Warranty Alignment: This failure occurred at 47.7k miles, well under the 60,000-mile threshold of the Supplemental Restraint System Limited Warranty. Known Defect: This repair is the subject of a specific Service Bulletin (RCM_a078), confirming it is a known manufacturing issue and not standard wear-and-tear. Safety Criticality: The SRS is a primary safety system. Forcing a customer to pay for a
... known defect on a life-saving system at such low mileage is inconsistent with Tesla’s safety mission. I have already paid the high invoice to ensure the vehicle was safe to drive. Tesla did not consider the goodwill repair request for known manufacturing defect that puts passenger at risk. Such safety related repair should be part of safety recalls.Read more

I was driving, there was a loud pop, then warnings popped up on the dash and I lost power. I was able to limp the car back to my house, because I had just left. I assume it is the ICCU. I’m reporting this because it is a known issue that Hyundai has not seemingly been able to repair. With the pedal to the floor, I got up to 25 mph. Luckily, I was near home and not on a highway.

My vehicle has a documented excessive oil‑consumption defect. After a 1,000‑mile oil‑consumption test performed by a Hyundai dealership, the engine was found to have consumed 2.8 quarts of oil, which is far above normal and poses a safety risk. The dealer referenced TSB 23‑EM‑008H and recommended a chamber cleaning but could not guarantee it would resolve the issue. Hyundai Motor America
... declined to cover the repair and refused to escalate my case. This level of oil consumption is consistent with known Theta II engine defects that have led to engine seizure, stalling, and fire in other vehicles. I am reporting this as a safety concern due to the risk of sudden engine failure while driving.Read more

This complaint concerns a 2016 Airstream Interstate motorhome that has experienced multiple safety-critical failures involving the electrical system, engine starting system, fuel/propulsion system, and electrically controlled exterior components. While driving at highway speed, the exterior power awning deployed without warning or user input. The awning repeatedly extended and retracted uncontrol
... lably, and the control switches were ineffective. I was forced to pull over on the freeway shoulder, cut electrical power to stop the motion, manually retract the awning, and secure it to prevent detachment. This created an immediate risk of collision and danger to surrounding motorists. Additional failures include complete engine non-start, generator failure, and a severe propane odor originating from the exterior propane system. The propane odor was strong enough to trigger the interior propane alarm, indicating a potential fire or explosion risk. The vehicle is now completely inoperable. The engine will not start and the ignition key will not turn. The vehicle cannot be driven and would require towing for inspection. These failures occurred progressively under normal use and were not caused by collision, misuse, or modification. This vehicle is a 2016 Airstream Interstate built on a 2015 Mercedes-Benz Sprinter chassis; the VIN corresponds to the Mercedes chassis model year.Read more

While driving, the vehicle experienced a sudden loss of propulsion and significantly reduced acceleration. At the same time, the climate control system stopped producing heat. A red warning message appeared instructing me to stop safely. The vehicle would barely accelerate, creating a dangerous situation while driving. This issue has occurred three separate times, including twice in the same day.
... Each occurrence happened while the vehicle was in motion. The condition resolved only after turning the vehicle off and restarting it. The issue is intermittent and cannot be predicted. The reduced acceleration and loss of propulsion placed my safety at risk, especially at highway speeds, as the vehicle could not respond normally to traffic conditions. The vehicle was taken to a Kia dealership and inspected. A manufacturer tech line case was opened, and testing was performed, including a smoke test. The dealership stated they were unable to duplicate the issue. No repair was made, and the manufacturer closed the tech line case despite the reported loss of propulsion events. The vehicle is currently being retained at the dealership because I do not feel it is safe to operate. The problem was not resolved.Read more

