I own a 2019 BMW 540 with B58 engine and I have begun noticing abnormal rattling noise from the engine area, which may indicate early stage failure of the oil pump assembly. There are widespread reports from other owners of failure of the oil pump rotor housing/adjusting ring due to heat-related degradation of plastic components. Failure of this component can lead to sudden loss of oil pressure, d
... rivetrain malfunction warnings, and ultimately engine seizure while driving. Sudden engine failure at speed presents a significant safety hazard due to loss of propulsion and potential inability to maneuver out of traffic. BMW has reportedly updated the oil pump design to an all-metal version around mid-2020, suggesting awareness of a design failure. I am submitting this report as a potential early indicator before catastrophic failure occurs.Read more

Yes. The vehicle has experienced multiple critical system failures, most notably involving a defective SX6 module, which is a central control unit responsible for coordinating key electronic, safety, and communication systems. Its failure has led to widespread issues, including persistent warning messages, driver assistance malfunctions, MMI system instability, and ultimately a complete high-volta
... ge system shutdown while driving. The repair is extensive, requiring full module replacement, software programming, and system recalibration across interconnected systems. The vehicle has remained inoperable due to significant delays in obtaining this part. This defect appears to be widespread among this vehicle’s make and model, as evidenced by numerous owner reports and forum discussions describing similar SX6 module failures with limited long-term resolution. The vehicle and all affected components are available for inspection upon request. The safety risk has been severe. The lane keep and lane centering system has shut off abruptly at highway speeds without warning, causing the vehicle to drift or swerve out of its lane and creating a high risk of collision. More critically, the vehicle suffered a complete high-voltage system failure while driving on public roads, resulting in a sudden and total loss of power without warning. This creates an extremely dangerous situation, as the vehicle cannot maintain speed or safely respond to surrounding traffic, significantly increasing the risk of an accident. Yes. The issues have been reproduced and confirmed by an authorized dealership, which also diagnosed a failed module requiring replacement. No. The vehicle and its components have not been inspected by the manufacturer, police, insurance representatives, or other third parties. Yes. Numerous warning lights, system notifications, and other symptoms began appearing within a few weeks after purchase. The vehicle was purchased in September 2025.Read more

Took care & kept up with car maintenance quite regularly, including oil and transmission fluid drain & fills. No signs of any issues. Drove it a little further (i was already super close to my destination) and within a minute the car is violently jerking. Loud metal bangs a
... nd clank sounds and lost power. RPMs up but can’t get into gear. It will jerk & move by itself at idle because can’t stay in park either. Still no warning message. Got it towed—dealership found NO transmission codes, but found one code, p0219, engine over speed; most likely from the RPMs spiking due to the slippage. The trans fluid level was ok but it was black and burnt, despite being serviced months ago. They confirmed it will need a total replacement transmission.Read more

After shutting the engine off for awhile, I was able to drive the vehicle with caution. I reached out to two Genesis service departments regarding my vehicle failing to accelerate while driving and neither had heard any previous complaints, and the earliest appointment would be mid April. There was no sense of urgency, as I'm left to hesitantly drive my vehicle at the risk of it happening again and getting rear ended. And when my vehicle does go in for service, I'm weary whether the problem will be resolved properly since the service department is unaware of the issue. In addition, I have had this vehicle for only 4 months and 1 month into it, I already took it to service because of fuel odor that had not gone away since the day 1. But the service department didn't find anything wrong with the vehicle, so nothing was done to it. I also swung by another Genesis service department, in case they happened to be familiar with the cause of the odor. My vehicle still has the fuel odor and now I'm wondering if that is related to my vehicle not accelerating. I am seeking your assistance to ensure that the problems with my vehicle are confidently resolved. And I wouldn't want this to happen to anyone else. My Mercedes Benz GLE450e Hybrid SUV has had a persistent but sporadic safety issue that suddenly causes the vehicle to go into "turtle mode" with a "performance extremely limited" warning when trying to accelerate quickly or sometimes when breaking quickly. In turtle mode, the car quickly loses acceleration, drops in speed and is then unable to accelerate properly. The sudden drop in speed and in
... ability to accelerate causes a major safety issue while driving - particularly on roads with other vehicles also travelling at high speeds. Last Friday, I was driving with my family on Interstate 95 when a tractor trailer truck did not see me beside it and started to switch into my lane. I tried to quickly accelerate to get out of the way before it hit us but my car went into "turtle mode" and the speed suddenly dropped by 15 or 20mph and would no longer drive correctly. Luckily, I was in the right lane and able to swerve onto the shoulder of the highway and narrowly avoid the large truck. Otherwise, if I had been in a middle lane, I would have either been hit by the truck or likely hit another car next to me. This issue has been sporadically occurring with my car for about 5 months now. I have had it into the Mercedes dealer three times and, each time, they say they think they fixed the issue. But, each time the same issue has returned. They said that Mercedes USA knows it is a big issue with their new hybrid SUVs but Mercedes doesn't yet know how to fix the problem and has been unwilling to publicly acknowledge the problem or offer it's customers any assistance. Doing a quick Google search online for "Mercedes hybrid performance extremely limited" also quickly brings up different online forums where other owners of this same car have had the exact same issue that Mercedes has been unable to fix or resolve. Driving a car that can suddenly lose the ability to accelerate is a major safety hazard and I would sincerely appreciate if the NHTSA would help to address itRead more

