The vehicle has experienced repeated electrical and engine-related malfunctions, including random engine shutdowns while driving and random no-start conditions. The thermostat and cooling system failed and were replaced in 2024. Multiple sensors have failed. The braking system remains noisy even after brake replacement. The lane detection safety system does not provide audible alerts. The vehicle
... is currently operational and available for inspection upon request. How was your safety or the safety of others put at risk? The vehicle has randomly shut off while driving on both highways and city roads in Azle, Texas. This occurred while my two children were in the vehicle. When the engine shut off, there was a risk of losing power steering and braking assist, which could have caused an accident, especially at highway speeds and in traffic. Has the problem been reproduced or confirmed by a dealer or independent service center? The thermostat and cooling system failure were confirmed and repaired by a service center in 2024. Electrical and sensor issues have occurred repeatedly but have not been fully diagnosed or resolved. Brake noise persists despite replacement. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? UNKNOWN. The vehicle has been serviced by repair facilities, but the manufacturer has not provided a definitive diagnosis for the engine shutdown and electrical failures. Were there any warning lamps, messages, or other symptoms prior to the failure? When did they first appear? Yes. Various warning lights and symptoms appeared prior to failures, including cooling system warnings and sensor alerts. Electrical issues and battery failures have occurred repeatedly, with batteries lasting only 2–3 years. Symptoms began after purchase and have continued throughout ownership. Additional details The brakes have been noisy even after replacement, and the lane detection system does not produce audible warniRead more

I am reporting a safety defect involving the front passenger door lock actuator on my 2021 Toyota RAV4 XLE Hybrid. The front passenger door will not lock or unlock using the key fob or the interior power door lock button. The door must now be locked manually. This defeats the vehicle’s automatic locking system and creates a safety and security concern. This condition prevents all doors from be
... ing secured properly when driving or when parked. A passenger may believe the door is locked when it is not, increasing the risk of unauthorized entry, theft, or passenger safety issues. The failure occurred during normal use and without warning. The vehicle has been properly maintained. I have learned that multiple Toyota RAV4 owners have reported similar door lock actuator failures, indicating a potential widespread defect. This issue occurred shortly after the factory warranty expired. Due to the safety implications, I believe this issue warrants investigation and possible recall or manufacturer repair program. “This failure occurred without warning and with no dashboard alerts.”Read more

While driving on a public road, the vehicle experienced multiple simultaneous, dangerous malfunctions: Dashboard Warnings: All lights illuminated, including engine (flashing), ABS, traction control, airbag, TPMS, and parking brake, which stayed on though disengaged. Engine & Transmission: Tachometer fluctuated wildly in park; engine revved without accelerator input. Shifting to drive/reverse cau
... sed unexpected “kicks.” Vehicle sometimes failed to move when pressing gas, requiring engine restart. Driving Issues: Speedometer read 0 or jumped inconsistently. RPM continued erratically while maintaining speed without accelerator. Steering became extremely difficult. I immediately determined the vehicle was unsafe to drive and returned home with it, which was only a couple miles away. Follow-Up: Hyundai advised dealership diagnosis; they suggested a backup camera module will resolve the issues I experienced in entirety, but also in the same email stated that they could not guarantee it would resolve all of the issues I experienced. This is the second major engine/powertrain failure: first approximately ~70,000 miles (full engine replacement), current approximately ~95,000 miles (approximately ~25,000 miles post-replacement). Recurrence indicates a serious, unresolved safety defect. I am deeply concerned for passenger safety, including children. Other owners report similar issues; the attorney general’s office advised reporting to NHTSA. Summary of Safety Risks: sudden loss of control/power, erratic acceleration, dashboard failures, steering resistance, unpredictable handling, recurrence of major defect. I am submitting this report to NHTSA to request investigation of these recurring defects. I do have video documentation of all of this, as well as ALL written communication with Hyundai National Consumer Affairs and the dealership. I am having a hard time uploading them because of the file limits. Please let me know if any of this will be helpful to you.Read more

