I am submitting this correspondence as a formal notification of a potential safety-related defect affecting BMW vehicles equipped with the B58 Technical Update (B58TU) engine platform, most commonly model years 2019–2021. The purpose of this communication is to request immediate review under BMW’s vehicle safety defect evaluation process due to an increasing number of oil pump failures capabl
... e of causing sudden loss of engine lubrication, unexpected vehicle disablement, and occupant exposure to hazardous conditions. The original B58 engine (B58B30M0, 2015–2017) utilized a conventional mechanically robust gerotor oil pump with predominantly metallic internal construction and demonstrated strong reliability in field operation. This component, constructed from reinforced thermoplastics including PA66-GF materials, operates within a chemically aggressive and thermally cyclic environment involving continuous exposure to engine oil additives, fuel dilution, condensation, and repeated heat cycling. While acceptable under accelerated validation testing, real-world operating conditions increasingly demonstrate premature degradation consistent with polymer embrittlement and fracture. Documented owner and technician reports indicate that under cold-start conditions — particularly in sub-freezing climates — elevated oil viscosity and startup load impose increased mechanical stress on the thermoplastic carrier. Fracture of this internal component allows debris intrusion into the pump mechanism, resulting in unstable oil pressure regulation. Commonly reported precursor symptoms include: • Inability to perform electronic oil level measurement via iDrive • Erratic or oscillating oil pressure behavior • Intermittent lubrication warnings or absence of warning prior to failure • Rapid progression to bearing damage and engine seizure Given the potential for sudden vehicle disablement without adequate warning, I respectfully request BMW of North America initiate a formal inquiryRead more

I was out of town and driving on the highway around 75 MPH, when the motor suddenly lost all power. I was driving in the far left lane approaching an overpass. The sudden loss made it next to impossible to make it off the overpass on a very busy freeway safely while trying to navigate many cars trying to pass me . The truck came to a complete halt on the white shoulder line, 50' off of overpass
... SH 31. The truck would not restart or do anything i could not push it any further off road. I excited the e truck and stood off the freeway for my safety, as the truck was on very edge of freeway. GMC Roadside assistance took my truck to nearest dealer. This occurred on a Friday, dealership gave me a loaner car to get home as i was from out of town. They stated it would probably be the following Tuesday maybe Wednesday before they could tell me anything. I never heard from dealership service department, so i called them on Thursday morning and was told my truck motor had seized up and a new motor was needed. Service advisor stated GMC had been contacted and paper work started and I would be getting a new motor. The service advisor also stated that he had called and left a message. We verified my phone number and he had correct number, however I still never receive a call or voice mail. I still have not received any documentation on the failure or a repairs needed.Read more

The 2024 Trailblazer developed a problem going into reverse at less than 17500 miles in December of 2025 when my daughter came home for Christmas. It would slam into reverse or freeze and then shift harshly. The selling dealer replaced the valve body in the transmission while it was in for normal service, the valve body was on order for 30 days in January. After I delivered the car back to my d
... aughter on January 26, 2026 which was 4 hours away in Northern Virginia the transmission continued to freeze and slam into reverse. The selling dealer advised not to drive it back to Virginia Beach, VA. The car was then dropped off at an Arlington, VA dealer and was there for 9 days. The servicing dealer said they could not duplicate the situation and returned the car after 9 days. My daughter stated that the transmission has continued to act up in reverse with freezing and slaming while trying to reverse in real world settings. The Trailblazer was dropped off for a 3rd time on Saturday, February 21st and since then we have not heard a word from the dealer. I called Chevrolet Customer Service and they assigned a case number to the problem with the transmission. The case number is 89420437 and Sarah at Chevrolet said she would be in contact with my daughter by Sunday, March 1st.Read more

