The adjustable rails holding the rear left seat in place no longer latch. It is available for inspection. This is the most common place for a car seat in this car. In the event of a crash the seat would slide forward. The seat belt attaches to the seat so there would be little restraint on the rear passenger. The mechanic who attempted a repair stated Mazda does not sell this part, only sells a re

While driving my vehicle on Monday, February 16, 2026 I heard a loud pop come from the front of the vehicle. Right after that smoke began to come through the hood vents. I stopped the vehicle to inspect and at that point the engine compartment was burning heavily. I could barely get out of the vehicle and away before the whole front end of the vehicle was engulfed with fire and heavy black smoke.
... It looked as if the fire started somewhere on the righthand side of the engine compartment because the right front tire was the first to blow out in the fire. The vehicle is with the insurance company to inspect, but it was in perfect running condition, all maintenance performed. There were no dash warning lights that came on, no odd sounds heard and the vehicle ran until I stopped and shut it office. There has to be something that was wrong with the manufacturing of this specific engine to make a loud pop and burn as hot as it did. I wanted to report this before the vehicle is sent away to be demolished due to it being a complete loss.Read more

After taking in to dealer for recall diagnostics and higher viscosity oil change a couple months later. Here is what dealer documented: CUSTOMER STATES CHECK ENGINE LIGHT IS ON WITH SERVICE TRACTION & SERVICE ESC MESSAGES. ENGINE IS RUNNING ROUGH AND WILL STALL. TOW IN. Valve Spring Replacement - Replacing the faulty valve spring on the cylinder 7 exhaust side is necessary as it is causing the exhaust valve to remain stuckopen, leading to engine misfire and over 80% leakage in that cylinder. This service can help restore proper engine compression and improve engine performance. Op.1 CEL -CEL: Customer states 1. replaced the exhaust valve spring and valve spring seal. reinstalled components with new seals and cleared dtcs. started engine and verified the engine starts and runs but having multiple cylinder misfires. reset engine fuel trims and brought engine to operating temperature and let it run for some time. other cylinder misfires went away but still found a dead misfire on cylinder 7. removed and swapped spark plugs with no change. noted the spark plugs have some exhaust build up on the tip due to the faulty exhaust valve spring. performed a static compression test and found the compression to be 175 psi and a running compression test with a smooth build up to 125 psi. checked spark and spark was good. tested the injector system using the AFIT test and found the test to pass indicating no faults with the fuel injectors. performed a cylinder leak down test and found no cylinder leakage coming from cylinder 7. put engine back together and cleared dtcs. reset the throttleRead more

The driver-side power sliding door latch, motor and cable system on my 2019 Honda Odyssey Elite failed. Honda performed Recall A88 (sliding door handle cable repair) in 2021. After this repair the door began intermittently failing to close fully and would close about 80% before the sensor triggered it to reopen. This continued until this week when the door suddenly stopped latching completely. The
... automatic door buttons no longer work and the door can only be moved manually. I believe my vehicle should also be included in recalls Z33 and O36 addressing sliding door latch failures affecting similar 2019 Odyssey vehicles. The failed components are available for inspection upon request. This defect creates a serious passenger safety hazard. Because the door will not latch, it opens while driving. The automatic function does not work and the door can be moved manually but will not latch. This creates risk of passenger ejection, especially children, and the possibility of the door opening during a crash. The vehicle is unsafe to operate. Yes. The failure was reproduced and confirmed by a Honda dealership that inspected and diagnosed the sliding door failure. The vehicle was inspected by a Honda dealership and I was charged a diagnostic fee. Honda refused recall or goodwill coverage because my VIN is not included. I request the repair be performed free under recalls Z33 and O36 and the diagnostic fee reimbursed. I am waiting for NHTSA inspection and request Honda be required to include my vehicle and repair it at no cost. Before total failure the door would close about 80% and automatically reopen. When it stopped working this week, the dashboard displayed a “Left Sliding Door Problem” warning and the vehicle produced continuous beeping while driving.Read more

