I am filing a safety complaint regarding a 2022 vehicle with approximately 47,000 miles that experienced a sudden and dangerous mechanical failure while in operation. While driving at approximately 37 mph, a dashboard warning message appeared indicating that the vehicle required attention but could continue to be driven for a short period. Shortly after the warning, the vehicle suddenly lost the
... ability to maintain proper gear engagement. At one point, no gear was engaged at all, creating a hazardous driving situation. I pulled over and turned the vehicle off. After restarting it, I was able to regain limited operation; however, the vehicle would not shift beyond second gear, significantly limiting its drivability and creating an unsafe condition in traffic. This vehicle has been consistently maintained at an authorized dealership according to all recommended service intervals, with no missed or delayed maintenance. The dealership later informed me that the failure was related to the mechatronic system and SOS system. I was also told that the repair would not be covered under warranty. This incident raises serious safety concerns due to the sudden loss of power and gear function while the vehicle was in motion. I am requesting that this issue be reviewed for potential defects that could affect vehicle safety and reliability. I am submitting this complaint so that the issue can be investigated and to help prevent similar incidents from affecting other drivers.Read more

It is not just this vehicle; it is ALL vehicles with adaptive cruise control and automatic emergency braking. When sensors get snow or debris on them, they shut the vehicles down without warning. I have had 3 times this winter when the roads were fine, cruise control set, and because another vehicle was kicking up road salt, the vehicle instantly shut off the cruise control without any warning to
... the driver, all 3 times I had traffic behind me at 55mph or on the highway at 70mph. The emergency braking is another issue, as it will bring the vehicle to almost a stop, and does not take into account if there are other vehicles behind it or not! How more people have not been killed because of these 2 systems is beyond me! BOTH of these systems and the vehicle they are on NEED to be recalled and either sensors removed or workarounds implemented, that at least give the driver some advanced notice before the cruise control is turned off or before a vehicle decides to hard-brake itself!Read more

On 4/14/26 my local Nissan dealership technician discovered that my rear lower control arm is severely corroded, with a hole rusted completely through the structural metal (see attached dealer photo). The technician flagged this as a critical safety risk. While Nissan previously issued a service campaign (P9324) and a 10-year warranty extension for identical failures in Altima and Maxima models in
... 'Salt Belt' states, my Rogue is currently excluded. I have been quoted $1,700 by an independent mechanic to replace these parts, and the dealership estimated the cost would be even higher. I am reporting this to document that this safety defect is present in the Rogue fleet and to urge an investigation. This corrosion creates an immediate risk of the control arm snapping, which could lead to a sudden loss of vehicle control.Read more

### Summary: Sudden Unintended Acceleration (SUA) from a stationary position. ### Vehicle Configuration: 2023 Tesla Model Y Long Range AWD. Battery Type: NMC. ### Incident Details: On April 19, 2026, at approximately 10:00 a.m., the vehicle was stationary on a flat surface at 6722 Vista Del Mar Ave, San Diego. I was in the driver's seat with the vehicle "On" but stopped, conversing with two w
... itnesses. My feet were completely clear of both the accelerator and brake pedals. ### The Event: Without driver input, the vehicle suddenly and forcefully accelerated forward. I immediately slammed the service brake with full force. The vehicle traveled approximately 20 feet before the brakes brought the car to a halt. ### Evidence & Gaps: Two eyewitnesses confirmed the vehicle moved autonomously without pedal interaction. A manufacturer data report was requested, but it showed a specific data gap for the exact seconds of the acceleration event, despite providing second-by-second logs for the rest of the day.Read more

