The components involved are the automatic emergency braking (AEB) system and the airbag system. During the collision, the AEB system did not activate, and the airbags did not deploy. The vehicle is available for inspection upon request. I was driving on a main roadway with no stop signs or traffic controls in my direction when another driver entered from a side street with a stop sign and pulled
... directly in front of me. I was unable to avoid the collision, and my vehicle was struck the other vehicle. The failure of the AEB system to engage and the airbags not deploying increased the risk of injury to myself and could have posed a risk to others involved. The issue has not been reproduced, and it has not yet been confirmed by a dealer or independent service center. The vehicle has been inspected by police and insurance representatives. The other driver was determined to be at fault. I am in the process of addressing the incident with the manufacturer. There were no warning lights, messages, or prior symptoms indicating any issue with the braking or airbag systems before the collision.Read more

I encountered two incidents of the same type. One was on Friday, March 27, 2026, the same day I picked up the vehicle from a local Chrysler dealership, where software updates and issues related to recalls were supposedly completed. The vehicle was at 38% battery level at the time of pick up from the dealership parking lot. After driving it for about 14 miles, the battery level depleted to <1% and

Then restarted and drove for less than two minutes, but again the same warning ("Stop Safety. Vehicle will shut off soon") displayed; I turned it off again. After taking a few minutes break, I restarted it and it was showing 1% battery; it gave me a little distance to travel until unfortunately the same warning displays. Fortunately, this time I was close to a place where I can charge the battery. The battery got charged to 100%, partly on the night of March 27th and the remaining in the morning on March 28th. The second incident was on March 29, 2026, on Sunday afternoon. As soon as the battery level reached <1%, the exact the same warning ("Stop Safety. Vehicle will shut off soon") was displayed. I slowly drove the vehicle to the side of the road and called my insurance company; I reported the issue and requested them to tow the vehicle to a nearby Chrysler dealership. As of now, the vehicle is at the dealership parking waiting for diagnosis. Based on my initial conversation with the customer service person, they don't know if this is considered part of any recall or not. Incident Overview: In late February 2026, at approximately 114,000 miles, this 2022 Rivian R1T suffered a comprehensive failure of its kinetic (hydraulic) suspension system. The failure occurred during normal on-road operation at approximately 60 mph in temperate (60°F) conditions. The vehicle has no history of off-road use or heavy hauling. Safety Risk & Critical Failure Mode: The suspension is
... a safety-critical system essential for vehicle stability, emergency braking, and predictable handling. A loss of hydraulic integrity in this system directly compromises the vehicle’s ability to maintain ride height and damping control. High-Speed Exposure: The initial failure manifested while traveling at 60 mph. Although a warning lamp was triggered, the vehicle’s functional safety logic failed to initiate a "limp mode" or restricted speed state. This allowed the 7,000lb vehicle to continue at highway speeds with a compromised high-pressure hydraulic circuit, posing an unreasonable risk of loss of control. Lack of System Redundancy: A localized hydraulic leak resulted in a cascading collapse of the entire architecture. Per the $10,662 service estimate, the repair requires replacing the front and rear dampers, hydraulic pump, manifold, and all fluid lines. This suggests a design-level vulnerability where a single-point failure in the hydraulic circuit necessitates a total system overhaul, indicating a lack of modularity or redundancy. Safety Protocol Gap: After diagnosing the leak, the authorized service center deferred to my personal judgment regarding operational safety. This forced an untrained consumer to make a critical safety determination on a high-pressure system while the vehicle remained capable of full highway speeds. Technical Confirmation: An authorized Rivian service center confirmed the failure on March 3, 2026, and determined a full system-wide replacement is required. All components are available for inspection. No prior warnings or symptoms were presenRead more

Per Acura service bulletin 23-022, Dated June 9th 2023, Acura extended a warranty on the rear steering actuators that were admitted to be “manufacturer error” parts, to extend 10 years from purchase date. In my opinion any part that is known to have manufacturer defects that effect the primary Rear wheel drive system that effects almost all safety systems on the vehicle to include limp mode on
... a highway needs to be replaced, this is a large safety issue. Fact; Honda admits manufacturers errors on parts. Fact; Honda recognizes the issue and issues a service bulletin. Fact; Honda extends the duration to repair the manufacturing defective part. Fact; Honda denies claims ( even when vehicle safety is a concern) after the 10 year deadline Fact; most average 2014 cars driven 12,500 miles per year have roughly 125,000 miles in 10 years and the manufacturer error was realized most likely due to usage. My 2014 now 12 years old has just recently realized the manufacturer error at 81,000 miles and Honda will not honor repairs for their errors. Facts; Google; “is it safe to drive my Acura RLX with a defective Precision all wheel steering actuator”…safety concerns realized. Facts; NHTSA is in place to keep the consumers safe and the manufacturers held accountable for repairs, let’s face it the service bulletin is just a way for manufacturers to appear proactive while turning out defective unsafe parts.Read more

