Randomly, and without warning, the instruments, gauges, and displays will switch into a language I do not speak or understand (Italian). Coinciding with the switch to Italian language, cluster and radio display settings revert back to default settings, auto door locks and paddle shifter becomes inoperative (with no menu to enable), pedestrian emergency brake (peb) lamp or forward collision warnin
... g (fcw) lamp turns on with no way to disable, units of measurement including speed, temperature, etc. all change, and many other random settings change as well. ***This is clearly a dangerous situation because of all the functions and gauges that change, and the fact it is difficult if not impossible to recover from this due to the language of the setting menus, and the complexity of discovering and navigating through all these changes. I have taken it to the dealer twice, and they say there is nothing they can do and to wait for a possible software update, but none has come and it has been over a year. When this happens, even if one is able to wade through all the settings menus and change setting back, some items cannot be reset (like the FCA off lamp). According to various Jeep owner forums, some dealers have been able to reset something and temporarily correct the issue, but they charge for this service, and the issue returns randomly again. Here is an example forum discussion, indicating many if not all vehicles are affected: [XXX] Many people on the forum believe this issue did not start until a software update a year or so ago. This is a well known and widespread problem, and there is even a DOT Bulletin on the issue: [XXX] Because this is dangerous (for settings/gauges/units to no longer be useful while driving), and it occurs randomly, I strongly believe this issue should be a manufacturer recall. I am available for any needs. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The frame of my 2006 Toyota Tacoma (VIN: [XXX] ) is severely corroded and compromised. I recently acquired the vehicle and discovered that it was part of a past Toyota recall/program for frame corrosion inspection and replacement. However, it appears the previous owner did not take the vehicle in for the service. The frame now shows advanced rust and structural weakness, which puts my safety and t

I am concerned that the recall program was not adequately communicated or tracked, and I am requesting that the NHTSA review this issue for potential reopening or expansion of coverage for affected vehicles like mine. Around [XXX], I began experiencing issues with the electric power steering system in my 2014 Ford Fusion. The steering became noticeably stiff and difficult to turn, especially at low speeds. A warning message appeared on the dashboard — something like “Service Trac” or a similar alert. Despite the warning and steering difficulty, I was able to continue driving the vehicle for about a week,
... but it became increasingly unsafe and physically demanding to steer. The vehicle has approximately 138,000 miles and was purchased from a dealership in Michigan just a few months ago. The issue has not been inspected by a Ford dealer, but an independent service center diagnosed the problem as a failure of the electric power steering system. I was quoted $2,400 for a new part or around $1,000 for a used one, not including labor. I checked my VIN using Ford’s official site and confirmed there are no open recalls or Customer Satisfaction Programs for the steering system on this vehicle. However, this issue appears to match the symptoms described in Ford recall 19V632000, which applies to some 2014 Fusions with the same steering design. My vehicle appears to be excluded from that recall. Given the widespread nature of this defect and the serious safety risk it poses, I believe the recall should be expanded to include more affected VINs. This failure could easily result in loss of control at higher speeds and represents a hazard to both occupants and others on the road. The vehicle's electrical system experienced a starter malfunction, which led to a complete failure of the starter. As a result, the car would not start, ultimately causing the battery to fail as well. This failure posed a serious safety risk. I could have been left stranded in the middle of traffic, increasing the risk of a collision that could have caused injury to myself and others. If the malf
... unction had occurred in a remote location, I could have been left stranded, placing me in a potentially dangerous situation. The issue was confirmed by the dealership. Although there is an active NHTSA recall related to this exact problem, my vehicle’s VIN was not included. However, it should have been—my vehicle was manufactured during the affected production period and experienced the same failure described in the recall. As a result, I had to pay out of pocket for these substantial repairs. The manufacturer failed to include my VIN in the recall, despite clear evidence that my vehicle was subject to the same defect. The manufacturer inspected the vehicle and confirmed that the failure involved both the starter and the battery; however, I was required to pay out of pocket for the replacement of both components. There were no warning lights, error messages, or other symptoms prior to the failure—the vehicle simply stopped functioning without notice. Further research revealed that many other owners have reported the same starter malfunction, frequently resulting in battery failure and the need to replace both components. Although the manufacturer is aware of this widespread issue, they have not issued a comprehensive recall for all affected vehicles. Had they done so, this failure—and the associated safety risks, inconvenience, and significant repair costs—could have been avoided.Read more

