First of all, I would like to say that I received an "IMPORTANT SAFETY RECALL" notice citing that "certain 2022 Chrysler Pacifica vehicles failed to perform" However, my car's problem, though similar, is not quite the same as the problem mentioned by the Federal Motor Vehicle Safety Standard" - rear visibility. My car's DRIVERS' SIDE VIEW mirror is the one that poses the problem. When
... I put the vehicle in reverse, the right-side view mirror adjusts so that I can see the right backside of the vehicle. This, unfortunately, does not occur with the left sideview mirror. I can only see the ground. This an obvious determent to the safety of not only myself, but others as well. When I reported this irregularity to Hunter Dodge, I was told that was just the way the mechanism worked. I was told that there was nothing they could do. No one bothered to check it out. No one seemed concerned. This has been an issue since I purchased the vehicle. I don't know if anyone else has this same issue; but I feel that I am not the only one. This functionality necessary for the prevention of serious injury to pedestrians and/or property. Thank you, in advance for receiving this information.Read more

UNKNOWN I received the notice that there was an active recall on my 2018 Honda about the 1.5 Fuel Pump. I have scheduled several appointments to fix this, only for Honda to cancel all appointments related to the recall, stating they didn't have the part. I was headed to work, and every light on the dashboard that could come up, came up. There were several warning lights from brakes, transmissio
... n, collision, engine, etc. I took the car to the dealership, and they discovered that the fuel injector was the culprit. I am outraged because they're denying there's a connection, and I believe otherwise.Read more

I am reporting a sudden and dangerous failure of the brake booster in my 2012 Toyota Prius. While driving, I experienced a loss of braking power, accompanied by several warning lights including ABS, Brake, and the Traction Control system. The brake pedal became extremely stiff and unresponsive, significantly increasing the stopping distance. This failure nearly caused an accident, as I was unable
... to stop quickly enough when traffic slowed unexpectedly. This issue happened without warning and is a serious safety risk. I later discovered this is a common and widely reported defect in 2010–2015 Toyota Prius models. Toyota issued an extended warranty (Customer Support Program ZE3) to cover this known brake booster defect — but only for vehicles within 10 years or 150,000 miles. While my Prius has well under 150,000 miles, it is just over 10 years old, which disqualifies me from coverage, leaving me with a repair estimate over $4,000 for a well-documented manufacturer defect. This seems grossly unfair, especially for responsible owners with low-mileage, well-maintained vehicles. Toyota’s refusal to issue a formal recall and instead offering only limited support puts millions of Prius drivers at risk. A brake system failure of this kind should qualify as a safety defect and trigger a mandatory recall, not a quietly limited program with arbitrary expiration dates. I urge NHTSA to investigate this issue further and consider a recall of the defective brake booster system in these Prius models to protect public safety and ensure fair treatment for affected owners. Thank you.Read more

The transmission started making a whirring and grinding noise so I took it into the mechanic who notified me that Toyota put out a TSB on this issue in February 2021. I leased this vehicle in April 2021 and it was never disclosed to me that I would experience a transmission issue down the line. At the end of my lease, I purchased the vehicle and again it was never disclosed to me of this issue.
... I immediately took the car into the Toyota dealership where they heard the noise and confirmed it is the transmission defect that Toyota previously identified. I am ordered to bring my four minor children from Las Vegas to Los Angeles minimum once a month and my fear is getting stuck on the 15 freeway at 75 miles an hour in the heat of summer. This is a huge safety issue and this matter should have been a recall. I understand from the Toyota service manager that Toyota did not redesign the transmission so even if I get a new transmission it will experience this issue again. At $11,000 for a new transmission or $4500 for the extended warranty, it isn't right that the consumer is on the hook for a defect Toyota knew about and didn't disclose. This is auto fraud and negligent. A car is a hefty investment and Toyota took advantage of the public. They should be held accountable.Read more

This is a supplemental update to Complaint #11656525 regarding NHTSA Recall 25V170 for my 2024 Tesla Cybertruck (VIN: [XXX] ). On [XXX], Tesla returned my vehicle after performing the recall repair. However, the results are visibly flawed on both sides of the vehicle. One side of the cant rail panel overlaps and is misaligned, while the opposite side is spaced too far apart, leaving a visible gap
... . The right-side gasket was not seated and required me to manually push it back into place. When I raised concerns, Tesla acknowledged that the misalignment exceeds their own internal tolerance threshold, and I was told that it was due to engineering limitations with the replacement parts. They also indicated I may need to return for new parts once a proper fix is available — implying the current repair may not be final or sufficient. This leaves me driving a vehicle that, in my view, remains structurally compromised, potentially vulnerable to panel instability, water intrusion, or corrosion — especially given Colorado’s weather and road conditions. The original recall involved panels detaching while driving. This level of misfitment does not inspire confidence in the safety or durability of the so-called remedy. Wait to schedule another appointment for another reinstall. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