#### The ABS Defect 12. Within seven (7) days of purchase, and with fewer than [NUMBER] miles driven, the subject vehicle experienced a catastrophic failure of the ABS pump containing the Electronic Control Unit (ECU) [citation:1 from previous analysis]. 13. This failure caused the ABS warning light to illuminate and completely disabled the ABS functionality, reducing the braking system to non-p
... ower-assisted, pre-ABS technology. 14. The failure occurred with no warning and despite the vehicle being operated under normal driving conditions. 15. Upon information and belief, the ABS pump/ECU unit contained manufacturing and/or design defects that existed at the time the vehicle left Defendants' control. #### The Airbag System Defect 16. On or about [DATE OF CRASH], Plaintiffs were involved in a motor vehicle collision while operating the subject 2026 Toyota Corolla. 17. Upon information and belief, the crash forces were of sufficient magnitude that the vehicle's Airbag Control Unit (ACU) should have deployed the front airbags. 18. The airbags failed to deploy, causing Plaintiffs to sustain enhanced injuries that would have been prevented or mitigated had the airbags functioned properly. 19. Upon information and belief, the Airbag Control Unit in the subject vehicle is of the same type or design as those implicated in prior class action litigation against Toyota involving defective ACUs manufactured by ZF-TRW Automotive . 20. Defendants have previously settled claims regarding ACUs that were vulnerable to "electrical overstress," which could disable the airbag system entirely during a crash, for $78.5 million . 21. Defendants failed to warn Plaintiffs that the 2026 Toyota Corolla might contain an ACU with these same vulnerabilities.Read more

FOUND SMALL AMOUNT OF METAL SILVER COLORED.’ The report also documents my reported intermittent power loss and references two photos of the debris taken during inspection. Despite finding documented metal debris consistent with recall R25B2 bearing failure, Infiniti Roseville is denying engine replacement claiming silver colored debris does not qualify. Nissan’s published recall criteria makes no such color distinction — it states metal debris qualifies for engine repair or replacement at no charge. Warning symptoms began December 5, 2025 including headlight flickering, high pitched engine whistling, VDC warning light January 25, 2026 — all documented with timestamped photos and video. I am reporting multiple safety-related issues involving my 2016 Chrysler 200 S. On 02/06/2024, after an oil change at a dealership, the underbody skid plate bolts were not reinstalled. While driving, the skid plate fell and I lost control of the vehicle, creating a serious safety hazard. No warning indicators were present prior to this failure. The issue was identified immediately after leaving th

In July 2022, following a battery replacement by the same dealership, the vehicle repeatedly lost power and stalled, leaving me stranded multiple times. I paid for diagnostic testing, but there is no documentation confirming proper diagnosis or repair. I was later advised incorrect hardware had been installed. On 02/27/2026, I was told the check engine light was on and diagnostic codes were cleared without repair. There is no documentation of the codes or repairs performed. I was also informed on 11/19/2022 that parts were ordered for a safety recall. From that date until 02/27/2026, I was repeatedly told parts were unavailable. I scheduled repair and dropped off the vehicle on 03/13/2026. I received notice that repair had begun, but later discovered the vehicle had not been serviced at all. No explanation was provided. Additionally, service records are inaccurate. A vehicle history report shows service on 03/17/2026 at a dealership I have never visited. The vehicle was not serviced anywhere on that date. These issues involve loss of control, repeated stalling, improper repairs, failure to complete a recall, and inaccurate reporting. Documentation is incomplete or missing. I am reporting a persistent and escalating electrical safety defect in a 2021 Ford Explorer. On February 28, 2022 (at approximately 10,542 miles), I formally reported a strong burning metal smell and the engine would be loud and idle high, there was abnormal noises and unusual electrical changes to the dealership (West Herr Ford). The dealer service advisor documented no problem found and told me
... not to worry about it. Over time, this unresolved electrical short and metal smell has resulted in a total system failure. In February 2026, the vehicle suddenly suffered a complete ‘black screen’ (APIM/infortainment failure) and a no-start condition. Most dangerously, the battery suddenly drains within days, which can lead to total loss of vehicle power while in motion, disabling power steering, braking assists and you can’t even put the car in neutral when this happens. I am gravely concerned that this unaddressed metal burning smoking/ electrical short issue poses a significant potential fire risk to myself, my passengers and other drivers. The same dealership has now had this car in there possession for 32 days and admits that what has happened is what happens while they have it and that they cannot find the root cause. Yet the previous service advisor from 2022, and the current service advisors ( gave the car back on 2/21/2026 and 3/6/2026 saying their was no problem) allowed the vehicle to remain on the public roads despite documented fire hazard symptoms.Read more

