I am reporting a recurring safety-related defect involving driver-assistance and steering-wheel controls on my 2022 Toyota Sienna Limited, VIN [XXX] ry 8, 2026, I brought the vehicle to Parkway Toyota in [XXX] bec [XXX] tion” warning appeared and cruise control stopped working. The dealer diagnosed the issue as related to the 12V battery, replaced the battery, and advised that the condition was
... resolved. The same malfunction later recurred. I returned to Parkway Toyota on March 18-19, 2026 for the same problem. I reported that the issue had reappeared after driving in cold conditions in Canada and that when the vehicle was warm there were no warning lights. The dealer diagnosed DTC U042381 (“invalid data received from instrument panel cluster”) and replaced the hybrid vehicle control ECU. I was told the repair was complete. Within about two days, the identical “LTA Malfunction” warning returned again and cruise control was again disabled. In addition, a broader failure then developed involving the steering-wheel-mounted controls: the steering-wheel buttons stopped responding, the instrument display became stuck on one screen and could not be changed, the cruise-control buttons stopped functioning, and the radio volume could no longer be changed from the steering wheel. This appears to be an intermittent but recurring defect affecting driver-assistance functions and vehicle controls while driving. The vehicle has now had two repair attempts for the same general condition, and the problem has not been resolved. Repairing dealer: Parkway Toyota, [XXX] . Repair dates: January 8, 2026 and March 18-19, 2026. In cold weather the doors will become unlatched and open. This has happened to all 4 doors, driver side, passenger side, rear driver and rear passenger. We will be driving and the doors will randomly become unlatched and fly open. We have small children and they could have been killed due to this incident. It is extremely unsafe and problematic. We are unbelievably fortunate that our children ha
... ve not been killed due to these incidents. It can happen at any moment and the doors will not close for several hours or days once they become unlatched. Typically when the weather warms up they will close again, which can take days. We have had friends/family allow us to use their garage to park the car so the car will warm up and we can re-close the doors, only to have them fly open again at some random time. This is a very dangerous defect that poses a significant health and safety, especially to children and babies. Additionally it is a liability as it can take several hours/days before the latches will close again. This requires the use of ratchet straps to hold the doors closed as the latches will not close (even if you attempt to reset the latches manually). However, having ratchet straps running from door handle to door handle to keep the doors closed poses other safety concerns. When I reached out to the ford mechanic I was told to replace the door latches as they may be defective. I had the driver side door latch replaced, but unfortunately the problem persists (about twice in the two months since it was replaced).Read more

What is happening with my vehicle is a major safety concern in my opinion for myself and for my family. I purchased the vehicle on February 18,2026 from Ryan Ford in sealy,TX with approx 32,800 miles. The first incident was on March 16 with 33,718 miles, my daughter and I went to leave the Lowe’s parking lot and my vehicle would not go in reverse. It however would go in drive, so after being pu
... shed back out of the parking spot we left for our home.(this part of not going in reverse I do have on video) Once on the feeder road and up to approx 45mph my vehicle went completely out of gear and had to coast to a stop on the side the road. Turned vehicle off and on and it went back in gear, after making it home I made and appointment with the dealership and dropped it off that day. I was informed no codes came up and the dealership could not get the issue to duplicate so I received my vehicle back. The second incident was on March 21 trying to leave the convenient store, I put my vehicle in reverse and pressed on the gas to go backward and nothing happened. Then I pressed on the gas again and it threw my vehicle in reverse and jolted my truck back several feet before I could press on the brake. Luckily no one or no object was behind me. The third incident was on April 3rd trying to leave Walmart. My truck would not go in reverse again. I pressed the gas pedal several times and out of nowhere again it threw itself in reverse and jolted my truck back again. (I also have this on video as well).Read more

