The night of the incident November 16th, 2025, the transmission in my daughter's 2018 Toyota C-HR failed suddenly while the vehicle was in motion. The car lost the ability to accelerate and then became completely immobile. The failure occurred without warning and with no prior symptoms. She could not shift into drive or reverse. My daughter, a [XXX] college student was driving at the time, and t
... he breakdown left her stranded in the dark on a roadway with limited visibility and no safe shoulder. She was in a semi-safe neighborhood. I called AAA as I am a premier member and it took 4 hours to get her car towed. This created a significant safety risk—both the danger of being struck by approaching vehicles and personal safety concerns associated with being stranded alone at night until almost midnight. I am several hours away (Canton, GA) and could not get to her safely. We specifically picked a reliable Toyota model to take to college so that this would not happen. The transmission and related components remain in the vehicle and are available for inspection upon request at my home. The car is not drivable and we are stuck with a costly repair. This appears to be a very common problem with this specific Toyota model even with low milage but no recall has been issues. After the vehicle was towed to Group 1 Toyota in Columbus, GA an inspection by the dealership was performed at a $200 cost to diagnose a transmission failure. A faulty CVT transmission that is not covered under any warranty and is $11,000 to repair. Attached is a report showing excellent condition of other car components. There were no warning lamps, messages, noises, or other symptoms leading up to the failure. The transmission malfunction occurred suddenly, with the first and only indication being the loss of power and inability to move the vehicle. Toyota addressed the issue by no longer using this type of transmission and discontinuing this model but will NOT offer a solution. Please help!! Escalating Safety Concern – Loud Clunking, Thumping, and Cracking from Front Axle at Low Speeds We are long-time loyal customers and currently own four identical Mercedes-Benz models. Unfortunately, one of our vehicles, a 2021 mb glb250 has recently developed severe clunking, thumping, and loud cracking noises originating from the front axle area, noticeable especially at low speeds. The proble
... m appeared suddenly and is rapidly worsening. After researching the issue, we’ve discovered that many owners of the exact same model and model year report identical symptoms. This appears to be a widespread and recurring problem that raises serious safety concerns, particularly as it affects the front axle and is uncommon in other modern vehicles of this class and young age. Given the potential safety implications and the growing number of affected owners, we respectfully request that Mercedes-Benz investigate this issue promptly, acknowledge it as a known concern, and provide an appropriate inspection and repair at no cost to us. We love the brand and have always trusted its engineering and safety standards, which makes this experience especially distressing. We would greatly appreciate your immediate attention and a clear response on next steps.Read more

This is deeply concerning, as gasoline fumes inside the cabin pose not only a serious health risk from inhalation but also a potential fire or explosion hazard. It’s unacceptable that a manufacturer would refuse to remedy a safety recall issue, especially one involving a known fuel system defect that could endanger both the driver and passengers. I’m left with a car that’s unsafe to drive and a manufacturer unwilling to take responsibility for a problem they’ve already acknowledged through the NHTSA.Read more

The failure appears to have originated in the passenger side engine compartment, but the exact component or system (fuel, electrical, or other) is unknown. The vehicle is a total loss from the fire but should be available for inspection at the tow or storage facility. Our safety and the safety of others were put at risk because the vehicle caught fire suddenly while driving with traffic on a publi
... c road; my wife was driving, I was in the passenger seat, and our dog was in the back, and if we had not noticed the smoke and pulled over, the fire could have led to loss of control, injury, or a larger fire. The problem has not been reproduced or confirmed by a dealer or independent service center because the vehicle is completely burned and not drivable. As of now, the vehicle has not yet been inspected by the manufacturer; a claim has been initiated with my insurance company, and I expect their representatives and/or a fire investigator to inspect the vehicle, and local emergency services responded to the scene. There were no warning lamps or messages on the dashboard prior to the fire; the only prior symptom was an abnormal odor from the exhaust that smelled like mothballs or ammonia, first noticed immediately after starting the vehicle at the beginning of the trip on November 23, 2025, followed later in the trip by smoke and then the fire.Read more

