The vehicle experienced repeated electrical wiring harness damage in the lighting, tire and connector areas due to exposed and insufficiently protected wiring near socket terminations. On two separate service visits, authorized dealership technicians confirmed that rodents chewed through multiple exposed wires. The affected wiring involves critical electrical systems. The connectors lack adequate
... shielding, loom, or protective insulation at exposed termination points. The insulation ends before the connector, leaving conductors vulnerable to environmental exposure and rodent access. Warning indicators illuminated on the dashboard, and the vehicle required dealer repair on both occasions. The damaged wiring was inspected and confirmed by certified technicians and is available for inspection upon request. This condition presents a significant safety risk. If a rodent partially damages a wire connected to braking systems, steering systems, high-voltage components, sensors, or engine control systems, failure could occur while the vehicle is in motion. A sudden loss of power, interruption of propulsion, or malfunction of safety systems at highway speeds could increase the risk of a crash. Because this is an electric vehicle, sudden electrical interruption may result in complete loss of propulsion or charging capability. If such failure occurs in remote areas, at night, or in extreme weather conditions, occupants could be left stranded in potentially hazardous situations. This issue has occurred more than once, and no permanent preventive solution has been provided. The design appears to leave critical wiring insufficiently protected from foreseeable environmental and rodent exposure, creating an ongoing safety hazard. I request review of whether the wiring design provides adequate protection for safety-related electrical systems.Read more

The vehicle was leaking coolant. The Mazda dealership Technician concluded it was a cracked radiator. There was no warning sign. Took vehicle in for an oil change and that's when the problem was discovered. Mazda completed service with replacing a new radiator on 2/12/26. On 2/15/26 the coolant is burning in/on engine. Component/System: Low-pressure fuel pump/fuel delivery system related to Recall 25S75. Vehicle is a 2022 Ford Explorer Hybrid. Description: My vehicle is subject to Recall 25S75 involving the low-pressure fuel pump that can cause loss of fuel pressure and engine stalling. The vehicle has stalled six times between about 15,000 and 35,000 miles. The issue is intermittent but ongoing. The vehicle is
... available for inspection. What failed or malfunctioned: The fuel delivery system appears to lose pressure consistent with Recall 25S75, leading to engine stalling and temporary inability to restart. Safety risk: One stall occurred while driving on a highway on 9/30/23 at about 15,000 miles. The engine lost power without warning and I had to coast to a gas station. Sudden loss of propulsion in traffic creates a serious crash risk. Other stalls happened in driveways or parking lots after the vehicle had been running. In several cases the vehicle would not restart for 20–60 minutes, which could be dangerous if it occurred in traffic or an unsafe location. Reproduction or dealer confirmation: Two incidents required towing to Ford dealerships. The vehicle has been taken to Ford dealers multiple times. Earlier incidents were not linked to a recall at the time. Now that Recall 25S75 has been issued, the symptoms match. Ford has not repaired it because the remedy is still under development and parts are unavailable. Inspections: The vehicle has been inspected by Ford dealerships and towed twice by AAA for diagnosis. No permanent fix has been made because the recall remedy is not yet available. Warnings or symptoms: Stalls often occurred after the vehicle had been running. The vehicle then would not restart for 20–60 minutes. In one event there were dashboard errors. There was not always a clear warning before a stall. Reason for complaint: The recall has been open since July 2025 with no remedy available. The vehicle continues to stall. I do not feel it is safe to operate.Read more

On January 28th 2026, I got red warning signs on dashboard said: "Stop vehicle and check power supply" and "12V battery voltage low. Stop safely". I was still in the parking lot, so the issue did not pose any safety issue. The vehicle towed to Hyundai dealership. They confirmed and pre-approved "Replace ICCU +fuse". The part is on 1 month back order. While the dealership initially did not commit