I have repeatedly received an error on my vehicle dashboard stating " 1 pedal drive fault: Press break pedal to reduce speed". I have taken my vehicle to the dealer twice for this and was told both times that it was not reproducible and therefore nothing that could be fixed. I was told there was an active recall and that it has been repaired. Today after coming to a complete stop I let go of the b
... reak and the vehicle suddenly forcefully lurched forward without me stepping on the accelerator almost resulting in an accident. After looking up the information online it seems other drivers have experienced this too and I was never warned about this by the dealer or its potential safety implications. I feel unsafe. On Interstate 65 4/4/2026 during light traffic, after a rain storm, however with no rain currently occuring, our vehicle experienced an event where the collision avoidance system dropped out speed from 70 miles an hour to 35, nearly causing a rear end clission with a vehicle behind us. Fifth wheel was purchased with a tire monitoring system included. It has not worked since we purchased it 8-19-2024. We have taken it twice to the dealer, Blue Compass and it still doesn't work. I am told the problem is they put the wrong sensors in the tires. Lippert and Grand Design are fighting about who's fault it is all while it still doesn't work. Not having the tire sensor/monitoring system
... working has created a safety issues for us. We have no idea if the tires get hot, start to lose air, all of which could happen without the system working correctly. That creates a safety issue because the camper is a fifth wheel 35' long weighing 12,000 lbs. I am reporting this because I have no where else to turn, the manufacture and camper company refuse to take care of the problem.Read more

I have a 2018 RAM 1500 Tradesman 2WD truck that the the ABS warning ligh as well as check engine, brakes warning light keeps staying on. I am very concerned about this as it may cause me to NOT be able to brake in time to. Also the cruise control does not work. I've taken it to several different mechanics as well as Autozone and I'm told the error code is C2200-49 which is the Anti-lock Brake Mod
... ule Internal- Internal Electronic Failure. My mechanic has told me there is NO PART available and has been on back order for the past year. My only recourse is to locate one from a junk yard and hope that it will work. I've done an exhaustive on-line search and haven't been able to locate the module for my year and model truck through any RAM 'MOPAR' dealership. I've been on several different message boards and have been told there are thousands of trucks out there with this issues. Please, help us by making RAM do a recall on this vehicle. My 2025 Ford Explorer pulls hard to the left, the steering wheel feels heavy, stiff, and gets hard to steer my wife and I are constantly fighting with the car to get it to steer right and stay in the lane. If I let it, I can keep the steering wheel straight and the car will just veer to the left Into the left lane, it’s dangerous, my wife won’t even drive it anymore. Then there are times while
... driving on curvy roads, the wheel or the cars steering will get stuck and if I wanted to, it will stay turning on its own around corners or curves. Also when we take a turn onto another street sometimes it gets stuck again, doesn’t come back like most cars do. We have had it in the shop more than 3 times for these problems. They have replaced the steering rack/gear, done alignments, but the problem still persists we are afraid that the steering is going to get stuck, that we won’t be able to pull it back into our lane and cause a major accident. We have brought it to the dealer but say they have done what they can and need direction from Ford. I don’t know if it’s related but there has been warning lights but it said it was for the parking brake. I have even been on rides with a service manager from a ford dealership and it has happened but, without an answer from ford they could not do anything. This problem has going on for months now and the dealership can’t fix it I bought it September 2nd, 2025 I can’t remember when it started exactly, October or November but it has gotten worse and worse it seems by the day. The first time we brought it in was in December 31st, 2025 that’s when the dealership changed the steering rack/steering gear I tried to upload files to this form but they are too bigRead more

What component or system failed or malfunctioned, and is it available for inspection upon request? The specific component has not yet been identified. The issue involves unexplained water intrusion into the vehicle’s floor pan under normal operating conditions. The vehicle is available for inspection upon request. How was your safety or the safety of others put at risk? Water intrusion into the
... cabin creates potential safety risks, including exposure of electrical systems to moisture, possible corrosion of critical components, and degradation of interior air quality (e.g., mold). These conditions could affect vehicle reliability and occupant safety. The last time that I drove it the brakes were engaging randomly and forcefully. Has the problem been reproduced or confirmed by a dealer or independent service center? The vehicle has been inspected by a dealership; however, the source of the water intrusion has not been identified or confirmed, and responsibility has not been accepted. Neither did the dealership provide requested documentation of the leak test. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The dealership has inspected the vehicle, and a claim has been submitted to my insurance carrier. Volvo corporate has opened a case but has not conducted a technical inspection or provided findings to date. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes, there were both warning lamps and messages, and the dealership serviced and claimed that it was 90% fixed and were just waiting for a small component. They said that I could resume driving it, then the problem got worse.Read more