I received a safety alert on my car screen: Front passenger safety restraint system issue. Service is required. The front passenger safety restraint system fault typically involves a malfunctioning occupant weight sensor in the passenger seat, which controls whether the airbag activates or deactivates EVGOER. This is a serious safety concern because the airbag may be disabled at that seat locatio

The issue is often caused by loose, broken, or unplugged harnesses or connectors under the seat, particularly related to the Occupant Classification System (OCS) Tesla Owners OnlineDigital EV Doctor. Many owners report this happens because wires under the seat get crimped over time due to Tesla's "easy entry" feature that automatically moves seats. After diagnostic troubleshooting from DTCs on a check engine light, it was discovered that the brake vacuum pump connected to the engine was completely nonfunctional. Previous symptoms included stiff brake, loss of brake power at random brake periods which could have cause and accident with other drivers, and finally malfunctioning sensors attributed to the exhaust system. This also led the vehi
... cle to be unable to pass state regulated Emissions testing because of the inability to maintain proper emissions levels. Brake Vacuum pump that was removed from vehicle to inspect and diagnose is still available for inspection. Recent findings show that Chevrolet/GM is aware of the issue in their vehicles but refuses to address or replace faulty brake vacuum pumps. It is a common problem amongst 2013-2022 Chevrolet Malibus. Vehicle that is in question was inspected by a certified mechanic shop with failed emissions test. Solid check engine light appeared once the brake had become harder to push and stop the car. OBD scans show DTC codes that were pertaining to MAF and MAP malfunctions in readouts and communications to the ECM. After further inspection, the ABS module had been acting as "backup" for hydrolic brakes system that had zero vacuum to power the brake booster, this could have caused the engine to stall, longer than normal stopping distances (stronger risk of rear ending other drivers), and multiple components and sensors to fail, requiring them to also be replaced.Read more

Upon purchasing the vehicle, I discovered multiple critical safety defects that were not disclosed at the time of sale and were confirmed by an independent EV specialist (Electrified Garage). These defects include worn and failing front and rear suspension components (control arms, thrust arms, and cracked/damaged bushings), a failing sway bar link boot, and a malfunctioning window that makes cont
... act with the internal wiring harness. The suspension defects create instability over bumps and uneven roads, producing clunks, shifting, and vibration that affect steering control and overall stability. These issues present a risk of suspension failure while driving, which could cause loss of control. The window/wiring contact also poses a potential electrical hazard if the harness becomes damaged. The independent inspection documented that these defects existed before I took possession of the vehicle. The dealer did not disclose any of these issues, and the defects were not visible during a standard test drive. I am reporting this because the suspension defects directly affect vehicle handling, safety, and crash-avoidance capability. I want NHTSA to be aware in case similar defects appear in other vehicles of the same model/year.Read more

On 11/20/25 I rented a 2026 Dodge Ram truck from Thrifty at the Wilmington, NC airport. My 17 year old daughter was with me. As I was sitting at a red light, I tapped the “comfort” button on the center screen to try to adjust the air conditioner. When I did, the audio system, which had been off, suddenly blasted a radio station to the maximum volume of the speakers. No warning. It was horribly
... painful and jolting. I tried turning down the volume using the volume dial. It didn’t work. Nothing would turn it down. The volume setting showed it was already at zero on the screen. It continued to blast us until I saw I could push the volume button to power off the screen. That stopped it but my daughter and I were both very shaken and upset. It was the loudest sound we’ve ever heard and it went on for at least 5-6 seconds. She was able to cover her ears, I was not because I was desperately trying to find a way to turn it off. I am thankful it happened at a red light or I’m afraid it would have caused an accident. I reported it to the rental agency immediately upon getting to our hotel. I was scared it would happen again and cause an accident, so I tried to do what they suggested, which was to try the volume controls behind the steering wheel. It did not work. We were exposed to this horrible volume at least 3 more times for closer to 10 seconds each before we were able to figure out how to turn off surround sound, which stopped the noise but enabled us to still use the screen. I returned the rental truck on 11/22/25 and reported it again to another agent. He confirmed the note was still in the system from when I first reported it and assured me they would look into it. My ears still hurt and my hearing is still muffled. I keep having to ask my daughter to repeat herself. Loud sounds, like announcements in the airport or on the plane are painful. I plan to get my ears checked when I get back home.Read more
