To Whom It May Concern, I am writing to formally report a serious and recurring safety issue with my 2024 Chevrolet Equinox, and to request immediate investigation and resolution. The vehicle has, on multiple occasions, unexpectedly gone into “reduced power” mode (also referred to as “limp mode”), drastically limiting acceleration while driving at full speed in traffic. This most recently
... occurred while my teenage daughter was behind the wheel, causing the car to completely shut off in the middle of the street. The first time this happened, I was traveling at approximately 55 mph on Highway 74 when the vehicle suddenly decelerated to about 35 mph without warning, despite my attempts to accelerate. The check engine light appeared only after the power loss. We narrowly avoided a serious crash. We had to engage hazard lights, swerve through traffic, and coast the vehicle home. The dealership was unable to replicate the issue during follow-up visits, and the issue stopped temporarily—but has now returned. This is more than an inconvenience; it is a critical safety risk to myself, my family, and others on the road. Details: Vehicle: 2024 Chevrolet Equinox Issue: Sudden power loss while driving; engine enters limp mode without warning Occurrences: Multiple (including full vehicle shutdown) over the course of ownership Dealership Response: "No issues found" – despite multiple visits Diagnostic Error: Dashboard reads “Engine Power is Reduced” Concern: Loss of power at high speed, dangerous stalling in trafficRead more

My wife RODE our 2018 Can Am Spyder 70 miles to have recall 24V728 completed on 4/23/25. Today is 6/24/25 and we still do not have vehicle back. Dealership pulled engine replaced output shaft put it back together and it will now not shift correctly. Dealership has been trying to figure out the problem and in contact with BRP. At first it was something about it not accepting updated programming
... , then it was something about an oil pressure sensor creating the issue, and now they say it is the HCM which I believe is hydraulic control module in the transmission. On May 7th in an email from BRP Customer Service this statement below was made by the customer service representative. "I have spoken with the dealership, and they have reassured me that the new issues you've mentioned are indeed linked to the recall. They are working diligently to resolve these matters and expect your unit to be repaired by the end of this week." It is now 6/24/25 and BRP is trying to walk back the statement made above by saying it is not related to recall simply because what they think the problem is, is not an inexpensive part. Fact of the matter is it was fully functional when my wife rode it down to dealership the work was done and now the transmission will not shift properly and what they think will fix the issue just happens to be inside the transmission the dealership just had torn apart. Based on other comments from other owners of this type of vehicle I am not the first one to have problems with this recall and I don't believe BRP has a dealer network entirely capable of doing the recall, and the BRP Customer Service rep has been of little help. I do have all the emails between myself and the BRP customer service rep if needed.Read more

I am reporting a serious and ongoing electrical and transmission issue with my 2021 Kia Telluride (VIN: [XXX] ). The problem began under 20,000 miles and has continued intermittently for years, progressively worsening. My vehicle is currently at ~99,600 miles and under extended warranty. The key safety concerns include: Transmission Lockout: The vehicle intermittently becomes stuck in Park and w
... ill not shift, even with the brake depressed. No-Start Condition: The vehicle will randomly become completely unresponsive—no dash lights, no infotainment, and no engine start. Electrical System Failure While Driving: Multiple safety systems fail mid-drive, including: Loss of power steering and air conditioning Random activation of ESC, TPMS, Collision Assist, Blind Spot, and Parking Brake warnings Infotainment screen turns off while in motion Component Failures Due to Electrical Shorts: Both the radio and the backup camera were diagnosed with shorts and had to be replaced under warranty. Brake Lockout Post-Shutdown: The brake pedal becomes very firm and the vehicle will not restart until hours later. Most concerning, my wife and children have been left stranded in the disabled vehicle on multiple occasions due to these issues. The car has been in the shop several times — around 50,000 and 80,000 miles — but no permanent resolution has been found. This is a clear and escalating safety hazard. The failures affect critical systems while driving and leave the vehicle unpredictably disabled. Kia has been made aware and an escalation is underway, but I am filing this report to ensure the NHTSA is aware of the pattern and potential danger these failures pose to others. The original engine in my 2023 GMC Sierra 1500 (6.2L L87) failed in November 2023 at only 2,872 miles. The dealership diagnosed engine knocking and discovered a damaged thrust bearing and crankshaft with metal debris throughout. The engine was replaced under warranty. In April 2025, GM issued Safety Recall N252494001 for the same engine model due to connecting rod and/or crankshaft manufacturing
... defects. According to GM’s bulletin, engine failure can result in sudden loss of propulsion and increase the risk of a crash. Despite already experiencing this failure and being within the affected model year and engine range, I have been told the recall remedy is not available to me because the campaign applies only to dealer inventory vehicles, and my recall status remains "Recall Incomplete: Remedy Not Yet Available." I requested a safety inspection of the replacement engine, but GM has refused, citing the incomplete status. When I escalated to GMC Customer Care, I was told they could not confirm whether my truck is safe or unsafe to drive. This is after already being stranded once with a family vehicle that cost nearly $80,000. I frequently drive with my family and feel I am being left at risk. GM appears to be prioritizing unsold vehicles while leaving current owners without support or assurance. The current engine has not yet failed, but I have no confidence it is safe, especially as it may include the same defective components described in the recall.Read more

