June 2020 I purchased the car brand new 2020 Hyundai Santa Fe SE 2.5L from dealership, after the first year I started having issues with my display monitor just shutting off and my car going idle at red lights or when I’m sitting in traffic. I’ve never been in an accident and I’m not a reckless driver to my standards besides occasional potholes, the car going idle happened 3 times before I r
... eached out to a dealership asking questions only for them to tell me, “it sounds like I’m using the start-Stop feature too much and it’s not recommended to be used often”. So I stopped using it completely. Fast forward throughout the years, it still happened about 5-10 times a year for the past few years, I’ve learned to hurry up and put it in park, give it gas, then put it in drive. Fast forward 2025, Feb I was passed due for an oil change about 300-500 miles the the oil light came on and when I check it myself the oil tank was completely empty, I assumed it was normal since I was passed due for an oil change, and a family friend told me that wasn’t normal, I follow the recommended oil change interval on my warranty paperwork 7,500 even set my car to it, after the oil change my car was fine. I took it in at 130,284 miles, fast forward June 2025, I start my car a morning for work & it’s at 135,658 miles, the oil light comes back on and I notice it’s completely empty again. 2nd time it’s happened this year, and two family friends that are licensed mechanics tell me that’s not normal especially if I’ve gotten an oil change this year & it’s seems like the engine is burning oil too quick. I took it in to get another oil change, tell them about the issue of it being empty when I’ve been this year and it doesn’t seem right, the dealership & a local quick oil change place tells me it’s due to me not bringing it in for regular maintenance. There are no lights on on my dashboard, and when I called the dealership to ask about using the lifetime powertrain warranty I have, I have to pay.Read more

On Thursday, June 5, 2025, I discovered a 2-inch crack at the bottom center of my windshield after my vehicle had been parked in my garage for three consecutive days without being driven. There was no known impact or external force that could have caused the crack. The vehicle remained stationary and protected from environmental elements during this time. On Tuesday, June 10, 2025, I drove the ve
... hicle to a glass repair company at the request of the dealership. While en route, I observed the crack rapidly expand from 2 inches to over 12 inches in length, extending upward and curving toward the driver’s side view area. The glass company inspected the windshield and confirmed there was no evidence of impact damage (e.g., from a rock or debris). Safety Risk: The expanding crack compromised the structural integrity of the windshield and obstructed the driver’s field of vision, posing a potential safety hazard. A weakened windshield may also reduce the effectiveness of airbag deployment and roof support in the event of a collision. Inspection and Confirmation: The issue was evaluated by an independent glass repair company, which confirmed the absence of any impact damage. The dealership referred me to this inspection. The manufacturer has not yet inspected the vehicle directly. Availability for Inspection: Yes, the cracked windshield is still available for inspection upon request. Warning Signs or Symptoms Prior to Failure: There were no warning lamps, messages, or other symptoms prior to the appearance of the crack. The crack was first noticed after the vehicle had been parked and unused for several days.Read more

On 05/27/2025, my rear back-up camera started to malfunction. Last week my dash started to malfunction electronically. When I would start my vehicle, all of the lights would start to turn on and stay on on my dash. My speedometer would not move while I was driving, so I could not see how fast I was going. I believe this is a severe safety issue for myself as well as others. The tachometer would ju
... mp up and down and would not function correctly. My dash stated that I had low tire pressure, that I was having problems with my traction control, that my BSD system was not working, that my ABS was not working, that my emergency brake was on even though it was not. It also stated that I did have functioning air bags, which is a severe safety hazard in the chance that I could've gotten into a wreck and they might not have worked. It also stated that I was not in Drive even though I clearly was. It also stated that I had no power steering while I was driving, before the light would turn off and I could properly steer again. This is a severe safety issue that could put me at risk. Finally, my check engine light turned on and stayed on. Yesterday on 06/09/2025 I was able to take it a vehicle maintenance shop so they could run a full diagnostic test. They discovered that I did not have power steering even though there was fluid due to the electrical problems with my dash. They also discovered the C-CAN buss was off and that my vehicle was losing communications with the TCM and HVAC. It was discovered that this was due to my rear view back-up camera malfunctioning, causing the C-CAN network to short out. They had to unplug my rear back-up camera to return everything to normal. I have found on hyundai recall forms that other years of hyundai models had similar problems and I believe that 2018 hyundai sonatas are also having the same problem. I wanted it to be made aware because online, people have stated they are having the same issues. Attached gif that shows part of problem.Read more

