I am filing this complaint to report a serious safety defect involving my 2022 Hyundai Ioniq 5. The vehicle experienced a sudden failure of the Integrated Charging Control Unit (ICCU), which caused the car to completely shut down and lose power while in motion. My family member, who was driving at the time, stated that the car lost power and shut off without warning on the road, creating an extre
... mely dangerous situation. Thankfully, they were able to pull over without incident, but this type of malfunction could easily result in a collision, injury, or worse. I later discovered that this issue is part of a widespread ICCU failure affecting Hyundai Ioniq 5 and other Hyundai/Kia EVs. My vehicle has been at the dealership for over a month, and Hyundai has informed me that the part required to repair the vehicle is backordered for 2–3 months, leaving me without my primary mode of transportation. This issue is not only a reliability concern but a serious safety hazard that warrants immediate investigation and intervention. A vehicle that completely loses power while driving is unacceptable, especially for a relatively new vehicle under warranty. I urge the NHTSA to investigate the ICCU failures in Hyundai Ioniq 5 vehicles and determine whether a broader mandatory recall is needed due to the loss of motive power and serious safety risk. Many owners are affected, and delays in repair parts are putting consumers in unsafe situations.Read more

We have been hearing a clicking or knocking sound in the front end of our 2104 Jeep Cherokee for a long time now. We had it into the local shop (we are relatively remote from the closest dealer) and they we're not able to find the source of the sound. Forward a bit and now that the sound has gotten louder and the 4x Service warning light has come on it has been identified and confirmed by the near
... est dealer that the Power Transfer Unit (PTU) is bad. This is a part that has been covered under many other Jeep Cherokee recalls but our VIN had not shown up on any of those recalls. We believe our PTU is faulty like the rest of them. I don't have an good estimate of when we started to hear the noise but I know we now have ~189k miles on the vehicle. I know it's higher milage but this has been a slow developing issue for a long time now. It's been off the road for a year now as parts were not originally available to fix this issue with the diagnosis was first made.Read more

I was driving from Colorado to Moab, Utah in Feb 2025 when my car’s engine started making a ticking noise. It had only about 80,000 miles on it at the time and had never had any major issues in the past. I have a safety concern on my 2023 Ford 250 SuperDuty Pickup. The front passenger seatbelt indicator does not engage when a person sits in the seat. The other 4 seats (front driver & 3 back seats) in the vehicle engage as soon as a person sits in the seat showing the seat red in the on the dash until the seatbelt is engaged and then turns green with a check mark on it. When a person sits
... in the front passenger seat the seat does not light up on the dashboard like there is no one sitting in the seat. Once the seatbelt is clicked in, the seat then turns to green with a check mark and then if the seatbelt is unclicked the seat will turn red. I took the vehicle in to be looked at. The Ford Dealership put in a new part and it reacted the same way. They then said that they checked and the seat was working as designed and there was no issue. I asked them to confirm that the airbags would still engage. They would not confirm that. I then called Ford's Hotline with my concerns, they said based on what they could see, the airbags would still engage. I feel that all the seats should react the same way and if a person is sitting in that front seat and doesn't use the seatbelt, the vehicle doesn't recognize that a person is in the seat. And if so, possibly the airbags do not work.Read more

I am writing to report a serious safety defect in my 2024 Acura ZDX concerning the Forward Collision Alert and Automatic Emergency Braking. The system exhibits hyper-sensitivity, leading to unwarranted and dangerous automatic braking events. On 3/17/25, at approximately 4pm PT, I was driving my Acura ZDX on US 101 at 65 mph. A vehicle in front of me changed lanes into my lane. There was ample spac
... e between my vehicle and the vehicle that changed lanes. However, the Automatic Emergency Braking engaged the brakes forcefully, bringing my vehicle to a complete stop in the middle of the freeway. The vehicle essentially stalled for approximately 5-10 seconds, and I was unable to accelerate or move forward during this time. This created a highly dangerous situation, as I was vulnerable to being rear-ended by oncoming traffic. This incident occurred even though I had disabled the Forward Collision Mitigation System through the vehicle's settings. The system's behavior indicates a severe malfunction or design flaw, as it overrides the driver's intent and poses a significant safety risk. The fact that the system engaged the brakes when it was supposed to be disabled is very concerning. This hyper-sensitivity and the inability to fully disable the Emergency Braking System are critical safety issues that could lead to serious accidents, injuries, or fatalities. I request that the NHTSA investigate this matter immediately.Read more

When we arrived home, we did research and decided to switch the RV brakes from Drum to Disc brakes based upon research and our belief that the brakes were simply not sufficient for the RV Trailer. We had an RV shop install a brand new disc brake (electric/hydraulic) brake system. When we went to pick up the RV, we tested the brakes and they continued to be very weak (despite working on the RV dealers test equipment and truck). We had the RAM dealership test the brake controller and they said that it was "operating", despite the fact that it is not triggering the necessary braking action. When I went on-line to research, I found many posts in the RAM owner forum that discussed this exact issue from many RAM owners. All had pursued repairs from their dealers, all to no avail. RAM denies that their controller is the problem, yet owners who go through the process of replacing their OEM equipment with aftermarket controllers all report that their brakes now operate as intended. These posts encourage folks who have similar problems to report to this website in the hopes that an investigation and recall would be triggered. I can't wait for the government to finally get convinced that this is a problem as I cannot go on the road with this brake controller in its current state. It is an incredibly dangerous condition, and I sincerely hope that you will take action to investigate and take the appropriate action with RAM/Dodge.Read more

Started car prior to leaving house. When entering car, placed car into run mode. Released parking brake and placed into drive. Service 4WD light came on with. Car was then parked, turned off for about 2 minutes in traffic. At that point car was restarted with no Service light. I began to accelerate and noise went away. Further driving brought light back on and off but no noise at this time. I have a concern this could happen at highway speeds resulting in a unessarcery car accident. Ford has a known design issue with the 10 speed transmission in this truck that cause the hydraulic clutch to leak fluid. This causes the truck to shift very harshly, miss gears, and randomly down-shift, or jump out of gear. This is an issue that prompted them to create a TSB specifically to address this issue. My truck only has 85,000 miles on it and I will most likely be needing a new transmissi
... on in result of this issue. This design flaw was recognized by Ford and was designed out of the newer version of the 10 speed transmission. This issue also can not be fixed by replacing the part like in kind. It requires it to replaced with a differently designed part. You do not know when the transmission is going to loose hydraulic pressure due to the clutch leaking fluid, it has happened when I have been pulling into traffic and it has also happened when I have been cruising down the road at maintained speed. It does not do it consistently. I have gone an entire month without having issues. As of late it is now doing it on a regular basis and I will have to take it to the dealership to be serviced. This issue is widespread across all vehicles with the 10 speed transmission from 2017-2020. TSB 25-2018. This isn't a repair that can be made by a consumer, it has to be made by a Ford dealership.Read more
