Within minutes of leaving the dealership, the vehicle stalled again in active traffic and had to be returned. The issue was escalated to the Ford Engineering hotline and a Field Service Engineer. February 17, 2026 – The dealership attempted to return the vehicle after performing a BECM reprogramming. Upon pickup, new issues were immediately observed including near-stall conditions and hybrid system malfunction, so the vehicle was left with the dealership again. February 18, 2026 - The owner brought forth concerns about Ford’s Technical Service Bulletin (TSB) 17-0015 which identifies potential damage to the traction battery interlock harness as a root cause for intermittent stalling and "Stop Safely Now" warnings. Although the dealership recorded Diagnostic Trouble Code B11D8—a fault directly related to the stall condition—the dealership later stated that no diagnostic codes relevant to the TSB were present and this component was not inspected. I am reporting a recurring safety issue with the vehicle’s auto engine stop/start system. When the vehicle comes to a stop, the engine sometimes shuts off normally (other time only the auto-brakes engage, but engine stays on), but then the system behaves unpredictably. The engine attempts to restart with a violent jolt, causing the entire vehicle to shake. This occurs even when I am fully sto
... pped with my foot on the brake. It feels like the vehicle is trying to lurch forward or restart against resistance. The issue is repeatable, happens in normal driving conditions, and creates a safety concern because the sudden jolt can startle the driver, cause unintended movement, or lead to loss of control in traffic. I believe this may indicate a defect in the auto‑engine‑stop system, engine mounts, starter system, or related control modules. Please investigate, as other owners may be experiencing the same unsafe behavior.Read more

Subject: Vehicle Fire Following ABS Recall Repair – Safety Complaint I am filing a safety complaint regarding a vehicle fire that occurred shortly after a recall repair was performed. Vehicle: Jeep (Grand Cherokee/2019) Mileage at time of incident: Approximate: 58,350 Approximately four weeks ago, my vehicle underwent a recall repair related to the ABS system at Westgate Jeep located in Rale
... igh, NC 27617. Following that repair, I experienced brake issues due to air in the brake lines, which required a return visit to the same service provider for correction. On April 5, 2026, the vehicle caught fire while parked in my driveway. The fire originated in the front driver-side engine compartment area, where the ABS and brake components are located. The fire caused extensive damage to the engine bay, wiring, and surrounding components, rendering the vehicle inoperable. Given the timing and location of the fire, I am concerned that the recall repair or subsequent brake system work was performed improperly leading to either a brake fluid leak or an electrical malfunction or defective parts that caused the fire. This incident presents a serious safety risk, as it could have resulted in injury or loss of life. I am requesting a full investigation into: - The quality and safety of the recall repair performed - Whether improper installation, damaged components, or fluid leaks contributed to the fire - Whether other vehicles repaired under this recall may be at similar risk The vehicle has not been altered or repaired since the fire and is available for inspection. Supporting documentation (repair invoices, photos, etc.) can be provided upon request. I respectfully request that this matter be reviewed urgently to prevent potential harm to other drivers.Read more

Reports suggest some Hyundai Ioniq models, including potential Ioniq 6 instances, may exhibit unexpected, brief acceleration following a software update meant to address a separate braking recall. This issue is often linked to the "N-shift" mode, where the car may accelerate for 2-3 seconds after the driver lifts their foot. I had that software updateRead more

The tires exhibit severe sidewall bulging, starburst cracking indicating internal structural failure, and at least one tire shows a deep gash with internal steel cords exposed. Three of the four original tires are affected. The fourth original tire was replaced in June 2025 after it failed during normal highway driving - the cause was attributed to a road hazard at the time but is now believed to be the same underlying defect. The replacement tire (a different unit purchased separately) shows no defects whatsoever. The safety risk is significant: three tires with structural sidewall failure were on an active vehicle for the first 6,808 miles of its life, including at highway speeds. One tire experienced an in-service failure. The remaining three deteriorated to the point a professional installer deemed them unsafe to mount. The failed tires are available for inspection. The problem has been confirmed by an independent tire service center (Discount Tire) who refused to install due to safety concerns. The manufacturer has not yet inspected the tires. There were no warning lamps or prior symptoms before the June 2025 in-service failure, and no visible signs of the sidewall damage were observed until the tires were removed from storage in April 2026.Read more

