In March 2026, I was driving on the freeway when the car shut itself off and put itself into neutral and accessory modes. There was no warning. This put the safety of myself and other drivers on the road at serious risk because I was unable to accelerate or adequately control my vehicle without warning. The dealer says the car "is operating as designed."

I am writing to formally raise a concern regarding the Power Transfer Unit (PTU) on my Jeep Cherokee, which currently has approximately 123,000 miles and has been driven fewer than 10,000 miles per year. Despite what I would consider normal usage, the PTU appears to be failing—an issue I understand is not uncommon for this vehicle. I have been informed that replacement PTU units are currently o
... n extended backorder, with dealerships reportedly facing a backlog of hundreds of pending repairs. This raises serious concerns, not only about parts availability but also about the apparent frequency of this failure. More importantly, I am concerned about the risk of continued operation while awaiting replacement. Based on my understanding, a failing PTU can place additional strain on the transmission and drivetrain, potentially leading to more extensive damage, overheating, or even sudden mechanical failure. Given that the part is unavailable for an extended period, I am left in a position where continued use of the vehicle may result in further costly damage and possible safety risks. I would appreciate clarification on the following: What steps can be taken to mitigate further damage while the PTU is on backorder? Whether there are any alternative repair options, including temporary solutions or access to remanufactured or used parts Whether any goodwill assistance, extended warranty consideration, or manufacturer support is available given the known issues and current supply constraints I rely on my vehicle for regular transportation and am concerned about both safety and the potential for escalating repair costs due to circumstances beyond my control. I would appreciate a prompt and thorough response outlining available options. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am reporting a safety issue with my 2007 Honda Odyssey. The left power sliding door latch assembly has failed, causing the sliding door to open while driving. The door does not consistently register as securely closed, and the vehicle’s alarm activates continuously, which has made the vehicle inoperable. This condition presents a serious safety hazard to the driver and passengers because the
... door cannot be relied upon to remain securely latched during normal operation. The issue has been ongoing for approximately six months and has progressively worsened. The failure appears to involve the door latch assembly responsible for securing the door and signaling that it is properly closed. I contacted the manufacturer, American Honda Motor Co., Inc., and requested assistance due to the safety nature of this issue. My request was declined unless I first obtain a diagnostic at my own expense, despite the condition being clearly identifiable and involving a critical safety component. I am requesting that this issue be documented and reviewed as a potential safety defect involving the sliding door latch system. The vehicle is located in New Jersey and is currently not in use due to this safety condition.Read more

On Saturday April 25 while driving my leased Ram 1500 Tungsten with the radio volume turned all the way down (no sound), I was changing stations on SiriusXM using the steering wheel button when the volume suddenly went to maximum. ... the dealership the following Monday, but they did not find any "codes" indicative of what may have caused the problem. The vehicle would shut off when idling at lights or in reverse and have delayed acceleration after stops. It also would shut off when idling in park. The vehicle gets stuck in gear or "limp mode" and will then suddenly fix itself and switch into the correct gear intermittently. There is often a delay when the vehicle goes into limp mode and the service engine wrench light coming on, so you don't know it's happened until you lose the ability to accelerate. I was almost t-boned when this issue first appeared as I did not realize the vehicle was
... stuck in 4th gear. I was at a flashing yellow light waiting to turn left across traffic. When I saw a gap in the oncoming traffic I proceeded to make my turn only to realize the vehicle had very little acceleration. Because it was stuck in 4th gear it had virtually no acceleration from a stop. Thankfully the oncoming vehicles were paying attention and noticed my vehicle crawling through the turn otherwise I would have been hit. It also can get stuck in lower gears and not shift up, so when accelerating to get onto the highway it will remain stuck in 1st or 2nd gear and as a result drive very poorly at highway speeds. I brought this to a Ford dealership and they had the truck for 3 weeks and told me they couldn't reproduce it and bring it back to them when it was more consistent since it is a very intermittent issue. Even though I showed them my Ford app with over 100+ notifications that stated the issue and they were able to see the vehicle codes they told me there was nothing they could do. I took it to another dealer and their recommendations were: Replacement of Trans Internal Wiring Harness (Molded Leadframe) & Pan Gasket & Filter, Transmission Flush The many notifications in the Ford app that I presented to the dealerships stated: "Powertrain Malfunction / Reduced Power - The transmission control module has detected that one of the transmission sensors is not sending its information to the module. Bring to authorized dealer as soon as possible" This issue has manifested itself many times for me and I have been going back and forth with Ford about it for months.Read more

