I am filing a safety complaint regarding my vehicle due to repeated mechanical failures that have caused it to become unreliable and unsafe. My vehicle has experienced ongoing issues despite multiple repair attempts. Most recently, the vehicle broke down again and left me stranded while I was transporting my mother to a critical cancer appointment and the 1st time had me stranded on the highway
... late at night. This situation created a serious safety concern, as we were left stuck in the vehicle without reliable transportation for the 3rd time. This is not an isolated incident. The vehicle has previously been in the repair shop for an extended period (over 50 days), for a separate issue that has been resolved. Despite these repeated failures and loss of use, the manufacturer, Stellantis, has declined a buyback request. I no longer feel safe operating this vehicle due to its unpredictability and repeated breakdowns. This poses a risk not only to myself and my passengers but potentially to others on the road. I am requesting that this issue be investigated as a potential safety defect.Read more

The vehicle infotainment system including the touchscreen display and Connectivity Master Unit malfunctioned. The touchscreen operates without driver input, the system freezes or glitches, audio activates unexpectedly, and phone functions behave erratically. The vehicle and components are available for inspection upon request. While the vehicle was being driven, the system initiated actions withou
... t driver input including unintended phone calls including calls to 911, sudden unexpectedly loud radio or audio volume, screen freezing, and loss of navigation. These events caused driver distraction and a startle effect while the vehicle was in motion, increasing safety risk to vehicle occupants and others on the road. The issue is ongoing intermittent and reproducible during normal driving conditions. The vehicle was presented to an authorized dealer multiple times. A software update was performed in an attempt to correct the issue, but the condition continued. The dealer advised that replacement of infotainment components such as the CMU or display may be required. The vehicle has been inspected by an authorized dealer on multiple service visits. The issue was also reviewed by the manufacturer customer experience case management team which acknowledged understanding of the malfunction but declined repair assistance. No police or insurance inspections have occurred. No warning lights or messages appear before the malfunction. Symptoms occur without warning and include random touchscreen activation, system freezing or rebooting, unintended phone dialing, and unexpected loud audio. The issue began after purchase and has continued despite repair attempts. Similar infotainment malfunctions including ghost touch behavior, screen freezing or rebooting while driving, unintended audio behavior, and dialing issues have been documented in manufacturer service bulletins affecting comparable vehicles, indicating this issue is not isolated.Read more

Incident date: Apr 10, 2026
This is a supplemental report to my previous filing regarding a 2023 VW ID.4 battery fire. On May 28, 2025, while being operated, the vehicle suffered a catastrophic thermal event. White smoke emanated from the front compartment followed by active flames. The vehicle was a total loss. Critical Safety Finding: A subsequent forensic examination conducted on October 29, 2025 (File 27739-01) by a cer
... tified fire investigator confirmed that after the initial fire was suppressed, the vehicle reportedly caught fire an additional time while being held at a tow/storage facility. Component Failure: The fire originated in the front compartment area. The failure is consistent with the "thermal runaway" risks identified in Recall 26V030, involving defective battery cell electrodes. Safety Risk: The multiple ignition nature of this failure poses a lethal risk to occupants, emergency responders, and tow/salvage facility operators. The fact that the vehicle reignited twice in storage, long after the initial suppression, suggests that current safety protocols or the scope of the existing recall may not fully address the volatility of this specific battery defect. Inspection Status: The vehicle was inspected by the manufacturer’s engineers and an independent forensic investigator. However, the manufacturer and the insurer are currently withholding the Photographic Report and Technical Field Report from the consumer. These records are vital to understanding the severity of the "multiple -ignition" event and should be subpoenaed or requested by NHTSA investigators to ensure the recall remedy is sufficient to prevent secondary fires in storage environments.Read more

My car stalls, jerks, and shuts off when in drive. More specifically when I turn it on, shift it in reverse, reverse, shift into drive. I press on the gas and nothing. Car doesn't move. Almost feels like something is going to fall out from under me. Every turn, bump, speed bump sometimes just at a stop then going into drive will create the loudest sound. Also sometimes it says I need to shift into park when I am in fact in park. When looking up common potential recalls my car checks almost every box that were stated online.Read more

Reporting a manufacturing defect in a 2022 Volvo XC60. The vehicle developed a metallic ticking noise in the timing area, audible at idle. Symptoms strictly match Volvo Technical Journal 36551.9.1 (NHTSA ID: MC-11015995-0001) regarding faulty camshaft seals. The noise disappeared when the oil filler cap was loosened, exactly as described in the TJ. An authorized Volvo dealer confirmed the defect a
... nd replaced the camshaft seals per Volvo’s protocol (Work Act No. ЗН-0131107). This repair completely resolved the issue, proving it was a manufacturing fault. The manufacturer refused warranty coverage citing 'salvage title' status, without proving any causal link between the title and this specific engine defect. Failure of timing components poses a significant safety risk, including sudden engine stall or oil leaks during operation. The owner was forced to pay out-of-pocket for the repair of a recognized factory defect. Supporting documents and video evidence are available upon request.Read more

The Auto Idle Stop system on my 2025 Honda Odyssey fails to restart the engine when the brake pedal is released quickly after a full stop. My safety and the safety of others has been put at risk multiple times because the vehicle stalled at traffic lights in active traffic, leaving me unable to accelerate through an intersection. The problem has been confirmed by Honda Marysville dealership. The dealer stated they could not repair the issue and advised me to modify my driving behavior as a workaround. No repair was performed. The vehicle has been inspected by the selling dealership. A message appears during the failure telling me to restart the car. The problem first appeared within the first year of ownership on a brand new vehicle with basically zero miles at purchase. The cause appears to be a defect in the Auto Idle Stop restart system that fails under specific braking conditions.Read more

While accelerating to highway speeds, the vehicle suddenly experienced a significant and continuous loss of power. Even with the accelerator fully pressed, the vehicle would not increase speed beyond roughly 50 mph. Based on the behavior and previous service history, the issue appears to be related to the engine, fuel delivery system, or overall powertrain. The incident created a serious safety c
... oncern for both the occupants and surrounding traffic. The vehicle could not properly accelerate, merge, or respond to normal highway conditions. Hazard lights had to be activated, and the vehicle remained in the slow lane for an extended period to avoid faster-moving traffic. The issue was consistent and repeatable throughout the drive. Multiple attempts to accelerate resulted in the same lack of response, and the loss of power persisted for the duration of the trip. The vehicle has not yet been inspected following this occurrence because when turned off and turned back on, the issue goes away and happens sporadically. It is also worth noting that the vehicle previously underwent a manufacturer-covered repair involving the high-pressure fuel pump, for which a warranty extension had been issued. This loss of power occurred after that repair was completed. No warning lights, alerts, or prior symptoms were present before or during the incident. Overall, the sudden loss of propulsion at highway speed posed an immediate safety risk, as the vehicle was unable to maintain appropriate speed or safely maneuver with traffic.Read more
