Component Affected: Service Brakes, Hydraulic – Brake Vacuum Pump Summary of the Problem: I am reporting a serious safety issue involving the brake vacuum pump on my 2018 GMC Terrain. The brake pump failed catastrophically ("blew up"), resulting in a sudden loss of braking power. This occurred without warning and posed a significant risk to my safety and others on the road. We were traveling t
... o vacation in Florida and thankfully had just gotten off the interstate. Details: The failure caused the brake pedal to become extremely hard to press, and the vehicle required a much longer distance to stop. We experienced no brake at the brake booster and a blown vacuum pump is due to a faulty vacuum pump which is a known issue for this model. Fortunately, no accident occurred, but the situation could have easily led to a collision. A certified mechanic confirmed that the brake vacuum pump exploded and needs to be replaced. I have incurred significant out-of-pocket repair costs due to this issue. We had to uber to our vacation room, get Walmart Plus to have groceries delivered to our room. We had to have Uber Eats deliver other supplies. We have had to walk to various places for dinner and entertainment because Avis and Budget had no vehicles available to rent and Enterprise was closed as we arrived on Saturday afternoon. I have checked for recalls and service bulletins using my VIN, but none are currently listed for this specific issue, despite similar problems being reported in other GM vehicles. Request: I urge the NHTSA to investigate this issue further. The failure of a critical braking component without warning is a serious safety concern. I believe this may be part of a broader pattern of brake system failures in GM vehicles and should be addressed through a recall or safety campaign. Also, the estimate for the repair is $2,500 so any reimbursement would be greatly appreciated. We just received this estimate but I can provide it at a later date.Read more

Police case : 2500011609 The failure of the airbags in my Toyota Camry placed both the driver and the minor passenger at significant risk of injury. While the airbags in the Ford vehicle deployed and presumably helped protect its occupants, my passenger was left unprotected, leading to sustained injuries that require ongoing medical treatment. This raises serious concerns about the reliability of
... Toyota’s airbag system. Collision between Toyota Camry 2011 with a 2005 Black Ford Freestyle. The Ford vehicle's airbags deployed, but the airbags in my Toyota Camry did not deploy, despite the severity of the crash. The airbag system in my Toyota Camry, specifically the airbag sensors and deployment mechanism, failed to activate during the crash. This failure prevented the airbags from deploying, which is critical given the impact severity. The vehicle was totaled due to extensive damage, and the airbag system in my Toyota Camry has not been inspected by the manufacturer yet. However, it should be made available for inspection by Toyota or a qualified independent service center upon request. The failure of the airbags to deploy exposed the driver and the minor passenger to increased risk of injury. The airbags are an essential safety feature meant to reduce injury during moderate to severe collisions, and their failure likely contributed to the severity of the injuries sustained by the passenger. The passenger, a minor, is now attending regular doctor appointments for treatment, indicating the serious nature of the injuries. In July 2025, my 2017 Ford Fusion SE (EcoBoost engine) was diagnosed with coolant intrusion into cylinder #4 by Sunrise Ford in North Hollywood, CA. This is a known issue with the 1.5L and 2.0L EcoBoost engines and is addressed in Technical Service Bulletin (TSB) 19-2208. The vehicle had only 76,000 miles at the time and had been regularly serviced and well-maintained. Over the past 1–2 years,
... I received repeated warnings during oil changes that my coolant levels were unusually low, though no leaks were found. In July 2025, the check engine light (CEL) appeared. The vehicle exhibited rough idle at cold start, but otherwise drove normally. I immediately brought the car to the dealership, where they confirmed the problem through diagnostics and plug inspection. The dealership quoted me $11,000–$12,000 to replace the engine (long block). Ford Customer Relations (CRC) was contacted and denied financial assistance, citing the age of the vehicle. They also acknowledged this is a known issue and suggested I file a report with the NHTSA, which I am now doing. This engine failure presents a serious safety risk. The CEL appeared without warning during normal use. A driver unfamiliar with the signs of internal coolant loss could continue operating the vehicle, risking a sudden loss of engine power, stalling, or further engine damage while in motion. In my case, the failure occurred quietly and was only caught because I acted quickly. Had I ignored the CEL or rough idle, engine failure could have occurred at highway speed. The issue was confirmed and documented by a Ford dealer. I am no longer in possession of the vehicle, as I was forced to sell it at a loss due to the cost of repairs. The vehicle is no longer available for inspection, but all related documentation has been retained.Read more

I experienced a second brake booster failure within 1300 miles of the replacement. The booster should be available for inspection, but I do not have it. The dealership has it. On May 9, 2025, at 38,349 miles, my booster failed, experiencing a very loud audible boom and insulation hanging under my car. It was taken to a Nissan dealership and they verified it was something they’d never seen befo
... re. It was replaced, then on June 23, at 39,630 miles, it failed again. I had my 4 year old daughter in the car the second time and thankfully was able to stop, but it could have been catastrophic in heavier traffic or at higher speeds. It was inspected by a second Nissan dealership. They advised there was oil in my brake fluid, but did not advise how it could have happened. They also said they aren’t sure what caused it and they’ve also never seen this before. One failure is rare, a second within 1300 miles is extremely rare. The symptoms, I feel, began in January. During a warranty strut replacement, I advised that my brakes were doing something strange and I wasn’t sure if it was the brakes or transmission. I was recommended to have a cvt flush and brake fluid change. I feel this was the booster, but the dealership said there are no symptoms of a failing booster. If those were not symptoms of the booster, then the first symptom began around May 5. When I would start my car, I noticed the brake pedal was stiff until I started it. It drove normal until I pressed the brake while idling in my driveway preparing to leave and heard a loud boom, followed by a hissing sound coming from my brake and a very stiff pedal. My brake is still very stiff when I go to start my car (as if I’ve pressed the brake multiple times, but I am pressing it for the first time). This is potentially the first symptom I experienced before the first failure, it’s a symptom that has not gone away since the first failure. And knowing it was unsafeRead more