Subject: Safety Concern – Infotainment Screen Malfunction (2022 Honda Accord) I am reporting a safety-related issue with my 2022 Honda Accord involving the infotainment display system. The screen is experiencing a “ghost touch” malfunction, where inputs occur without any physical interaction. This causes the system to randomly change settings, switch screens, activate functions, and interf

I purchased the truck in 2021 from a chevy dealer with 32k miles on it, truck looked great and performed great, at 42k miles it started to be sluggish and it would vibrate between 30-40 and 60-70 as i feathered the gas pedal to maintain speed, but if i gave it more gas or take my foot off the pedal it would stop vibrating, I've taken it to 2 chevy dealers, the first one said it drives within GM sp

I looked online and many people have similar problems. ALSO: The paint around my front wheels is peeling off down to the primer, I've had many vehicles and never had this happen to me, It's just flaking off, When i had the truck at the chevy dealer for maintenance and told him about it, He said GM will not fix it and they know about the issue. Now these trucks are not cheap, It seems like once they sell the vehicle GM washes their hands of it, I guess that's why they only give you 36,000 mile warranty because they don't even trust their product to last.Read more

The Power Conversion System (PCS) in the Tesla Cybertruck is experiencing a high rate of hardware-level failure (specifically the PCS2 module) that poses three distinct safety risks beyond the loss of AC charging. Loss of Steer-by-Wire Integrity: The Cybertruck utilizes a full steer-by-wire system and a 48V low-voltage architecture. The PCS is responsible for managing the 48V bus and charging the
... LV battery. A hardware failure in the PCS (common alerts: PCS2_a094, PCS2_a137) threatens the stability of the 48V system. Any sudden collapse of the 48V bus while in motion could result in a total loss of steering control and vehicle maneuverability, creating an immediate risk of a high-speed collision. Loss of Propulsion and Stranding: PCS failure frequently leads to "limp mode" or a "no-start" condition where the vehicle cannot engage Drive or Reverse. Experiencing a loss of propulsion while in active traffic or being stranded on a highway shoulder without the ability to move the vehicle creates a severe life-safety hazard for occupants and other motorists. Failure of Visibility Systems: Critical safety components, including the primary windshield wiper and exterior lighting, are powered via the 48V system managed by the PCS. Hardware instability has led to reports of these systems failing during inclement weather, severely compromising driver visibility. Tesla has implemented software bypasses (OTA updates) to allow DC Supercharging when the AC hardware fails, but this is a "band-aid" that does not address the underlying hardware defect—likely MOSFET degradation. With replacement parts on backorder for 8+ weeks, owners are forced to operate vehicles with compromised electrical architectures. I am reporting this as a systemic hardware defect that requires an official safety recall.Read more