The Multimedia Interface (MMI) infotainment system in my 2020 Audi A4 frequently and unexpectedly reboots during driving. Each reboot causes the display to go black, disabling essential safety features including the backup camera, rear obstacle sensors, navigation, and hands-free phone connectivity until the system fully reloads. Restoring operation requires manual interaction, such as entering a
... PIN and repeatedly using the touchscreen, which distracts the driver and creates unsafe conditions. This malfunction significantly jeopardizes safety, particularly while driving in reverse, as the abrupt loss of the backup camera and obstacle detection sensors occurs without warning, substantially increasing the risk of collision or pedestrian accidents. Additionally, during normal driving, the sudden reboots disable navigation guidance and hands-free phone operation, which are essential for compliance with state laws regarding distraction-free driving. The need for manual input to restore these functionalities causes further driver distraction, compounding the potential for hazardous situations. The issue initially appeared sporadically over a year ago but increased dramatically around February 2025, now occurring as often as every few minutes. Performing a factory reset provided no improvement. Many owners report identical problems on forums like AudiWorld and Reddit, specifically mentioning the early-2025 onset and similar software versions. Several related Technical Service Bulletins, including TSB 2067021, TSB 2076404, and TSB 2069112, suggest software updates to address infotainment malfunctions. My vehicle underwent scheduled maintenance at an authorized Audi dealer in December 2024; no MMI abnormalities were reported then. Due to the sudden and unexpected nature of these system reboots and the disabling of critical safety and compliance functions, this defect should be recognized as a significant safety risk requiring immediate corrective action from Audi.Read more

The MMI screen in my 2020 Audi A5 Sportback randomly reboots while driving, with no consistent pattern. These reboots occur regardless of whether a phone is connected—via USB-C, Bluetooth, or not connected at all. The issue is highly distracting and often occurs during critical moments, such as while reversing (disabling the rear-view camera), when cross-traffic alerts are active, or while movin
... g forward (disabling the parking sensors). What’s particularly concerning is that the issue has become more frequent over time. Initially, it might happen once during a 30-minute commute. Now, it can occur 3–4 times in quick succession, making the system increasingly unreliable and disruptive. Each reboot interrupts essential, hands-free safety features such as Bluetooth phone connectivity and GPS navigation via Android Auto or Apple CarPlay—functions designed specifically to promote safe driving by reducing the need for manual interaction. In addition, the system frequently prompts for the key user to log in and continually checks remote access rights, adding even more distraction while driving. There should be a recall for this! It seems to be happening to a lot of 2020 A4 and A5 owners from my research on the internet. I have tried rebooting and restoring factory settings, but the issue still persist!Read more

I am writing to formally request reconsideration for including my 2017 Kia Forte in the existing recall (NHTSA Recall No. 21V-260, Manufacturer Recall No. SC210) affecting similar 2017-2018 Kia Forte models. Despite my vehicle not being initially included, I believe it should be reconsidered due to the following reasons: 1. Similar Engine Issues: The recall pertains to certain 2017-2018 Kia Forte
... models manufactured at the Kia Mexico plant between June 5, 2017, and July 24, 2017, equipped with 2.0L Nu MPI engines. My vehicle, manufactured at the Kia Mexico plant, is equipped with the same 2.0L Nu MPI engine and has experienced similar issues as described in the recall, including abnormal engine noise and warning lamp illumination prior to engine failure . 2. Consistent Maintenance Record: I have maintained my vehicle consistently, adhering to the manufacturer’s recommended maintenance schedule. The Carfax records demonstrate regular oil changes, brake inspections, tire rotations, and other necessary maintenance performed both at authorized Kia service centers and independent shops. The most recent maintenance was conducted at 112,530 miles, just before the engine failure occurred . This demonstrates that the failure was not due to neglect or lack of proper maintenance. 3. Premature Engine Failure: My vehicle’s engine failed before reaching 116,000 miles, which is significantly premature for a well-maintained engine of this type. Given the similar defect characteristics — oil pressure issues caused by foreign particles in the oil pump variable relief plunger — I strongly believe that my vehicle’s failure aligns with the recall conditions described . 4. Safety Concerns: The defect described in the recall poses a significant safety risk due to the potential for loss of motive power while the vehicle is in motion. My experience aligns with this risk, as the engine failure occurred without sufficient warning, putting me and others at risk.Read more

