The Variable Geometry Turbocharger Actuator failed on my 2019 2500HD Silverado. There was a significant decrease in power. This was most noticeable on uphill grades where speed would drop to between 35 and 40 mph on a 75mph speed limit highway. On several occasions, vehicles came close to hitting me because of my slow speed.

I was driving along in my 2013 MB GL450 with the M278 engine when the engine stopped. I had it towed to my home and checked it out. I had a suspicion the engine was seized. I eventually had to towed to my closest MB dealership for evaluation. The engine was seized. Their further investigation revealed that a wrong oil filter had been installed when the oil was changed 5 months and 3000 miles
... earlier. I tried to find out what this wrong filter would cause. I asked the dealership and then MB and got no response. I suspect if this filter caused the issue it would have been an oil pressure issue. The car gave no indication of anything wrong prior to engine seizing. I got no message on the display and when switching on the switch there is no indicator on the dashboard to indicate anything about oil pressure. There is no "oil can" that pops up red indicating an oil pressure issue. To this day have no idea why the engine seized other than an improper oil filter.Read more

The moonroof spontaneously shattered when I was driving on the freeway which sounded like a gunshot. There were no external factors such as debris from other vehicles and the temperature outside was relevantly constant at about 78F. No other cars were close to me or in front of me. For almost the entire 2 years I had my car, nothing has ever damaged the moonroof glass. I do want to note an observa
... tion that may or may not be linked to the potential safety defect: I do recall hearing strange pebble-like noises occasionally on the freeway & off the freeway at speeds of ~35-65 mph. It sounds like crumbs falling down the sides/top of the car in a horizontal pattern. I observed this as early as 1 month into ownership. Upon closer inspection after stopping to inspect my vehicle, the glass shows to have shattered/blew up & outward in cube shaped pieces and the pattern of the glass appears spiderweb-like, not originating from an impact point. The fracture pattern of the remaining glass appears to match a smooth edge breakage pattern which indicates stress failure due to a spontaneous glass breakage. The dealer has denied coverage due to the technician 'finding no evidence of structural damage.' Yet, my moonroof shattered upwards and there is not evidence of an impact crater. My car is still under warranty and under 2 years old and currently is only at about 20,500 miles. The dealership quoted me ~ $2500+$185 diagnostic. I will be elevating to Toyota corporate to request a closer inspection. Based on research, I discovered numerous reports of similar spontaneous sunroof failures in Toyota vehicles. The failure presents a high risk for public safety because the glass could fall into the eyes and face of the driver at highway speed. Additionally, drivers could be startled & swerve on the road at high & low speeds, on the freeway/streets. There were no warning signs/alerts. One moment I was driving to work on the quiet road & the next I heard what sounded like a gunshot.Read more

My 2015 Ford Transit 350 equipped with the 3.2L diesel engine experienced a failure of the high-pressure fuel injection pump at approximately 72,000 miles. The pump generated metallic debris which contaminated the entire fuel system. The vehicle began experiencing drivability problems including loss of power and intermittent stalling. The vehicle was diagnosed by a Ford dealership, where metal co
... ntamination was found in the fuel system. The repair required replacement of the high-pressure fuel pump, injectors, high-pressure fuel lines, and fuel rail, along with flushing and cleaning of the fuel tank and fuel system. The total repair cost was approximately $11,500. This failure appears to match the defect described in Ford safety recall 16S32 for certain 2015–2016 Ford Transit vehicles equipped with the same 3.2L diesel engine, where scuffing in the fuel injection pump can create metallic debris that contaminates the fuel system. My vehicle was built July 16, 2015, essentially within the same production period described in the recall, but my VIN was not included in the recall population. Because this failure required replacement of most of the fuel system components and created a sudden loss of power risk while driving, I am reporting this issue so it can be investigated and considered for inclusion with similar failures.Read more

I bought this 2022 Jeep Renegade in 2023. Upon changing the oil today (3/13/2026) my mechanic found a rear main seal leak. Is this covered for repair under a recall? Also the engine stalls when I try to take off, jerking

