I need to open a safety case for my vehicle. I am concerned with my vehicle. Car was having trouble starting on 4/16/2026 while parked at home giving a "full accessory power active" error. I called Ford and I was told it could be a battery issue. Triple A was called and help was dispatched. The Triple A representative did not jump start the battery and instead gave directions to power up the car
... back up with no electrical current necessary. Car started after trouble shooting it and it was tested through the night just like Triple A directed. The following day on 4/17/26 the car had no issue starting and all was normal. While testing the vehicle it started to produce a strong burning rubbery smell. I opened the hood and there was a leaking sound followed by a light smoke and leakage near the battery. A video was taken if needed. The smell was extremelly strong and I am reporting this safety concern as the VIN associated with the vehicle currently has an unrepaired recall with no available remedy yet by FORD - 25V467. The safety risk associated with this recall number is fire under hood due to possible injector crack that could lead to fuel accumulating on top of the engine. I want this issue recorded as I am concerned with the safety of my family.Read more

My Volvo's XC90 brakes have failed twice. At around 95,000 miles, my brakes stopped working, and then I had them repair it. They charged me close to $3,500, which they said they were able to cover under warranty, and I only had to pay the deductible, which I don't remember. In less than nine months, at about 104,000 miles, the same problem persists. Both times I was driving the car, by God's grac

Issue: Intermittent infotainment failure – MOST network connection lost I am reporting a recurring electrical failure involving the infotainment system in my 2019 Honda Odyssey. The system intermittently loses all audio and displays a “MOST network connection lost” message. At times, the screen also goes black and reboots. This issue has occurred repeatedly over time, especially after exte
... nded driving (e.g., long trips). The audio will stop working completely, and the system may or may not recover after restarting the vehicle. Multiple repair attempts have been made, including replacement rear entertainment system. Despite these repairs, the issue continues to occur. Based on the error message and symptoms, this appears to be a failure within the MOST communication network, potentially involving the amplifier or related components. This creates a significant distraction while driving, as the system becomes nonfunctional and unpredictable. I am unable to access hands free phone capability and navigation features. I am also unable to access the heating and cooling features in the car when the screen is black. This has been happening since about one week after I purchased the vehicle in 2021. I am concerned that this may represent a broader defect affecting multiple vehicles.Read more

I am writing to report a compliance issue regarding Hyundai Recall 292 (NHTSA 26V-034). My vehicle has been grounded due to the safety risks outlined in this recall (specifically the failure of side curtain airbags to meet ejection mitigation standards for third-row occupants). Despite the vehicle being rendered unusable for safe transport, Hyundai is refusing to provide or reimburse a rental veh
... icle. This is a direct contradiction to the Reimbursement Plan submitted by Hyundai to the NHTSA on March 2, 2026, which states that the manufacturer will provide owners of affected vehicles reimbursement for out-of-pocket expenses incurred to obtain a remedy for the recall condition. Since a final remedy is currently "Not Available," the only interim "remedy" to ensure occupant safety is to provide a rental vehicle. By grounding the vehicle but refusing to cover alternate transportation, Hyundai is effectively forcing owners to continue driving an unsafe vehicle or incur significant financial hardship, which undermines the entire purpose of the safety recall. I request that the NHTSA investigate Hyundai’s refusal to honor its reimbursement commitments for grounded vehicles under Recall 292 and ensure that all affected owners are provided with safe, manufacturer-covered transportation until a permanent fix is installed.Read more

My vehicle is a 2023 Volkswagen Atlas. It has a serious and recurring safety issue where the vehicle shuts off unexpectedly while driving. I have a car covered by the Theta II Settlement. Engine, fuel system. Metal shavings found in oil and oil is diluted with fuel and oil if overfilled 1 inch above full mark on dipstick upon pickup from dealership. The vehicle is available for inspection at my residence; however, it is currently grounded as a known safety hazard. The vehicle is exhibiting symptoms of imminent engine failure (audible
... knocking, P0011 code, and oil dilution). Despite being shown evidence of this(photo and video of oil overfill and watery viscosity) , the manufacturer instructed me to continue operating the vehicle, placing myself, and other drivers at risk of a high-speed collision or vehicle fire. The symptoms (P0011 code and engine noise) were confirmed by an independent mechanic on March 25th 2026. However, during the official Bearing Clearance Test (BCT), the dealership intentionally overfilled the engine oil by 3 quarts (documented via photograph) to muffle mechanical noise and artificially pass the diagnostic and issued a pass on all inspections despite the oil hazard. Hyundai Corporate Case Manager Nikulmar Dhabi has admitted on record that the dealer was out of compliance by "being outside the standard notification time" yet they refuse to provide a safety-compliant remedy. Warning Symptoms & Appearance: Symptoms first appeared months ago when I repeatedly got a P0011 code, however On march 24th 2026 I received an engine light with a P0011 and the car going into limp mode while i drove. Warning signs included a persistent Check Engine Light (P0011) and audible metallic knocking. Upon checking the oil myself after the dealer "inspected" it, I discovered the oil was 3 quarts over the "Full" line, which is a deceptive practice used to mask rod bearing failure.Read more

