**NHTSA SAFETY DEFECT COMPLAINT** **Vehicle:** 2022 Harley\-Davidson Street Glide CVO **VIN:** [XXX] **Odometer at beginning of trip:** 9,768 miles **Current odometer:** 9,914 miles **Miles traveled during trip:** Approximately 146 miles I am reporting a potentially serious safety defect involving the throttle/grip control system on my 2022 Harley\-Davidson Street Glide CVO\. During a recent tr
... ip, I was traveling approximately 65 mph on the freeway when I attempted to apply additional throttle and the motorcycle did not respond to the throttle input\. This created a serious safety situation because I was traveling at highway speed and suddenly had no normal throttle response\. I pulled the motorcycle safely to the side of the roadway\. **The motorcycle remained running and idling\. I did not shut the motorcycle off and I did not turn off the key fob\.** While the motorcycle was still running at idle, I was able to re\-establish the connection/function between the throttle grip and the motorcycle\. Once throttle response was restored, I was able to continue riding\. Approximately 10 miles later, the same loss of throttle response occurred again\. I again pulled over, and while the motorcycle remained running at idle, I was able to re\-establish the throttle/grip connection and regain throttle response\. This problem occurred a total of **six times during the trip**\. On one occasion, I was able to get the throttle to respond by physically moving/sliding the throttle grip back and forth\. I also observed that the throttle grip could move in and out approximately **one\-quarter inch very easily**, which did not appear normal to me\. I was not using cruise control when these incidents occurred\. The motorcycle did not stall or shut down during these incidents\. The engine remained running at idle while the throttle grip intermittently failed to provide the expected throttle response\. repeated loss of throttle response while traveling -chsracters INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIONRead more

On August 27, 2026, after riding approximately 6 miles and less than two hours after taking delivery of this brand-new 2026 Harley-Davidson CVO Street Glide ST (5 miles on the odometer from the factory), VIN [XXX], I came to a complete stop at [XXX]. When I attempted to turn right after stopping, the handlebars suddenly locked or jammed, preventing the motorcycle from turning. The bike continued o
... n a semi-straight path toward the curb, and I collided with a small tree. The motorcycle sustained minor damage consistent with a low-speed incident, but I suffered a cut on my left cheek, significant swelling in my right knee, and minor scrapes on my hand and leg. I was transported by ambulance to Memorial Hermann The Woodlands Medical Center. The police documented the incident under case number XXX. Harley-Davidson of The Woodlands never offered a test ride prior to delivery; I was apparently the first person to ride it. The following day, after hearing my account, the sales manager acknowledged that the steering stem/fork tube (the "triple tree") could be related to a factory defect, along with other factors he said required investigation. I had the motorcycle towed to the dealership's service department on XXX, for evaluation. Since then, the dealership has not provided any written explanation regarding the cause. Communication has broken down: the salesperson stopped responding, the sales manager did not answer emails, and I received only one call from the service department providing a repair estimate, without addressing the safety-related factory defect. The motorcycle remains at the dealership awaiting evaluation. This motorcycle had zero miles on it at delivery, and the dealership did not offer a prior test ride. I am concerned the handlebars might lock again, at higher speed, causing serious injury or death INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

While traveling at highway speeds in heavy traffic, the motorcycle suffered a sudden and catastrophic electrical failure. Without any advance warning, the TFT dashboard display went completely blank, the engine immediately lost all power, and the motorcycle quit running while in the middle of a busy highway.Because the engine stalled abruptly, I experienced a dangerous loss of forward momentum in
... active traffic lanes. The complete loss of electrical power simultaneously disabled vital safety systems, including the headlamp, taillamp, brake lights, hazard lights, and anti-lock braking system (ABS). This left me riding a dark, dead motorcycle with no way to signal my emergency to surrounding drivers. I was forced to coast across multiple lanes of high-speed traffic without brake lights to reach the shoulder, narrowly avoiding a rear-end collision. A diagnostic inspection later confirmed that the failure was caused by a burned-out alternator stator, which failed to charge the battery and starved the motorcycle's engine control unit (ECU) of power until the bike shut down entirely.Read more

Aftermarket motorcycle seat manufactured by Corbin Pacific falls off motorcycle while riding or dismounting. This is specific to the Harley-Davidson Pan America but unrelated to Harley-Davidson in terms of manufacture. I am not in possession of the seat as it was returned to Corbin Pacific. They replaced the seat but the replacement exhibited the exact same behavior. In short, the seat can fall of
... f or dislodge from the motorcycle while riding in a standing position (which is common on gravel or dirt roads) or when dismounting the motorcycle. This can place the rider in grave danger. Per Corbin Pacific's tech support manager, Christopher Michael, what I described to him over the course of multiple phone calls and emails was "impossible". Detailed photographic evidence as well as a video showing the problem was disregarded by Corbin Pacific as they told me that viewing such material constituted a breach of their email security policy. The first seat was returned to their facility in Florida for inspection, the second seat was returned to their facility in Hollister California. They remain in possession of both seats.Read more