The engine oil light came on during late September or early October of 2024 while on the highway. There was a severe loss of power. I was able to get the car back to my residence and changed the oil filter and oil. The error persisted, I then changed the oil pressure switch to see if that was the issue, but it did not resolve. In approximately December of 2024, a friend who is a mechanic and has t
... housands of hours of experience offered to look at the issue. He pulled the engine and found that the oil pump belt and timing belt were severely degraded and had clogged the oil screen. He replaced the two belts and cleaned the screen hoping it would resolve the issue. In approximately late may or early June 2025 I got the car back and by the beginning of August the engine had seized due to damage received when the oil pressure had failed due to a clogged screen back in 2024. Thankfully I was not on the highway when the engine seized.Read more

I purchased a 2025 Subaru Forester Hybrid VIN [XXX] on July 26, 2025 from Schumacher Subaru, 3031 Okeechobee Blvd. West Palm Beach, Florida 33409. On March 11, 2026 I filled the gas tank to 100% capacity. The ambient temperature was 72 degrees Fahrenheit. I parked the vehicle in our garage with the door closed. Two hours later my wife notified me that there was a strong smell of gasoline throug
... hout our four bedroom home. I opened the door to the gas filler recess and verified that the gas cap was correctly closed. Gas was spilled into and over the recess area below the gas cap, down the right side of the right back quarter panel and in a large puddle on the garage floor. I backed the Subaru out of the garage and onto the front driveway. Using multiple sheets of paper towels we mopped up the spilled gasoline from the vehicle and garage floor. I placed all the soiled paper towels in a sealed double garage bag and put it in our garden away from the house. I called Schumacher Subaru and talked with Dylan Wagner who sold us the car. I was shocked to hear that he knew of the gas cap defect and also knew that we should not fill the tank more than 50%. I asked him why he didn't notify us and he said that Subaru was going to notify us at the end on March 2026. In my view this is clear cut negligence on the part of Schumacher Subaru of West Palm Beach, Florida. While driving home from Costco with my entire family in the vehicle (4 children and my pregnant wife), I noticed the engine misfiring while traveling on Interstate 80 and mentioned to my wife that something felt off with the vehicle. After exiting I-80 onto Route 46 East at the Denville exit, the truck reached the first traffic light where the vehicle rumbled violently, shook, and completely shut
... off. The engine would not restart. Dashboard lights illuminated but the engine would not turn over, and the hazard lights would not activate. This created a dangerous roadside situation with traffic approaching from behind at approximately 50 MPH. I exited the vehicle and stood in the roadway directing traffic to prevent an accident while my wife attempted to restart the vehicle and called 911. Police arrived and a tow truck was dispatched. The tow truck driver attempted to jump start the vehicle, but it would not start. The vehicle had to be winched onto a flatbed tow truck.Read more

At approximately 1750 miles the truck transmission began to shift in lower gears extremely violently. Every time that truck would shift in this manner, it would cause a startle effect and divert my attention away from driving to trying to determine what was going on. I immediately took the truck to my local Ram repair shop where they found metal shavings in the transmission oil pan. They told me I
... would need a completely new transmission. This is unacceptable with a truck with only 1750 miles on it. The dealership said they would need authorization to replace the transmission and not sure if one would be readily available. This is the fourth time this vehicle will have been at this repair facility. The first three times were for a fault code called Turbo under boost. Way too much downtime for a truck with only 1750 miles. We had to postpone our vacation since this is the only vehicle I own that’s capable of towing our boat and trailer. Because of all these issues, we have not been able to tow the trailer one time with our new truck!!!!!! When I asked the dealership what they thought the problem was they said quality control at the factory and a subpar designed for the product. If Ram doesn’t change the design of this transmission, even with a new transmission installed, I am scared this may happen again and then cause an accident.Read more

