The engine system in the 2021 Ford F-150 is malfunctioning due to excessive oil consumption, in the 5.0L Coyote V8 engine. This is leading to no oil registering on the dipstick; the vehicle and its engine remain available for inspection upon request as they are in the owner's possession and have not been disassembled or repaired. This issue puts the safety of the driver, passengers, and other ro
... ad users at risk by increasing the chances of engine seizure, overheating, or sudden power loss during operation, which could result in loss of vehicle control, especially at high speeds or while towing, potentially causing accidents, breakdowns in traffic, reduced braking efficiency, or even fires from overheated components, though no such incidents have occurred yet. The problem has been reproduced and confirmed by technicians at Ford Spitzer in Hartville and Valvoline in North Canton, OH, who documented the oil consumption during service visits, consistent with Ford's diagnostic procedures. The vehicle has been inspected by the dealership, which is in communication with Ford corporate for further guidance on warranty repairs, but no inspections by police, insurance representatives, or other third parties have been reported. The main symptom observed is the consistent lack of oil on the dipstick, technicians documenting the engine is as low as 2-3 quarts, indicative of rapid consumption. No warning lamps or dashboard messages have appeared yet, but they could activate if oil levels become critically low, and the exact onset timeline ties to routine maintenance after several thousand miles.Read more

I own a 2023 Ram 2500 equipped with the 6.7L Cummins diesel and 68RFE automatic transmission. The vehicle has experienced ongoing transmission malfunction issues including extremely harsh shifting, intermittent RPM flare during gear changes, delayed engagement, and sudden lurching during acceleration and deceleration. The issue occurs most commonly during low-speed acceleration and normal driving
... conditions. The transmission will sometimes flare RPM between gears and then slam into the next gear. At other times the vehicle will lurch forward unexpectedly around approximately 35–45 mph. The behavior is inconsistent and unpredictable. Some drives appear normal while others produce severe shifting events. This condition creates a safety concern because the vehicle can surge or jerk unexpectedly during normal driving and while towing. I regularly tow a small enclosed trailer and these sudden shift events create instability and loss of smooth power delivery. The vehicle has been inspected multiple times by an authorized Ram dealership. The dealer has attempted repairs including transmission diagnostics and valve body replacement. The condition has continued despite repair attempts. The vehicle has also remained out of service at the dealership for extended periods of time while the manufacturer attempts to diagnose the issue. No warning lights consistently appear prior to the failure. The symptoms occur intermittently after the vehicle reaches operating temperature and sometimes after driving several miles. This issue has been confirmed by the dealership and the manufacturer has been involved in the case. The vehicle remains unrepaired after multiple attempts. Because this is a heavy-duty truck that frequently tows loads, the sudden harsh shifting and unpredictable transmission behavior creates a potential safety risk to the driver and surrounding traffic.Read more

Also should note While driving, the vehicle becomes excessively loud and behaves as if it is stuck in the wrong gear, accompanied by a significant loss of engine power. An onboard diagnostic alert has indicated a malfunction in the electronic control of the engine throttle or automatic transmission. After going through a touchless car wash with undercarriage spray and clean, the vehicle stalled 5-10 minutes later. This occurred after the wash was completed and driving away. Suddenly the engine began to lose power, stumbled, and then died while driving. If I had to guess this was a sensor or some electrical component that may have been affected by the water spray from underneath. There were no
... check engine or other warning lights when this occurred. Attempted to restart the engine. After a few seconds longer than normal it started and appeared to run fine. Drove it home afterward. If the water caused the issue there are serious implications for driving in rain, through puddles, or snow as that water can migrate up into the engine compartment during driving (would be especially dangerous at highway speeds). If it makes any difference, the vehicle did idle fine for a few minutes after leaving and stopping outside the touchless car wash. Only after a walkaround, removing trash from the vehicle, and then driving away did the stall occur.Read more

Since owning the vehicle, I have experienced a range of serious transmission issues. These include harsh or delayed gear shifts, inconsistent downshifts, and transmission slipping. The vehicle also frequently jerks or lurches during gear changes. Of particular concern is the vehicle’s behavior during downshifting. At times, the transmission downshifts abruptly and causes the vehicle to lunge for