All of this has been going on for a while but it's gotten to the point of dangerous. The Shift to Park keeps coming on preventing me from shutting the vehicle off even though it's, in fact, in Park. Another time I was almost hit in the side when this randomly happened on crossing the highway. This is absolutely absurd! I have had MUCH older vehicles without all these issues with many many more miles! I can't count the amount of times this has caused us serious hazards not to mention almost costing me my JOB! Interestingly enough, the same year/make/model has been recalled for the exact same issues yet for whatever reason GMC says MY model "was not affected by the recall". Funny how it's having the EXACT same issues as the ones on the recall list! GMC needs to fix this before they cause people to die! I'm a healthcare worker and this is disgusting!Read more

May 14, 2025 Dear NHTSA, I am submitting this complaint regarding my 2017 Jeep Wrangler Unlimited Sahara, which has only 60,000 miles and has already required several major repairs that raise serious safety concerns. To date, I have had to replace three brake calipers and the emergency brake, at a cost of over $2,500. The calipers were seizing, and the emergency brake would not release proper
... ly, both of which posed clear risks while driving. I understand through forums and peer discussions that other owners have faced similar issues. In addition, my vehicle now has an oil leak that will cost over $700 to repair, and the windshield washer fluid hose burst, requiring another replacement. These repeated failures on a relatively low-mileage vehicle suggest underlying quality and reliability issues. I believe these concerns warrant review, particularly the safety-related brake defects. I respectfully ask NHTSA to investigate whether these failures are part of a broader pattern that may justify further scrutiny or a formal recall. Thank you for your attention.Read more

My 2016 GMC Sierra 2500 with a 6.6-liter Duramax diesel V8 engine suffered a complete engine shutdown—an issue that is both hazardous and expensive to repair. Fortunately, mine failed in a parking lot. No warning lights or symptoms were present. Failure is instantaneous and can occur any time rendering the vehicle inoperable on high-speed highways, Intersections, etc. Failure always leaves vehi
... cle occupants stranded, creating an unsafe and dangerous situation. My vehicle was towed to a GMC dealer. Repair costs could be as high as $15,000. The 6.6-liter Duramax diesel V8 engine is equipped with a Bosch ‘CP4’ high-pressure fuel pump. These pumps are unreasonably fragile and prone to catastrophic failure causing instant contamination of the entire fuel system with metal shavings. General Motors has agreed to pay $35 million to settle with owners following a class action lawsuit that accused the company of installing defective Bosch CP4 high-pressure fuel pumps in certain Chevrolet and GMC pickup models. This action covered only owners in California, Florida, Illinois, Iowa, New York, Pennsylvania, or Texas.Read more

The truck went to the dealer for recalls on March 21, 2025. The Truck is still within the bumper-to-bumper warranty. Recalls consisted of tailgate fix, TCM & ECM updates. The truck was returned to me approximately a week later. I noticed a significant loss of power. I took the truck back to the dealer and told them about the issue. They had it another 2 days and returned it to me, saying there was
... nothing wrong. Approximately a week later, I was driving down the road and the check engine light appeared on the gauge cluster. I immediately drove the truck to the dealer, and the DTCs were emissions-related. The service advisor, Jeff, told me to keep driving the truck, and it might go away. I drove the truck home, 20 mins away from the dealer, and as I pulled into my driveway, a message on the gauge cluster said, "Add engine oil." I notified the Dealer of the issue immediately. The next morning, I added a gallon of oil, and there were still no signs of oil on the dipstick. At that time, I noticed there was no coolant in the reservoir, but engine oil residue, and a collapsed upper radiator hose. The truck was towed to the dealer later that day. After inspection, the dealer said the oil cooler had failed. They sent me the service bulletin, but that bulletin only says, "oil and coolant mix," nothing about the disappearance of the fluid, and no explanation of the emissions codes. The dealer and GM's corporate office are refusing the share the diagnostic data on how they have concluded that the oil cooler has failed. Their solution is to replace the oil cooler and see what happens. This truck has 1000 miles left on the warranty, and the parts/procedures expire at the end of the bumper-to-bumper warranty. Recap: GM will not share diagnostic data, and they are refusing to extend coverage, yet their remedy is to experiment with my $70,000 truck, and the moment this truck is out of warranty, they are going to wash their hands of it. That is not acceptable.Read more

The Ford F150 has suddenly downshifted at highway speed and gone into limp mode. A maintance light shows momentarlly. I am working with local Ford dealerships to have the issue addressed. They suggest that it may be addressed under a field campaign that has expired. The sudden downshift and limp mode is a safety issue in traffic. The problem happens then goes away.
