I own a 2019 Honda HR-V that has been consistently maintained at Honda dealerships, including proactive transmission fluid changes every 30,000–40,000 miles. At approximately 104,700 miles, during a routine maintenance visit, the dealer diagnosed a failing CVT transmission. Technicians identified metal debris in the transmission, indicating internal deterioration. This condition is consistent w
... ith Honda Technical Service Bulletin 21-047, which acknowledges CVT-related issues in 2016–2019 HR-V models and provides extended coverage for affected vehicles. However, my vehicle was deemed ineligible for coverage because the failure was identified 27 days after the 7-year in-service deadline. Given that transmission deterioration is progressive, it is likely the issue began within the coverage period. Despite this, Honda declined to provide any goodwill assistance. I was quoted approximately $11,000 for the repair. This issue also raises safety concerns. A failing transmission can result in loss of power, hesitation, or inability to accelerate, particularly when merging into traffic or crossing intersections. These conditions increase the risk of an accident, especially if failure occurs unexpectedly while driving. This failure occurred despite proper and proactive maintenance and at a mileage where a transmission failure would not typically be expected.Read more

On Friday May 1, 2026 while parked at my home, the rear glass lift gate burst into multiple shards. I have taken a few photos of the outside as I found it. In my surprise, I failed to photograph the glass in the interior of the vehicle. I have owned this car since new, since near the end of July of 2025. I had unloaded some groceries from the back area, raising and lowering the large metal li
... ft gate. The glass lift gate was NOT used. As a matter of recording, I have only lifted the glass once or twice since owning the vehicle. The main metal lift gate is the unit used most often. Apparently, this is a known issue with several reports online. What I found online seems similar in that no impact or accident occurred to cause the glass to shatter. What is odd, instead of a typical safety glass experience of a "spider web" type of fracture and the glass staying intact, this was "exploded" with the glass found in multiple shards both in the car and outside on the ground. When I called my local Chevrolet dealer, they denied knowledge of this as a problem within the fleet. Because this is a use issue and weather issue, I am having to personally pay for repair. The new window will cost almost $1000.00 to repair.Read more

First symptom I noticed was the car smell while it was running. I used a device to see what codes of any would pop up on a scan. It came up with codes tying into the fuel system. I then went to go fill up on gas couple days later and when going to start it back up it wouldn’t start. When it did start it would say engine losing power or steering losing power and then just shut off. I then looked
... up what that usually means and it made me look towards the alternator. I was told by a mechanic an easy way to see if your alternator was good was to start car and while running disconnect the negative on battery and if it stays running the alternator was still good. So I did that and it stayed running. I then looked up recalls for my vehicle and seen everything it was describing for the fuel system seemed to be happening. I am now filling this form out to get my vehicle looked at and fixed. I have a wife and 2 young daughters that we drive to school. It is not safe to be driving and hoping that us and others on the road will get to there destination safely.Read more

Dear NHTSA, I am writing to formally report a dangerous design oversight and safety hazard regarding the trunk lid support mechanism on the Nissan Altima. On [Date of Incident], while the vehicle was parked, a gust of wind caused the trunk lid to slam shut unexpectedly. The metal door latch struck me directly in the frontal region of the skull, resulting in an injury that required medical monitor
... ing for potential brain trauma. The current design relies exclusively on torsion bars or gas struts to maintain an open position. These components are prone to wear and environmental fatigue, and they lack a mechanical "positive lock" or secondary safety catch. Consequently, the trunk lid acts as a sail in windy conditions, easily overcoming the lift support and turning the heavy lid and metal latch assembly into a falling hazard for any consumer loading or unloading the vehicle. While Federal Motor Vehicle Safety Standards (FMVSS) focus heavily on internal trunk releases (Standard No. 401), there is a significant gap regarding the prevention of sudden, gravity-induced or wind-induced lid collapse. I am requesting that the NHTSA: Investigate the frequency of "unintended closing" injuries related to Nissan Altima trunk lift systems. Evaluate whether the lift-support specifications for this model are sufficient to withstand common wind gusts. Mandate a mechanical locking mechanism or more robust secondary safety catches to prevent these lids from becoming dangerous projectiles. While operating the vehicle on both interstate highways and local roadways, an intermittent malfunction occurs involving the All-Wheel Drive (AWD) system. The instrument panel displays a warning message stating, “Service All-Wheel Drive System.” When this warning appears, the vehicle automatically disengages from AWD and reverts to Two-Wheel Drive (2WD). Once this occurs, the system does not a
... llow the driver to manually return the vehicle to AWD. This issue has occurred on more than three separate occasions. Notably, there have been instances where the warning indicator illuminated immediately upon starting the vehicle, prior to any extended operation. When the malfunction is present, the vehicle exhibits abnormal driving behavior, including a noticeable jerking or hesitation that feels consistent with a transmission-related issue. The failure is unpredictable in nature, with no clear pattern or triggering condition identified. This creates a significant safety concern, particularly when operating the vehicle at highway speeds or in conditions where AWD capability may be necessary for proper traction and control. An appointment has been scheduled with an authorized dealership for diagnostic evaluation; however, at the time of this report, the vehicle has not yet been inspected or repaired, and the issue remains unresolved.Read more