I own a 2025 Audi Q7 and have experienced repeated malfunctions related to the vehicle’s driver assistance and safety systems. However, the dealership often reported that they could not duplicate the problem or that no fault codes were found. Despite this, the warnings continue to appear and the problem remains unresolved. Because these systems are related to safety features such as emergency braking and driver assistance, I am concerned that the malfunction could create a dangerous driving situation. The vehicle has multiple documented service visits and I also have video evidence showing the warnings when they occur. I am reporting this issue because the malfunction involves critical safety systems and has not been properly resolved by the dealership.Read more

The vehicle produces a ratcheting / clicking sound from the front axle area when turning the steering wheel, especially during low-speed turns such as pulling out of a parking space or making sharp turns. The sound appears to originate from the front drive axle or CV joint area. The noise is repetitive and occurs when the vehicle is under light acceleration while the wheels are turned. This prob
... lem has been widely reported by other Honda Prologue owners and appears related to defective front drive axles or CV joints. There are reports and a current lawsuit alleging that the 2024–2026 Honda Prologue vehicles have axle defects causing clicking or ratcheting noises when turning. I am concerned that the drivetrain component may fail prematurely and potentially affect steering or vehicle control. Because this involves the axle and drivetrain, it may represent a safety risk if the component degrades or fails while the vehicle is in motion. I am submitting this complaint so the issue can be investigated for potential safety defects affecting Honda Prologue vehicles.Read more

My 2010 Volkswagen CC became completely inoperable because of the active fuel pump control module recall (NHTSA 17V509). The vehicle stalled and nearly caused an accident. The check engine light came on just before I noticed a burning smell coming from the backseat of my car immediately after smelling that the car suddenly stalled out in the middle of a busy intersection and the engine would not s
... tart back up. I had to push it to a gas station parking lot across 3 lanes of traffic. I lifted up the backseat and found that the fuel control module was overheated and melting on top of the fuel pump so I carefully and properly disconnected it from the vehicle. The next day I had the vehicle towed to Lokey Volkswagen in Clearwater, FL. Today 3/14/2026 They did the recall repair and told me my vehicle is safe to drive. Today 3/14/2026 I drove the car 1 mile from the dealership when the exact same thing happened again the car stalled out, and I had to carefully maneuver to the shoulder of hwy us 19 s narrowly missing vehicles in the other lanes. The vehicle was then towed back to lokey Volkswagen. It is pretty clear that the issue was not resolved, and that this fuel control module failing may have affected other parts of the fuel system/wiring and or the fuel control module failed again. I have pictures of the fuel control module, and I gave the initial melted/failed fuel control module to the service department and they took it. The dealership service department is stating that this may be to do something other than the recall repair and that I may be liable for the diagnostic and towing charges. This is a very serious safety issue that almost cost me to get in accidents on the highway twice. I feel that they (Volkswagen) needs to be held responsible for rectification and to find out if there is a connection between this recall and the continuing failure of the vehicle to operate safely.Read more

On June 9, 2025, I was involved in a motor vehicle accident while driving my 2016 Honda Accord. During the crash, the seatbelt system failed in a way that caused severe force to my body rather than restraining me properly. The seatbelt detached and violently pulled my body, resulting in significant injuries. The force from the seatbelt failure ruptured my right breast implant, which required surg

I have since undergone surgery and continue to experience ongoing medical issues including nerve symptoms and physical therapy. In addition, the airbags did not deploy during the collision, which raised additional concerns about the vehicle’s safety systems. Because of the unusual behavior of the restraint system during the crash, I am reporting this as a potential safety defect. After researching online, I discovered other drivers reporting seatbelt and safety-related issues with similar Honda Accord model years. Because the seatbelt system is a critical safety component designed to protect occupants during a crash, I believe this incident should be documented and reviewed for possible defects. This report is being submitted so that the issue can be investigated and documented for public safety.Read more