While ascending a steep hill in San Francisco, the engine began operating roughly and the check engine light came on and began flashing. Pulled over as soon as possible, let the engine cool, and confirmed in the operator manual that the flashing light indicated severe misfire and possible loss of engine power. After cooloff and restart, the flashing check engine light went off, but a scanner rev
... ealed a pending P0302 error code, indicating a misfire in cylinder 2. On the first restart of the engine, it started roughly before settling down, and I could hear loud water sloshing. On the short drive home, I could continue to feel the engine running roughly on each startup cycle (periodically on the drive since this is a hybrid), indicating to me a blown head gasket. In my research, this seems to be a common issue with this engine or model year (see NHTSA Preliminary Evaluation PE25016 or Bissell v. American Honda Motor Co., Inc. et al). I'm confident that if I continue to drive it, another misfire like this on a hill in San Francisco could easily cause a loss of power, steering, or braking action. I will be taking it in for inspection at a Honda service center. The service representative at my local service center (Berkeley CA) indicated this is a common issue on this car that they see regularly. No prior mechanical issues or engine roughness with the vehicle, or anything that would've indicated an issue prior to this misfire and leak. Regularly serviced as a leased vehicle from Honda dealership until it was purchased 2 years ago from the dealership with 70,000 miles. No major maintenance since purchase besides oil changes, rear differential fluid changes, air filters, and tire replacement.Read more

On February 6, 2026, while driving my 2014 Dodge Durango at normal speed, the driver’s seat headrest suddenly and unexpectedly deployed/exploded, striking the back of my head. The force of the impact caused my head to snap forward, resulting in soft-tissue and muscle injuries to my neck and upper back. I sought medical evaluation following the incident and was diagnosed with soft-tissue injury/
... whiplash. Treatment has included diagnostic imaging, prescription medication, and follow-up care for ongoing symptoms. I was able to continue driving the vehicle directly to a Dodge dealership, where staff confirmed that the headrest had exploded. This is the second headrest failure on this vehicle caused by a defective plastic component; the passenger-side headrest failed previously in August 2023. The vehicle is unsafe to operate until the headrest is replaced. Due to the part being out of stock, the repair is expected to take more than two months. Prior to this incident, the only indication of a problem was a letter sent by Dodge in August 2020 stating that the warranty on this part was extended from 3 years to 10 years due to a known defect. Given the repeated failures and risk of injury, this condition represents a safety defect that should have been addressed through a recall.Read more

Oil pump failures have been reported across multiple production years, and affected vehicle population boundaries remain unclear. When I informed them of the incident, I received a text asking whether an over-the-air update had been performed prior to the stall. I confirmed the vehicle had received an update within the past three months. I was then told there is a bulletin stating that after an over-the-air update, the vehicle may suddenly lose communication with multiple modules and stall. The foreman indicated this was likely the cause and that they were consulting Technical Assistance. I asked when it is considered safe to drive a GMC vehicle following an update, as I was never warned that the vehicle could suddenly lose power and stall without notice. I was told they were investigating further. Yesterday, I was informed that engineering directed them to replace the ECM (Engine Control Module) and the Fuel Pump Control Module, with the ECM expected to arrive next Wednesday. I have not been given a confirmed timeline for completion. This appears to be a serious and known issue within GMC, yet there has been no recall and no warning provided to consumers. I was unknowingly driving a vehicle susceptible to sudden and complete power loss. This failure placed my life, my family, and the safety of others at significant risk.Read more

I stopped at a red light in the middle of a 50 mph intersection when the light turned green The engine completely shut off, and it didn’t want to turn back on. There was no warning lights or anything before this happened. The first tow truck driver that came by was told that he was there “in order to do a jumpstart” He ran a diagnostic on both of the batteries and said that they were perfectly fine. Another tow truck had to be dispatched since the first wasn’t equipped to take the truck. In total we were stuck in the middle of the intersection for almost three hours. The amount of times we almost had someone crash into us during that time was very concerning. Dashboard and lights were all working perfectly fine. Auto start stop technology seems to have shut down the motor and was not able to get it to turn back on . This vehicle is a safety hazard.Read more