Incident date: Jan 23, 2026
I am writing to formally express my concern regarding serious and repeated safety-related failures with my 2022 BMW X5 xDrive45e, VIN [XXX] . I purchased this vehicle in 2022 through a custom build, at a purchase price exceeding $80,000, with the expectation of reliability, safety, and premium quality consistent with the BMW brand. In October 2025, while my wife was driving with my son in the ve
... hicle, the hybrid component safety box failed without warning, causing the vehicle to stall on the roadway. This created a dangerous situation for my family. The vehicle was towed to the dealership, repaired, and returned to me on November 4, 2025. Unfortunately, on February 3, 2026, a second serious incident occurred. After my wife picked up my son from school, the vehicle began jerking violently while driving, and a strong sulphur-like odor entered the cabin. Fearing for their safety, she immediately stopped driving. The vehicle was again towed to BMW of Jackson. I was informed today by the service advisor that the failure is due to another hybrid-related component, specifically the EME Hybrid Inverter, and that BMW North America is declining to cover this repair under warranty. These repeated hybrid system failures are deeply concerning, particularly given that both incidents involved my wife and children and posed clear safety risks. As a result, both of my children are now fearful of riding in the vehicle, and this situation has caused significant emotional distress to my family. Given the nature of these failures, the vehicle’s purchase price, and BMW’s emphasis on safety and engineering excellence, I find it unacceptable that a critical hybrid component failure would not be covered under warranty—especially following a prior hybrid system failure less than four months earlier. I am requesting that BMW North America review this matter promptly and provide a reasonable resolution, including warranty coverage for the EME Hybrid Inverter repair. Honda issued a warranty extension covering piston ring deterioration that causes engine misfires (DTCs P0301–P0304) due to oil fouling. I experienced this exact condition on 3/11/25 prior to issuance of the warranty extension, resulting in an engine misfire and loss of drivability. To restore basic operation of the vehicle, an independent repair facility replaced a fouled spark plug, documentin
... g the misfire and oil fouling condition (including the P0301 code). I elected the minimum necessary repair to restore drivability, as the warranty extension had not yet been issued and the vehicle could not be operated safely without immediate service. Honda has acknowledged that this repair was temporary and consistent with the covered condition. Honda has denied reimbursement for this repair because the piston ring or short block replacement identified in the campaign was not completed. Honda has also stated that it will not perform the covered repair unless the misfire code is currently active or stored and the check engine light is illuminated. Honda representatives have acknowledged that this condition commonly recurs after a temporary spark plug repair, but have advised that no repair can be performed unless the issue resumes. This creates a situation in which the vehicle has a known covered defect, but access to repair is delayed until failure reoccurs, potentially outside the warranty extension mileage or time limits. I am submitting this complaint due to concerns that these requirements may prevent timely access to emissions-related repairs for a known defect and may discourage reasonable interim repairs necessary to maintain safe vehicle operation. Honda’s requirement that I continue operating the vehicle until the misfire reoccurs raises safety concerns. The prior failure resulted in loss of drivability, and I am uncomfortable transporting my family while knowingly operating a vehicle with a documented defect that Honda has acknowledged is likely to recur.Read more