Vehicle: 2020 Kia Sportage Issue: Excessive Oil Consumption — TSB #TSB_ENG222 I am reporting a recurring issue involving excessive engine oil consumption in my 2020 Kia Sportage. The engine is consuming oil at an abnormal rate between scheduled oil changes, requiring me to add approximately 2.5 quarts of oil every month to maintain safe operating levels. This issue first began approximately 6 m
... onths ago and has been ongoing since. The affected component is the engine system, specifically related to abnormal oil consumption. The vehicle is available for inspection upon request. This issue poses a serious safety risk to me and my children. If the oil level drops too low, it can lead to sudden engine failure, loss of power while driving, or engine seizure. This creates a dangerous situation, especially when driving at highway speeds or in traffic, increasing the risk of an accident. The problem has been consistent and reproducible. It has been confirmed by an independent service center, which verified that the vehicle is consuming oil excessively beyond normal operating standards. I have brought attention to this issue through service evaluations, but no permanent repair has been provided to resolve the root cause. Prior to the issue, there were no clear warning signs. In some instances, the oil warning light only illuminates after oil levels have already dropped to a critically low level, limiting my ability to prevent potential engine damage or failure. This ongoing defect raises serious concerns about the safety, reliability, and longevity of the vehicle.Read more

Parking brake pad failure has occurred twice. Last month at 31,668 mi, truck was taken in for servicing a warning light due to TPM failure (all replaced), and I asked for parking brake inspection because I was pushing pedal to the floor in order to engage. Mechanic said "Left rear prkg brake shoe has section missing." He had never seen that before and I exclaimed that it happened first in Apr 2021
... at 18,770 mi. I am not sure which side it was since it was not written on the service invoice, but I suspect the right side because during a Nov 2021 oil change the invoice included "adjusted rear rh parking brake." OEM replacement pads were installed both times. I have the latest failed pad which shows about 2 in of material missing from the shoe substrate, not the 2021. Kearny Mesa Chevy repaired this in 2021; Walt's Danville Service in 2026. No safety issue, but could have been a problem in event of brake failure an need to stop vehicle by applying parking brake that would not hold. I asked KMChevy if this should be a warranty repair or a recall issue and they refused to consider in 2021. It cannot be coincidence that this happened to Chevy OEM equipment, I suspect to both sides. I paid ~$500 each time and feel that Chevy corporate should compensate me.Read more

My vehicle currently has only 27,500 miles, yet it has been producing a loud and disturbing thud or clunk whenever the vehicle encounters bumps in the road. This problem began earlier this year and the ride quality has progressively deteriorated. I brought the vehicle to Subaru dealer for diagnosis and was informed that the rear stabilizer bushings required replacement and was provided with part
... number 20464AN000. I was told they are back-ordered. It's been almost two months with no part in sight. This is the same part being allocated to new 2026 Outback production while existing customers are left driving defective vehicles. That priority is indefensible and unacceptable. Aside from being unpleasant, the noise raises legitimate concerns about the safety and durability of a relatively new vehicle.Read more

I spoke to the service manager at Hub City Ford located at [XXX] about the rubber around windshield was dry rotting and had came completely off. I was told that there was a recall that would fix the issue last year and so I waited for the call. During this time the vehicle started making this very loud whistling noise once the vehicle got to 40 mph. I was contacted n October of 2025 about getting
... the window fixed. We scheduled an appointment and someone came to fix the issue however once they left I notice that they only fixed to left and right side and did not fix the top and bottom of window so I called the manager back. The manager insisted that this was the only parts they were going to fix and if we wanted the rest fixed we would have to pay out of pocket which would’ve cost around $800. This is very concerning to me because why would they only fix half of the problem with no explanation on why the rest would not be fixed. Since then I have called to complain about the loud whistling that is still there and now water is leaking inside of vehicle. Since the leak started we have has problems with the electrical system and windshield wipers coming on with no rain. I really need help because this is the only vehicle I have to drive to my doctors appointments as an elderly [XXX] ), wheelchair bound female. If the vehicle electrical system were to fail I would be stranded wherever it breaks down because my vehicle carries my wheelchair on a lift. I am also considered fully disabled and so I only receive Social Security as my primary income. I can not afford the $800 cost of fixing a problem that is a clear malfunction of the vehicle versus wear and tear problem. I have not where else to turn and since I live in [XXX] now my means of getting around f consist of my daughter using my vehicle to take me to appointments because I no longer have a driver’s license. I am dependent of my vehicle and the rain in BEH is getting worse. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)Read more
