Hello, I am reporting a safety issue involving my Ford F250 vehicle related to Safety Recall 25S75 (low-pressure fuel pump failure). Approximately one month ago, while driving on the freeway at approximately 65 mph, my vehicle experienced a sudden loss of power. This created a dangerous situation as I was traveling at highway speed and the vehicle became unable to maintain normal operation. The

The front collision detection system malfunction directly compromises my ability to rely on the vehicle's built-in safety features to prevent or mitigate crashes. Additionally, the infotainment screen has frozen and reset while the vehicle was in motion. I have also been locked out of my vehicle twice while the engine was running, which created a dangerous situation. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. On July 25, 2025, a technician at Georgetown Infiniti attempted and failed to connect their own phone to the CarPlay system, confirming the defect was in the vehicle and not my personal device. The collision system malfunction has been documented across multiple service visits at both Georgetown Infiniti and Grubbs Infiniti. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes. The vehicle has been inspected and serviced by authorized Infiniti dealerships, specifically Grubbs Infiniti and Georgetown Infiniti, both of which are manufacturer-authorized service centers. Most recently addressed 2/4/20226. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. I purchased the Hyundai NEXO with the expectation that it would provide reliable transportation supported by Hyundai’s service network and the hydrogen fueling infrastructure promoted alongside the vehicle. As a consumer, I was attracted to the NEXO because of its innovative fuel cell technology, environmental benefits, and Hyundai’s commitment to zero-emission vehicles. However, my ownership experience has been extremely difficult and concerning due to both vehicle performance defects and the lack of support required to maintain and operate the vehicle safely. One of the most concerning issues with the vehicle is that the regenerative braking system does not appear to function properly. Regenerative braking is an important component of the NEXO’s efficiency and safety systems. When it does not operate correctly, the vehicle does not slow down in the expected way, and the energy recovery system that is supposed to recharge the vehicle’s systems during deceleration does not function as intended. This raises concerns about both vehicle performance and braking predictability. Based on research, the degradation of light emittance takes about 5-6 years to reach a dangerous visibility issue. Owners are not interested in spending $1,600 every ~6 years just to have minimum visibility while driving. This is dangerous. This is serious. This is consistent across all 2013-2015 RDX models. Sadly, my father's 1999 Ford F150 with 250K miles has original headlights which shine about 10 times brighter than this "current day" Acura. I attached a .pdf with comments from a certified Acura technician who also owns one of these vehicles. I own (2) RDX vehicles that my daughters both drive. I am considering not allowing them to drive at night in the rain because there is literally ZERO visibility. Component: Engine/Powertrain/Electrical System Incident Description: I am reporting a recurring, life-threatening "Loss of Motive Power" (Dead Pedal) in a 2024 Hyundai Tucson (Alabama-built). On multiple occasions, the vehicle has failed to respond to throttle input during high-risk maneuvers, specifically while merging into 45+ MPH traffic. The vehicle "chugs" and stalls, refusing to exceed 15 MP