The Advanced Driver Assistance Systems (ADAS) on my Genesis GV60 called Highway Driving Assist (HDA) would randomly turn off without warning ever since I purchased the vehicle. ... Yesterday when I was driving in the carpool HOV lane in the Los Angeles area where some sections of the freeway have lane markers that are very close to the center divider and the car decided to randomly turn off ADAS without warning which resulted in a car swipe against the center divider and damaged the front bumper as well as a broken side rearview mirror. I did a random search online and found lots of complaints from other Genesis/Hyundai/Kia owners who have similar issues am surprised why nothing has been done about it so I decided to report this. In early November 2025, while towing our 2024 Grand Design Momentum 344M M-Class fifth wheel toy hauler to a family Thanksgiving gathering, the rearmost driver-side leaf spring fractured and failed during travel. The trailer was towed by a 2024 Ram 3500 dually, properly rated for this unit, operated within all load ratings, and driven at normal cautious speeds at the time of failure. The failure r
... endered the trailer immobile and unsafe to move. It has remained in storage since the incident because the damaged suspension cannot support safe relocation. A leaf spring is a primary suspension component that is not customer-serviceable and not owner-maintained. The remaining leaf springs on the unit were inspected and are in good condition, indicating the failure was isolated to the defective spring rather than a result of abnormal use, overloading, or environmental exposure affecting the suspension system as a whole. This points to a manufacturing defect in the failed component. The trailer was purchased new from an authorized dealer (Blue Compass RV) in October 2024. The spring failed only weeks after the one-year factory warranty expired, at an extremely low service age for a primary structural suspension component. Safety concern: This failure happened at low speed, allowing a controlled stop without incident. Had it occurred at highway speed, likely consequences include sudden loss of trailer stability, axle and tire damage from the collapsed corner, trailer sway or jackknife, tire blowout, and potential loss of control of the tow vehicle. A loaded fifth wheel toy hauler of this size presents a severe hazard to occupants and surrounding motorists if suspension fails at speed. I am filing this report so NHTSA can track potential defect trends on Grand Design Momentum fifth wheels. Other owners of this model and model year may be at similar risk. ... nted the failure, yet the same problem still occurred less than 30,000 miles later. The vehicle has low mileage, has been properly maintained, and should be available for inspection upon request, depending on whether the dealer or repair facility retains the parts. 2. This created a major safety risk. My teenager was driving the vehicle when the engine died, which put them in immediate danger. A sudden engine failure can cause loss of power, rough running, misfiring, or stalling in traffic, at intersections, or at highway speeds, leaving the driver unable to accelerate or move out of the way of other vehicles. This was not just an inconvenience; it created a very serious risk of a crash and potential injury to my child, passengers, and others on the road. 3. Yes. The problem was confirmed by the dealership service department. They diagnosed the failure as the 19B37 coolant intrusion issue and stated that the engine needs to be replaced. They also acknowledged that the prior CSP repair should have fixed this problem. 4. The vehicle has been inspected by the dealership/service center. Ford has also been contacted regarding the issue. The vehicle experienced a major engine failure involving the 6.2L engine. The failure appeared sudden and premature and is not consistent with normal wear and tear for a vehicle of this type. Prior to the failure, there were intermittent performance concerns and warning indicators suggesting potential engine or drivetrain issues. The vehicle had previously undergone repair related to similar conc
... erns, indicating a possible recurring problem. The failure created a potential safety risk due to the possibility of loss of power while driving, which could increase the risk of an accident, especially at highway speeds or in traffic. The issue has been confirmed by a service provider, and the vehicle requires significant repair. General Motors has been notified but has declined repair assistance, classifying the issue as wear and tear. While no active recall is currently associated with this specific vehicle, similar vehicles with the same engine platform have documented issues, raising concerns about reliability and safety. The failed components are available for inspection upon request.Read more