On August 12, 2026, at 5,826 miles, my 2022 Harley-Davidson FXLRST M8 117 suffered catastrophic engine failure while traveling 65 mph on the freeway. I lost motive power and had to reach the shoulder, creating a serious crash hazard. About five miles into the ride, severe metallic clanging/pinging began. The engine shut down, locked completely and would not restart. No oil-pressure or other warni
... ng light illuminated. No engine/oil DTC was stored; only a battery fault was found. Harley-Davidson of Sacramento performed the 1,000-mile service at 1,004 miles and returned the bike at 1,007. Its invoice documents an oil/filter change and five quarts of oil. The engine failed 4,819 miles later. Afterward, I found the oil extremely low. Less than one quart was recovered from a roughly 5-quart system, with no external leak observed. Two independent mechanics, including a former Harley-Davidson technician, disassembled the engine. A rear-cylinder exhaust-valve head had separated from its stem, causing extensive piston, cylinder/liner, spark-plug and internal engine damage. The springs and keepers remained intact. The failed parts are preserved for inspection. NHTSA data I reviewed shows about 244 M8-era complaints involving stalls, power loss or engine/powertrain issues; about 81 hard mechanical/oiling failures, including about 15 sumping/oil-pump complaints. Harley has made numerous M8 engineering changes, including exhaust-valve and valve-spring changes for 2023 and later cylinder-head revisions. In 2026, approximately 88,000 M8-family motorcycles were recalled for an oil/breather defect. Given the freeway power loss, seizure, dangerously low oil without warning, confirmed exhaust-valve separation and broader M8 failure history, I believe this warrants a formal safety investigation.Read more

They told me over the phone that yes, the power steering issue I experienced could be due to the active recall. The dealership visually inspected the recalled part and said it was fine and my vehicle doesn't qualify to get new parts. They then stated that if I wanted them to look into a power steering issue, it would cost me $149.95 per hour. I was the only occupant and I was uninjured. There was nobody on the road at the time. Thankfully the woods got rain recently or it would have caused a massive forest fire. Problem reproduced: No Inspection: No one has inspected the car yet. Warning: There was no other warning other than described above and the truck was behaving perfectly normal. Other: The vehicle was taken to Tri-Lakes Ford in [XXX] to replace the high pressure fuel pump (another recall). I am reporting a serious safety concern involving my 2022 Nissan Rogue equipped with the 1.5L engine. While driving in normal traffic, my vehicle suddenly displayed an "Engine Malfunction" warning with no prior indication of an engine problem. Immediately after the warning appeared, the vehicle rapidly lost power and could no longer maintain the speed of surrounding traffic. I had very limited ab
... ility to accelerate or safely maneuver, creating a significant risk of being struck from behind or becoming stranded in an active travel lane. There was no advance warning that a catastrophic engine failure was imminent. The vehicle ultimately became inoperable and was towed to an authorized Nissan dealership. The dealership diagnosed the engine with coolant leaking into the cylinders and determined that replacement of the engine assembly, turbocharger, and catalytic converter was required at an estimated cost exceeding $10,000. My vehicle had previously experienced engine-related concerns and was included in Recall Campaign R25D1. That recall was completed, but less than 30 days later the engine failed catastrophically. Based on this sequence of events, I am concerned that the existing recall may not identify or address all engine conditions that can result in sudden loss of motive power and catastrophic engine failure. I have since learned of numerous publicly reported complaints from other owners describing similar failures involving coolant intrusion, sudden loss of power, and complete engine replacement in 2022 Nissan Rogue vehicles equipped with this engine. I respectfully request that NHTSA investigate whether these engine failures represent a broader safety defect and whether additional corrective action is necessary to protect the driving public.Read more

