The original B58 engine (B58B30M0, 2015–2017) utilized a conventional mechanically robust gerotor oil pump with predominantly metallic internal construction and demonstrated strong reliability in field operation. Beginning with the B58 Technical Update introduced for 2018 production (B58B30O1), BMW redesigned the lubrication system by integrating the engine vacuum pump and oil pump into a combi
... ned variable-displacement assembly. This redesign introduced a thermoplastic internal adjusting ring (carrier) responsible for regulating oil pump displacement through rotor eccentricity control. This component, constructed from reinforced thermoplastics including PA66-GF materials, operates within a chemically aggressive and thermally cyclic environment involving continuous exposure to engine oil additives, fuel dilution, condensation, and repeated heat cycling. While acceptable under accelerated validation testing, real-world operating conditions increasingly demonstrate premature degradation consistent with polymer embrittlement and fracture. Observed Failure Mode Documented owner and technician reports indicate that under cold-start conditions — particularly in sub-freezing climates — elevated oil viscosity and startup load impose increased mechanical stress on the thermoplastic carrier. Fracture of this internal component allows debris intrusion into the pump mechanism, resulting in unstable oil pressure regulation. Commonly reported precursor symptoms include: • Inability to perform electronic oil level measurement via iDrive • Erratic or oscillating oil pressure behavior • Intermittent lubrication warnings or absence of warning prior to failure • Rapid progression to bearing damage and engine seizure In many cases, drivers receive insufficient warning before catastrophic lubrication loss occurs. Vehicles may become disabled without notice, including during highway operation or winter driving conditions, leaving occupants stranded in potentially dangerous environRead more

My 2025 Audi RS3 unexpectedly shifts into park and engages the electronic parking brake while the vehicle is in motion at low speeds. This issue has occurred multiple times, has been documented on at least nine occasions, and has undergone four (4) separate service attempts within the first 900 miles. The issue remains unresolved. On multiple occasions, the vehicle became immobilized in active t
... raffic. In one incident, the vehicle shifted into park while making a left-hand turn across a busy intersection, leaving the vehicle stopped in the roadway with oncoming traffic approaching at high speeds. The vehicle would not move and required the driver to exit and perform a full restart before normal operation resumed. In another incident, the vehicle again shifted into park while exiting a parking lot and entering traffic, leaving the vehicle partially in the roadway and creating a hazardous condition for the driver and oncoming traffic. A fault related to the electronic parking brake system was identified and a component was replaced; however, the issue persisted after repair. The dealership has confirmed that the issue can be reproduced but stated that the vehicle is operating as designed. The explanation provided was that shifting body weight while seated and properly restrained may cause the system to behave as if no driver is present. The dealership also indicated that similar behavior could be reproduced on other vehicles during testing in a parking lot environment, rather than under normal driving conditions. Despite this explanation, the behavior results in an unexpected loss of vehicle mobility in traffic, which presents a serious safety risk. There were no consistent warning indicators prior to the events. The vehicle remains available for inspection.Read more

My 2018 Ford F‑150 XLT has the 10R80 automatic transmission with severe issues including harsh shifting, delayed engagement, slipping between gears, and unpredictable shift timing. Symptoms occur during normal driving and create a serious safety hazard. Ford dealer replaced transmission with a remanufactured 10R80, part number JL3Z‑7000‑ERM, and is listed in Ford’s Equipment Safety Recall
... 25SB3 (dated 10/28/2025) for missing internal bearings and unsafe shifting behavior. When the Ford dealer replaced the transmission on 02/26/2025, they recorded mileage as 70,009 miles, 9k miles lower than the actual mileage of 79,009. This discrepancy creates a false service record and misrepresents when the failure occurred. It also affects warranty determinations, recall eligibility, and the accuracy of the vehicle’s service history. The mileage history shows a consistent progression documented by two independent auto shops (see attached files). This mileage discrepancy impacts warranty and goodwill eligibility, recall-related determinations, accuracy of Ford’s internal service history and Carfax/AutoCheck reporting. Despite the replacement, the transmission continues to exhibit the same dangerous symptoms. As a disabled veteran, I rely heavily on safe, predictable vehicle operation. Delayed engagement and slipping created an elevated risk when operating this vehicle. The dealer informed me that Ford has not provided a fix and that they cannot take further action. I have stopped operating the vehicle due to the severity of the transmission defect and the safety risks it creates. I request that Ford correct this mileage entry and ensure the digital service record reflects the accurate mileage. I also request NHTSA investigate the transmission and mileage matters, as the vehicle is unsafe to operate and Ford has not provided a remedy. Thank you in advance.Read more