Safety Issue: Defective interior cabin camera causes Tesla Full Self-Driving (FSD) to disengage abruptly and without adequate warning at freeway speeds. I am a [XXX] driver. I purchased FSD capability with this vehicle in December 2018. In October 2025, I had the FSD hardware upgrade (HW2.5 to HW3) installed at Tesla's Encinitas, CA service center (B.A.R. License [XXX] ). Within days, FSD would
... became completely unusable due to a recurring "Cabin camera unavailable — schedule service" alert that instantly disengages the system. The disengagement occurs on the freeway at highway speed, without meaningful advance warning. As a [XXX] driver, the sudden loud alarm and loss of assisted driving is genuinely alarming — it is not immediately clear in the moment whether the vehicle has lost power or experienced a critical failure. This is exactly the kind of dangerous situation driver assistance systems should prevent, not cause. Tesla's own diagnostic confirmed the defect (Diagnostic Invoice No. XXX, November 27, 2025, Encinitas service center): "Cabin camera unavailable" alert verified Intermittent stream exit faults detected in the selfie/cabin camera Low-voltage circuit and wiring checks were normal — ruling out wiring Conclusion: fault is within the camera module itself; technician recommended replacement The defect is FSD-specific. The cabin camera works correctly for Sentry Mode, live security viewing, and all normal driving. It fails exclusively when FSD is engaged and demands sustained driver monitoring. The camera housing also becomes extremely hot to the touch during FSD use, consistent with reports from other 2018 Model 3 owners describing identical symptoms after their HW3 retrofits. Unresolved Safety Defects & Request for Buyback/Replacement - 2024 Dodge Hornet GT Dear NHTSA I am writing to formally demand a resolution regarding a persistent and life-threatening safety defect in my vehicle, purchased from Gary Barbera Autoland. Without any warning, the V8 L87 6.2 liter engine suddenly seized up, began shaking with significant reduced power. I was fortunate that I could drive slowly to safety and was not on the expressway. In my research, I understand this is related to a design/manufacturing issue with the connecting rods and crankshafts in this type of engine. As I investigated the issue, I found that 600,000 vehicl
... es were recalled with the recall either completely replacing the engine or upgrading the oil type from standard to a thicker, higher-viscosity 0W-40 full synthetic to provide better protection under high stress, along with a new oil cap and filter. In our case, we were directed to change the oil and even with that change, we experienced this dangerous engine failure which cost us $12K ! For that cost of repair, we elected to have the engine replaced for about $1K more to get a 100K mile warranty on the engine. However, our additional concern with the replacement engine is they gave us a remanufactured version of the L87 and we have not found confirmation that they have adequately addressed the design/manufacturing flaws so may be vulnerable to the same seizing issue. As a direct result, a bucket containing my water hose, regulator, 90 degree fitting, etc were ejected from the compartment and on
... to the roadway at highway speed. This is a significant escalation of the safety issue previously reported. This is no longer limited to a door opening condition—this event resulted in unsecured cargo being discharged into a public roadway, creating a hazard to other motorists. This failure occurred despite extensive prior repair efforts, including: Four separate Service Response Team (SRT) visits Approximately 10 weeks at the Grand Design factory for structural and frame-related repairs Multiple inspections and repair attempts involving both Grand Design RV and Lippert Components The cargo door opening condition has persisted throughout this entire repair history and has not been resolved. This repeated failure following extensive repair attempts demonstrates that the manufacturer’s inspection and repair procedures have not corrected the defect.
