatically reenables at each re-ignition; there is not permanent override, which is EXTREMELY dangerous. It is not practical to manually disable the system on each drive because you need to navigate multiple system menus to adjust the setting. Google and user forums confirm this collision avoidance system is a widely experienced and complained about safety issue for Hyundai and it should be investigated. People will likely get killed due to this “safety feature”. Ideally Hyundai should push a recalibration for those who want this feature AND enable a permanent override option in the system settings for those who don’t.Read more

Replaced the frozen suspension last year. Now that it’s cold again, similar codes are being thrown with new ones: Service Trailer Brake System, Service Air Suspension System, Service Electronic Braking System. To note, I’ve had a “Check Fuel Cap” code for years. It’s tied to 3 sensors along the fuel tank and absolutely nothing to do with the fuel cap itself. For the air suspension sensor
... s (WHICH I’VE ALREADY REPLACED ONCE), and the fuel cap sensors— they all run nearly $300 a pop and that isn’t including labor. I personally cannot financially keep up with the vehicle’s maintenance costs. The fact that Canada had a class action lawsuit against FCA for the air suspension but we don’t is baffling. I pray nothing comes from these defective parts because I will be suing.Read more

Incident ~ 5:40 pm, Geneva NY Weather ~ 10 F, not snowing. Background. On or shortly after being started, the vehicle had exhibited prior incidents of extremely rapid engine acceleration not initiated by the operator. Most recently the day before. The incident: I was in the process of very slowly parking the vehicle in a narrow laneway, and positioning the vehicle behind and in line of one other
... , both parallel to the laneway, in a small area that had been cleared of snow, leaving the laneway open. First, I stopped the vehicle opposite of the parked vehicle's driver side door. Second, I reversed and backed up and positioned the vehicle behind that vehicle. This involved firmly stepping on the brake to stop the vehicle once well behind the parked vehicle. Third, while keeping my foot firmly pressed on the brake pedal, I moved the shift from reverse to forward. At that time, the front wheels were still fully turned to the left. Immediately on completing the shift to forward, my vehicle made a very rapid acceleration. My foot had remained firmly on the brake and had not touched the accelerator pedal. Despite my foot being firmly pressed on the brake pedal, my car surged forward, retracing the path that I had followed while backing up. I could hear the noise of the accelerated engine, and feel the fighting of the brake as the car surged uncontrolled for about four feet and then finally stopped. My foot had been fully depressed as hard as possible on the brake the entire time and did not touch the gas pedal at all. The owner of the parked vehicle observed the incident and expressed concern since the surge was dramatic, and stopped with the right corner of the front bumper of my car within a couple of inches of the left corner of the parked vehicle. The driver's car mat was in the correct position and did not interfere in any way with the operation of the vehicle. I did not inadvertently step on the accelerator. No warning lamps or messages. Cause Unknown.Read more

My engine makes a loud rattling noise upon start up and then a more discreet rattling while driving because of the cam phaser.

My 2016 Kia Optima experienced a sudden catastrophic engine failure in July 2025. The failure is consistent with widely reported Theta II engine rod bearing defects that cause engine seizure and stalling. The vehicle did not receive the Knock Sensor Detection System (KSDS) update prior to my ownership, and I was never notified of this critical safety-related update. The engine failure occurred w
... ithout warning and could have resulted in loss of power while driving, creating a serious safety risk. This vehicle model year has a documented history of engine defects and class action settlements related to premature engine failure. I believe this represents an ongoing safety defect. I am requesting that NHTSA review this failure as part of its investigation into Kia Optima engine defects.Read more

While driving I'm having difficulting staying in the center of my lane because my steering wheel is not allowing minor steering adjustments without sticking in place temporarily. I need to apply more pressure than expected to turn the wheel before it unsticks. My local Honda dealer recently replicated this behavior on a test drive and is recommending I replace my EPS gearbox, outlined in a ser
... vice bulletin issued by Honda related to this symptom (Service Bulletin/Manufacturer communication: 23-037). Honda has agreed to pay 50% of the repair since I am out of warranty - my mileage on my vehicle is a little over 41K. I am asking Honda to pay for the full repair given the related EPS saftey recall they have issued, but they have not agreed to do this yet. There were no warning lamps or messages prior to experiencing these steering problems.Read more

After driving in snow the rear lift gate opens unexpectedly when walking behind the vehicle. This happened while the car was parked in my garage and I walked behind the car causing the rear hatch to open pinning me between the vehicle and the garage door. I was able to push it closed, howevere, if this happened to my wife or my grandson they could have been injured because they would not have the
... strength to push it. This happened again after our snow last week causing it to open while the garage door was open. The tailgate crashed into the garage door damaging the underside of the spoiler. I was told after the initial incident when I braught the vehicle in for service in December that Nissan was aware of this problem but there was no solution.Read more