Component/System: Engine cooling and lubrication system. Coolant was found mixing with engine oil (milky oil), indicating internal engine failure (I was informed it could be oil cooler/housing, thermostat, head gasket, or related components). Vehicle is currently at an authorized Mercedes-Benz dealership and available for inspection. The front left inner lining of my front break pad also "failed"
... inspection and the other 3 front break pads say "caution" and I never received a notification or warning about my breaks. Safety Risk: Coolant contamination of engine oil can cause sudden engine failure or loss of power without warning, creating a serious safety risk while driving, especially at highway speeds. There was no indication that engine oil was mixing with coolant, check engine light failed to illuminate/ no diagnostic warning illuminated. The condition was discovered incidentally during routine maintenance, which prevented any opportunity for corrective action prior to the failure. Confirmation: The condition was confirmed by an authorized Mercedes-Benz dealership during routine maintenance. The dealership stated the failure was not caused by owner misuse.Inspection: Vehicle has been inspected by an authorized Mercedes-Benz dealership (manufacturer representative). No police or insurance inspection occurred, as there was no accident or external event. Warnings/Symptoms: No check engine light or diagnostic warning ever illuminated. The issue was discovered incidentally after I received a low battery voltage notification and a routine maintenance reminder indicating service was due in approximately 1,000 miles, which I arranged promptly. The manufacturer will not perform full diagnostic teardown to determine the root cause unless I pay approximately $3,000 out of pocket. The 2018 Model X experienced a sudden and complete loss of power and became totally inaccessible. All doors and access points were inoperable and the vehicle could not be unlocked, disconnected, powered on, or placed into any service or tow mode. Emergency front-trunk release cables had to be used and external 12-volt power applied at the jump terminals to wake the vehicle. The inability to access
... the vehicle was unexpected and alarming. Children, pets, elderly, and others could have been inside of the vehicle. Thankfully, they were not. Tesla inspected the vehicle and stated the failure was related to the battery coolant heater. The failure resulted in a complete vehicle immobilization and access failure. I had previously experienced restart warnings on the dashboard, but I did not expect a total lockout and loss of access. The inability to access the vehicle was unexpected and alarming. It may be a known safety issue. It should be recalled. This condition presents a serious safety risk because occupants could be trapped inside the vehicle and the vehicle cannot be accessed or moved without external power. In extreme heat or cold, or in a medical or emergency situation involving a child or pet, this failure mode could create a life-threatening situation. Importantly, even after multiple requests, I have not received a copy of the diagnostic report, or anything other than informal emails or calls. Tesla has not provided information regarding this failure other than broad statements, and visuals of the location of the battery pack, in an attempt to deny warranty coverage. The Tesla reports a High Voltage System alert currently. The vehicle is subject to Tesla’s Battery and Drive Unit Limited Warranty. Tesla has stated that the failed component was the battery coolant heater, but has refused to cover the repair under warranty. Tesla confirmed the failure but has not provided the diagnostic report, fault codes, or vehicle logs supporting its determination.Read more

I was driving and the engine abruptly stalled. The truck self-shifted to Neutral. Once stopped, I shifted to Park to attempt a restart. It would not crank. Once I realized it would not restart I tried to shift to Neutral so I could push it to a subdivision road about 50 yards away only to find out my transmission has no ability to shift to Neut
... ral (the dealer confirmed this later). So I was stuck on the road for hours just over the crest of a hill with people coming up on me and my family at 50mph. It was a windy 10 degrees so we could not wait on the side of the road where we would have been safer. This is how people get killed; being stopped where people can't see you and do not expect you to be stopped. Why can I not get the truck off the road? If this happened on an 8-lane highway how would I get to a shoulder, even after traffic had stopped behind me? On top of all that, the problem with the truck ended up being an engine bearing failure (per the dealership service dept) that is a known problem with this engine and truck build date. General Motors needs to proactively diagnose and get these engines replaced before someone gets hurt. There is a TSB issued for the bearing issue but when I asked my dealer about it a month ago, before the failure, they acted like they knew nothing about it and told me to bring it in if I hear noises. If they had offered to check the crankshaft endplay when I asked about it ( as the TSB suggests), I may not have gotten stranded and my family's safety put at risk. There were no warning lamps, and no strange noises before the engine seized. My fuel economy had decreased by about 20% in the last month but that may be from cold weather operating conditions. I have a separate electrical issue that I've been chasing down and I assumed this stall condition was related to that. But the dealer confirmed this was a bearing failure.Read more

This is happening after the initial recall for the steering was performed by Mazda on the vehicle. Car is running out of oil quickly but there are no visible leaks. Have had the oil changed a couple of times since then and have had to add oil in between because the oil light comes on when I make turns. Had the oil changed at a local oil service station and they said it was almost empty.