Engine oil is disappearing in my 2020 Kia Telluride. This has been occurring for an extended period of time ( Since 2020) Early on, I was not driving frequently and did not immediately recognize the severity of the issue. However, about a year ago, when my work required significantly more driving, I began noticing that a large amount of oil was missing between checks, at times approximately half
... of the engine’s oil capacity. I initially suspected the plastic oil filter housing, which can crack or leak if it is over torqued during oil changes. To address that potential issue, I purchased an aluminum oil filter housing and had it installed. While that repair resolved one concern, it did not stop the ongoing oil loss. At times, when I check the dipstick, there is little to no oil registering, indicating the engine is operating on residual oil. When I remove the oil filler cap, smoke is visible coming from the opening. There are no signs of external leakage, no oil on the ground, and no oil leaking under the engine. This strongly suggests the engine is consuming oil internally. This is a serious safety concern. If the oil level drops unexpectedly while driving, the engine can suffer sudden damage, lose power, stall, or fail without warning. Because I regularly travel for work between Florida and Georgia, including highway driving, I now carry bottles of oil in the vehicle and feel compelled to monitor and top off oil frequently out of concern that the engine could fail or stall while driving, potentially leading to a crash, injury, or loss of life. Based on my research, I understand I am not the only Kia Telluride owner experiencing this type of abnormal oil consumption or unexplained oil loss.Read more

Make: Toyota Model: Corolla (likely hatchback/sedan) Year: 2026 VIN: [XXX] Mileage at Incident: 80 Date of Purchase: February 17, 2026 Date of Incident: February 19, 2026 Description of Defect: While moving slowly in reverse on dry pavement (no throttle, no sharp steering, no brake misuse), the vehicle suddenly spun out of control—yaw/skid like rear wheels lost grip or stability control malfunc
... tioned and steering wheel locked. No prior warning. The spin caused a collision with another vehicle and nearly resulted in serious injury/death to the driver/owner (myself, [XXX] ). Suspected Causes (based on symptoms and similar reports): • Possible ESC (electronic stability control) or yaw sensor failure. • ABS/traction control pulsing/locking wheels unexpectedly in reverse. • Electronic power steering (EPS) glitch—wheel felt “fighting” or unresponsive during the event. • No fault codes pulled by dealer yet, but similar low-speed reverse spin-outs reported in 2025-2026 Corollas (e.g., NHTSA complaints on “ABS ghosting” or “rear-end slide”). Additional Info: • Dealer (Toyota of Hollywood, 6920 W Sunset Blvd, Los Angeles, CA) has the vehicle awaiting inspector. • Demand letter sent/hand-delivered today (February 21, 2026) under Song-Beverly Act—safety defect, one occurrence. • No insurance claim filed—this is manufacturer liability, not driver error. • Photos of skid marks/collision available if requested. What I Want NHTSA to Do: Investigate for potential recall—safety risk is high (uncontrolled spin in reverse at low speed). This is the second such incident in recent Corollas I’ve seen reported; could affect others. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

70-80k mileage the oil filter housing gushed coolant fluid. Replacement at dealership quoted at $4,000. Dealership indicated they replace part with current part and do not upgrade to an all aluminum to prevent future problems. Employee also stated they are seeing more of these issues of late. The housing has plastic components that fail easily. There was a 2020 bulletin warning of similar failures
... for this type of engine and vehicle but BMW has not issued a recall. If this draining of coolant is not detected early enough, the engine could overheat and fail, creating a traffic hazard and safety concern to passengers. Local certified mechanics quoted $2,500 for replacement using an aluminum part. These two quotes were from different mechanics and were based on phone conversation, so additional charges may have applied once they began work.they both opined if part is not replaced with aluminum, the part will fail again prematurely. My 2019 Ford F-150 (2.7L EcoBoost) is subject to Emission Recall 24E12, which addresses a defective PCM calibration that Ford acknowledges "could, over time, reduce the effectiveness of the emissions control system and the monitoring accuracy of the catalytic converter's efficiency." COMPONENT: Catalytic converter system and powertrain control module (PCM). Vehicle is available for inspection at R
... ich Ford in Albuquerque, NM, where it has been serviced for this issue. TIMELINE: Prior to the 24E12 recall repair, my truck had intermittent P0420/P0430 codes (catalyst efficiency below threshold). The check engine light would illuminate and then clear on its own. I brought the vehicle to Rich Ford for the 24E12 PCM reprogram. Within approximately one month of the recall repair, the same catalytic converter codes returned, except now the condition is constant and no longer clears. This timeline is documented in my FordPass app and the dealer's service records. SAFETY RISK: The constant check engine light prevents the vehicle from passing Bernalillo County emissions testing, which is required for registration in Albuquerque, NM. I am unable to legally register my vehicle due to a condition caused by a manufacturer-acknowledged defect. DEALER CONFIRMATION: Rich Ford diagnosed the recurring P0420/P0430 codes. They initially attempted to charge me $300+ for extended diagnostics. They are now consulting with Ford about whether a catalytic converter replacement will be covered under the recall. PRIOR SYMPTOMS: The intermittent check engine light with P0420/P0430 codes existed before the recall repair but was not constant. The recall repair worsened the condition from intermittent to persistent. PATTERN: Ford's recall documentation states the MIL "may illuminate even though a catalytic converter repair may not be necessary," yet numerous owners report being quoted $1,900-$2,100+ for cat replacement at their own expense after the 24E12 reprogram. Ford corporate confirmed the $300Read more