After the recall work was performed, the vehicle began consuming engine oil at an abnormal and excessive rate between oil changes. Most concerning, during a recent check, the engine oil level was found to be extremely low, with no oil registering on the dipstick. This condition presents a significant risk of engine damage or failure and raises serious safety concerns. Prior to the recall service,
... the vehicle did not exhibit any abnormal oil consumption. The issue has only occurred after the recall was completed, which strongly suggests a direct connection to the repair.Read more

The CVT transmission failed on my 2018 Toyota C-HR, which I purchased in 2022. The vehicle stalled in the middle of traffic on a day with elevated temperatures while my special needs child was a passenger, creating an immediately life-threatening situation. The car showed no signs of any issue prior to this failure. I attempted to have the vehicle inspected multiple times before the failure occurr
... ed, but this defect is not detectable without a deep diagnostic inspection and by the time that level of inspection is needed, the transmission is already in the process of failing. There are no warning lights or symptoms until complete failure occurs. When I contacted Toyota, the representative acted as though this was an isolated issue and not a known defect. This is false. This is a viral, widespread problem that the vast majority of 2018 Toyota C-HR owners are experiencing and actively reporting across forums, consumer complaint sites, and to NHTSA itself. Toyota is fully aware of this defect. Upon seeking an independent inspection, the mechanic showed me the same transmission from another vehicle of the same make and compared it to mine, explaining that the C-HR uses a larger CVT transmission, making a used replacement approximately $6,000 alone and advised that given the repair cost relative to the car’s value, purchasing a new vehicle may be the only realistic option. I am now completely without transportation due to what is clearly a premature manufacturing defect on a 7-year-old vehicle, causing severe financial hardship. Notably, Toyota Canada has already issued a recall and extended the warranty to 10 years/100,000 miles for this exact same defect on the same model. U.S. owners are being denied the same protection and quoted $10,000–$14,000 for a known manufacturing defect. I am requesting NHTSA investigate and issue a U.S. recall for all affected 2018 Toyota C-HR vehicles immediately.Read more

I am the original owner of a 2023 Jeep Wrangler Sport with approximately 16,240 miles. The vehicle is completely unmodified. I am reporting a serious safety defect involving the steering and suspension system. When driving at highway speeds (50 mph and above), the vehicle experiences a violent front‑end oscillation whenever it encounters a bump, uneven pavement, or bridge expansion joints. The s
... teering wheel rapidly shakes left to right, and the entire vehicle shudders for 5 to 10 seconds. The oscillation does not stop until I slow the vehicle down to below 45 mph. This issue is reproducible and occurs frequently on rough or uneven road surfaces at highway speeds. During these events, I temporarily lose the ability to maintain proper steering control, creating a dangerous situation for myself, my passengers, and other drivers. This condition is commonly referred to as “Death Wobble,” and it makes the vehicle unsafe to operate. Given the severity of this defect and the safety risk it poses, I am requesting that NHTSA investigate this issue and that Jeep/Stellantis address the defect under the New Vehicle Limited Warranty (3‑year/36,000‑mile basic warranty). The front‑end suspension components—including the track bar, steering damper, ball joints, and related parts—should be inspected and repaired as necessary to eliminate this hazardous oscillation.Read more

We were on the way home from a soccer tournament in [XXX] on rt. XXX and 4 hours into our trip the car's fuel pedal stopped responding and our speed started descending rapidly. The engine light was on with the vehicles stability system, Hill assistant, emission system, power system, TSA..... All of them were lit. Scared I put hazards on and pulled over on the side of the road. I waited about 20