Technical Context: This vehicle is currently subject to NHTSA Recall 23V-526 (Hyundai Campaign 246) for electronic controller damage that can cause "CAN communication disruptions." I believe the "Dead Pedal" failure is a direct result of these communication disruptions between the engine and transmission control modules. Furthermore, Hyundai has issued TSB #24-AT-001H-1 for "Hesitation when accelerating," yet the authorized dealer (Ed Voyles Hyundai) has been unable to remedy the defect after 84 cumulative days out of service. Safety Risk: This is not a convenience issue; it is a critical safety failure. The unpredictable nature of the power loss makes it impossible to safely judge gaps in traffic. The vehicle effectively becomes a stationary object on active roadways without warning. Manufacturer Response: The manufacturer has been formally notified via certified legal counsel. Despite 79 consecutive days in the current repair attempt (since Jan 16, 2026), the manufacturer has failed to provide a fix or acknowledge the severity of the safety risk. An informal third-party evaluator attempted to dismiss the claim despite the vehicle's documented inability to maintain motive power. The lights are hit and miss - sometimes they won't come when pressed or sometimes won't go off when they are pressed to go off. The problems according to the dealer cannot be reproduced - even though I have videos of the issues. When there is an error code on the vehicle and I have taken it in they say that there are no codes - which is a sing to me that the car is malfunctioning as they say the codes should be there. The vehicle for two days this year would make the information call repeatedly and did this roughly 100 times in two days. It finally stopped the day I went to take it in to the dealer. The dealer replaced the radio after my first week due to infotainment and backup camera issues. The dealer replaced it with a recalled part and it is recorded in my Repair Orders (ROs). They said it was the last radio in the entire US and world and they couldn't replace it again. I didn't find out that it was recalled until October 2025 as I was researching the recalls and reviewing my ROs and noticed the same part listed on my ROs. I have taken the vehicle in multiple times and they have not replaced or provided a viable solution that fixes the multiple issues. I paid for the vehicle and extended warranty it was $45k. After a Hyundai dealership installed the Knock Sensor Detection System (KSDS) update to prevent engine bearing failure and potential fires in certain Theta II, Nu, and Gamma engines, my Elantra GT is stuck in limp mode and will no longer accelerate. Hyundai acknowledged that the engine does qualify for the Theta II extended warranty but will not explore that possibility until I have them replace the cat for $3,300, which we both know isn't going to solve anything. I have contacted Hyundai Corporate Customer Relations, and even the BBB, who refuses to qualify my car for their Engine Replacement class action lawsuit because the engine hasn't seized up yet, which will never happen since the update is preventing it. Even lemon law lawyers refuse to assist me because the car is too old. Since this is the only car I have, I am a nervous reck whenever I have to use it. The vehicle has not yet been inspected for this issue by a dealership, manufacturer, or third party. No police or insurance reports have been filed. I plan to take it to the dealership tomorrow (4/4/2026). There were no warning lights or messages before or during the event. Oil was being diluted by fuel seeping in from the injectors and the high pressure fuel pump. Vehicle was brought in twice to fix the problem. During the 24-27 Feb 26 repair the high pressure fuel pump was diagnosed and replaced. Current symptoms include: •Intermittent loss of communication between modules •Repeated warning messages, including “Service Shifter” •Fault codes that temporarily clear but return after driving •Unpredictable and inconsistent vehicle system behavior The condition sometimes temporarily improves but the faults and warnings consistently return. This creates a safety risk due to unreliable vehicle operation, including potential loss of shifting capability, starting issues, or failure of electronically controlled systems while driving. The failure is ongoing and intermittent but reproducible. The vehicle has not been involved in any accidents related to this issue. 2/24/2026 Sitting at red light, very busy road, vehicle cut off and said push to start. Vehicle would not start and would not shift out of park to push out of road. Very dangerous to be stranded in moving traffic. Had to be towed to dealer and it's still there waiting on engine replacement.

The transmision failed with 120,000 miles, Code P0776 "solenoid B Performance/stuck Off, vehicle goes into limp mode and it shifts really hard but never goes past 3rd gear, the recomended fixes by the shop are replace solenoid pack whith NO garantee that it would fix the issue for $3,000, or replace transmision with a rebuilt one for $9,000. The contact and her family were unable to exit through the front door due to the fire and used the sliding kitchen door to exit the residence. The fire department was called to the scene. Per the contact, three fire trucks were at the scene. The fire department informed the contact that the fire went up to 3,000 degrees. The fire department concluded that the fire started from underneath the driver's back panel. The contact stated that the gas tank was underneath that area and believed the gas tank may have contributed to the fire. The towing company towed the vehicle to a towing yard, and it was later picked up and towed to an unknown location. The insurance company was notified and declared the vehicle a total loss. The manufacturer was made aware of the failure and advised the contact that an inspection was needed on the vehicle and asked for the location of the vehicle. The harness is adjusted as tight as possible with the splitter plate pulled all the way to the cam lock. After monitoring the infant in the seat for a few moments, they shifted and slouched down into an unsafe position in the seat, compromising a safe airway and positioning for a newborn.