I am filing a safety complaint regarding serious and recurring issues with my 2024 Jeep Grand Cherokee that create unsafe driving conditions. Shortly after purchasing the vehicle, I began experiencing intermittent electrical malfunctions while driving. Without warning, multiple dashboard warning lights and signals activate simultaneously. During these events, I lose confidence in the vehicle’s
... reliability and safety, as it is unclear whether critical systems are affected. On at least one occasion, the issue forced me to pull over to the side of the road due to safety concerns. The vehicle would not return to normal operation immediately, and I had to repeatedly turn the vehicle off and on for approximately 20 minutes before it reset. In addition, the check engine light has been on for a significant portion of the time since I purchased the vehicle. I have taken the vehicle to the dealership multiple times, but the issue has not been permanently resolved. I am also concerned about multiple safety recalls affecting this vehicle. Some of these recalls currently have no available remedy, which increases the risk and leaves me driving a vehicle that may have known safety defects without a fix. These ongoing issues pose a serious safety risk to me and others on the road. I am requesting that this matter be investigated, and that appropriate action be taken to address these defects. The vehicle is a 2019 Lincoln Navigator. The component that failed is the transmission. The vehicle experienced a sudden transmission failure resulting in loss of normal drivability. The issue is currently available for inspection at the repair facility upon request. This failure created a safety concern due to loss of power, hesitation, and unreliable shifting, which can increase the risk of col
... lision, particularly in traffic or during acceleration. The problem has been confirmed by the dealership and independent repair facilities. Multiple repair professionals indicated that this failure is consistent with a known issue addressed by a Ford Technical Service Bulletin (TSB), suggesting this is not an isolated incident. The vehicle has been inspected by the repair facility. The manufacturer has not directly inspected the vehicle to my knowledge. There were no significant prior warning indicators that would suggest imminent failure. Any symptoms prior to failure were minimal or not clearly indicative of a major transmission issue. Based on information from repair facilities, this appears to be a known defect pattern rather than normal wear or maintenance-related failure. The use of a TSB instead of a recall raises concerns that a broader safety-related defect may not be adequately addressed. This failure occurred after proper maintenance and outside any initial waiting period for warranty coverage. The issue appears systemic and warrants further investigation.Read more

I am reporting that Hyundai’s Recall 296 remedy appears inadequate and that Hyundai closed the recall on my vehicle even though the underlying rear-seat safety hazard remained unresolved. My complaint involves the second- and third-row powered seat system, including the fold/stow and walk-in functions. My core safety complaint is that the second-row seat continued to move under powered operatio
... n despite resistance from a properly installed child seat using LATCH. I also reported that the third-row seat could still collapse under powered operation despite the presence of an object. This is a crush/entrapment hazard, especially for children. I presented the vehicle for this same rear-seat safety concern multiple times. Even after Hyundai and the dealer treated the recalls as completed, the condition remained unresolved and I returned the vehicle again for the same issue. Hyundai and the dealer changed and used inconsistent completion dates while continuing software/update activity and then still failed to acknowledge my real-world safety concerns after the supposed repair. I also reported additional post-update problems including fluctuating cabin air temperature, blower speed fluctuation without input, and reduced ventilated-seat cooling. Hyundai ultimately took the position that those systems were operating normally and instructed the dealer to release the vehicle. My complaint is not merely that Hyundai issued a recall. My complaint is that Hyundai treated Recall 296 as completed, changed the vehicle’s seat functionality by disabling or limiting previously available automatic features, used inconsistent completion timing, and still failed to correct or acknowledge the underlying safety defect I continued reporting. I am asking NHTSA to review whether Recall 296 is actually correcting the defect in service, or whether affected vehicles remain unsafe after Hyundai marks the recall complete.Read more

At 70 mph the adaptive cruise control system applied the brakes hard enough to engage the abs system even though my lane of travel was clear. There was a car on the exit ramp to the right applying their brakes not in my path that I believe my vehicle picked up. There were several vehicles behind me and it almost caused an accident. There needs to be a way to disable the adaptive cruse on this vehi
... cle for safety concerns. This has happened to me on multiple occasions and I reported it to the General Motors hotline with absolutely no feedback on the issue.Read more

Tail light on the drivers side was hyper flashing and upon inspection found that it is not working. When I removed the tail light to replace the bulb I noticed that the whole tail light assembly was full of water. The bulb is inside a housing that is inaccessible, so bulb cannot be replaced without replacing the whole tail light assembly. Leaking tail light assembly causing failure of turn signal