Incident Description: On [XXX], I took delivery of a brand-new 2026 Harley-Davidson road glide from a certified franchise dealership. Within 15 to 20 minutes of departing the dealership, under normal riding conditions on a well-maintained roadway, the motorcycle suffered a sudden, catastrophic mechanical failure. I observed a large cloud of white smoke trailing heavily from the motorcycle. I immed
... iately pulled over to the shoulder and shut the engine down. Upon inspection, the entire right side of the motorcycle was saturated in motor oil. The oil was actively ejecting from the air cleaner housing and rapidly accumulating on the ground in a large puddle. Safety Hazard & Impact: The volume of motor oil ejected from the air cleaner assembly created an immediate, severe traction hazard. Because the air cleaner is situated on the right side of the motorcycle directly upwind of the rear tire path, the atomized and pooling oil thoroughly saturated the right side of the chassis. Continued operation would have inevitably coated the contact patch of the rear tire, introducing an incredibly high probability of an unrecoverable rear-wheel slide (low-side crash), leading to catastrophic vehicle damage and severe bodily injury or death. Resolution & Dealer Action: The motorcycle was recovered by the dealership. Due to the immediate mechanical safety failure and discrepancies with delivery mileage, I demanded the bill of sale be rescinded and my financial assets/trade-in vehicle returned. The dealership reluctantly complied and unwound the deal, but they completely refused to provide a mechanical explanation or diagnostic report for why a brand-new vehicle suffered this severe oil-expulsion failure. That makes OnStar's role even more damning — they understand the problem, can't fix it, and don't even know their own system well enough to walk you through the workaround. Here's the complete final draft: The navigation system on my 2025 Chevrolet Equinox EV will silently change my set destination without notification or driver consent. I will enter a destination, and the system will reroute me
... to a different location — a ChargeAmerica charging station — without alerting me that it has done so. I only discover this has happened after I am already committed to the new route, potentially in an unfamiliar or unsafe area. This behavior appears to be driven by a pre-configured commercial partnership between GM and ChargeAmerica, prioritizing that network regardless of its proximity to other available charging options or the driver's intended route. The system cannot be overridden or disabled by the owner. On one occasion this silent rerouting directed me approximately 40 miles in the opposite direction of my destination, bypassing closer charging options, leaving me critically low on charge and nearly requiring a tow. This has affected me on two separate occasions. I have had extensive discussions with OnStar regarding this issue. OnStar representatives confirmed that this behavior is occurring but stated they have no ability to fix it. They were also unable to direct me to the charging menu setting that enables routing to Tesla Superchargers — a workaround I had to locate myself. Additionally, some Tesla Supercharger locations are restricted to Tesla vehicles only, with no warning provided by the navigation system. I used my remaining range to reach one such station only to discover I could not charge there. The vehicle is available for inspection upon request. No warning lamps appeared. The system functioned as designed — the design itself is the safety hazard.Read more

The problems we are having are the same they were having. The vehicle runs rough, hesitates, vibrates & shakes a lot. It is not very safe in traffic. There is a place for a dipstick to check transmission fluid. We have confirmed with 3 parts stores, 2 mechanics & finally with Nissan, they did not make a dipstick for this car or several other models. One way to check the transmission fluid is to jack up the car, remove a port on the transmission & fill it until if overflows (Cost is approximately $1,100.00, vs a dipstick costing less than $30.00 retail for comparable cars) Another way to check is to jack up the car, drain the transmission fluid, change the filter, replace the seal & cover, then pour in the exact correct amount of transmission fluid through the dipstick hole. However, the only way to know it is actually full is to leave the above-mentioned port out until the fluid overflows. We have had to replace all spark plugs 2 times plus 8 ignition coils in the 2-1/2 years we have owned it & driven it only 28,000 miles.Read more

The driver's seat inboard airbag assembly and internal structural wiring harness failed due to a direct short circuit, completely deactivating the vehicle's supplemental restraint system. General Motors has a known engineering bulletin for this issue (#23-NA-085), noting that sharp internal seat frame components slice the airbag wiring harness. Rather than permanently fixing this structural design
... flaw while under warranty, dealership technicians repeatedly executed temporary, non-compliant field patches, including plastic zip-ties and Velcro tape inside the cushion. These temporary patches continuously failed, leaving the airbags disabled. Furthermore, the vehicle was sold under "Certified Pre-Owned" designation with an active, uncompleted 2020 manufacturer Customer Satisfaction Program Campaign (#N202310850-02) for this exact airbag circuit defect. Component failed & available for inspection: The driver's seat inboard airbag assembly and internal structural wiring harness failed due to a direct short circuit. The components are available for inspection inside the vehicle at the servicing dealership. Safety risk: Safety was put at extreme risk because active Diagnostic Trouble Code B14DC completely deactivates the airbags. This means the airbags may fail to deploy in a collision, leaving the driver completely unprotected in an accident. Confirmed by dealer: The problem has been explicitly reproduced and confirmed by an authorized franchise dealership across seven separate service visits spanning from August 2025 to June 2026. Inspected by manufacturer/others: The components have been physically inspected multiple times by authorized franchise dealership technicians acting on behalf of the manufacturer. Warning lamps & symptoms: The vehicle displays "Service Safety Restraint System" warning message on the dashboard. The warning lamp first appeared on August 26, 2025, at 78,472 miles while the vehicle was strictly within its CPO warrantyRead more

