I am experiencing a recurring issue with my 2021 GMC Sierra where the dashboard displays a “Shift to Park” warning even though the vehicle is already in Park. This typically occurs when I attempt to shut off the vehicle. The doors unlock as expected, confirming the truck is in Park, but the warning prevents me from turning off the ignition immediately. After a few seconds, the warning usually
... disappears, allowing me to shut off the truck. This issue is more frequent in cold weather, suggesting a temperature-sensitive electrical or mechanical fault. Based on research and community reports, this appears to be a known issue across multiple GM vehicles, often linked to a faulty micro switch or wiring harness in the shifter assembly. I am concerned about the safety implications, especially if the vehicle fails to shut off properly or if the issue worsens over time. GM has not issued a recall for this problem, and I believe it warrants further investigation due to its widespread nature and potential risk.Read more

Bucking/shuddering transmission issues with 8F35 transmission. Vehicle shudders under 40MPH. This is a software issue from Ford and should be a RECALL and not a TSB (TSB 21-2389), since this is an issue that will eventually lead to transmission failure due to no fault of the owner. Ford needs to be held accountable for their own faulty parts, rather than charging for a software upgrade that is not

Coolant intrusion defect. The engine coolant leaks into the combustion chamber, due to a failure in the engine block design. TSB 19-2346 issued by Ford addresses this known and serious issue for my 2018 Ford Edge, 2.0L EcoBoost engine. In 2024, I purchased a used, 2018 Ford Edge with 49k miles. A few months after I purchased this used vehicle at a dealership, my Check Engine light came on. The v
... ehicle showed other symptoms of white smoke, coolant loss, misfiring and rough idle. I brought my car to a local dealership (not the same dealership that I purchased the vehicle) and they confirmed the coolant intrusion defect. When I purchased the vehicle, it didn't come with a warranty. I tried getting help from by calling Ford Customer Service, and they offered no goodwill assistance and no coverage assistance at all. I ended up having a new engine put in my vehicle for $7,000. This was done by a local car repair shop, not done at a dealership. I now have to pay a loan for $7k on top of my monthly car payment that existed before the new engine. Although my vehicle is no longer under warrant, I understand this issue stems from a documented manufacturing defect and has affected a wide range of Ford models. I've taken good care of my vehicle and it has been maintained regularly. I want to retain confidence in Ford as a brand that stands behind its products, but I didn't get anywhere with Ford after submitting a case and speaking with a representative and also a manager. I don't know what else to do in trying to get reimbursed for the replacement of the engine of $7,000. I would like to be added to any potential class-action. On [XXX], I was driving my recently purchased 2018 Tesla Model S (VIN: [XXX], mileage approx. [30,536]). The vehicle was purchased directly through Tesla’s app in late August 2025 and delivered to me the same day of the incident. Approximately 4.5 hours after delivery, while driving with my son, white smoke began to come out of the cabin air vents. Within minutes, the smoke turned black and rap

No modifications had been made to the vehicle, and it was sold to me as inspected and certified by Tesla. The sudden appearance of smoke from the vents and rapid escalation to a full vehicle fire posed a severe danger to me, my child, and others on the road. I am reporting this to NHTSA to document the incident and to request that the agency investigate whether Tesla Model S vehicles have underlying defects that may cause dangerous cabin fires shortly after use. To:BMW of North America, LLC Customer Relations & Services P.O. Box 1227 Westwood, NJ 07675 Subject: Request for Compensation Due to Prolonged Safety Recall – 2025 BMW X7 (VIN: [XXX] ) Dear BMW Customer Relations, I am writing to formally raise my concern regarding my 2025 BMW X7, which has been subject to an open safety recall since August 2024. It has now been over 13 months without a remedy f
... rom BMW. This vehicle was leased through BMW Financial Services with the intent of being used as part of my business operations. Due to the unresolved recall and the associated safety responsibility issues, the vehicle has been restricted from use and rendered inoperable for its intended purpose. Despite this, I continue to make full lease payments, which has placed an unnecessary financial burden on my business for an asset that cannot be utilized. Given the prolonged delay and the material impact on my business, I respectfully request that BMW provide appropriate financial relief or compensation, which may include: •A credit or reimbursement for lease payments made during the period of inoperability, •The option to terminate or exchange the lease without penalty. This situation has been ongoing for well over a year, and I believe BMW has a responsibility to ensure that customers — especially those who rely on their vehicles for business purposes — are not left bearing the cost of a vehicle that cannot be safely used. I appreciate your immediate attention to this matter and look forward to a fair resolution. I am experiencing ABS issues on my truck. After visiting a local auto parts store to get an OBD scan, the code says it’s a bad ABS module. The messages on the dash display say to service the ABS system, service the electronic braking system, and service the trailer brake system. From my research, I have discovered that many people are experiencing this same issue on similar vehicles . So much in fact that the parts for repair are unavailable at the dealership and online warehouses. The cruise control is disabled while experiencing this issue and braking performance is affected while driving. I’m afraid that my brakes will lock up on me in a hard stop event and lead to an incident.Read more

