This complaint is submitted to supplement and clarify a prior NHTSA submission regarding Safety Recall 23TA15 for VIN [XXX] . The prior submission was brief and did not fully capture the timeline and circumstances described below. I purchased a 2021 Toyota Sienna (VIN [XXX] ) from Avondale Toyota on [XXX]. At the time of purchase, I was not informed of any active or pending safety recall affecting
... the vehicle. After returning from Arizona to San Diego with my family, I attempted to list the vehicle on a rental platform on December 26, 2026. At that time, the platform flagged the VIN as having an active safety recall (Safety Recall 23TA15) and prevented the listing. This was the first time I became aware that the vehicle was subject to a safety recall. Despite multiple attempts to obtain clarification from Toyota Brand Engagement regarding the VIN-specific recall activation date, Toyota declined or failed to provide written confirmation. Whether the recall was active prior to the sale or applied shortly thereafter, the vehicle was placed into use without proper disclosure, warning, remedy, or guidance. As a result, my family and I unknowingly operated the vehicle, including interstate travel from Arizona to San Diego, without awareness of a potential safety risk. Had appropriate disclosure or timely notification been provided, I would not have used the vehicle or exposed my family to potential harm. In addition, the recall rendered the vehicle unavailable for its intended business use, resulting in loss of use and financial impact while the remedy was unavailable. I am submitting this supplemental complaint to ensure the full timeline, safety exposure, and lack of disclosure related to Safety Recall 23TA15 are accurately documented. On Feb 3, 2026, I purchased a used 2023 Ram 2500 with ~40,107 miles from a large national used car dealer for ~$46,000. I drove the vehicle with my son as a passenger. On Feb 4, I had it inspected by a third-party shop. Both upper ball joints were worn beyond spec — the wheel moved in and out by hand on the lift, indicating imminent wheel separation risk. All 4 shocks and both front struts were
... leaking oil. All 4 tires were cupped from prolonged suspension failure. Front brakes near end of life. Estimated repairs: ~$7,000. These components do not fail at 40K miles under normal use, indicating prior abuse/neglect that existed before sale. The dealer's pre-sale inspection failed to catch an obvious, critical safety defect. I wanted to keep the vehicle and have it repaired. The dealer refused to authorize outside repairs, insisted on doing it themselves, but could not even look at it for over a week. They did not request a copy of the inspection report documenting the defects. I was forced to return the vehicle the same day. The ball joints posed a wheel separation hazard. My son and I were exposed to this risk on public roads. This raises serious concerns about the adequacy of this dealer's vehicle inspection process.Read more

I have a 2026 F-250 diesel with 6K miles on it that caught fire about 3-5 minutes after using it to jumpstart a UTV. Jumping the UTV went just fine as expected, and I disconnected the cables and shut the truck hood with no sparks, smoke, or flames at
... the time. Within 3-5 minutes, I noticed there were flames coming from inside my truck at the center dashboard area. We attempted to extinguish the fire ourselves, but the fire department was ultimately called. If my son had not gotten out of the truck to get into the UTV with me, he would have been in the back seat when the fire started. The truck was pulled close to my barn when the fire started, which caused damage to the barn. A fire investigator hired by my insurance, and the fire marshal came out to my home to inspect the truck, and both determined it was not my fault the truck caught fire. There were no warning labels indicating the drivers side battery should not be used to jump start vehicles. The fire investigator stopped at a local ford house, and none of the employees were aware the drivers side battery should not be used to jumpstart vehicles either. I found information online that mentions fires occurring in 2025 & 2026 due to cracked fuel return lines, and engine heater block problems. The fire investigator stated if I was able to find that information online, that the manufacturer likely knows there are problems, and chose to sell the vehicles anyways. My truck is currently waiting to be inspected by a Ford Engineer to try and determine the cause. This fire put myself, my son, barn, and animals at risk. If Ford knows about any potential fire hazards, they should have never released the truck for sale. Due to the fire, I had smoke inhalation, and visited my PCP a few days after. I'm still waiting for the fire departments report.Read more