2021 Hybrid CR-V - within the last 10 months the vehicle has twice lost engine power while driving on expressways with just enough time to pull off the road. In the same period of time, at least 6 times, when initially starting the engine, the engine runs roughly and will not accellerate. So far, this has happened in cold or cool weather but not extremely cold weather. In an effort to restart th
... e system, we then shut the system off, wait a few minutes and try restarting. Occaisionally, the engine will start normally after this "reset". More often, if the "reset" doesn't work, we shut off the heat/cooling system (to keep the system from calling for the engine power) and restart the system in EV Mode. The car will begin moving in EV Mode. The engine then starts and runs normally when accellerating beyond the EV system's capacity to accelerate. We understand this problem does not register in the car's computer as a "fault" once the car's engine operation returns to normal. Therefore, the dealers are unable to diagnose the problem. We have read of many other people having the same experience without any satisfactory correction from the dealerships or mechanics. Because this is an intermitant and unpredictable failure, and because the engine and system operates normally for long periods between incidents, the error must be computer/electrical and not related to the fuel pump. We have also replaced the aged engine battery and cleaned the cables. This did not correct the problem. Any assitance in resolving this issue is appreciated. We are concerned that the system will fail completely at the most inopportune time, leaving someone stranded on the highway or being a road hazard on a highway.Read more