On [XXX], my 2022 GMC Yukon Denali XL (VIN [XXX] ) experienced a complete loss of propulsion while I was driving on [XXX] in El Paso, Texas, with my wife and two young children in the vehicle. Mileage was approximately 50,364. While in motion, the vehicle was displaying abnormal behavior, concerned the vehicle would stall on the highway, I exited at the Geronimo exit. As the vehicle slowed, it shu
... t down entirely. Multiple restart attempts failed. The dashboard cycled on and off, displaying a "Service 4WD" warning while the vehicle was in 2WD mode. The transmission locked, and the vehicle became inoperable in the middle of the roadway with traffic passing on both sides. OnStar SOS was activated to communicate the unsafe location. The El Paso Police Department was requested to divert traffic. A city tow vehicle arrived after approximately fifteen minutes; my family was assisted across the roadway. Roadside assistance did not arrive for approximately three hours. This was the second loss-of-propulsion event on this vehicle in less than twelve months. The first occurred in March 2025 and resulted in engine replacement under NHTSA Safety Recall 25V274 (L87 Engine Loss of Propulsion). The [XXX] failure occurred after the recall remedy had been applied and the engine was replaced. The vehicle was towed to the dealer who replaced the throttle body, intake air duct and a service item titled "Repair open ground circuit". I am filing this report to document a second loss-of-propulsion event on a vehicle that already received the remedy under Recall 25V274, and to ensure NHTSA has visibility into the potential pattern of recurring or inadequate remedy on the L87 platform. I am submitting this complaint regarding a persistent and unresolved issue with the left front axle of my 2022 Jeep Cherokee X, which has been present since the day I purchased the vehicle new. Shortly after purchase, I brought the vehicle to the dealership due to a noticeable clicking sound coming from the front end. The dealer retained the vehicle for several days but stated they were unable to
... identify the source of the noise, although they indicated that a repair had been performed. Immediately upon leaving the dealership, the clicking sound was still present. I returned the vehicle, at which point it remained in their possession for approximately one week. During that visit, components in the left front end were replaced. Approximately one year later, while pulling out of a driveway under normal driving conditions, the left axle failed completely. The vehicle had to be towed back to the dealership, where it remained for another week. I was informed that the CV joint had been replaced. Now, roughly one year after that repair, I brought the vehicle in for a routine oil change. During this visit, the service manager showed me that grease is actively leaking from the same left front axle. I am also once again hearing the same clicking noise that initially prompted my first visit. This vehicle has low mileage, is driven approximately six miles per day, and is used primarily for local driving. It has never been used off-road and has not been subjected to any abnormal driving conditions or impacts. Given the repeated failures involving the same component, despite prior repairs, I am concerned about the safety and reliability of this vehicle. Additionally, this ongoing issue has significantly diminished the vehicle’s value, making it difficult to sell. I am extremely disappointed that a vehicle of this age and usage history continues to experience recurring axle failures without a lasting resolution.Read more

On two occasions, the automatic emergency braking system engaged potentially causing an accident. The first occurred at an intersection where I was attempting to turn left from a stop. My passenger was a witness to this jarring event. The weather was clear and sunny. Both incidents occurred with an odometer reading of <3500 miles. My local Hyundai dealer has attempted to recreate the phantom breaking, but were not able to recreate the circumstances and therefore not know what system required repair/replacement. On April 3,2026 we are starting back from a road trip to Florida from Kentucky (starting odometer 69,199 miles)oil changed prior to leaving in our 2022 Honda pilot. Stopped to get gas and transmission alert code popped up-it would not go into any type of gear. We had no warning previously to this. We were still 3 hours from home, had to get it towed to nearest Honda dealership where they stated
... it was an electronic module failure but transmission would have to be replaced entirely. Car was out of standard power-train warranty of 60,000 miles d/t this road trip (odometer now over 70K) and we had no extended warranty on the vehicle. We requested assistance for transmission replacement from American Honda. We filled out the required form on 4/9/26 explaining we are a loyal Honda customer(own an additional Honda-CRV), had good service records, etc. It was assigned to a case manager on Fri 4/10-they called left voicemail that it was under review and asked us to call back. We immediately called back on Friday 4/10 and left message-no call back. The following week we called back everyday to American Honda and finally got our case manager over a week later on Monday 4/20-said it would be 5 more days. Heard back today 4/24/26 that Honda denied our assistance request saying it was out of warranty and there are no known manufactorer defects or recalls at this current time. Made us wait 3 weeks to decide we will have to eat cost of a new transmission in a car that is less than 4 years old and <100K miles on it. Very disappointing in Honda corporation especially since there are numerous known complaints, civil law suits, etc on various online forums and in the national news of same issues and they aren't taking any responsibility leaving owners with very high repair expenses. Our transmission will be over $8000 to replace out of pocket.Read more