Hyundai issued recall 292 "A VEHICLES THAT FAILS TO COMPLY WITH THIS STANDARD MIGHT HAVE INCREASED RISK OF INJURY TO THIRD ROW OCCUPANTS DURING CERTAIN CRASHES INCLUDING A ROLLOVER." on January 22, 2026. As of the time of this filing, February 4, 2026, there is no remedy available. The Palisaid I own is impacted and we are unable to safely use the vehicle as intended for 3 of the 7 vehicle seats.
... I have contacted Hyundai Corporate and was assigned a case manager; as of February 3, 2026, his response was "I completely understand that one of your primary reasons for purchasing this vehicle was to accommodate your family using the third-row seating, and I truly apologize on behalf of HMA for the inconvenience this situation has caused. At this time, because the vehicle remains operational, we are unfortunately unable to offer compensation or an alternate vehicle until an official remedy becomes available. Please know that we recognize how frustrating this experience can be, and we appreciate your patience while we await further updates." I do not consider the vehicle "operational" when occupants of the seats may be more injured during a crash.Read more

System/Component Malfunction: Systemic failure of the onboard computer (Infotainment/Connectivity Module) and Advanced Driver Assistance Systems (ADAS), specifically the Collision Avoidance/Auto-Braking system, Rear-view Camera (FMVSS 111 non-compliance), and the 12V Battery management system. All components remain in the vehicle and are available for inspection. Safety Risk: The vehicle exhibits
... frequent "phantom braking" where the emergency collision avoidance system abruptly activates without obstacles present, creating an immediate risk of collisions. Additionally, the "hands-on steering wheel" sensor malfunctions, triggering emergency stopping procedures while driving. The backup camera intermittently fails (black screen), causing a total loss of rear visibility. These issues lead to unpredictable vehicle behavior in high-speed traffic. Confirmation & Inspection: These issues began to be reported to an authorized Volvo dealer within the first year of ownership (2022-2023). Most recently, a total vehicle failure occurred due to a drained 12V battery, which the dealer service department confirmed was caused by a known software module defect. Despite multiple "software updates" by the manufacturer, the safety-critical ADAS malfunctions (phantom braking) have recurred three times in the last week following the most recent dealer service.Read more

NHTSA Vehicle Safety Complaint Report I am submitting this complaint regarding a serious safety defect involving the rear seatbelt in my 2025 Hyundai Tucson Limited. The vehicle currently has approximately 6,000 miles, and this issue has created an extremely dangerous situation for passengers. While traveling out of town, the rear passenger seatbelt suddenly jammed and would not retract or relea
... se properly. The belt became completely stuck and unusable. Three adults attempted to safely unjam the seatbelt, but it would not retract or function as designed. Because the restraint system failed, the rear seating position became unsafe and unusable. We were forced to arrange alternate transportation for our passenger because it was not safe or legal for them to ride without a functioning seatbelt. This failure represents a major safety concern. A seatbelt is a critical safety component that should never lock up or become permanently jammed under normal use, especially on a nearly new vehicle with very low mileage. The inability to secure a passenger created a hazardous situation and could have resulted in serious injury if an accident had occurred. I am contacting Hyundai Customer Affairs today to formally report this safety issue. I will also be taking the vehicle to the dealership for inspection and repair, as I do not feel it is safe to drive the vehicle until the defective seatbelt is properly repaired and confirmed safe. Given that seatbelt failures directly impact occupant safety, I believe this issue requires immediate investigation. A restraint system that becomes stuck and cannot be used is unacceptable and poses a significant risk to drivers and passengers. I am requesting that NHTSA review this defect, investigate potential manufacturing or design problems, and determine whether other vehicles may be affected.Read more

I had parked my vehicle in front of my house on Friday, January 30, 2026 at 1:30a.m. I had turned on my vehicle's engine via Acuralink app at 5:20p.m. on that same day. so that it can be warmed up to go to work. I then contacted my landlord and super since the vehicle was parked in a location where it was visible to my building cameras but no one was near my vehicle during the time the rear windshield had broken. I proceeded to do my research online and discovered that this is a constant occurrence with Acura RDX models from years 2012-Current. The issue is with the defroster when the vehicle engine starts. I took my vehicle to Paragon Acura where they proceeded to take pictures of the vehicle and the damage and replaced the vehicle with an aftermarket windshield, stating the issue is with the windshield which is incorrect, the issue is with the defroster. They assured me this will not happen again due to them using aftermarket glass and this is a one time goodwill repair. Glass is still in my trunk door as I was unable to leave the vehicle for cleaning. I explained to Acura this can not happen due to having 3 young children who place their school bags in the trunk of the car, it could have shattered while they were putting their bags in the trunk as they do daily. There was absolutely no warnings or trigger before the glass had broken or given any signs of breaking.Read more