FEDERAL REPORT ON TECHNICAL OBSTRUCTION AND WARRANTY FRAUD (THETA II SETTLEMENT) TO: National Highway Traffic Safety Administration (NHTSA) FROM: [XXX] VIN: [XXX] REPAIR ORDER (RO#): XXX VEHICLE LOCATION: Hyundai Shop - [XXX] HYUNDAI CORPORATE CASE: #XXX STATE COMPLAINT (AZ AG): XXX I. CHRONOLOGY OF EVENTS AND EVASIVE TACTICS All documentary evidence has been formally submitted to Hyundai Motor
... America’s National Consumer Affairs Department, addressed to Case Manager Bryan, at [XXX] and via the official email: [XXX] . STEP 1: Falsification of Title Status (Code A99) Hyundai initiated its denial by alleging that the vehicle held a "Salvage" title status, citing internal code A99. EVIDENCE: I possess a Certified and Stamped Motor Vehicle Record (MVR) from the Arizona MVD, as well as my Clean Title. Mr. Bryan categorically asserted that code A99 "cannot be removed," thereby deliberately disregarding an Official State Certification. STEP 2: Allegation of Negligence (Karen) Corporate representative Karen claimed via telephone that the vehicle's failure was attributable to "negligent maintenance." EVIDENCE: The Official WebDCS report—printed by Mr. Ismael Barragán (Service Drive Manager)—completely refutes this claim; the Blue Link service history indicates "0 FINDINGS." STEP 3: Fabricated Diagnosis and Breakdown of Improper Charges After being compelled by Mr. Bryan to take the vehicle to the dealership for a diagnosis, the narrative shifted to an "Oil Pump Failure." Mr. Ismael Barragán (Service Drive Manager) presented a billing estimate to the customer under Repair Order (RO) #XXX, itemized as follows: Mechanical Diagnosis: $193.00 USD Engine Replacement/Rebuild: $11,980.58 USD TOTAL DEMANDED FROM CONSUMER: $12,173.58 USD TECHNICAL EMPHASIS: The Blue Link report (showing 0 Findings) serves as irrefutable INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I was told that continued driving could worsen the condition, leading to overheating and further engine damage. After that diagnosis, the vehicle’s drivability worsened, and it became unsafe to rely on due to misfires and loss of acceleration, especially on the highway. The selling dealership refused to assist, claiming the defect was not present at the time of sale; Honda America acknowledged the head gasket concern but denied a goodwill claim, citing the high cost of the issue and lack of NHTSA enforcement on this issue. An independent repair shop then provided a second inspection/estimate consistent with the same head-gasket repair scope. The vehicle and repair records are available for inspection. No crash or injury occurred, but the defect caused a sudden loss of power and overheating while driving; I was stuck on the highway on two occasions with my hazard lights on after acceleration completely halted. Warning symptoms (the engine light) only appeared when the car suddenly stopped accelerating. The radio head unit randomly reboots. This occurs usually when the car is initially started, and you are unable to utilize the backup camera or the camera freezes. Rear cross traffic breaking also fails and can cause an accident. I have also had issues with the powertrain, where the vehicle refuses to start, with a red lobster error message saying Service Electrical System.

2013 BMW 328i, N20 engine. Timing chain guide assembly and oil pump drive chain failed catastrophically. Vehicle is available for inspection, however the authorized BMW dealership has already made clear they will accept no responsibility, and I would bear the expense of towing it two hours to their facility. The failure is a known issue with a class action case, but BMW took no responsibility, and
... falsified information stating the vehicle showed no signs of an issue when I brought my vehicle in for inspection. Eighteen months before failure, steam came from the engine compartment on a humid day, later connected to the water pump electrical connector recall. Following this, the vehicle ran consistently hotter than normal and lost noticeable power. Twelve months before failure, an intermittent oil pressure warning began triggering under load at highway speeds, particularly on inclines above 70mph. I brought the vehicle to Sandia BMW in Albuquerque, NM and reported all symptoms when I was contacted by them for the recall. The service advisor falsely claimed elevated operating temperature was optimal, suspected a faulty sensor, and returned the vehicle without offering further diagnosis or making me aware of the impending issue, all classic signs of the engine faults. Eight weeks later, the oil pressure light activated on a highway, followed immediately by a drivetrain malfunction warning, a violent grinding sound, and complete loss of power. The vehicle was towed and has not restarted since. I contacted BMW of North America's recall line. A case was opened and an advocate stated BMW would accept no responsibility because the class action was closed and I'mpast 70K miles. I raised the dealership's direct culpability. The advocate stated a dealership representative would contact me. No call was ever received. This failure is consistent with BMW Service Bulletin SI B11 03 17. A engine safety recall is warranted regardless of age to protect the public.Read more