Year: 2022 Make: Chevrolet Model: Silverado 1500 Engine: 2.7L TurboMax (L3B) VIN: [XXX] Mileage at Failure: 119,139 Description of Problem: 2022 Chevrolet Silverado 1500 2.7L L3B TurboMax engine is completely inoperable due to Cylinder 4 exhaust roller follower failure causing exhaust camshaft failure and bent exhaust valves. Vehicle shakes severely, will not accelerate, check engine light on,
... and has gone into limp mode. Diagnostic codes P0304 (Cylinder 4 misfire) and P2101 (electronic throttle actuator fault triggered by engine shaking and limp mode) were recorded. All external components including spark plugs, coils, and injector harnesses were inspected and ruled out as causes. Failure is confirmed mechanical and internal. This failure was diagnosed and documented in writing by an authorized Chevrolet dealership on March 12, 2026 (Repair Order XXX, Service Advisor [XXX] ). The dealer's written estimate states verbatim: "Failed Cylinder 4 exhaust roller follower failure causing exhaust camshaft failure and bent exhaust valves. Cylinder head replacement required to correct this concern. Direct cause of engine noise and misfire condition." Dealer repair quote: $10,371.34. A second independent shop confirmed the same failure and cited Active Fuel Management (AFM) and Direct Fuel Injection (DFI) as known contributing design factors in this engine's oil consumption and internal wear pattern. That shop quoted between $11,876.88 and $14,897.35. This is a known pattern in the L3B engine platform. Consumer Reports has documented a 4% catastrophic failure rate in this engine. Multiple NHTSA complaints exist for this same failure across 2019-2022 model years. GM has issued technical service bulletins but has not issued a recall. I am reporting a safety concern involving BMW vehicles equipped with the B58 Technical Update (B58TU) engine, commonly found in model years 2019–2021. These vehicles utilize a variable-displacement engine oil pump containing an internal thermoplastic adjusting component responsible for regulating oil pressure. Field failures are increasingly being reported in which this internal component fractu
... res, resulting in sudden loss or instability of engine oil pressure. The failure frequently occurs during cold-start operation in low ambient temperatures. When the component fails, oil pressure regulation becomes erratic or is lost entirely. In many cases, drivers receive little or no actionable warning prior to engine damage or vehicle disablement. Reported precursor symptoms include inability to electronically measure engine oil level, abnormal oil pressure behavior, or sudden drivetrain malfunction warnings. Failures may rapidly progress to engine shutdown or severe mechanical damage caused by oil starvation. Loss of lubrication can cause the vehicle to become disabled without warning while driving or shortly after startup. This creates a safety hazard, particularly when vehicles become immobilized in active traffic or during winter conditions the cold is when they fail the most, where occupants may be exposed to freezing temperatures while awaiting assistance. Replacement of the oil pump requires extensive disassembly of the vehicle and repairs commonly exceed $6,000–$8,000 for just the pump, assuming the motor is unharmed. Owners are currently unable to determine whether their vehicles contain the original thermoplastic oil pump design or a later revised version utilizing metal internal components, as VIN-based parts information now identifies updated components in the car even when no replacement has occurred and the old components are actually in the car. Also, BMW has actively removed the plastic part number out of all dealer networks as well. They are aware.Read more

I do not know what it was but it impacted the front skid plate and seems to be no other damage from the impact. There was no contact with the bumper, wheels or otherwise. There was no sudden change in speed or loss of control at all. The most dangerous result
... of this was the deployment of all the airbags (except passenger front airbag, there were no passengers). A police report was filed with Culpeper County Sheriff (case# 2603-0033) and my insurance agency (USAA claim # 016632631-800). The shop where the vehicle was towed (Ace Collision in Madison, VA) expressed surprise at the total deployment of the air bags given the scenario. There were no warning lamps on, the vehicle was in perfect operating condition and all recalls have been addressed according to CarFaxRead more

My vehicle was inspected by a mechanic after experiencing sudden loss of power and stalling while merging onto the expressway in active traffic. At the time of the incident, the vehicle became unable to accelerate properly, creating a serious safety hazard due to surrounding vehicles traveling at highway speeds. The mechanic determined that the fuel pump had failed. During inspection prior to rep
... lacement, the mechanic identified metal shavings present in the fuel system, which appear to have originated from the fuel pump. These metal fragments contaminated the system and pose a risk of further engine damage. This failure appears to be related to a known issue involving fuel pump defects in Ford vehicles. The design and/or function of the fuel pump may allow internal deterioration that leads to system-wide contamination and potential engine failure. This issue presents a serious safety concern due to the risk of sudden stalling and loss of power while driving, particularly during high-speed merging conditions.Read more

My 2018 Volkswagen Tiguan experienced a failure of the airbag control module caused by water intrusion through the HVAC access area and failed seam seal tape at the cowl support. The dealership confirmed that water leaked through a factory seam, accumulated at the airbag module, and caused the module to fail. The airbag warning light is illuminated, and the system is inoperable. This failure matc
... hes the same defect pattern covered under Volkswagen’s recall campaigns for the Atlas and Atlas Cross Sport (water intrusion leading to airbag control module failure), but my VIN is not included. The root cause and safety implications are identical: water entering through a body seam/HVAC area and compromising the airbag system. This is a safety‑critical issue that disables the airbags and creates a risk of injury in a crash. I am requesting NHTSA review this defect for potential recall expansion. Dealership Findings: • Water present at airbag control module • Water leaking through HVAC access hole • Failed seam seal tape at cowl support • Water accumulation damaging the airbag module • Cowl trim cracked and repaired • Recommended replacement of airbag control module and seam sealing tape • Estimated repair cost: $1,377 + tax Safety Concern: Airbag system is disabled due to water intrusion from a factory seam. This creates a risk of injury or death in the event of a collision.Read more