Safety complaint: intermittent uncommanded rear-seat folding while driving; rear (3rd-row) seat belt anchorage integrity. Issue 1 — 2nd-row right seat: Since the purchase, the right-most rear passenger seat in the 2nd row intermittently activates its seat-folding motor while the vehicle is moving. In each event, a passenger was seated in that seat with the seatbelt fastened. The motor engages (
... audible whirring), and the seat attempts to fold fully forward; the passenger’s body prevents a complete fold, so the seat partially collapses while occupied. This has never occurred when the seat is unoccupied. The most recent documented event times provided to the manufacturer for electronic log review (CST): • Fri Jan 23: 7:40 AM; 7:41 AM • Mon Feb 2: 7:26 AM; 7:29 AM; 7:53 AM; 7:54 AM The manufacturer requested these exact times to check vehicle logs, but reported they could not find evidence in the logs and could not reproduce the issue during service/test drive. Issue 2 — 3rd-row seat belts: During a service visit on Feb 3, 2026, the manufacturer found that the 3rd-row seat belt anchors were not secured to the vehicle (documented as a safety concern) and performed a repair/adjustment to secure the left 3rd-row seat belt, then validated its operation afterward. The manufacturer had also previously documented (March 17, 2025) that the 3rd-row seat belts were not properly mounted and were unsafe/operational for passengers, but took no action in March 2025. No crash, fire, or known injuries to date. Request to NHTSA: Please log and review this as a potential safety defect involving (1) uncommanded activation of a rear seat-folding mechanism resulting in partial seat collapse while the seat is occupied and the vehicle is moving, and (2) rear (3rd-row) seat belt anchorage/installation integrity, based on recurring real-world events and manufacturer service documentation.Read more

A recurring malfunction occurs with the infotainment/radio and rear backup camera display. The rear backup camera view remains stuck ON while driving forward in DRIVE at speeds over 20 mph, including highway speeds. The system also cycles between screens (including switching to the camera view), and the infotainment freezes and becomes unresponsive (loss of screen/control). Audio playback and hand
... s-free calls become staticky/distorted. These events create a serious driver distraction and safety risk while driving. This has occurred multiple times, including during long-distance driving, and has been significant enough that I have had to pull over to the side of the road multiple times. In multiple incidents, the system did not recover until I pulled a fuse to reset/shut down the system so I could continue driving safely. The unit remains powered ON after the vehicle is turned OFF and exited. There is also a history of battery drain/no-start events requiring jump starts. No warning lights or warning messages are displayed before or during the failure; the system becomes unresponsive and behaves unpredictably. The problem is intermittent and has occurred multiple times over the life of the vehicle, including after a head unit replacement. Dealers have documented the concern on repair orders; during the most recent visit the dealer noted no diagnostic trouble codes (DTCs) stored.Read more

While driving on a highway, the vehicle intermittently suffers a sudden loss of engine power during acceleration, accompanied by a flashing check engine light. The most serious event occurred while merging into traffic when the vehicle failed to accelerate normally and briefly lost power, creating a risk of being struck by faster-moving vehicles. Pulling over and restarting temporarily restores no
... rmal acceleration, but the problem recurs. The malfunction appears consistent with an engine misfire/powertrain fault. An OBD scan has shown misfire-related diagnostic codes (random/multiple misfire and a cylinder-specific misfire). The condition is intermittent and often occurs after approximately 10–45 minutes of driving. The vehicle remains available for inspection. The vehicle has been presented to authorized service departments multiple times. Dealers reported they were unable to duplicate the issue during road testing and no lasting repair has been completed. The manufacturer has been contacted and the vehicle has been evaluated through authorized service channels, but the underlying cause has not been identified. This remains a safety hazard because it reduces acceleration in traffic. Incident list: • 10/25/2025 (~12:25 PM; ~3,108 miles): Loss of power while merging; flashing CEL; restart temporarily resolved. • 10/27/2025–11/04/2025: Intermittent recurrence, usually lasting a few minutes then clearing. • 11/05/2025 (~8:01 AM; ~3,641 miles): Flashing CEL and misfire/loss-of-power symptoms; taken for service. • 11/26/2025 (~10:06 AM): Another episode consistent with the same pattern. • 12/17/2025 (~8:21 AM): Another episode, including during travel to a service visit. • Ongoing: Episodes after ~10–45 minutes; sometimes requires restart to restore drivability. Observed codes: P0300 and P0301. Safety risk: loss of power during acceleration/merging increases risk of rear-end or side-impact collision.Read more