While driving at 45 MPH with my wife in the front passenger seat, on a two lane road (one way each direction) in Wake Forest NC, no cars were in the front or rear of my vehicle , sunny clear weather, the following happened: Driving normally, then all of a sudden, the car proceeds to, what felt like emergency braking and then a split second, the dashboard signals come on , different symbols, then
... a red image appears on the dashboard (for a split second) then it brakes hard a second time (within a split second) then everything clears and the car proceeds to drive normally. The braking action felt like a severe brake pulsing, then a split second release and then another spilt second brake pulsing and then everything cleared and the car continued to drive normally. The braking was so severe, my wife and I lurched forward towards the windshield, held back by our seat belts. The braking occurred suddenly with nothing near the vehicle. The emergency braking came in the first pulse, then followed by a extremely brief release(split second) then another quick emergency braking action(split second), then multiple lights and warnings quickly appeared on the dash board ending with a split second red symbol (so brief i could not determine any of the warnings or lights). Then the car drove normally after that. If any vehicle had been behind us, a rear accident collision could have taken place. This happened on Saturday, February 14th at approximately 2:30pm on Forestville Road, in Wake Forest, NC. This was an extremely dangerous situation. As this is now a Sunday, the Nissan dealer will be contacted tomorrow and arrangements will be made to bring the vehicle in for an inspection ASAP. While driving this vehicle under normal conditions, the engine developed abnormal ticking/knocking noises, loss of power, and misfire symptoms that progressively worsened. The vehicle was frequently used to transport children. Due to the nature of the failure and unpredictability of progression, continued operation posed a serious safety risk. An authorized dealer later confirmed that the engine
... suffered a catastrophic internal valvetrain failure involving collapsed lifters/rocker arms and severely damaged camshafts. The dealer technician explicitly stated this failure mode is “typical” for this engine design and documented progressive internal damage affecting both cylinder banks, not an isolated component. This failure mode is not sudden; it develops gradually over tens of thousands of miles and can lead to sudden engine power loss, stalling, or seizure without warning. A loss of propulsion at highway speed, during lane changes, intersections, or while transporting children creates a clear risk of crash, injury, or worse. There was no evidence of driver abuse, lack of maintenance, lubrication failure, or misuse. The dealer confirmed oil condition and service history were appropriate. The defect appears to be internal, latent, and manufacturing-related, consistent with numerous consumer complaints, technical service bulletins, and ongoing litigation involving this engine family. Had the vehicle continued to be driven, the failure could have escalated to: •Sudden loss of engine power at speed •Inability to accelerate to avoid hazards •Engine seizure causing loss of steering assist •Increased crash risk with child passengers onboard The vehicle was removed from service due to safety concerns. This defect is repeatable, predictable, and well-documented, yet not subject to recall at this time. I am reporting this to support investigation into a systemic engine defect that presents a serious safety risk to drivers and passengers.Read more

The rear window of this car has separated and when I contacted the manufacturer they indicated a service bulletin was issued but they are not liable for the repair. The window has become more and more separated and I'm using glue to hold it on but it's becoming undrivable due to the risk of it separating while on the road. While this has not yet caused an accident, I believe this is one waiting

While driving, the vehicle loses power, hesitates, and jerks, then stalls and shuts off without warning, especially in extreme heat and stop-and-go traffic. The failure occurs more frequently when the fuel level drops below approximately half a tank. After stalling, the vehicle will not restart until it cools down for 20–30 minutes, then temporarily operates normally before the issue repeats.
... Warning symptoms prior to failure include loss of acceleration, hesitation, and engine overheating. A fuel pump–related fault code has been detected. The vehicle has stalled while driving, creating a serious safety risk due to sudden loss of power in traffic. This condition matches NHTSA Recall 23V858000, which states the fuel pump may fail and cause engine stall while driving. The fuel system component believed to be involved is available for inspection upon request. The issue has occurred multiple times and is reproducible under similar conditions.Read more

I am reporting a failure of the remedy for Recall S55 (NHTSA 16V-529). The manufacturer’s authorized repair, a software-update only, failed to correct the underlying hardware defect in the transaxle jumper harness. As a result, the wires in the harness physically melted due to the high electrical resistance as stated in the S55 recall, which caused a total loss of motive power while driving and
... rendering the vehicle immobile. This created a significant crash risk as we were suddenly left completely immobile and stranded in the middle of busy traffic, not to mention a fire hazard. Prior to the total failure, the vehicle exhibited electrical symptoms including sporadic starting, locks locking and unlocking by itself, improper revolution, and harsh transmission shifting. In addition to the melted wires in the transaxle jumper harness, there is now oil pooling under the vehicle, signaling further critical damage caused from the recalled part's failure. In the aftermath we are now assessing our federal rights to repair from the manufacturer. The dealership and manufacturer claim the recall is 'completed' because of the initial software update, but the physical defect remains and has worsened into a hazardous state. I am requesting that NHTSA investigate the adequacy of the S55 software-only remedy, as it does not prevent the hardware from failing, leaving countless in a dangerous situation. Any help or advice you could provide on how to proceed would be greatly appreciated. Thank you so muchRead more

I am submitting this complaint regarding a significant safety-related failure in my 2024 Acura ZDX that occurred immediately after a dealer-installed software update. I was subsequently informed that the update caused a complete failure of the unit and that ful
... l replacement of the infotainment module is required. As a result of this failure, the following systems are nonfunctional: Hands-free Bluetooth (unable to make or receive calls legally and safely while driving) Audio/radio system Navigation system Climate control interface (controls integrated into the display) Phone charging capability The loss of hands-free Bluetooth functionality presents a direct safety concern, as drivers are required in many states to use hands-free systems while operating a vehicle. Additionally, the inability to access navigation and certain vehicle settings increases driver distraction. The climate control integration within the infotainment display further compounds safety concerns. These systems were fully operational prior to the dealership performing the software update. A routine manufacturer-issued update should not result in total system failure requiring hardware replacement. I am concerned that this issue may represent a broader defect affecting other vehicles equipped with the same system. I request that this incident be reviewed for potential safety implications and possible investigation.Read more