While driving at a speed just under 45 mph, my Mazda CX-70 suddenly displayed and chimed “Hybrid System Malfunction. Have Vehicle Inspected.“. I ended up signing up for another lease a couple of years later with this current CX-70 PHEV, as I was told that the “bugs” have pretty much been worked out so I would give it another try. This warning continued to stay on as I drove for a few minutes so I immediately took the car straight to the Mazda dealership service department where it was kept 24 hours. The dealer said that the issue resolved itself - something to do with not driving the vehicle interchangeably enough between gas and EV mode. The photo was taken once I slowed down to stop at an intersection since it took me 15 minutes to drive to the dealer.Read more

Recently my car's check engine light and the message "Service Stabilitrak" started coming up on my car, then my car started shaking while idling. I quickly took the car to my mechanic who assessed it, replaced my spark plugs, which did not solve the issue, and recommended I go to the dealership. In between these appointments my car was consistently shaking while idling and I would have difficultie
... s accelerating especially after coming to a complete stop and often while making turns, this worsened after adding fuel to my car. The nearest Chevrolet dealership did a compression test and found that the psi in cylinder 1 was only 85, while the rest of the cylinders are 180 psi. They reported "internal engine damage causing misfire on cylinder 1" and quoted the repairs and replacement engine at $9500, almost double the value of the car if the engine was working. The codes P0300 "Engine Misfire Detected" and P0301 "Cylinder 1 Misfire Detected" were the two relevant codes in the report. However the car only has 49,860 miles on it. I see that other people have reported this issue as well and it continues to happen. Additionally, if the manufacturing issue or defect has not been rectified what are the chances of this happening again down the line with the same issue? This is a clear manufacturing defect in the engine of these cars that make them a safety hazard on the road. There is no telling when and where my engine may completely give out despite the car being low mileage and well taken care of. GM and Chevrolet should and must rectify this issue.Read more

I recently discovered that the brake light does not activate when applying the front brake lever. I became aware of the issue only after noticing that the motorcycle’s cruise control system would no longer engage. Upon researching the cruise control malfunction, I found numerous reports from owners indicating that failed front or rear brake pressure switches were the root cause. In almost all c
... ases, the faulty brake pressure switch prevented the brake light from functioning properly. Online searches reveal widespread owner reports across multiple model years describing identical failures related to defective brake pressure switches. These failures impact both brake light operation and cruise control functionality. The safety implications are extremely serious. Motorcyclists may unknowingly operate their motorcycles without functioning brake lights, significantly increasing the risk of rear-end collisions. Owners may be completely unaware their brake lights are not functioning. In my case, I only discovered the defect because the cruise control stopped working. Had that not occurred, I may have continued riding without realizing my brake light system had failed. I also found reports indicating that other owners filed complaints with NHTSA and were subsequently contacted regarding dealership repairs facilitated through KTM. This suggests the issue may already be known within the manufacturer and regulatory system. Given the repeated reports across multiple KTM models and years, I respectfully request that NHTSA investigate this defect and issue a safety recall . Brake light failure caused by defective brake pressure switches presents a substantial and potentially life-threatening safety hazard to motorcycle operators and surrounding traffic. Good faith repairs by KTM should be mandated for all owners.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The flexible brake hose on my 2015 Ford Edge failed/broke shortly after I purchased the vehicle. The vehicle also began experiencing engine performance issues, including a rough idle and rough driving condition after the check engine light came on. The vehicle and failed brake hose should be availabl
... e for inspection upon request. How was your safety or the safety of others put at risk? The broken flexible brake hose created a serious safety risk because it could affect the vehicle’s braking ability and increase the risk of losing control or being unable to stop safely. The rough engine performance also caused concerns about the reliability and safe operation of the vehicle while driving. Has the problem been reproduced or confirmed by a dealer or independent service center? The issue has been observed and experienced consistently while driving the vehicle. The brake hose damage and rough engine performance were noticeable after the vehicle was purchased. (If a mechanic has looked at it, you can add: “An independent mechanic/dealership confirmed the brake hose was damaged.”) Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? I have taken it to the dealership and they said nothing about it but I got it diagnosed at autozone and the code came up right away. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Approximately 2–3 weeks after purchasing the vehicle, the check engine light came on and the vehicle began running roughly. Around the same time, issues related to the broken flexible brake hose were discovered. Symptoms included rough driving performance and concerns with the braking systemRead more