Vehicle: 2026 Honda CR-V Issue Type: Parts – Design Vulnerability / Theft-Related Defect I am submitting this complaint regarding the front radar/calibration sensor located behind the Honda “H” emblem on my 2026 Honda CR-V, which was stolen due to what I believe is a serious design vulnerability. The sensor is positioned in a manner that makes it exceptionally easy to access and remove, ev
... en without specialized tools. As a result, my vehicle was targeted shortly after purchase, the sensor was stolen, and my vehicle was rendered unsafe and undrivable due to disabled safety and driver-assist systems. This is not an isolated incident. I have since learned that this specific sensor placement has become a known and repeated target for theft, impacting Honda owners nationwide. The accessibility of this critical component creates an unreasonable risk of theft, repeated financial loss, vehicle downtime, and safety system failure for owners. To make matters worse, the replacement sensor is currently on national backorder, leaving me without my vehicle for an extended and indefinite period. I purchased this vehicle expecting reasonable protection of essential safety components and did not anticipate that a core system could be removed so easily and leave the vehicle unusable. I am requesting that Honda: 1.Formally document this as a design vulnerability 2.Review and redesign the placement and protection of the front sensor 3.Provide guidance, support, or remediation for affected owners 4.Consider goodwill assistance due to the financial and logistical burden caused by this design issue This complaint is being submitted not only for resolution of my situation, but to ensure Honda addresses a preventable design flaw that continues to place owners at risk. I look forward to Honda’s response and a meaningful resolution.Read more

The rear window has shattered twice (2023, 2026). Both times, the vehicle has been parked in a lot with cameras. After reviewing the cameras no one was near the vehicle. After the first time, I became aware that this was an issue with my model. I contacted the manufacturer and they expressed that my year was not affected by the defect. It just happened again. I contacted the manufacturer again, th
... ey expressed, that in order for them to do anything a recall has to be put in place and to report it to you. In addition, I do not put on the rear defrost. I have been driving around with shards of glass in the rear door. Safelite has expressed, due to how the manufacturer built the door, they can not remove all the glass. Also, every time the door is lifted pieces of glass fall out.Read more

I can have the vehicle available for inspection. I have to hold the truck door shut while driving it when the issue happens. There has been no confirmation of the issue by a dealer or independent service center. There has been no inspection of the problem by the insurance company, manufacture, police or others. This year during cold weather the issue started. I bought the vehicle used. ---------
... ----------------------------------------------------------------------------------------------------------------------------------- I have a 2015 f150 and when it gets cold i can open my driver's side door, but when i go to close the door, it won't latch. The inside door handle is really loose, like it's not attached to anything. Once the vehicle heats up, the door handle gets stiffer, like it is connected and I'm able to close the truck door. This only happens when it's extremely cold outside. I was told by a ford dealer that this is not a recall door actuator problem. They think it may be the door cable that connects to the inside door handle. They stated it may or may not be a warranty replacement and not a recall. They say it would be around $149 to diagnose the door problem, and if it's not a warranty replacement, I will have to pay the diagnosis cost and repair cost. They say the only way to know if the door repair is under warranty is for them to go inside the door and diagnose. So this is my concern. I have an extended warranty on the truck and it runs out at 147,000 miles. I'm at 144,000 miles. If I take the vehicle to them and they take the door apart and then say it's not a warranty or recall issue, this could cost a lot of money. With all the problems that the F150 has with the door lock problems when the temperature gets cold, it seems to me that Ford should know right away that my truck door problem is a common problem and it should be covered immediately under a recall or warranty. I bought the truck used. Let me know your thoughts on this.Read more

the front monocular camera is defective causing the safety features (adaptive cruise control, collision warning, lane shift warning, auto brights, auto braking) to not function at all. This seems to be a known issue with many honda owners. The warnings show every time I start the van. This issue has been verified and the repair is costly.

Steering wheel sticks when making gradual turns in curves when around the 11 o’clock and 1 o’clock positions. Living in a state with snow and ice makes driving scary, not knowing if my steering wheel will stick in a curve. Took it to a repair shop, but they said it would need to be looked at by the dealership.