Dear NHTSA, I am submitting a formal complaint regarding a 2019 Buick Regal, which I purchased as a used vehicle in 2023 with approximately 19,000 miles. Since owning the car, I have added only 14,000 miles, yet I have experienced multiple significant and ongoing issues that raise serious concerns about the safety, reliability, and warranty service associated with this vehicle. Vehicle Problems:
... Suspension Issues: Shortly after purchase, the car has had persistent suspension problems, which negatively affect stability and ride quality. The dealership has replaced multiple suspension components, but the issue continues and remains unresolved. General Motors (GM) initially agreed to cover these repairs even if my warranty expired and also stated I would qualify for a vehicle buyback. They have now backtracked and are refusing to honor that commitment, despite the ongoing unresolved safety issue Electrical and Transmission Malfunctions: I have reported intermittent electrical and transmission issues to the dealer. These include erratic shifting, hesitation, and occasional system malfunctions. Each time, I have been told they cannot replicate the problems, so no repairs have been made. These issues are intermittent, but they occur while driving and are potentially dangerous. Delays in Repairs and Warranty Coverage: The dealership delayed parts ordering and repairs for my suspension concerns until after my warranty expired. This delay appears to have been intentional, potentially to avoid honoring warranty coverage. Refusal to Service Due to Location (Puerto Rico): I live in Puerto Rico, and both GM and the dealership have stated that they are limited in what they can do for me because of my location. For example, I was told they cannot order a replacement windshield to Puerto Rico, and that what they can do based on a buyback or warranty coverage is limited. ... one as well. I had the same issue when I got my oil changed amongst other issues no issues detected. I went to the dealership to get an oil change and the code P00B0-49 was showing the dealership asked if I wanted to pay $265 to diagnose the vehicle I stated I would not because these issues didn’t happen overnight and there could be potential recalls not addressed when they should have been.Read more

Dear Sir/Madam Masked Oil Pressure Warning in 2016 Nissan Titan XD (5.0L) I am writing to report a concerning and potentially dangerous defect observed in a 2016 Nissan Titan XD equipped with the 5.0L Cummins V8 diesel engine. As a Nissan Master Technician with extensive experience in vehicle diagnostics and repair, I believe this specific defect could be a contributing factor to the well-docume
... nted crankshaft breaking condition prevalent in these vehicles, which NHTSA is currently investigating. The specific issue I have identified and thoroughly documented on my own 2016 Nissan Titan XD is as follows: Observed Defect: The engine oil pressure gauge consistently reads "normal" (approximately 1/2 scale) at all times, including with the Key On, Engine Off (KOEO). Crucially, the corresponding oil pressure warning light does NOT illuminate during the normal Key On, Engine Off bulb check or at any other time, despite the engine not running. Diagnostic Findings (Summary): Preliminary diagnostics, including unplugging the oil pressure sensor, indicate a likely fault with the oil pressure sending unit itself, causing it to send a false "normal pressure" signal. This specific sensor defect fundamentally compromises the vehicle's critical oil pressure monitoring system. ( the engine is running ok, no other symptoms) Potential Safety Implications and Link to Crankshaft Failure: This defect creates an immediate and severe safety risk because it completely masks any actual low oil pressure condition that the engine may develop. Documentation: I have well-documented photographic evidence of the current occurrence described condition (gauge reading, non-illuminated warning light KOEO) which I can provide upon request to support this report. I urge the NTSB to include this specific oil pressure sensor/warning system defect in its ongoing investigation into the 2016 Nissan Titan XD 5.0L Cummins engine failures Sincerely, [XXX] Nissan Master Technician ASE-1104-7529 INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIONRead more