Vehicle: Ford EcoSport Issue: Overheating / Unsafe to Drive After Recall Repairs Dealership: Bolton Ford Dates: Vehicle at dealership since December 31, 2025 Complaint Text: My Ford EcoSport has been at the dealership since December 31 for multiple recall repairs. My 2024 Jeep Wrangler 4xe is subject to Recall 78C (NHTSA #25V-766) regarding sand contamination in the engine. As of April 2026, the manufacturer has no available fix for this safety hazard. Simultaneously, the vehicle suffers from per

Despite a repair attempt on March 3, 2026, where the 12V battery was replaced under warranty, the vehicle continues to display 'Aux Switches Temporarily Unavailable' errors accompanied by constant audible chiming while driving. These combined defects create an extreme and unreasonable safety risk. Because the manufacturer has admitted the engine is a fire hazard but cannot provide a repair, the vehicle is effectively unmarketable and unsafe for operation. This happens every single day, not just today, but are two separate issues related to my brakes. 1- squealing brakes. Every single day and for several miles before they warm up. The first time I brought this issue to the attention of the dealership, they replaced the brakes, and there was no squealing for just a few days. I forced the dealership within a week to let them know, I had to bring it ba
... ck in, and they said they were fine and would just squeal. To this day, I have no idea if this is safe or not, but haven’t been able to receive a message that they squeal for any specified reason, which does not ease safety concerns. 2- my app told me that I had only 18% brake pad life remaining when I was around 11k miles. I took it in and they said they just reset the sensor without fixing the brakes because they didn’t know why it was doing that - its now clear to me that this isn’t an accurate reflection of true brake life measurements and is most likely just based off of miles driven instead of a true measurement, as advertised. I love this car, but am super disappointed that my brakes are so consistently loud - it’s both embarrassing and worrisome. Component: VISIBILITY / SUN VISOR Vehicle: 2023 Toyota Corolla Both the driver-side and passenger-side sun visors in my 2023 Toyota Corolla have failed within approximately two years of normal use. The visors no longer remain in their intended position and instead fall downward unexpectedly. When this occurs, the visor drops directly into the driver’s field of vision, creating a significant safe
... ty hazard by obstructing forward visibility while driving. This condition can happen without warning and requires the driver to physically move the visor out of the way, causing distraction and increasing the risk of an accident. The vehicle has not been modified, damaged, or subjected to abnormal conditions. The visors were used only as intended. Sun exposure is a normal and expected condition for this component, and failure under these circumstances indicates a potential defect in materials or workmanship. Both visors failing in the same manner suggests a systemic issue rather than isolated wear and tear. The vehicle was taken to a Toyota dealership, where I was initially told the repair would be covered under the manufacturer’s warranty. However, the claim was later denied and classified as “wear and tear” due to sun exposure. Given the function of sun visors, this explanation is not reasonable. This issue presents an ongoing safety concern due to impaired driver visibility. I am submitting this complaint so that this defect can be reviewed and tracked for potential broader impact.Read more

I am submitting a formal safety complaint regarding a defect in my 2020 BMW X3 that has resulted in the failure of a critical emergency safety system. The vehicle displayed an error message indicating an “Emergency Call System Malfunction.” I brought the vehicle to BMW of Murrieta on March 30, 2026, for diagnosis and repair. Upon inspection, the service center determined that the Telematic C
... ontrol Unit (TCU) had sustained water damage. This damage was attributed to water intrusion originating from the vehicle’s shark fin antenna. This is a known issue documented by BMW in internal Service Information Bulletin SIB 65 12 22 (dated October 5, 2022). Despite acknowledgment of this known defect, BMW declined to cover the repair costs for both the antenna-related water intrusion and the resulting damage to the TCU. This failure presents a significant safety hazard. The emergency call system is designed to automatically notify emergency services in the event of a collision. With this system inoperable, occupants may be unable to receive timely emergency assistance, increasing the risk of serious injury or death. Given that BMW is aware of this defect and its potential to disable a critical safety feature, I believe this issue warrants further investigation and should be considered for a mandatory safety recall.Read more

