I received a recall notice for my 2013 Kia Optima, VIN #: [XXX] related to engine issues and an increased risk of fire. After receiving the recall notice, my vehicle began smoking and briefly caught fire while in operation. I immediately shut the vehicle off, and the smoke and fire stopped. For safety reasons, the vehicle was towed by my insurance company to my aunt’s residence, which was the
... closest safe location at the time. The vehicle was unsafe to drive. No injuries occurred, and the fire department was not called because the fire stopped once the vehicle was turned off. I believe this incident is related to a known safety defect and recall involving Kia Optima engine fires. I have contacted Kia multiple times regarding this issue, and there have been delays in addressing or repairing the vehicle. Hyundai model Ioniq 5 (all years and trim styles) have a known issue with ICCUs (Integrated Charging Control Unit) that fail. This component controls charging of the 12 volt battery, and in turn electrical systems. Hyundai replaced my ICCU, but I am concerned it may fail again, as the underlying issue does not seem to be known or addressed. When the ICCU fails, the vehicle can suddenly lose the ab

I received an alert on the dash stating there was an issue with the blind spot monitoring/rear cross traffic system. It was erroring off and on for that entire drive. When I got home, I googled the issue and it appears it's a known issue with '21 and some early '22 broncos where they factory installed the blind spot module wiring harness too close to the rear bumper. Over time, the wires rub an

I have an extended warranty and it's at the dealership today. This is a manufacturing defect and should be covered (even if out of warranty) in my opinion. We will see what kind of story the dealer can come up with to wiggle out of it being covered under warranty.Read more

My 2023 Mazda CX-30, which is currently under full manufacturer warranty, was brought to the dealer on October 10, 2025, specifically to address a safety defect involving white chemical particulates blowing through the HVAC vents. The dealership confirmed the failure of the "evaporative coating", which resulted in unidentified chemical byproduct being discharged directly into the passenger cabin.T
... he vehicle remained out of service for 103 days due to this manufacturing defect. Upon receiving the Repair order (RO# 77004584) today, January 20, 2026, I discovered a significant safety concern that violates standard warranty repair protocols: the technician admitted to receiving two separate heater unit boxes that were damaged during shipping and subsequently 'transferred all salvageable items "from one damaged unit to the other to make one complete unit". As a consumer with a vehicle under warranty, I am entitled to a repair using new, undamaged factory parts. I am extremely concerned about the structural and functional integrity of a "reconstructed" HVAC system assembled from damaged components. Furthermore, despite the confirmed chemical emission, no professional decontamination of the interior was performed. The manufactures is now using financial threats of storage and rental car fees to force me to accept this substandard, "cannibalized" repair.Read more

To Whom It May Concern, I am submitting this formal safety complaint regarding my 2021 BMW X3, which is currently subject to an active safety recall involving a potential engine starter defect that may result in a vehicle fire.BMW of North America has notified me that water may enter the engine starter, causing corrosion and overheating, which can lead to a fire even when the vehicle is parked and
... turned off. BMW has advised owners, including myself, to park the vehicle outside and away from structures due to this risk.Since receiving this recall notice, my vehicle has displayed a check engine light, and I have personally experienced a burning or fire-like smell coming from the engine compartment. These symptoms have caused serious concern for my safety and the safety of my child.I contacted BMW regarding this issue and was informed that there is currently no available remedy to repair the defect. Despite the known fire risk and the warning signs my vehicle is exhibiting, BMW has not provided a temporary repair, replacement vehicle, or other permanent solution. This vehicle is my only means of transportation. I am a single mother and rely on this vehicle daily to transport myself and my child to work, school, and essential activities. Being advised to continue driving a vehicle with a known fire hazard, while also being instructed not to park it in my garage, places an unreasonable and unsafe burden on my family.I am not refusing a reasonable solution. I am open to trading my vehicle into a comparable BMW model while maintaining the same existing car note or loan terms. My primary concern is having a safe and reliable vehicle for myself and my child, given the unresolved safety defect and the lack of an available repair.I am submitting this complaint so that the National Highway Traffic Safety Administration is aware that consumers are currently operating vehicles with a known fire risk and no available remedy. I OWN A 2018 GMC SIERRA DENALI ULTIMATE, The truck has the 6.2 engine which had the lifter issue as stated with the current situation. At the time, the truck had 37,000 miles when the issues started. I purchased the truck from Andy Mohr with 8806 miles on it in November of 2019. I worked overseas for the DOD so that’s why the miles are low. After I got it fixed, ($4500 dollars at Hare Chevrolet
... ) I experienced the transmission shake while driving home from work with my children in it. The transmission was shaking so bad the whole cab was rocking. That repair was $3,300. Now, the truck has piston slap and rod issues and currently at Hare Chevrolet for service. I was told I need a full rebuild ($12,000 dollars or a new engine $14000 dollars). As a retired Air Force veteran, I didn’t expect issues like this, especially when I baby the truck and it currently has 40,530 miles. I had regular oil changes and service, yet now stuck with a high class truck that has serious issues that seem to be ignored by GM and Andy Mohr GMC. Andy Mohr sold me an extended warranty at time of purchase but I paid for a year and a half that wasn’t usable. If I had that warranty I would be covered now.But besides that, it’s a Denali, I shouldn’t be having a new engine at 40,530 miles. I am happy that there is an investigation but it needs to be widened because my truck was right before the new model rollout but I am having the same issues. If needed, I have all my receipts and information and will provide the service rep at Hare. The reason why its at Hare vs Andy Mohr is due to me catching them not fully servicing my truck and joy riding my Camaro ZL1 after an oil change before I went home for a funeral.Read more