This vehicle is subject to two open safety recalls for which no repair remedy is currently available from the manufacturer: 1. Recall 24V957 (24S78) Dec 20, 2024 involves the high-pressure fuel pump and powertrain control system. The recall states the fuel pump may fail and cause extended cranking/no start or loss of motive power while driving. Loss of motive power while in traffic or while towing
... increases the risk of a crash. The manufacturer has stated that parts and/or software are not available to repair this defect. The vehicle is regularly used for towing a recreational trailer, which increases the safety risk if loss of power occurs during operation, especially at highway speeds or while merging. 2. Recall 25V686 (25SA8) Oct 10, 2025 involves the 360-degree/rear-view camera system. The recall states that the rear camera image may become dark in certain reversing situations when front and side cameras are exposed to significantly more light than the rear camera. Reduced visibility while reversing increases crash risk, particularly when maneuvering a large pickup truck. The manufacturer has acknowledged both recalls but has not provided a repair remedy within a reasonable timeframe. The powertrain-related recall has remained unresolved for an extended period. The vehicle has not been repaired because the manufacturer states parts/software are unavailable. At this time, the defect conditions have not resulted in a confirmed failure event, but the vehicle continues to be operated with open safety recalls that cannot be remedied. The safety risk remains present due to lack of available repair. Due to the prolonged unavailability of a repair remedy for a recall involving potential loss of motive power, I am unable to safely use the vehicle for its intended purpose of towing, as doing so would increase the risk of a crash if the defect occurs.Read more

I am submitting this complaint to report a safety concern involving Tesla’s Full Self-Driving (FSD) system. On [XXX], while my vehicle was operating under FSD in Las vegas NV, the vehicle was positioned in a clearly marked left-turn-only lane. The roadway had visible pavement arrows indicating a required left turn. Despite these clear lane-direction markings, the FSD system proceeded straight
... instead of turning left. The maneuver occurred abruptly. I did not receive a clear or timely takeover warning prior to the deviation. The transition from a left-turn lane into a straight path happened quickly, leaving extremely limited reaction time (estimated less than one second). As a result of the system’s unexpected behavior, a collision occurred with another vehicle traveling straight. At the time of the incident: Weather conditions were clear. Road markings were visible. There were no obstructions blocking lane markings. I was attentive and looking at the road. The police report assigned fault to me as the driver; however, the vehicle was actively operating under Tesla’s FSD system when it disregarded the clearly marked left-turn-only lane. I am concerned that this behavior represents a potential safety defect involving: Lane interpretation errors Failure to follow traffic control markings Insufficient takeover warning timing If this issue is systemic, it may pose a broader safety risk to other road users. I respectfully request that NHTSA review this matter as a potential safety defect related to Tesla’s Full Self-Driving system. On the way to the dealer, the car completely turned off in the middle of traffic and became a brick and had to be towe

The car shutdown/lost control on Rt 1 in Massachusetts and was fortunately able to steer off the road into a parking lot before it became a brick in the middle of a parking lot, not in a spot or anything. The dealer expects this is a DC converter problem but the car is a complete brick and cannot turn on or be moved around at the dealer until the part arrives. We've been told the part is on back order for months and there is no timeline for when it will be repaired.Read more

The system that failed is the headlight design. I was driving home this evening in a moderate snowstorm, and the snow accumulated and completely blocked the headlights. It was nearly impossible to see the road in the dark without the lights providing adequate lighting. This put the safety of my wife and me (the passengers), and all other cars on the road at risk. This can't easily be reproduced wi

This car is unusual because the wipers are located in a crevice far below the top of the hood where it meets the windshield. The first time we had an issue, it had snowed and snow collected in the crevice area. The wipers were turned on, but the drivers side one didn't move in sync with the passenger side one. They eventually "tangled" and we stopped them. The car was taken for service and supp
... osedly repaired by replacing one of the wiper arms that had bent. It has snowed again. Before the snow fell, the crevice was mostly covered with a shield to keep snow from falling into it. The wipers were put into the "snow position" but when used again today, the wipers have again failed to operate in sync. The drivers side wiper doesn't move fully and the passenger side wiper does. Car wipers should work when it snows, but especially when precautions have been taken. This is extremely unexpected and atypical.Read more

The Michelin tires used for this particular model have a foam issue. This foam is put by Michelin to reduce road Noice and they are coming apart thus creating a rattle in the car making it a hazard while driving on the highway.