The frame of the back window is cracking allowing water to come in through the back glass. This is causing the headliner and other components such as air bags and other electrical components to get wet and damaged. This could cause risk of fire or failure of safety components within the vehicle such as air bags, cameras and other sensors. With water intrusion comes the high risk for mold to devel
... op and cause serious health risks. GMC is aware of this issue and will not agree to repair it. A GMC dealer has inspected the glass and said it was a faulty seal but since it's not in the 3 year warranty window, it's not covered. Independent auto glass companies state they have seen this issue many times and the glass seal is faulty. GMC should do the right thing and fix this issue. This devalues the vehicle and the GMC brand and poses serious risks to driver. I have created a case with GMC and submitted photos, videos and dealership estimates for proof.Read more

On Friday, 1/30/26, My daughter, granddaughter and grandson were heading out of town and tried to lower the 3rd row seat on the driver's side to put our luggage in the back. My [XXX] granddaughter was buckled in the second row seat. The 2nd row seat button was pushed. My granddaughter started screaming about the seat coming down on her. We kept trying to stop it, it finally stopped (we were l
... ucky). I ran to my granddaughter and she was completely squashed in the seat and could not get out. She is tiny, roughly 40 lbs, the opening was roughly 6" to 8". It was not easy but I was able to pull her out. We did not touch the button or try to move the seat until I arrived back home on Saturday night, 1/31/26. My husband pushed the button and was able to place the 2nd row seat back into place. This was really scarry and with all the sensors the Palisade has, I am not sure how this happened. Prior to purchasing this car, we did our research on the safety and reliability of 2026 SUV's. What is the length of time Hyundai has to recall and repair? This is really a liability to everyone. The vehicle is experiencing excessive engine oil consumption. The engine was found by the dealership to be low by 3 quarts of oil after about 1000 miles during an oil consumption test, indicating abnormal oil loss. The suspected failure involves internal engine components, possibly piston rings, allowing oil to enter the combustion chamber and burn. This condition creates a safety risk because the
... engine can become critically low on oil, without warning, potentially leading to engine damage, loss of power, or engine failure while driving. The problem has been reproduced and documented by the dealership during oil consumption testing. However, there are inconsistencies in the dealership’s reporting, as later documentation stated that no oil consumption was found despite earlier records showing excessive oil loss. This vehicle has been inspected multiple times by an authorized dealership and is currently part of an open case with the manufacture. The vehicle was also held at the dealership for approximately three weeks without clear documentation of work performed. There were minimal warnings indicators prior to the issue. The vehicle showed rough engine performance, which was consistent with oil fouling the spark plugs. No clear warning light reliably indicated the severity of the oil loss before the condition was discovered during testing.Read more

2023 GMC Yukon XL 6.2L V8 experienced progressive engine defects starting in first year. At 7,445 mi (Jan 2023), first dealer visit for engine issues. At 10,315 mi (Apr 2023), Cyl 2 fuel injector failed (internally open), vehicle TOWED to dealer, replaced under warranty. At 14,982 mi (Aug 2023), identical Cyl 7 injector failure (internally open), replaced under warranty. Two identical failures on
... different cylinders within 15,000 mi indicates systemic defect. By 30,409 mi (Aug 2024), dealer performed engine decarbonization due to carbon buildup from fuel injection issues. Dealer recommended additional decarbs at 38,077 mi and 42,577 mi. At 56,028 mi (Mar 2026), engine suffered catastrophic failure: piston ring failure causing excessive oil consumption and engine destruction, requiring full engine replacement. This matches GM's known 5.3L/6.2L V8 piston ring defect (subject of $150M class action settlement). Defective injectors caused incomplete combustion, carbon buildup on piston rings, ring tension loss, and catastrophic engine failure. Vehicle also has unfixed suspension defect (camber bolt/alignment). Vehicle visited dealer 8+ times. Engine failure poses serious safety risk from sudden power loss at highway speeds. Vehicle available for inspection.Read more
