Vehicle frequently stalls or engine stops suddenly. Engine will "knock" and suddenly stall when engaging the clutch while providing throttle to begin riding. It will stall when engaging the clutch to downshift into lower gears while coming to a stop. This has happened at busy roadways at red lights, stop signs, and even when slowing down to navigate a turn. Engine may stall while in neutral and th
... rottle is applied. Typically a short rattle or what sounds like valves knocking, is followed by a backfire, prior to stalling. There is no way to prevent stalling/engine stopping unless RPM's are kept over 3,000. I have fallen over or crashed offroad several times, when navigating obstacles or negotiating tight turns or turn-arounds in the trails, dirt roads or forest. Bike stalled when attempting to navigate over a small log, resulting in bike crashing and pinning me underneath. Dealer performed update to most current fuel/mapping software but this did not make a difference.Read more

Action: The bulletin initiates a stop-sale order for affected vehicles and provides instructions for inspection and repair, which may include replacing the connecting rod bearings, crankshaft, or short/long block. Status: The bulletin has been revised. Version 1 was issued on November 9, 2023, and was replaced by Version 2 on February 9, 2024. XXX XXX XXX XXX XXX 11/13/2025 National Highway Traffic Safety Administration Office of Defects Investigation 1200 New Jersey Avenue SE Washington, DC 20590 Toyota Motor Sales, U.S.A., Inc. 19001 South Western Avenue Torrance, CA 90501 Subject: Complaint Regarding Torque Converter Shudder – 2018 Toyota RAV4 AWD To Whom It May Concern, I am submitting this complaint regarding
... a serious transmission defect in my 2018 Toyota RAV4 AWD, specifically a torque converter shudder that has been documented by Toyota technicians. The vehicle exhibits pronounced shuddering between 25–50 mph under light throttle, consistent with the torque converter failures previously acknowledged by Toyota in earlier RAV4 models (2013–2015). Toyota has extended warranty coverage for torque converter replacement in those earlier models, yet my 2018 RAV4—despite showing identical symptoms—has been excluded. The dealership has quoted me $9,000 for a full transmission replacement, even though the issue is well-documented and appears to be systemic across multiple model years. This situation raises serious concerns: - Toyota is aware of the defect but has limited coverage to select VIN ranges. - The cost of repair is excessive and disproportionate to the defect. - Consumers are left without recourse despite clear precedent for warranty extension. I respectfully request: 1. That NHTSA investigate whether the torque converter shudder in 2016–2018 RAV4 models constitutes a broader safety defect. 2. That Toyota extend its warranty program to include affected 2018 RAV4 vehicles. 3. That Toyota provide reimbursement or coverage for necessary repairs to restore safe vehicle operation. I am prepared to provide service records, dealership documentation, and correspondence confirming Toyota’s acknowledgment of the defect. Please advise on next steps and ensure this issue receives the attention it deserve INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

On Th, Nov 6 ~1700, the 25 Equinox EV being driven encountered an electric drive failure and brake failure, including friction brakes. This Equinox EV is essentially still brand new with 3900 miles and no accident history. The vehicle was driven off the EB SR 528 exit ramp to Narcoossee Rd in Orlando and driven to the stop light at the WB on ramp. After the light changed and began driving, the
... vehicle console reported an exception with drive assist. At this time the driver attempted to slow down to take stock of the situation and the vehicle accelerated, at which time the driver took their foot off the brake pedal and attempted to identify a place to take the vehicle off the roadways. Thankfully a stoplight at McCoy Rd was green and the driver continued through that light and while attempting to brake, the vehicle accelerated instead. The driver then coasted, maneuvering the vehicle was into the Vista Palms commercial center. The vehicle, unresponsive to braking, propelled through a stop sign into the Kohl's parking lot at which time the driver began looping around the lot until the weight of the vehicle brought the motion down. While the vehicle was stopped, it rotated through warning messages indicating..forward collision, brake assist is reduced, steering assist is reduced, then finally Service Transmission Now Unable to Shift Soon. The vehicle was towed by AAA to AutoNation Chevrolet Airport in Orlando. Technicians found codes U1610 and U1639, and found K160 connecter unseated. It was reseated and greased. The concern is that this should have been locked and unable to be unseated. Is this an indication of manufacturing problem or operating problem? All braking was dysfunctional. The paint on my 2020 Toyota RAV4 (Blueprint color) is peeling and deteriorating prematurely, on the hood. The vehicle has been properly maintained and has not been in any accidents. This suggests a widespr

The peeling paint not only affects appearance but also exposes the metal body to rust and corrosion, which could eventually compromise the vehicle’s safety and structure. Toyota has not issued a recall or offered repairs for this known problem, and many owners are being asked to pay out of pocket for costly repainting. I’m filing this report because the dealership where I purchased and have my vehicle regularly serviced failed to notify us of an active safety recall related to the brake system on our Toyota truck. The dealership is Classic Toyota – located at 2301 W Mercury Blvd, Hampton, VA 23666. The recall was issued in February 2025, yet the vehicle has been serviced by the same Toyota dealer multiple
... times since then, including an oil change and a state inspection. The dealer never mentioned the open recall or offered to repair it. I bought the truck prior to the recall in July of 2024. We only learned of the recall recently through the Toyota app, not from the dealership. This is unacceptable given that brake recalls pose a serious safety risk. I am requesting an investigation into this dealership’s failure to notify or perform the recall repair as required by federal law.Read more

I took the vehicle to the dealership, and they diagnosed a Power Transfer Unit (PTU) failure that requires replacement. This is a serious safety concern, as the vehicle could lose the ability to drive, especially in certain road or weather conditions. After researchin
... g the issue, I discovered that PTU failures appear to be a widespread problem in Jeep Cherokees, yet there has been no effective recall or permanent fix. Owners are being put at risk due to potential loss of power while driving.Read more