I am reporting a potential safety defect involving the panoramic sunroof of my 2026 Kia Sportage, purchased new in August 2025. On February 19, 2026, at approximately 4:45 PM, while driving around 65 mph, the panoramic sunroof suddenly shattered without warning. The failure was preceded by a loud, explosive sound, after which the tempered glass fractured and collapsed inward into the passenger co
... mpartment above the driver’s seat. There was no observable external impact. I did not see any debris or object strike the vehicle, and the glass had no prior damage. Weather conditions were mild (approximately 75°F), and the vehicle was operating under normal highway conditions. Glass fragments fell into the cabin, including onto the driver’s seat and interior surfaces. The sudden noise and glass intrusion created a safety hazard, including risk of injury and driver distraction while traveling at highway speed. The vehicle was taken to a Kia dealership the same day. I was later informed by the dealership in San Angelo, Texas, that the damage would not be covered under warranty and that I would be responsible for the repair. Given the lack of observable impact and the inward failure of the glass, I am submitting this report for evaluation as a potential safety-related defect. The incident raises concerns due to the sudden failure at highway speed, inward breakage into the occupant compartment, and occurrence in a vehicle less than one year old. I can provide photos or documentation upon request. Submitted by: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am reporting a recurring transmission/drivability issue with my 2024 Mercedes-Benz S580. The vehicle has been experiencing a harsh, abnormal shift between approximately 3rd and 4th gear at around 25–35 mph during normal acceleration. The shift feels like a sudden jerk or hesitation and does not feel consistent with normal operation. The vehicle has been taken to an authorized Mercedes-Benz de

The dealership later confirmed in writing that the harsh shift was duplicated and documented on the repair order. Despite prior service visits, the problem continues to occur intermittently. It has happened again after the most recent service. In addition to the shifting concern, the vehicle has previously experienced power-related issues and steering/driver-assist warnings. There are not always warning lights before the harsh shift occurs. The vehicle remains available for inspection. After 8 months of driving my car brand new I started experiencing multiple intermittent malfunctions in the dash board & infotainment system. I have documentation of DTC codes current in my ECU. These electrical malfunctions were at one point affecting my alternator which I also have documentation of. The car has lost power yet the engine stays running while I am driving. My handsfree is intermittent everyday. I am now way past my warranty & extended warranty due to following dealerships advice to drive the car as normal and bring back. These ongoing malfunctions create unreasonable safety risk which I believe falls under 49 U.S.C. Chapter 301 — the National Traffic and Motor Vehicle Safety Act. A third party diagnostics shop told me my car would have to be taken apart to find the grounding issues as this is clearly a manufacturing defect. These repairs have cost me money and I refuse to keep paying these dealerships if they aren't willing to resolve the core problem. I get constant push back from each dealership. I've already engaged with Nissan consumer affairs, a Lemon Law attorney, and filed with my State AG, AG recommended while undergoing an IDR that I file a complaint here as well. Please help.Read more

The manufacturer (warranty service provider) informed me of this safety recall on February 10, 2026 and advised me to get it repaired soon. I called the dealership where I purchased the new vehicle to set up an appointment for repair: Jim Browne Chrysler Jeep Dodge Ram in Dade City, FL. However, their service department was not available, even though there was approximately 15 minutes remaining for their scheduled hours. The representative told me they consistently leave early. I called back when the service department was finally available and was told they are unable to repair the issue related to the safety recall. I notified the manufacturer of the problem with Jim Browne Chrysler Jeep Dodge Ram. The representative was unable to help me and escalated the issue to someone else. I was finally contacted this week by a different representative from the manufacturer, but he has not been able to help either. I was advised to contact your organization and then an attorney if your organization cannot help in this matter. I am appalled by the service I’ve received regarding my Ram truck situation and that the dealership and manufacturer do not care about safety issues. I’ve never had problems like this with my other automobiles.Read more
