Vehicle: 2026 Toyota RAV4 XSE Hybrid Mileage at time of incident: 145 miles While driving on the highway with my family, I was passing a semi-truck when the vehicle suddenly and aggressively applied the brakes without any warning. The braking was strong enough that the vehicle shook violently and nearly caused me to lose control. This placed my family and surrounding drivers in immediate danger.

We took it back to the second dealer and they did the additional repair authorized by Acura and submitted as a warranty repair. This "visual-manual distraction" is a direct result of the system's inability to process commands, making safe operation impossible while attempting to use vehicle controls. VISIBILITY FAILURE (FMVSS 111): The failure impacts the digital rearview mirror and the safety camera feeds. These cameras frequently glitch, lag, or freeze, depriving me of necessary rear and surround visibility while moving. This is a violation of federal safety standards for rearview visibility. SYSTEMIC ELECTRICAL FAILURE: This "slowly dying" system is also interfering with the vehicle's communication bus, causing the power door locks to malfunction. This creates an entrapment or security risk. I have reported these issues to General Motors/OnStar. Despite their awareness of these "electrical gremlins" and the existence of limited service updates (like Program #N242435631), no permanent fix has been provided. This is a systemic safety defect that requires a mandatory recall to prevent accidents caused by driver distraction and visibility loss. The system needs updating to Android 14. It is Android 12 now. This is the fix. Gm want me to pay $250 for a hour for a macanic to update it. Gm was updating over the air, but it was causing the battery on the vehicle's to prematurely die under warranty. ... mbly went bad and had to be replaced. Upon the mechanic installing the new fan assembly, I had numerous fuses blown and that is when my blinker and hazard light issue started. This is a major safety concern as signaling the direction that you're turning is very important to other drivers. Additionally, it is against the law not to use signals. I'm constantly in fear that someone is going to rear in me because of my inability to signal when switching lanes and or turning. Additionally, my back up camera stopped working shows a black screen with a notice to contact the dealer. Another issue I have is that my door ajar light is illuminated all the time. I often feel unsafe and fear that the door may just come open at any time, I am unable to lock my doors with the internal lock , which means my doors are left unlocked while I'm in the vehicle. Also, the interior lights stay on for awhile when car turned off, thus putting more strain on the battery, which I've just replaced. Plus, the dome light will not go off until you accelerated to about 20 mph, however, when putting the car in reverse there's no way to get the lights to turn off in this causes visibility issues at night. That in conjunction with my backup camera not working makes it a very dangerous situation when going in reverse. I also detect the odor of gasoline which lingers in my vehicle for some time after getting fuel. I'm scared that there's a leak and if so the potential of a fire is very concerning. Incident date: Mar 6, 2026
Due to icy roads, the Genesis slid into a curb at a slow rate of speed. The vehicle did not go up on to the curb and the speed was very slow. There were scratches on the wheel, but no other body damage or contact. Four airbags violently deployed in a spectacular overreaction which could have caused injury. I am concerned for public safety. The auto manufacturer refuses to respond to me.