The malfunction involves the windshield wiper control system, including the rain-sensing activation module and associated control software. The system activates windshield wipers without any driver input under dry and clear conditions. The vehicle remains available for inspection upon request. Safety was compromised due to sudden and uncommanded wiper activation occurring during active driving co
... nditions, including highway and nighttime operation. This results in temporary loss of visibility as the windshield is smeared by debris, requiring driver distraction to manually control or disable the system while operating the vehicle in traffic. Initial complaints regarding this issue were made by [XXX] during dealership service visits; however, those concerns were not consistently documented in the official repair records at the time they were reported and were initially dismissed or not formally recorded by service personnel until later intervention ensured the issue was entered verbatim on the service order. The issue has been reported and addressed during multiple dealership service visits; however, the condition has not been reliably reproduced and permanently corrected by the dealer despite documented repair attempts, including sensor replacement and manufacturer-level software (ODIS) updates. The malfunction has persisted after these interventions. The vehicle has been inspected by an authorized Volkswagen dealership service department. No inspection has been conducted by police, insurance representatives, or independent third parties. No consistent warning lamps or dashboard messages were present prior to the failure. The activation occurs unpredictably and without prior warning indicators. The issue first appeared intermittently at lower mileage and has continued through recent operation. The defect persists after multiple repairs and manufacturer intervention, indicating an uncorrectable electronic system failure causing an unresolved safety risk. Vehicle Information • Make/Model: Nissan Rogue (2026) - purchased as a lease in August 2025 • Mileage: 7,810 • VIN: [XXX] • Date of incident: 4/20/26 • Location: Highway travel (Pennsylvania) Engine stall / sudden loss of power while driving While driving my brand new 2026 Nissan Rogue at approximately 75 mph on a highway, the vehicle suddenly and completely stalled without warnin
... g. The engine lost all power while the vehicle was in motion, resulting in an immediate and dangerous reduction in speed. There were no prior signs of malfunction before the stall. The vehicle became unable to maintain highway speed and effectively lost propulsion while in active traffic conditions. This created a serious safety hazard, as the sudden loss of power at highway speed placed me and surrounding vehicles at risk of a collision. I had to maneuver the vehicle to safety while attempting to avoid traffic. The vehicle is brand new and should not experience a complete engine stall under normal highway driving conditions. This incident raises a significant concern regarding a potential defect in engine performance, fuel delivery, transmission control, or electronic power management systems. Because the failure occurred at highway speed without warning, it represents a critical safety risk that may affect other drivers if it is a systemic issue. Actions Taken After Incident • Vehicle was brought to dealership / service center • Case opened with manufacturer (Nissan) • The vehicle was towed and currently getting the engine replaced I am requesting that NHTSA: 1. Review this incident as a potential safety defect 2. Determine whether similar complaints exist for this model/year 3. Investigate whether this issue warrants a safety recall or manufacturer investigation 4. Incident date: Apr 7, 2026
On April 7, 2026, I was driving my 2023 GMC Yukon Denali Ultimate on the interstate at approximately 70 mph, accelerating to pass a car so I could get off on my exit. Without warning, the dashboard displayed "Engine shut off, press start button" and the engine lost power. I coasted off the interstate to the shoulder safely. I was alone in the vehicle. ... worker attempt a jump. The vehicle would not restart. I called Moses GMC of Huntington, WV, and they dispatched a tow through GMC Roadside Assistance. I waited approximately 2 hours on the shoulder for the tow. After the tow, Pete, the Moses GMC service manager, called and said nothing was wrong with the vehicle, that it only needed an oil change. He said it started right up and he drove it into the bay. Moses performed the oil change. When the technician test drove it afterward, the engine fully shut down on the hill leaving the dealership property. My husband Zach happened to drive past and saw the Yukon stopped with hazard lights on at the top of the exit. Pete called that evening and said, "I think there actually is something wrong with it." On April 10, 2026, Pete confirmed the failure is the GM 6.2L V8 L87 engine defect covered under NHTSA Recall 25V-274 (connecting rod/crankshaft manufacturing defect). Full engine replacement required, estimated 6-7 weeks. This vehicle was previously inspected by Moses GMC under Recall 25V-274 in August 2025 and cleared at that time, approximately 8 months before the April 7 shutdown. Vehicle had ~38,000 miles, garage kept, full maintenance records, no prior engine issues or warning signs. No crash, no injury, no other vehicles involved in any of the three failure events.Read more
