I am submitting a safety complaint regarding a recurring infotainment system failure involving the 2025/2026 BMW X3. While driving, the center display screen suddenly blacks out, which creates a serious safety hazard. This issue first occurred with my personal BMW X3. More concerning, the exact same failure occurred again while I was driving a BMW X3 loaner vehicle provided by the dealership. Thi
... s indicates the problem is not isolated to a single vehicle but may be systemic. I was able to capture video evidence of the incident while driving, which clearly shows the screen going completely black: [XXX] When the screen blacks out, access to navigation, vehicle settings, driver assistance displays, and other critical information is lost while the vehicle is in motion. This creates a dangerous distraction and increases the risk of an accident. Despite previous service visits, the issue has not been resolved, and the occurrence in a second vehicle raises serious concerns about the reliability and safety of BMW’s infotainment system in the X3. And most importantly, this is the brand-new redesign BMW X3 2025 and 2026 model. Not any previous model, which raises more concerns. I believe this defect warrants investigation, as it directly impacts driver safety and may affect multiple vehicles. I am uncomfortable continuing to drive the vehicle under these conditions. Thank you for your attention to this matter. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Location: Intersection of [XXX] and [XXX] Lakeville, MN Date/Time: [XXX], at approximately [XXX] Vehicle: 2025 Kia K4 (911 call placed at 10:08 am) On [XXX], while traveling toward the roundabout at [XXX] and [XXX], my vehicle experienced a sudden and violent unintended acceleration. Despite having my foot completely off the gas pedal, the engine began to accelerate the vehicle rapidly on its ow
... n. I immediately attempted to regain control by pumping the brake pedal repeatedly with significant force. These efforts had no effect; the vehicle continued to accelerate faster and faster. As the speed increased, the vehicle began to swerve, and I realized I was losing control while rapidly closing the distance between myself, the roundabout, and the vehicles ahead of me. Fortunately, the right lane was clear. Because the vehicle was already veering in that direction and the brakes were failing to slow the car, I made a split-second decision to steer into a large snowbank to avoid a high-speed rear-end collision. The force of the impact with the snowbank was substantial, resulting in the front body panel and a lower front component being torn off the vehicle. The Lakeville Police and Fire Departments responded to the scene and were required to dig the vehicle out of the snow pile. I have attached the official police report to this statement. Had I not maintained a safe following distance or made the decision to steer off the road, the result would have been a catastrophic multi-vehicle collision. Adaptive Cruise Control (ACC) caused dangerous unintended acceleration. This model of vehicle has an ACC that maintains following distance from a leading vehicle. It will decelerate to a full stop and will resume after a full stop while maintaining a proper following distance when prompted by a brief push on the accelerator. At the beginning of this incident, my ACC was set to about 40 MPH and I
... was following the car in front of me. At a traffic light, I came to a full stop behind the leading car for 10-15 seconds. At this point, I realized that the left lane turn signal changed to green which would allow me to proceed. Since there was no traffic in that lane, I decided to move from my lane to the open left lane. I turned the steering wheel to go around the car in front of me into the left lane. I then briefly touched the accelerator. The result was that the car accelerated at a high pace. Since my foot on the accelerator initiated the event, it took me a second or so to move my foot to the brake and disable ACC. During the acceleration, I was able to steer into the left lane and make the subsequent left turn without incident. I'm unlikely to make this mistake again. However, I'm pretty certain that this sequence is easily repeatable should you want to test for it. This ACC behavior seems dangerous. I expect that resuming ACC while following a vehicle will result in acceleration. But if the car is pointed in a new direction while it is stopped, it seems inappropriate to allow the ACC to resume by tapping the accelerator. A better ACC behavior would disable the"resume after a full stop" functionally if the wheel is turned more that a few degrees after an ACC controlled full stop.Read more

NHTSA Complaint Update / Addendum I am submitting this update to my existing complaint regarding Safety Recall 25S49 (NHTSA Recall No. 25V695) for my 2015 Ford C-MAX Hybrid (VIN: [XXX] ). Ford Motor Company has now formally stated, in writing through the South Carolina Department of Consumer Affairs, that: • The rearview camera defect may cause a blank or distorted image while in reverse •
... The condition increases crash risk • Parts are not currently available in sufficient quantities • No repair will be available until Q2 2026 or later • No loaner vehicle or rental reimbursement is authorized • Owners are expected to continue operating affected vehicles indefinitely Due to this recall and lack of remedy, my vehicle was removed from service on October 14, 2025. As of December 29, 2025, the vehicle has been unusable for 76 days, resulting in $2,209.32 in documented lost income based on historical earnings of $29.07 per day. Ford has acknowledged the safety defect but is offering no interim solution while delaying the repair for potentially over six months. I am concerned this creates an unreasonable safety and economic burden on affected owners and may represent a broader issue impacting many consumers. I am requesting that NHTSA review Ford’s prolonged delay, lack of interim remedy, and refusal to provide alternative transportation while a known safety defect remains unresolved. After 110,000, the engine started using oil and it began to knock. According to the history of these engines, it will lead to engine failure. They have a record of being a lemon engine and there is not a engine of this type that this does not happen. The engine needs to recalled and replaced, They are the Theda II GDI series of 2.0L and 2.4L four-cylinder engines from Hyundai/Kia.

The car is experiencing the same problem reported on technical service bulletin SA187 (Blind spot detection (BSD) system logic improvement (SA 187)) BUT KIA states my VIN number is not listed under the service bulletin and therefore I have to pay $189 to get a software upgrade done. The "Check BSD system" illuminated in the instrument cluster has been cleared twice by my mechanic and still comes
... back after a few days. To me this issue should have been made a recall because of its criticality... the BSD system is a core safety feature of cars and reduces accidents substantially. There was no way for me to know my car has this problem unless it is made a recall. I have NOT had any accident related to this issue but i had to pick an incident date to complete this complaint. I have called KIA customer service at 1-800-333-4542 twice to find out whether the VIN number should be included in the service bulletin and the answer was no and whether i should report this to NHTSA with the possibility that certain VIN numbers were left out of the service bulletin in error. No answer as to what to do to investigate this was given by KIA. That's why i'm reporting this on here. The service bulletin has been attached to this complaint. My question to NHTSA is whether certain vehicles were left out of the service bulletin in error and whether the manufacturer is liable for the repair at no cost to the consumer. If NHTSA finds out the manufacturer is liable will you notify them to pay on a case per case basis or will you make this service bulletin a recall? Again this is SAFETY HAZARD that can cause crashes and accidents. [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Additionally, the contact had hired a third party inspector, who had inspected the RV before it was driven off the dealer's lot. The inspector had informed the contact that the RV was missing safety pins and locking mechanisms, and that the roof was damaged; however, the dealer advised the contact that the RV was safe to drive without the parts and the parts would be provided later. The contact stated that all the beds of the RV had detached, and the beds frames were flapping against the walls while in transit, causing damage to the beds frame and to the walls. The contact stated that the failure had caused the whole RV to shudder while driving 40-50 MPH, which required the contact to exit the highway a. The RV was taken to the nearby RV dealer. The dealer detached the beds and laid them flat on the floor. The RV was later taken to General RV Center(14000 Automall Dr, Huntley, IL 60142), where the roof was rebuilt; however, the contact requested a trade-in due to safety concerns. The dealer depreciated the RV and rolled the depreciation into the payment of the new RV that the contact had purchased. The manufacturer was notified of the failure, but the manufacturer put the blame onto the dealer and referred the contact to the dealer for assistance. The failure mileage was 0.Read more