Two of my catalytic converters have failed twice. The first time was at the end of January 2024, the second time was at the end of March 2026. All regular maintenance on the vehicle has been completed at a Ford dealership. The only explanation I have received for the failure of the catalytic converters was "the parts were defective." I have owned five Ford trucks in my lifetime and this is the fir
... st time the catalytic converters have failed, and they have failed twice on a five year old truck. This is, however, the first hybrid truck I have ever owned. Perhaps there is a defect in the design of the motor that the general public may not be aware of? Hopefully you can look into this situation to prevent someone else from going through the drama that I'm having to deal with. Lightening has already struck twice and I'm just praying that it will strike the third time for me before the two year warranty runs out on these catalytic converters that were just installed in my truck.Read more

My 2025 Toyota Tacoma engaged the emergency braking/pre-collision system system while my vehicle was going under 10 mph on a steep road in winter conditions. My vehicle was completely under control before the system engaged and there was no risk of a collision with any vehicles in front of me. The emergency braking system engaged and rendered my vehicle completely out of control. I tried desper
... ately to pump the brakes in hopes I could regain control of the vehicle, but the brakes were fully locked. Had the brakes dis-engaged, I could have easily avoided the collision. The car not only did not decelerate, it accelerated down the hill while the brakes were locked for several seconds. My vehicle eventually slid into another vehicle that had been parked on the side of the road. From doing extensive research I believe a perfect storm of events caused this malfunction. Previously on this day, several inches of snow had fallen. The road had not been plowed and cars were parked on the side of the road. There was also a curve in the road near my vehicle's collision where the other vehicle was parked. My windshield was also still defrosting in the uppermost section near the rear view mirror. It is also possible the radar block on the front of the vehicle had snow or ice on it. It was also actively snowing. From what I have read, any of the above factors can cause a malfunction in the emergency braking pre-collision system. I had never been in an accident in over 20 years of driving and my vehicle did the least safe maneuver it could have in the given situation. There is no circumstance where one would purposefully slam the brakes on a snowy/icy down hill road if they know how to drive in the snow (even if you were going under 10mph as I was). At the most basic level of failure, I can't imagine this system should ever engage if going as slow as I was. Also, the system is designed to decelerate (was accelerating) and I was pumping brakes in effort to disengageRead more

I am submitting this complaint as a follow-up to three prior NHTSA complaints regarding this same vehicle: Complaint #11714848 (submitted 2/1/2026) Complaint #11714852 (submitted 2/1/2026) Complaint #11716040 (submitted 2/5/2026) These prior complaints document a progression of serious electrical, charging, and vehicle control issues that have worsened over time. Initially, the vehicle experien
... ced widespread software and electrical failures affecting core driving and safety systems, including loss of power while driving, unintended system activation, blank instrument displays, and failure of safety-related features such as the SOS emergency system. These issues escalated to include severe charging system failures, inability to reliably charge the vehicle, dramatically reduced range, and intermittent propulsion loss. On February 4, 2026, the vehicle became completely inoperable, displaying multiple critical warnings including “Drive system error,” “Electrical system not working correctly,” and inability to start or move the vehicle. The vehicle was towed to the dealership on February 5, 2026 for repair. I have now been informed by both Volkswagen and the dealership that they have been unable to identify or implement a repair for these issues. Despite this, they plan to return the vehicle to me without performing any substantive repairs. I have been told that diagnostic fault codes have been cleared and that the vehicle is now considered “safe” to drive. However, no root cause has been identified, and no repairs have been made to correct the underlying defects that led to loss of power, system failures, and complete inoperability. The issues previously reported remain unresolved and include: Loss of power while driving Dashboard and infotainment displays going blank while driving Vehicle shifting into park unexpectedly Cruise control activating without driver input Charging system failures preventing reliable charging Repeated warnings indicating unsafe operRead more