I did not observe any warning lamps, messages, or unusual symptoms before the event, and this had never happened before.Read more

I am writing to report a serious safety concern regarding the “Consider Taking a Break” driver alert system in my new 2026 Hyundai Ioniq 5 Limited, which currently has 515 miles. This notification is presenting itself in a way that I believe creates a distraction and potential hazard while driving. The alert appears frequently and without clear justification, even when I am fully alert and in
... control of the vehicle. Rather than enhancing safety, it draws unnecessary attention away from the road and contributes to driver frustration. From a safety standpoint, any system that interrupts driver focus—especially without accurate or consistent triggering criteria—can increase risk rather than reduce it. This is particularly concerning in real-world driving conditions where attention needs to remain uninterrupted. Due to this issue, I am seriously considering returning the vehicle, as I do not feel confident in a system that behaves unpredictably and may compromise safety. I strongly urge Hyundai to: - Investigate this behavior immediately - Issue a recall or software update to correct it - Provide owners with the ability to disable or customize this alert I have also included NHTSA on this email, as I believe this warrants broader review as a potential safety defect. Please advise on next steps and whether a fix is currently in development. Thank you for your prompt attention to this matter. Multiple warning lights were turning on then turning off in their own, typically 2-3 days later. However while on, all ADAS showed errors and did not work. Most recent time, had an elevation changed (over 6000 ft), lights came cascading on parked safely. Next morning, car would not accelerate/was extremely jerky. Turned off waited turned back on, while lights were on car felt to be running fine.
... Slightly off sounding idle. DTC showed multiple misfires and error codes with a recommendation to replace fuel injectors. Dealer was surprised given relatively low milage (109000 miles) for this issue to occur.Read more

My 2025 Hyundai Palisade is currently subject to multiple safety recalls affecting occupant safety. These include a recall involving seat belt buckle assemblies in the front and second row seating positions, as well as a separate recall involving the third-row seating area where the vehicle may not meet federal safety standards for occupant protection and may increase the risk of injury during a
... rollover crash. At this time, there is no available remedy for the third-row safety recall. Although no crash or injury has occurred, I regularly transport my children in this vehicle and am concerned about operating a vehicle that has been identified as having an increased risk of injury in a rollover crash without any available fix. The vehicle is available for inspection, and I have scheduled a dealership appointment to address the recalls that currently have available remedies. However, the primary safety concern remains unresolved due to the lack of a repair for the third-row issue. This situation raises concerns about occupant safety and the ability to safely transport passengers.Read more

Incident Date: February 10, 2026 Mileage: 1,260 On the morning of February 10, 2026, my 2026 Chevrolet Equinox EV (1,260 miles) would not start. The instrument cluster simultaneously displayed three warning messages: 1)“Service Battery Fault — Vehicle Won’t Restart,” 2)“Charging is unavailable,” and 3)“Charge Vehicle Soon.” The vehicle could not be placed in neutral and required do
... lly tow from my garage, as the front wheels were locked and could not roll freely. AAA and I measured the 12V battery at 11.8 volts. AAA technician made attempt to jump 12 volt battery with no success. He made the observation that it was very rare for an EV not to start once the 12 volt battery was jumped. The vehicle was towed to Folsom Chevrolet (Repair Order #558923, opened February 10, 2026). The technician performed a global reset, BECM reprogramming, standalone radio programming, and a manual reset procedure. The repair order was closed with “confirmed no further concerns,” however the root cause — identified in the repair notes as a possible HV module communication fault — was never confirmed diagnosed or repaired. Seven days later (February 17, 2026, RO #559167), the vehicle returned to the dealer with overnight 12V voltage loss, starting at 12.12V and continuing to drop. A parasitic draw test found an 11mA draw. After a second overnight test the dealer closed the repair order as “no concerns found” without identifying or repairing a root cause. Safety concern: A vehicle that cannot be started, cannot be charged, and cannot be placed in neutral poses a serious safety risk — particularly if such a failure occurs on a roadway rather than in a driveway. The inability to shift to neutral prevents the driver from clearing the vehicle from traffic. The root cause of this dual electrical failure has never been confirmed repaired despite two repair attempts.Read more