Incident date: Aug 1, 2025
My 2018 Honda Odyssey (VIN: [XXX]) experienced a catastrophic engine failure due to a spun connecting rod bearing in Cylinder 4. While driving, the engine suddenly began making a loud knocking/tapping noise. I immediately drove the vehicle to the Honda dealer, and as I was pulling into the service bay, the engine completely seized. The dealer confirmed that the engine suffered a spun bearing in C
... ylinder 4, with severe crankshaft and connecting rod damage. The failed parts and metal debris in the oil system are available for inspection if requested. This failure occurred without any prior warning lights, messages, or check engine indicators. There was no advance notice other than the sudden onset of knocking just before failure. The defect caused loss of engine power and risk of stalling in traffic, which could have easily resulted in a crash or fire. This type of failure is consistent with Honda Recall 23V-751, which covers connecting rod bearing defects caused by crankshaft machining errors, and also matches the ongoing NHTSA investigation into premature rod bearing failures in Honda Odyssey and other models. However, my VIN is currently excluded from the recall. I request that NHTSA review this case as potential evidence that the recall should be expanded to include additional Odyssey vehicles. On rare occasions (about once every 500 miles), when uniformly applying force to the brake pedal, the deceleration suddenly ceases and the vehicle seems to jerk forward. To a passenger, it feels like the driver's foot slipped off the brake pedal, but the driver continued to provide the same uniform force to the brake pedal and lost all deceleration. Typically, the driver is startled and presses
... significantly harder on the brake pedal jerking all occupants forward. The brakes re-engage and continue the deceleration at a much higher rate. Toyota addressed this problem with TSB 4724 which involved reprogramming the brake actuator assembly. This corrected the problem for about 3 months, but it is now occurring again. When the car was taken to the Toyota dealer again, the technician stated that TSB 4724 was cancelled, he could not duplicate the action (even though he was told it only happened on rare occasions), and claimed that the "brakes function properly as they should." Fortunately these loss of braking incidents have only occurred on dry pavement so that the driver did not lose control of the car when slamming on the brakes to compensate for the loss of deceleration. Coming down a hill on a snow covered road could have caused the vehicle to slide into an oncoming lane. It is my speculation that there is a null spot that exists during certain circumstances which causes there to be a "gap" between when this hybrid vehicle transitions between the dynamic hybrid brake and the friction wheel brake. Both braking systems independently work fine, but the transition between them (on rare occasions) does not occur continuously. This causes the driver to experience an unexpected loss of braking which, under the right conditions, could cause the driver to loose control of the forward motion of the car. As an engineer, I feel it is unacceptable for a safety system (braking) to not properly operate 100% of the time! On a Toyota owner's site, others have seen the same.Read more