Genesis towed my car to a dealer in Mountain View CA. I did notice that I was leaking radiator fluid in the days prior to my incidents but when I checked the overflow tank, it was full. Again, there were no check engine lights on the dash. The Mountain View Genesis service manager replicated the issue and discovered that a small rock had come through the bottom grill and damaged the intercooler radiator. He unequivocally stated that "it's a design flaw in the lower grill. The grill has such a wide opening that foreign objects can enter through the lower grill." He went on to say that the intercooler radiator sits below the regular radiator and is made of a softer metal than the regular radiator which makes is more susceptible to damage. He also said that another Genesis GV70 came into his shop the same day with the exact same issue. The cost of the repair was $2,200 dollars to replace the intercooler radiator. (the intercooler was on backorder which may be demand) I opened a case with Genesis and they say that the repair is not covered under a warranty. Safety Complaint – Sudden Oil Loss and Engine Failure – 2014 Chevy Equinox Hello, I am reporting a serious safety issue involving my 2014 Chevrolet Equinox with the 2.4L Ecotec engine. My vehicle began consuming oil at an abnormal rate without any dashboard warnings in early 2025. The oil level would drop dangerously low between regular oil changes, and the problem escalated quickly. I had
... to carry oil and a funnel at all times to avoid engine damage. Despite proper maintenance, the engine failed and required replacement on March 27, 2025, at a cost of $11,879.28. The replacement engine includes a 3‑year or 100,000‑mile warranty, but it is the same Ecotec design. Approximately one month after the replacement engine was installed, I experienced another incident requiring service. This occurred despite the engine being brand new and under warranty, raising concerns that the defect may still be present and that the vehicle may remain at risk of sudden failure. This sudden oil loss and subsequent incident created a safety hazard, as the engine could have seized while driving. I am submitting this complaint so NHTSA can track the defect and evaluate whether further investigation or recall expansion is needed.Read more

The component that failed appears to be the DC-DC converter and/or related charging system within the vehicle’s electrical system, which resulted in complete failure of the 12-volt battery system. The vehicle displayed a dashboard warning stating: “Electrical fault detected. Please find a safe place to stop.” Shortly after this warning appeared, the vehicle lost electrical stability and th
... e 12-volt battery completely discharged. The battery would not hold a charge even after attempted recharging. The vehicle became inoperable and required towing to an authorized Land Rover dealership. The vehicle is currently in the possession of an authorized Land Rover dealer and is available for inspection upon request. It has not yet been repaired. Safety Risk This failure created a significant safety risk because: The warning appeared while the vehicle was being driven. The electrical system is critical to power steering, braking assist, stability systems, lighting, and other safety components. A sudden loss of electrical power while driving at highway speeds or in traffic could result in loss of critical vehicle control systems. The vehicle became fully inoperable after the fault, creating the potential for stranding in unsafe conditions. If this failure had occurred at higher speed or in heavy traffic, it could have placed the driver, passengers, and surrounding motorists at risk. Reproduction / Dealer Confirmation The vehicle is currently at an authorized Land Rover dealership. The dealer has indicated that they have a significant number of other Land Rover and Range Rover vehicles in queue with similar DC-DC converter / electrical system failures. However: The vehicle has not yet been formally diagnosed. It has not yet entered the service bay. There is no estimated time for inspection or repair due to part shortages. The dealer has acknowledged that this appears to be a known and recurring issue. Inspection by Manufacturer or Others The vehicle has been inRead more

The bolt related to this recall inevitably broke on 11/23/25 at 43,842 miles. Ford of Upland took my truck and provided me with a loaner vehicle for about a month and a half, until demanding that I return the loaner vehicle. I had no choice but to take my truck back as I do not have a second vehicle, and they didn’t offer me a solution. At the time of writing this, I have put ~800 miles on the t
... ruck since the bolt sheared off. It is now making a terrible noise from the rear end, which I have expressed to Ford of Upland, Colley Ford, and Ford Customer Service.. with no resolve. Ford Customer Service told me over the phone that they are “hoping” to have the parts to fix my truck in Q3 of this year (2026). I purchased this truck as it fit my everyday needs. I often tow which made this truck a good candidate. As I understand, the truck is not currently safe to drive, and definitely not capable of towing. Not having a capable truck is unacceptable. It seems that Ford is incapable of providing a reasonable solution. The truck has been “out of service” since 11/23/25, nearly 3 months at the time of writing this.Read more

Two of the same faults occurred within the same month of each other. The first: while driving home from work on the highway, while heavily raining, without warning every alarm light possible engaged, windshield wipers went full speed, vehicle would not shift, and my steering locked up (not to be confused with lost power steering). After about a mile I was able to encourage the Jeep off the side
... of the highway. While waiting for my tow, with the E Brake set and in Park, the Jeep kept trying to crawl forward on its own. I was unable to turn the vehicle off as it would not respond, in addition to an alarm warning saying not to turn off the Jeep. The second occurrence was in the parking garage at work; I pulled through the parking stall, put the Jeep in reverse to back up and the same fault took place. No steering, wipers on full speed, every alarm on the dash was engaged. Thankfully I was not a highway speed. I was able to turn off the Jeep this time after about 5 minutes of attempting to.Read more