Incident Date: December 11, 2025 Vehicle: 2026 Tesla Model Y Location: DFW Airport, Irving, TX System: Full Self-Driving (Autopilot / FSD) Description: On December 11, 2025, my 2026 Tesla Model Y was operating under Full Self-Driving while exiting an airport when the vehicle struck a gate arm, causing property damage and windshield/body damage. No injuries occurred, but the collision happened whil
... e the FSD system was actively controlling the vehicle. Following the collision, I requested the vehicle operational and FSD engagement data from Tesla for the incident timeframe. Tesla provided a CSV dataset; however, the FSD engagement and autonomy decision-layer data were almost entirely missing, despite the vehicle being in motion and presumably under FSD control. On follow-up requests, Tesla stated they are unable to provide additional autonomy data and that “Tesla does not collect all your vehicle data,” despite marketing the system as Full Self-Driving and collecting extensive telemetry. This raises a safety concern because: 1.A collision occurred during FSD operation. 2.Tesla is not providing complete autonomy data for safety assessment. 3.There appears to be no transparency into FSD decision-making, object detection, or control authority at the time of impact. 4.Owners, insurers, and potentially regulators cannot review how FSD behaved during a safety-related event. I am submitting this complaint so NHTSA is aware that: (1) A collision occurred under FSD control, and (2) Tesla refused complete operational autonomy logs for evaluation.Read more

I was driving on a 2-lane road early in the morning with light snowfall. There was about 1" of snow on the road, with the roads not yet plowed or treated as it was the beginning of the snowfall. About 15 minutes into my trip, I was on a relatively flat and straight stretch of the road and let up on the gas paddle to avoid going too fast -- was probably traveling ~35 mph. Although the road was s
... traight and flat at that point, my car suddenly started fishtailing, slowly turned 180 degrees and sliding to the other side of the road, ending up on the opposite edge of the road. When it hit the edge of the road, it tipped over on its right side, as apparently the dirt at the edge of the road was not as slippery as the road, and the SUV having a high center of gravity. Since my car was an AWD hybrid vehicle, I believe that the triggering event was the vehicle shutting down the gas engine and switching to EV mode when I started coasting (that usually happens when I'm driving <40 mph and the vehicle is sufficiently warm). But I did not look at the dashboard to see if the EV mode switched on at that point. In addition, while I did not sense any skidding prior to this event, it's possible that there may have been a slick spot right at that point in the road. But it seemed to me that when the power to the front wheels was reduced from the gas engine shutting down (as I believe was the case), the transition to electric motors taking over the front drive created some perceived traction issues which triggered the electric motors in the back to try to compensate for the perceived traction issues in the front. But the rear drive ended up creating a significant differential where the right rear wheel was driving much harder than the left rear wheel, creating the car spin and slide, which the traction control did not correct. I did not apply the brakes nor gas once the car started skidding. The car was 10 years old and was a total loss as a result of tipping on its side.Read more

The rearview camera intermittently fails when the vehicle is shifted into reverse. The image flickers, becomes distorted, upside down, or goes completely blank. This is a known issue covered under Recall #59C, but the dealer stated there is currently no available remedy. The failure makes backing up unsafe and unpredictable.

While using the vehicle’s BlueCruise driver assistance system, the driver monitoring system generates frequent false driver-attention alerts despite normal posture and active attention to the roadway. The system often fails to recognize attentive driving unless the driver deliberately exaggerates head position or facial expression. Routine and unavoidable behaviors—including briefly touching t
... he face, wiping the nose, or checking side-view mirrors for traffic—frequently trigger alerts, even though these actions are part of safe driving. These false positives create alert fatigue and increased cognitive load, forcing the driver to divert attention from driving to manage the monitoring system itself. The intrusiveness of the alerts becomes distracting and undermines the intended safety benefit of the system. Due to the frequency of false alerts, the driver is often compelled to disengage BlueCruise entirely. I am concerned the current driver-monitoring calibration may inadvertently increase distraction for attentive drivers and warrants review. This issue has been reported to the manufacturer and logged under Ford Product case number [XXX]. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