My car was backed into July 4, 2024, unoccupied and had damage done to the hood and front light and bumper. The person who hit the car had insurance and I took it to a local collison company for the repairs In November 2024. (Insured would not return calls to insurance company so it took this long for repairs) I began taking it to Volvo OC in December because the sensors were constantly lighting
... up and beeping constantly with no one around me. I took it to Volvo many times from December to August 2025. They would have my car for a week or two and think they fixed it and it was not fixed. After Volvo sent me a note saying that Corporate was not going to pay to do anything and they were done with trying to fix the car I took it back to my collison company. When Volvo last had it they told me that the wiring harness was not the correct part number and that the collision company installed the wrong one. I called them back and they explained that they had to call Volvo Mission Viejo and gave them the VIN number and they installed what was sent. I took it back to the collision company and they called and ordered another harness and they received the exact same harness. They installed that one and the sensors still do not work correctly. They then called Volvo Mission Viego again and they were told there was a cable that was needed if a car was in an accident and 2 sensors needed to be attached to the cable and the other sensors to the harness. So they did that. And still the same issue. At that time Volvo told me that the sensors in my car were not the correct sensors and that they were not the sensors that came with the car. The collision company said they removed the sensors from the harness but attached the exact same sensors during the repair. So both Volvo and the collision company have left me hanging and this is a safety issue. If I am looking one way for traffic and the beeping goes off it is startling to think someone is coming from the otherRead more

On [XXX], while driving my wife to the RDU airport, our Odyssey's engine failed to restart after an Auto Idle Stop (AIS) at a busy intersection in Greensboro. Instead of starting, the dashboard displayed a Collision Mitigation System Problem alert, the Drive ("D") indicator light started flashing, and then the dashboard displayed a "Shifting to Park: Apply Brake Pedal" message. We were stranded at
... a busy stoplight, at the front of a dedicated left-turn lane, unable to drive or shift to park as instructed. The only way to resolve the issue was to hold the Start/Stop button for several seconds to shut the vehicle off. And the vehicles behind us had to swerve around us because our malfunctioning vehicle was blocking the lane. This problem has recurred semi-regularly since [XXX], forcing us to manually disable the Auto Idle Stop feature on every drive to prevent being stranded in traffic. An independent mechanic found no stored diagnostic codes but identified relevant Honda Technical Service Bulletins (23-008 and 23-009) addressing similar AIS failures. A Honda service center misdiagnosed the issue as a faulty battery, but subsequent testing by the independent shop confirmed the battery was in excellent working order. The actual Auto Idle Stop malfunction remains undiagnosed. The vehicle and its Auto Idle Stop system remain available for inspection. This issue appears to be part of the ongoing NHTSA Engineering Analysis EA25-004. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

But that didn’t solve the issue. The vehicle shut down and I was left in the middle of the road until I could get help. I took it to the shop and I was told coolant had leaked into the cylinders and the I would be

When going over roads with small bumps that are spread a few feet apart, pressing the brakes unexpectedly engages the ABS. This causes the braking distance to increase drastically and also requires the brake pedal to be pressed with excessive pressure to even stop. This happens consistently and has nearly caused us to rear end the vehicle in front of us.

This is a complaint regarding intermittent camera blindness, driver assistance failure, and dismissed diagnostics on a 2023 Tesla Model Y. These issues have impacted critical safety systems including: •Pillar and rear-view cameras •Autopilot and Traffic-Aware Cruise Control (TACC) •Park assist and visualization systems Despite multiple service visits and clear documentation, Tesla has faile
... d to diagnose or remedy the malfunction, attributing the issues to external weather factors despite my repeated statements that the problems occurred in clear conditions. The vehicle has also generated camera blocked/blinded alerts, even when the lenses were clean, and service technicians confirmed the B-pillar camera was blocked during 8.7% of the drive cycle — a serious figure given that Tesla’s driver-assist systems depend entirely on vision-based processing. The vehicle has shown alerts related to Automatic Emergency Braking being disabled, which Tesla attributed to a “false trigger.” Regardless, the vehicle may have operated without key crash-avoidance systems enabled at unknown times. I live near Santiago Canyon, a winding road I specifically raised concerns about in relation to Tesla’s camera-only system. A recent fatal crash in that same area involving a Tesla with severe post-collision fire damage has made this complaint more urgent. My concerns appear predictive and ignored. Timeline of Complaints •12/20/2023: Rear camera issues begin. •12/22/2023: Initial service request opened. •12/31/2023 – 1/4/2024: Repeated documentation of pillar camera blindness and system failures. Tesla dismisses as fogrelated. Invoice #3000S0008955083 issued (Irvine, CA). •1/4/2024: I confirm continued camera blindness after service. •1/5/2024: I formally tell Tesla I believe the issue is due to a software update and is broader than one sensor. •7/11/2024: New concerns about GPS failure and power trunk documented in invoice #3000S0010537222 (Lake Forest, CA).Read more