On January 8, 2026, my 2014 Ford Explorer (VIN: [XXX] ) caught fire without warning. The vehicle was parked in my garage prior to departure. When I pulled the vehicle out to leave, I noticed something burning on the garage floor. I briefly returned inside to extinguish that small fire, and when I turned back, the vehicle itself was on fire. I immediately moved the vehicle onto the grass away from
... a nearby tree to reduce the risk of further damage or spread. The fire department responded and extinguished the fire, and police were also on scene. The vehicle was effectively a total loss. There was no collision or prior accident involved. The fire appeared to originate in or around the engine compartment. The vehicle was properly maintained, and to the best of my knowledge, no unauthorized modifications were made. I purchased my vehicle 6 months ago. I had no prior issues outside of a rough drive or a very slight burning smell from time to time. 12/2025 I found out through my auto insurance that my vehicle had a active safety recall (25S76) so I scheduled with my local Ford to get the temp software PCM update done until the remedy becomes available. Immediately after the software update I started getting a
... lot of system error codes; keys not detected, airbag fault, Tailgate open (when it wasnt), auto hold system fault, auto start/stop system fault, etc.. During this time I had issues with my break system "softening" like the breaks wanted to go out OR the break peddle locking up on me and being unable to move/drive. Delays in acceleration or spurts of acceleration which was uncommanded. This happened in active school zones making it even more unsafe and dangerous to drive. I took it back to my nearest dealership, and they could not replicate the issues but kept my case open and said to inform them if things changed. Things gradually got worse. I have video recording of my car accelerating without my foot on the peddle and my breaking peddle softening to the point of not working properly as well as hardening and locking up. I have photos of the errors when I was able to safely due so. I have full documentation that is not listed here (due to the amount of words allowed). I took my vehicle back and has been with Frontier Ford in Anacortes WA for 5 days. I have contacted Ford as well. I am unable to get a rental due to both Ford and the dealership both telling me to go to each other for help. I have no update as my vehicle is still in testing. They were able to replicate some of the issues but have no update outside of that. I have no way to work or take my children back and forth to their classes. My Ford dealership said this software has caused similar issues with other vehicles but my issue was a more severe case. I am unsure on how to move forward with this.Read more

I am requesting formal escalation and review of my case due to a repeat failure of a safety recall component that has rendered my vehicle unsafe and not safely operable. However, this is not a new or unrelated issue — this is a repeat failure of the same safety-critical component addressed under the recall. This condition presents an ongoing safety risk, and the vehicle cannot be used safely in its current state. I have already filed a formal complaint with NHTSA documenting this repeat failure and Ford’s refusal to repair the issue. At this time, I am requesting: Review by a supervisor or escalation specialist Consideration for goodwill or recall-related assistance Resolution of this repeat safety defect without cost to me Please provide: Confirmation that this case has been escalated A written response explaining Ford’s position Any options Ford is willing to offer to address this safety concern I am requesting this review in good faith and hope Ford will stand behind its safety recall repair when the same defect has clearly reoccurred. Starting in October 2025 at about 131700 miles and continuing into Jan 2026 my 2016 Ram Eco Diesel would periodically loose power steering capabilities . In December , a ( SERVICE POWER STEERING! ) message did light up. Since dealer in my opinion gave me the old “heave hoe” in October and November about this issue, I made appt with independent mechanic shop. They diagnosed I needed new rack and pinion assembly in just one day! Repaired on 1/12/26 at 132817 miles. I do have truck back and it is performing fine. I have owned it since May of 2019 . This truck was built in June of 2016 in Michigan! Thank goodness I had access to other vehicles to drive during this issue. Had I been in bad weather or icy roads , I would not have been able to control vehicle!!! I do feel this Eco should be part of the electronic power steering recall!!!Read more

... elerate past 25 mph, and it felt like the car was dragging. I did not know what was happening. The “Check Electrical Vehicle System” warning returned but gave no indication of whether it was safe to drive or what to check in the electrical system. Driving very slowly, I made it out of the military base (my workplace), parked outside of a closed establishment, and called my husband. He told me to drive further up the road to a well-lit shopping area. I exited the parking lot and entered the intersection of a four-lane highway – and suddenly, when I was in the center about to turn in the direction of traffic, the car stopped and would not respond to the gas pedal. I could not accelerate and was stuck in the middle of the highway. After several frantic pushes on the gas pedal, I was able to coast downward to the shoulder and exit the vehicle. My husband and I had brought the car to the dealer in February 2025 for the December 20, 2024 ICCU recall (NHTSA Recall 24V-868, Hyundai Recall 272). The dealer merely inspected the car and updated its ICCU software. Per Hyundai guidance, it did not replace the ICCU itself. It is taking no apparent steps to do so for 2023 Ioniq 5 owners, leaving us in danger. The seriousness of this problem is compounded by the lack of adequate warning when it occurs out of the blue. The generic “Check Electrical Vehicle System” indicator on the dashboard did not indicate what precisely in the electrical system to check, how serious the issue was, or how much of the 12-volt battery power was left. Hyundai must rectify this issue before others are hurt.Read more