I have 2020 Chevy 1500 with L87 engines on it. We have the exact same issues that the 2021 1500 recall is having. Loses power, noisy engine when at start. Paradise Chevrolet in Temecula told me that they were going to replace the engine per GM but then GM only auth the repair. 8 barring kits, 8 rings 8 piston kits lot of bolts and gastkit and seals. Talked to GM every tho it’s the same engine th

I failed from the dealer. Had to get the truck towed to the dealer cause the car wouldn’t turn on and all this happened with only 68,833 miles.Read more

Vehicle had "front passenger safety restraint system fault" multiple times intermittently. Serviced by Tesla service center in Dublin, CA who confirmed the problem. Documentation on the invoice stated that there was an open service bulletin related to this issue and was related to "the occupancy sensory in the seat was not functioning as expected." Upon inspection by the technician they found "an
... open connection in the seat harness and seat cover that can cause intermittent alerts." Additional repair notes indicate that this was to "address a service bulletin related to the front passenger seat's occupant classification system. We identified the need for an update to ensure proper functionality. To resolve this, we retrofitted the system in the front passenger seat as per the bulletin requirements." The fact that there is an open service bulletin for this issue indicates that it is not isolated to just this vehicle. An issue with the occupant sensors of a front vehicle seat directly impacts whether the air bags will deploy in a crash, therefore this is a serious safety risk.Read more

Dear Transportation Safety Board: On January 12, 2026, at noon, I was involved in a traffic accident while using Tesla's Full Self-Driving (FSD) function on the [XXX] between [XXX] and [XXX] . I am the complainant's wife. The accident was caused by a malfunction of FSD, which has had a tremendous impact on my family and my husband. He suffered a head injury, followed by intracranial hemorrhage a
... nd hematoma, rendering him unable to work. The accident has resulted in frequent cramps in his hands and feet, dizziness, headaches, and drug allergies due to the need for long-term medication. I need your help. Tesla does not state in its app that the product cannot fully drive itself; instead, it advertises that its FSD can drive independently in various road conditions without human intervention. This has misled the driving habits of many Tesla owners and caused immense harm to me and my family. I earnestly request your assistance in helping this helpless woman resolve my husband's problem. Thank you very much. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I do believe the part that is failing on the 2022 Jeep Wrangler Unlimited is the “Steering Dampner” and anyone at anytime is MORE than welcome to look at and inspect touched or feel it. This is SAFETY RISK has the entire vehicle starts to shake and wobble uncontrollably and without warning. If not for quick reflexes we would have been in a chain reaction crash. Was able to release gas and c
... oast to under 50mph making lots of people behind me unhappy. The dealer said he repaired dampner when we had 4,000 miles, now again at 16,700 miles it’s back and we almost ended upside down in a river. Either Jeep or NHTSA must need people to die before they can make truly effective repairs or just remove this model form inventory. I believe it has to be more than just a dampner and the front suspension parts should all be looked at. There was ABSOLUTELY no warnings, lights, sounds, or indication that we were almost plunged to our death. Angels had to be riding with us. Let’s get this fixed!! PLEASE 🙏🏻Read more

I made a turn onto a highway. I got up to 45 mph after my turn and my car stalled. It felt like a was reared ended, my car came to just about a complete stop. It then began to shake and would not accelerate past 5mph to get me over to the side of the road. The check engine light came on. On 2/28/2026 my engine came on, I took it to Kia and they replaced my spark plugs, I got my vehicle back on 3/1
... 0/2026. At the beginning on 2026, Kia had my car for over a month for the same engine issues but said nothing is wrong with the vehicle. Back in December 2024 I had this exact issue of the car jolting and stalling. Kia said it was the transmission. I paid to have that replaced but it seems the vehicle is still having all of the same issues. My life was put at risk driving this 2021 Kia Seltos. I am very lucky that the cars around me were paying attention and did not crash into my car that came to a complete stop with vehicles going 65+ around me. Others lives could have been at risk had they crashed into my car. My car is once again back at Kia, with the engine light on again.Read more