Remember, I noted this is a Ford Service Manager! Ford has issued a recall (24S37); however, the prescribed solution is to have the Powertrain Control Module (PCM) software updated and that prescribed fix actually makes the sudden downshifting happen more often. This is the cheapest, band-aid solution that Ford hides behind so they can say that they are addressing this issue. A second Ford dealership down the road actually admitted that they have identified that the recommended software update actually makes the problem worse and they practically refuses to perform the software update for their customers. After several discussions with Ford's Recall Assistance Team and several discussions with a local dealership's service manager, Ford is not willing to do anything more, despite this major safety issue not only not getting resolved, but actually has gotten worse due to their band-aid approach of a software update. In my case, if further diagnosis is to be performed, I have to be the one that covers that cost and also I would be the one to cover the cost of repairs. To me, this is completely unacceptable. The rational that Ford uses to deny accountability is that the vehicle's computer doesn't register any DTC codes when the incident occurs.Read more

Vehicle: 2019 Hyundai Elantra VIN: [XXX] Mileage at failure:163,799 Issue: Complete premature engine failure due to a piston rod fracture through the engine block Hyundai’s Response: Dealership (Murdock Hyundai Lindon, 530 Lindon Park Drive, Lindon, Utah 84042) reviewed vehicle maintenance history, noted a single vehicle owner, and regular maintenance history, conducted a review of valvetrain a

Two independent Murdock Hyundai dealership service agents and an independent mechanic reviewed the preliminary findings and believed that my engine failure matched other similar Hyundai recalled engine failures. However, Hyundai Motor America declined to have the dealership perform the necessary tests (i.e. bearing clearing test/cylinder bore test) that would have verified the failure. The dealership also noted previous goodwill assistance for past customers, in similar situations, due to the known manufacturing and safety-related issue. However, in my situation, Hyundai Motor America denied goodwill assistance. This exclusion appears arbitrary and unfair, as the failure in my vehicle aligns with the documented recall defect covered in the Hyundai issued Recall # 21V301000 for 2019 Hyundai Elantras). Furthermore, Hyundai has extended warranties and provided goodwill repairs to other customers with similar failures and similar mileage, yet they refused to provide assistance in my case, citing “time in service and/or mileage of the vehicle.” However, this is not a typical wear-and-tear failure caused by mileage. Additionally, if all Hyundai Elantra consumers were made aware of potential engine issues (at the time of purchase and/or through other means), consumers could address the concern within warranty mileage. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No, it flew off on the highway and has not been reported yet. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warning at all that this was going to break loose. I also have an issue with the PTU. I was driving home from the airport and at a a stoplight, I noticed a ticking noise when I stopped at the exit of the parking lot. I noticed it again closer to home, after a fifty mile drive. When I stopped on the off ramp from the highway, I noticed smoke coming from under/behind my explorer. It smelled like antifreeze, but when I arrived home, and checked under the vehicle, I found black residue on the PTU and what looked like leaks from the axle seals that were not there for the last five months I've owned the vehicle. The vehicle smoked heavily when I stopped on the exit ramp, and when I went to accelerate into the traffic round-about, the vehicle lagged before moving, with an audible clunk and a lurch when it finally moved. Any reasonably hard acceleration produces a noise and will usually lag before moving. It may make a ticking noise when at idle cold, but is more pronounced the hotter the drivetrain gets. First, when the truck is at a stop (red/stop sign), acceleration fails, resulting in a sudden, hard jolt into gear or a complete loss of power. Near rear end collision occurrence avoided several times. Second, while driving 0-30MPH there is a constant hard swift into gear causing a forward/backward jolt of all passengers. Last, and most significant, is the sudden loss of power experienced with hi
... ghway driving. While driving 75-80MPH, sudden deceleration occurs. The gears shift spontaneously from 8/9 down to 1. Speeds are not capable of being manually controlled, by the driver, at that time. What happens next, is either a sudden jolt into gear following deceleration or complete Loss of power. Collision was avoided by pulling off to the side of the road. This sudden deceleration occurs also when attempting to merge into highway traffic. Here again, a rear end collision was narrowly avoided. The truck was serviced to evaluate the cause of this issue, by independent service station technician (vehicle no longer under warrant). They stated that the transmission was the problem and, after consulting with Ford, a computer upgrade was performed. This unfortunately had no impact on the above problem, despite following the recommended "break in" time. Currently the truck is not operable as the transmission is too unpredictable and erratic, rendering it unsafe to drive. This truck currently has only 46,000miles. Ford customer service was notified of this problem who in turn suggested we contact NHSA regarding this complaint. Incidentally, a family member with the same year, make and model truck had the transmission replaced, just this month, because of similar power train operational issues. I encourage the NHSA to evaluate this matter, as POWER TRAIN FAILURE is a known issue with this particular vehicle. Most importantly, this is a concern which poses a significant impact on overall public safety.Read more