Perhaps 1 year into owning this car the apple carplay connection started going in and out-- it takes perhaps 10 to 15 seconds for it to start working again. I've tried different phones and cords. I have brought the car into a dealership and they
... cannot find any issue with the car. At the recommendation of the dealership I switched the cord to an official apple cord, but when I did this the connection would be lost and I'd have to reach down, disconnect the cord, and reconnect it to get the screen to come back on-- I have since switched back to an Anker cord. There have been many drivers with the same EV6 reporting this issue in Kia FB groups. Now that the car is about 4 years old, it happens very frequently (one time 6 times in a 10 minute span). The safety issue comes in as one, some people may be reaching to disconnect and reconnect their cord while driving, which can be a distraction and may lead to increase in accidents, two, if someone is on a call they may disconnect the cord and switch to speaker, which again is a distraction, and three, I have also been using the navigation system through the carplay, and when the map goes out my turn is going up it can cause me to hesitate on whether to turn or not (and perhaps if the screen comes on at the last minute, make a quick turn, which can increase risk of collision).Read more

Vehicle experienced a catastrophic mechanical failure characterized by a percussive "shotgun blast" noise from the rear chassis, followed by an immediate, total loss of braking power (pedal to the floor) and a violent loss of directional stability. This incident represents an aggravated safety failure of known GMT900 platform defects. The technical chain of causality began with the factory exhaust
... manifold fasteners suffering a metallurgical shear, a universal defect documented under GM Bulletins 19-NA-218, PIP5075, and PIP4701. This shear created a high-velocity, high-heat exhaust leak directed at the brake master cylinder and vacuum assembly. This "heat soak" event compromised the engine-mounted mechanical Brake Vacuum Pump (Recall N192268490 / NHTSA 19V645) and destroyed the internal seals of the Master Cylinder (Special Coverage 15143 / NHTSA 11V599), resulting in total depletion of vacuum assist and hydraulic pressure. Simultaneously, the "shotgun blast" and violent swerve were caused by structural failure consistent with Rear Brake Caliper Bolt Fracture (GM 13247 / NHTSA 13V342) and Rear Axle Shaft/Pin Fracture (GM 14892 / NHTSA 14V819). This mechanical collapse caused the caliper to strike the wheel, resulting in a catastrophic rim blowout and immediate loss of tire pressure. Despite the vehicle having aftermarket long-tube headers installed, the root cause of the heat-related brake failure was the failure of the OEM GM manifold fasteners which snapped off in the cylinder head, as documented in the aforementioned bulletins. Under the Magnuson-Moss Warranty Act, the manufacturer remains liable for these pre-existing safety-critical defects. The vehicle is at Glenn Polk Chevrolet for a 901-series Product Allegation Investigation (Case #90199492). This report is filed to demand federal oversight into these life-threatening defects. The combo of failed fasteners, rim blowout, and compromised braking created a vehicle that was both unable to be steered or stoppedRead more

Upon vehicle turning off, I had no option but to "coast" off to the left side, which thankfully, freed me from traffic for the most part. If anyone who have been a foot over, they would've hit me. I have had numerous electrical issues with this vehicle and have taken it to the dealership but they have been "unable" to duplicate the
... issue. Vehicle was immediately taken to the dealership (and they still have as of this incident report submission). They told me last night that they will keep it again today and test drive it again to see if they can get it to do anything but if it doesn't show anything new, they have nothing to diagnose and will give me my car back. After requesting a copy of repair orders and a list of the history codes in my car, I am beside myself that anyone thinks giving me this vehicle back to drive is ok. See the history code report in documents uploaded. These are MAJOR safety failures in a large vehicle.Read more

My Toyota hybrid AWD vehicle experienced corrosion of the rear traction motor cable, which Toyota has acknowledged in Service Campaign 25TD01. This corrosion affects the electrical connection to the rear drive motor and can result in loss of AWD propulsion capability. Loss of rear-wheel propulsion significantly reduces traction and stability, particularly in snow, ice, and wet conditions. Drivers
... may believe the vehicle still has AWD capability when in fact the rear drive system is compromised. In addition, the vehicle’s adaptive cruise control system repeatedly turned off unexpectedly while driving and displayed “unavailable” warnings. These malfunctions occurred alongside the electrical issues related to the rear traction motor cable corrosion, suggesting the defect may affect multiple vehicle systems. Toyota has acknowledged this defect through Service Campaign 25TD01 but has not issued a safety recall. Given the potential for loss of propulsion and malfunction of driver-assistance systems, this issue should be investigated as a safety defect.Read more