It was quite terrifying and looking back I should have called 911, this was a first time ever I have been in a situation like that with my daughter. We got home at night. In the morning I called AAA to see if the battery was bad, they said no. The dashboard was cleared, no lights appeared and when I went to the Honda dealer they couldn't a identify the specific problem. They told me it could be a few things and told me to do repairs for $5000.00 not knowing if it would fix the problem. This first incident happened in summer 2025. I stopped driving the car on a highway and long distances, the dashboard lit up few more times but always went back to normal. A couple of weeks ago, when I again called AAA to check my battery, I was advised to go to AutoZone when the board lits up again. So I did today and was told it was code P0087 and had to do with the fuel pressure pump. So I went to my mechanic to diagnose the problem and they are in the process of repairing it. But after some research I found online that there has been a recall on Honda Pilots 2016-2018, but my car didn't qualify. So I called Honda manufacturing explaining my situation and warning them that the same thing happened to me and they said there was nothing they could do if my car's VIN # wasn't included. Following a 25-day repair at a certified collision center due to an earlier accident, the vehicle's Full Self-Driving (FSD) and Active Safety Systems began failing. The vehicle repeatedly failed to keep speed and failed to stay centered in the lane. On March 25, 2026, while traveling at low speed on a public roadway with FSD actively engaged, the system failed to maintain a stable path and incorr
... ectly steered the vehicle into a curb, causing property damage to the wheel. This failure put my safety and the safety of surrounding traffic at risk by executing an unpredictable steering maneuver that the active safety systems failed to prevent. The vehicle was taken to the manufacturer's service center twice with this specific complaint prior to the curb strike. The service center investigated and officially concluded that this was not a software issue, stating that they could do nothing else. They deferred the resolution back to the collision shop, citing likely camera/sensor physical misalignment from the 25-day body repair. The collision shop is now refusing to review the vehicle or inspect the hardware, claiming lack of visible exterior damage despite the internal service team stating this is a latent hardware alignment defect. The vehicle is currently in the same failing condition and available for inspection upon request. No warning lamps or error messages appeared on the screen prior to the failure or the curb strike. The symptom of unpredictable lane tracking first appeared immediately upon receiving the vehicle back from the collision repair earlier this year. Note: I have a formal data privacy request pending with Tesla to pull the exact chat logs showing their refusal to service this hardware defect. I will provide these to the investigator upon follow-up.Read more

Office of Defects Investigation National Highway Traffic Safety Administration 1200 New Jersey Avenue SE Washington, DC 20590 Re: Safety Defect Complaint — Ram ProMaster (2018–2025) — Cowl Water Intrusion into Engine Compartment To Whom It May Concern, I am writing to formally report what I believe to be a serious, systemic, and unresolved design defect in the Ram ProMaster van, affecting
... model years 2018 through at least 2025. I am requesting that the Office of Defects Investigation open or expand a formal investigation into this matter. Nature of the Defect: The windshield cowl on the Ram ProMaster is designed in a manner that permits significant water intrusion directly into the engine compartment. Water enters through the cowl seam and surrounding gaps, bypassing any drainage system, and flows onto critical engine and electrical components including but not limited to: the serpentine belt and pulley system, the engine fuse box, transmission components, and wiring harnesses. I brought my vehicle to two separate authorized Ram dealerships. The second dealer confirmed — based on testing conducted on multiple ProMaster vans dating back to 2018 — that this water intrusion behavior is consistent across the model line. In other words, the dealer has effectively confirmed that this is not a defect isolated to my vehicle, but a design characteristic of the ProMaster platform itself. This confirmation does not reassure me. It alarms me. A design that has permitted consistent water intrusion onto safety-critical engine and electrical components across a major commercial vehicle platform for over a decade is not an acceptable engineering standard. Given the size and weight of these vehicles — many operated commercially and carrying passengers or heavy cargo — the consequences of power steering loss, electrical failure, or mechanical failure are severe. We have tried dealing with Stellantis as well. Any help would be greatly appreciated. [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIRead more

Incident Overview: In late February 2026, at approximately 114,000 miles, this 2022 Rivian R1T suffered a comprehensive failure of its kinetic (hydraulic) suspension system. The failure occurred during normal on-road operation at approximately 60 mph in temperate (60°F) conditions. The vehicle has no history of off-road use or heavy hauling. Safety Risk & Critical Failure Mode: The suspension is
... a safety-critical system essential for vehicle stability, emergency braking, and predictable handling. A loss of hydraulic integrity in this system directly compromises the vehicle’s ability to maintain ride height and damping control. High-Speed Exposure: The initial failure manifested while traveling at 60 mph. Although a warning lamp was triggered, the vehicle’s functional safety logic failed to initiate a "limp mode" or restricted speed state. This allowed the 7,000lb vehicle to continue at highway speeds with a compromised high-pressure hydraulic circuit, posing an unreasonable risk of loss of control. Lack of System Redundancy: A localized hydraulic leak resulted in a cascading collapse of the entire architecture. Per the $10,662 service estimate, the repair requires replacing the front and rear dampers, hydraulic pump, manifold, and all fluid lines. This suggests a design-level vulnerability where a single-point failure in the hydraulic circuit necessitates a total system overhaul, indicating a lack of modularity or redundancy. Safety Protocol Gap: After diagnosing the leak, the authorized service center deferred to my personal judgment regarding operational safety. This forced an untrained consumer to make a critical safety determination on a high-pressure system while the vehicle remained capable of full highway speeds. Technical Confirmation: An authorized Rivian service center confirmed the failure on March 3, 2026, and determined a full system-wide replacement is required. All components are available for inspection. No prior warnings or symptoms were presenRead more