... is issue aligns with a known defect in Kia’s Theta II engines, which have been recalled or covered under extended warranty in many cases. However, Kia has denied any support, stating my vehicle is “outside the factory warranty” and disqualified due to not completing a knock sensor software update I was never notified about. How Safety Was Put at Risk: My daughter was in the vehicle at the time, and we were placed in a vulnerable position with a failing engine, unable to safely continue driving. A sudden loss of power could have resulted in an accident if we had been in traffic or on the highway. This defect is a serious safety hazard, especially for families with young children. The vehicle has not been inspected by Kia directly since they declined to assist once they determined the factory warranty had expired. I am happy to make the vehicle available for inspection upon request. Warning Lamps or Messages: There were no warning lights or indicators prior to the onset of the knocking and failure. The symptoms began suddenly and escalated rapidly. My Request: I am requesting an investigation into Kia’s handling of these failures, particularly in cases where families were not properly notified about knock detection software updates and are now being denied support for what is clearly a known and widespread engine defect. I purchased a 2018 Jeep Grand Cherokee on [XXX] unaware that the vehicle was subject to a federal safety recall and stop-sale order. The recall (NHTSA 24V-838 / Manufacturer 94B) was issued on November 7, 2024, for a defect in the ABS control module that may disable the Anti-lock Braking System (ABS), Electronic Stability Control (ESC), and cause unintended vehicle movement even while stationary.
... A stop-sale order (VIR 2024-11-14) was issued by Stellantis on November 14, 2024, prohibiting the sale or delivery of affected vehicles until repaired. Despite this, the dealership sold me the vehicle while the recall was still open and unrepaired. In the months following my purchase, I began experiencing issues with the Electronic Throttle Control (ETC). On June 15, 2025, I paid $770 out-of-pocket for ETC sensor repairs after a loss of throttle response. The vehicle has not yet been inspected by the manufacturer, but this defect may be linked to the broader electronic issues cited in the recall. No warning lights appeared in advance. I have been driving this vehicle daily without knowing it could malfunction in a way that puts myself and others at risk. I only learned of the recall on June 22, 2025, when I searched the VIN through NHTSA’s database. ... CCU failure and replaced both the ICCU and fuse on 6/17/2025. However, the replacement was not a redesigned part—it was the same model that failed. I purchased this vehicle in March 2025 as a Certified Pre-Owned with ~12,000 miles. All recall-related ICCU updates had been completed before this incident. There were no warning signs before the failure. This failure could have been dangerous had it occurred on a highway, in extreme weather, or far from help with my [XXX] daughter with me. I no longer feel safe driving this car. Hyundai’s recall remedy is clearly not effective, and they continue replacing defective ICCUs with the same flawed units. They have not disclosed the root cause or implemented a permanent fix. I urge NHTSA to: Investigate ongoing ICCU failures despite recall compliance. Mandate a stop-sale of affected vehicles until a real fix is found. Require Hyundai to disclose the root cause. Consider a buyback program if no safe solution is available.