... elow the eye, resulting in small open wounds, which needed to be cleaned and disinfected. The problem has occurred twice under normal use. I reported the first incident to General Motors (GM) on May 15, 2024, but did not receive a response. On Sept 22, 2025, I reported the second incident that occurred on Sept 2, 2025, to GM, at which time they asked several questions and requested photos of my vehicle, which I provided. On Sept 22, 2025, GM requested I take my vehicle to my dealership, J.B.A. Chevrolet, for inspection. I took my vehicle to JBA on Sept 29, 2025, and was told that GM wanted photos of my vehicle, as well as photos of another 2024 Trax on the Lot for comparison. At that time, JBA had to wait for another vehicle to come in. Unknown if JBA or GM attempted to reproduce the issue during an inspection. On Oct 24, 2025, GM telephoned and issued a final determination, which I requested in writing and received on Nov 11, 2025. GM stated that because the design was consistent across Trax vehicles, and based on the facts provided, they were unable to assume responsibility for damages and suggested I resolve the issue with my insurance company. I did not ask for damages, I asked for corrective measures to avoid future injuries. I submitted a request for reconsideration on Dec 2, 2025. On Dec 16, 2025, a totally different GM office contacted me, which did not yield any results. Although the injuries were minor, the repeated nature of the harm indicates a foreseeable risk to future passengers and to other Trax owners. Uploading 10 pages in PDF formatRead more

The vehicle in question had approximately 5000 miles when low oil light came on, it was 2 quarts low. this was after the first oil change at around 3500 miles. I had the dealer do an oil consumption evaluation. They check the oil every 500 miles and confirmed it was using 2 quarts every 2500 miles. They informed me GM says this is normal. I cannot disagree more with that claim. GM knows they
... have a problem with the 6.2-liter engine. I have researched the issue and found they have a problem with the crankshaft machining and the oil delivery port beveling that delivers lubrication to the connecting rod bearing appears to be too large that causes the motor oil not to consistently keep the bearing lubricated. This causes the engine to be an "oil burner" and premature failure of the bearing. They think putting a heavier viscosity of oil in will delay the inevitable engine failure, which makes me believe they are aware of the faulty manufacturing and want to run from their warranty responsibilities.Read more

I brought my vehicle to the Jeep of concord for a fuel pump relay recall and oil change on 11/12/25. Prior to 11/12 the vehicle did not experience any of the issues described below. On 11/13/25, a dealership representative sent an inspection video stating the vehicle was in immaculate condition following the recall repair. However, within less than 24 hours, the check engine light illuminated, and
... additional issues began occurring intermittently. After the recall work, the vehicle began experiencing intermittent warning lights, including service four wheel drive, traction control, and check engine. These warnings appear unpredictably and are sometimes accompanied by loss of vehicle functions, creating unsafe driving conditions. The vehicle has also experienced overheating since these issues began. Sometimes turning the vehicle off and then on would temporarily correct the problem if it would recrank. The failures occur intermittently, making them difficult to reproduce or diagnose, and they pose serious safety risks, including the potential for sudden loss of power, steering assistance, or drivability while the vehicle is in motion. On multiple occasions, the issues have left me stranded. I returned the vehicle to the dealership on 1/8/26. On 1/9/26, I received an inspection video from the dealership identifying a transmission issue for the first time. The rep stated they “double-checked” the recall wiring but confirmed that no manufacturer engineer inspected the vehicle, despite my request to escalate this as a STAR case due to safety concerns. The intermittent warning lights, overheating, and transmission concerns remain unresolved and Stellantis refused to escalate the issue. The dealership currently retains possession of my vehicle. I am concerned the recall repair may have been performed wrong or inadequately inspected, resulting in ongoing intermittent safety-related failures affecting multiple vehicle systems. I am requesting that NHTSA investigate & confirmRead more

I was involved in a severe motor vehicle collision in a 2017 Hyundai Sonata. The airbags failed to deploy during the crash. I sustained serious injuries and was transported from the scene by EMS. This vehicle has known safety recalls, and I believe the airbag non-deployment may indicate a safety defect.

I am reporting a safety defect in the 2025 Toyota Highlander Hybrid Nightshade involving the multifunction turn signal switch. The interior trim adhesive on the steering column has experienced a chemical breakdown (adhesive migration), causing the turn signal stalk to become stiff and bind. This obstruction makes it difficult to engage the turn signal and occasionally prevents the signal from "aut
... o-canceling" after a turn. The dealership has acknowledged the adhesive failure by authorizing the replacement of the steering column covers but refuses to replace the contaminated turn signal switch. Operating a vehicle with a binding turn signal switch is a direct safety hazard, as it compromises the driver's ability to communicate intent to other motorists. Additionally, the dashboard material is experiencing systemic delamination and producing a white crystalline powder (crystallized adhesive). I am requesting an investigation into the heat-resistance standards of the interior bonding agents used in the 2025 Nightshade trim, as these materials are failing and compromising safety-critical electronic controls.
